      • 트레드밀 운동시 흡기근 테이핑이 폐활량에 미치는 영향

        이 연구는 트레드밀 운동과 키네시오 테이핑의 효과에 따른 폐활량의 변화를 알아보기 위해 26명의 비흡연자가 참가하였으며 키네시오 테이핑을 적용하지 않은 그룹과 키네시오 테이핑을 적용한 두 그룹으로 무작위로 배정하고 각 군들을 주 3회 6주간의 트레드밀 훈련을 실시하였다. 실험 전과 후, 스파이로미터를 사용하여 키네시오 테이핑의 적용 따른 폐활량 변화 효과를 측정하였다. 이 연구의 결과를 종합해보면 키네시오 테이핑을 적용 그룹에서 적용하지 않은 그룹에 비해 FVC, FEV1에서 유의한 증가를 보였다. 이러한 결과는 건강한 성인에서 키네시오 테이핑을 적용한 경우 키네시오 테이핑을 적용하지 않고 트레드밀 운동을 한 경우보다 폐활량의 향상에 효과적이라고 생각되어진다. The purpose of this study was to investigate the variation of vital capacity(VC) according to the effects of kinesio taping with treadmill exercise. Twenty-six non-smokers were participated in this research and these subjects were randomly assigned into two groups. To measure the VC variation effects of kinesio taping, spirometer was used. The collected data were analyzed statistically by using a paired Mauchly test and repeated measure ANOVA. The results of this study were as follows; A group: treadmill with kinesio taping B group: treadmill without kinesio taping. Between A and B, there were significant differences. In the case of A group, there was 11.66% increase of VC, during 3 weeks experiment(p<.01). In the case of B group, there were significant differences, 3.35% increase of VC, during 3 weeks taping intervention(p<.01). After 6 weeks experiment, the improvement of VC shown a significant difference with intergroup (p<.05). From this result, it was revealed that treadmill exercise with kinesio taping was effective to improve VC to healthy adult than treadmill exercise without kinesio taping.

        두개골 및 두개봉합부 초기발육과정에서의 전사조절인자인 Msx2와 DIx5의 역할

        두개봉합부의 조기융합으로 일컬어지는 craniosynostosis는 두개봉합부에서의 골아세포의 조기분화 및 석회화의 결과로 나타나는 선천성 발육이상이다. 최근 유전학적 연구에 의하면 homeobox gene인 Msx2의 변이에 의해 Boston-type craniosynostosis가 야기되며, 또한 Dlx5 homozygote mutant mouse의 표현형에서 두개골의 골화지연을 포함한 다양한 두개안면부위의 이상을 발견하였다는 보고가 있었다. 게다가 Msx2와 Dlx5 homeodomain protein의 상호작용에 의해 성숙골아세포의 표지자인 osteocalcin의 전사를 조절할 수 있다는 사실이 알려져 있다. 이러한 일련의 결과들은 Msx2, Dlx5 및 osteocalcin 유전자들이 두개골의 골화과정과 두개봉합부의 형태발생에 중요한 역할을 담당하고 있음을 제시해주고 있다. 두개골의 성장과 두개봉합부의 형태발생시 이러한 유전자들의 기능을 알아보기위해 mouse의 태생기 (E15-E18) 동안 osteocalcin, Msx2, 및 Dlx5 유전자들의 발현양상을 조사하였다. Osteocalcin은 E15부터 두정골의 골막에서 관찰되었으며, 발생시기가 후기일수록 강한 발현양상을 나타내었다. Msx2는 시상봉합부의 미분화간엽조직과 osteogenic front에서 강하게 발현되었으며 경막과 hair follicle에서도 관찰되었다. Dlx5는 osteogenic front를 포함한 두정골의 골막에서 강하게 발현되었으나 시상봉합부의 미분화간엽조직에서는 발현되지않아, Msx2와는 발현양상의 차이를 나타내었다. 이 결과들을 종합해볼 때, Msx2와 Dlx5 유전자는 두개골과 두개봉합부의 성장발육과정에 중요한 역할을 담당하고 있으며, BMP signaling은 두 전사조절인자들을 조절하므로써 두개골의 골화과정과 두개봉합부의 형태발생 및 유지에 관여하고 있음을 제시해주고 있다. 특히 BMP signaling에 specific downstream gene인 Msx2 및 Dlx5의 발현양상의 차이는 골아세포의 분화시 이들 유전자가 각각의 독특한 기능을 가지고 있음을 시사해주고 있다. Craniosynostosis, known as a premature fusion of cranial sutures, is a developmental disorder characterized by precocious differentiation and mineralization of osteoblasts in the calvarial sutures. Recent genetic studies have demonstrated that mutation in the homeobox gene Msx2 causes Boston-type human craniosynostosis. Additionally, the phenotype of Dlx5 homozygote mutant mouse presents craniofacial abnormalities including a delayed ossification of calvarial bone. Furthemore transcription of osteocalcin, a mature osteoblast marker, is reciprocally regulated by the nomeodomain proteins Msx2 and Dlx5. These facts suggest important roles of osteocalcin, Msx2 and Dlx5 genes in the calvarial bone growth and suture morphogenesis. To elucidate the function of these molecules in the early morphogenesis of mouse cranial sutures, wer have first analyzed by in situ hybridization the expression of osteocalcin, Msx2 and Dlx5 genes in the developing parietal bone and sagittal suture of mouse calvaria during the embryonic (E15-E18) stage. Osteocalcin mRNA was found in the periosteum of parietal bones from E15, and gradually more highly expressed with aging. Msx2 mRNA was intensely expressed in the sutural mesenchyme, osteogenic fronts and midly expressed in the dura mater during the embryonic stage. Dlx5 mRNA was intensely expressed osteogenic fronts and the periostem of parietal bones. To further examine the upstream signaling molecules of transcription factor Msx2 and Dlx5, we have done in vitro experiments in E15.5 mouse calvarial explants. Interestingly, implantation of BMP2-, BMP4-soaked beads onto the osteogenic fronts after 48 hours organ culture induced etopic expressions of Msx2 and Dlx5 genes. On the other hand, overexpression of TGFβ1, GDF-6, -7, FGF-2, -4 and Shh did not induce the expression of Msx2 and Dlx5. Taken together, these data indicate that transcription factor Msx2 and Dlx5 play critical roles in the calvarial bone and suture development, and that BMP siganling is involved in the osteogenesis of calvarial bones and the maintenance of cranial sutures through these two transcriotpn factors. Furthermore, different expression patterns between Msx2 and Dlx5 suggest their specific functions in the osteoblast differentiation.

