      • DC-SIGN expressiOn m Hofbauer cells may play an important role in immune tolerance in fetal chorionic villi during the development of preeclampsia.

        Immune tolerance at feto-maternal interfaces is a complex phenomenon. Although maternal decidual macrophages are well-known immune cells, little is known about fetal-derived macrophages (Hofbauer cells) within chorionic villi. Preeclampsia (PE) is a major cause of maternal mortality in the field of obstetrics, and the innate immunological role of maternal decidual macrophages is well known. In this study, we assessed the differential phenotypes and marker expressiOn m fetal macrophages, known as dendritic cell- specific ICAM -grabbing nonintegrin (DC-SIGN) -positive Hofbauer cells. We compared Hofbauer cell properties between normal and PE placenta chorionic villi and performed sequential staining of DC-SIGN, CD14, and CD68 to evaluate the existence of Hofbauer cells. Furthermore, to evaluate the immunological function of these cells, we stained the cells for CD163, a marker of immunoregulatory type 2 (M2) macrophages. Additionally, we examined the expression of the immunosuppressive cytokine interleukin (IL) -10, which is known to be produced by M2 macrophages. DCSIGN +/CD14+, DC-SIGN +/CD68+, and CD163+/DC-SIGN + cells were quantified based on photomicrographs. The results showed that CD14, CD163, DC-SIGN, and IL-10 levels were significantly downregulated in PE compared with normal. Additionally, CD 163 +/DC -SIGN+ Hofbauer cells were significantly less frequent in PE than in normal. DC-SIGN Hofbauer cells produced IL-10 at lower levels in the PE than in the normal. Thus, we speculate that fetal-derived Hofbauer cells may play an important role in normal pregnancy with immunosuppressive effects based on their M2 macrophage characteristics to maintain immune tolerance during pregnancy. Additionally, in PE, these functions were defective, supporting the roles of these macrophages in PE development.

        원발성 자궁내막 편평상피암 1 예

        Primary endometrial squamous cell carcinoma is extremely rare, with only 64 cases repo- rted in the literature. In 1928 Fluhmann proposed three criteria for differentiating primary from secondary endometrial squamous cell carcinoma:(1) no coexisting endometrial adenocarcinoma (2) no connection between the endometrial tumor and the squamous epithelium of the cervix, and (3) no squamous cell carcinoma of the cervix present. Several authors have modified these criteria in their case presentations. Kay accepts the presence of an in situ cervical carcinoma, if there is no connection between cervical carcinoma in situ and the endometrial tumors. We have experienced a case of simultaneous carcinoma in situ of the cervix and endomet- rial squamous cell careinoma. This case was treated with chemotherapy(cisplatin, 5-fluoruracil) and external radiation following extended hysterectomy and bilateral salpingooophorectomy We report this case with a review of the concerned literatures.

        자궁경부암 환자에서 측정한 혈중 Interleukin-6 및 Tissue-Plasminogen Activator 농도

        1992년 8월부터 1993년 2월까지 연세대학교 의과대학 산부인과에 내원한 침윤성 자궁경부암 환자군 30명, 자궁경부 상피종양 환자군 20명 및 대조군 10명을 대상으로 혈중 IL-6 및 t-PA를 측정하여 다음과 같은 결과를 얻었다. 1. 연령분포는 침윤성 자궁경부암환자군의 경우 32세에서 72세까지로 평균 50세, 자궁경부 상피내종양환자군은 26세에서 60세까지로 평균 39세, 대조군의 경우 23세에서 53세까지로 평균 39세이었다. 2. 각 군간의 IL-6의 검출률은 각각 6/30(20%), 3/20(15%), 1/10(10%)로 침윤성 자궁경부암환자군에서 증가한 경향을 보였으나 유의한 차이는 없었으며 t-PA경우 전례에서 검출되었다(p$gt;0.05). 3. 각 군 사이의 평균 혈중 IL-6 측정치는 각각 30.33 pg/ml, 29.20 pg/ml, 12.45 pg/ml로 통계학적 의의는 없었으나 대조군에 비하여 침윤성 자궁경부암 및 자궁경부 상피내종양 환자군에서 증가된 경향을 보였으며, t-PA측정치는 각각 6.15 ng/ml, 4.61 ng/ml, 5.00 ng/ml로 세군간의 유의한 차이는 없었다(p$gt;0.05). 4. 병기에 다른 각군간의 IL-6측정치는 I기 28.00 pg/ml, II기 33.50 pg/ml이었고 III기이상에서는 검출이 되지 않았다. t-PA는 각각 6.89 ng/ml, 4.59 ng/ml, 7.09 ng/ml로 세 군간의 유의한 차이는 없었다(p$gt;0.05). 이상의 결과로 IL-6 및 t-PA 의 단독