        원발성 피부 CD30 음성 대형 T세포 림프종 1예

        Primary CD30 negative large T cell lymphomas consist of CD30 negative large tumor cells without prior or concurrent mycosis fungoides. Clinically, it presents with solitary, localized or generalized plaques, nodules, or tumors and most cases show an aggressive clinical course. The histologic apperance may be identical to that of classic mycosis fungoides undergoing transformation into large cell lymphoma and large tumor cells represent at least 30 percent of the total tumor cell population. We experienced a case of CD30 negative large T cell lymphoma and report it with a review of literatures.(Korean J Dermaton 2003;41(10) : 1402~1405)

      • 일개 대학병원에서 일년간 분리된 반코마이신 내성 장구균의 임상 분자역학적 연구

        목적 : 한강성심병원에서는 2000년 3월 반코마이신내성 장구균이 처음 분리되어, 원내전파를 막기 위한 노력을 하였으나, 분리가 지속되었다. 따라서 이에 대한 좀더 자세하고 객관적인 자료를 얻기 위해 본원에서 분리된 반코마이신내성 장구균에 대한 임상분자 역학적 연구를 시행하게 되었다. 방법 : 2000년 3월부터 2001년 2월까지 한림대학교 한강성심병원에서 분리된 장구균을 대상으로 디스크 확산법, 최소발육억제 농도등의 측정을 통해 반코마이신 내성여부를 확인하였으며, 반코마이신내성 장구균을 대상으로 PFGE를 시행하여 형별 분석을 하였다. 결과 : 분리된 균주는 총 32균주로 모두 E. faecium이었다. 항생제 내성 검사 결과 모두 Van A형이었다. PFGE 결과 1a형이 8검체, 1b형이 5검체, 2a형이 4검체, 2b형이 4검체, 3a형이 2검체, 3b형이 5검체였고, 그 외 4, 5, 6형이 각각 1검체씩이었다. 총 32균주 중 56%(18/32)의 균주가 같은 시기, 같은 병실에서 동일한 PFGE 형을 보이면서 분리되었다. 결론 : 반코마이신내성 장구균의 PFGE 분석 결과 여러 종류의 반코마이신내성 장구균이 유행하였음을 알 수 있다. 그러나 같은 기간, 같은 병실에서 분리된 균주의 PFGE 형이 같은 것이 상당수 있었다. 이는 반코마이신내성 장구균의 원내 전파가 있었음을 의미한다. 따라서 반코마이신내성 장구균의 전파의 발생 및 전파 방지를 위해서는 보다 효과적인 감염관리 활동이 필요할 것으로 생각된다. Background: Vancomycin-resistant enterococci(VRE) have been a rising problem worldwide. In March 2000, VRE was first isolated from a patient in Hangang Sacred Heart hospital. Although efforts to prevent transmission of VRE were performed, isolations continued. So molecular epidemiological study of VRE was done. Method : The minimal inhibitory concentration(MIC) to vancomycin and teicoplanin, and pulsed field gel electrophoresis(PFGE) pattern of VRE isolated from March 2000 to February 2001 were evaluated. Results : 32 VRE were isolated. All of the VRE were Enterococcus faecium and showed Van A resistance phenotype. We found out that a significant number of VRE isolated during same period and in same room, were same PFGE patterns. (18 cases/ 32 isolates= 56%) Conclusion: This study demonstrated the spread of VRE of same PFGE patterns. It suggests the nosocomial spreads of VRE.

      • 담금원료에 따른 고추장의 숙성기간 중 품질변화

        Kochujang was prepared with different raw materials, and with and without sake. Quality changes during fermentation of Kochujang were measured in terms of pH, titratable acidity and amino nitrogen content. pH of Sunchang Kochujang gradually decreased as the increase of fermentation time, while that of Sachun remained almost constant. Titratable acidity was the highest in Sunchang, and was similar in Sachun regardless of glutinous rice and wheat flour as a starch source. Amino nitrogen of Sunchang increased drastically by 10 days of fermentation and then increased gradually. Amino nitrogen of Sachun with glutinous rice was gloater than that with wheat flour. pH of Kochujang without sake decreased drastically by 10 days and then stayed almost the same by 60 days of fermentation, while that with sake increased slightly as the increase of aging time. Titratable acidity of Kochujang with sake decreased sharply by 10 days, increased gradually by 30 days and then stayed at that level. Kochujang without sake showed greater decreases in titratable acidity by 5 days, increased gradually by 20 days and then reached equilibrium. Amino nitrogen was a little higher in Kochujang without sake than that with sake.

        The structure of sodium trans-bis(methyliminodiacetato) chromate(Ⅲ), C_10H_14O_8N_2CrNa, has been elucidated with two space groups P 1-; a=8.9856(6), b=13.308(1), c=6.261(1)Å, α=76.38(1), β=110.361(9), r=107.394(6)˚, V=662(2)Å^3, Z=2, μ=0.47mn^(-1), d=1.831Mg/㎥, R=0.0295 for 1901 independent reflections with F^2>2σ(F^2), and A2/n; a=13.65(5), b=6.261(1), c=16.85(6)Å, β=113.05(1)˚, V=1324(7)Å^3, Z=4, R=0.0287 for 1013 independent reflections with F^2>2σ(F^2). There are two different asymmetric half molecules in triclinic system and one asymmetric half molecule in monoclinic system. The structures of three different molecules are very similar in conformations. The four carboxyl O atoms and the two imino N atoms in each molecule produce a distorted octahedral coordination geometry around the Cr atom. The average distances of Cr-O(carboxyl) and Cr-N(imino) are 1.956 and 2.061Å respectively. Molecules are held together by ionic forces. In this paper, it has been also shown that the cell constants, Miller indices, atomic coordinates, lattice directions and reciprocal lattices of the two space groups can be mutually transformed.

        두경부 마사지가 중환자실 환자의 수면과 불안에 미치는 효과

        Purpose: This study was to apply head and neck massage to patients in intensive care unit and to inventigate the effect of that massage on sleep and state anxiety. Method: The subjects in this study were 27 patients who were admitted in medical intensive care unit. The study was performed from June thru September of 2005 on the One-group pretest-posttest design and the sleep, state anxiety of the subjects were measured before and after head and neck massage. For data analysis, paired t-test and Pearson correlation coefficient were utilized. Result: The first hypothesis that the subjects might have a better sleep after being exposed to head and neck massage was accepted. The second hypothesis that the subjects might feel less state anxiety afrer being exposed to head and neck massage was accepted. The third hypothesis that the sleep of the ICU patients maight be correlated to their anxiety was accepted, as there appeared correlation between their sleep and anxiety. Conclusion: Head and neck massage is identified as one of independent nursing interwentions to improve the sleep of ICU patients and ease their anxiety, and it is necessary to apply it to clinical practices.
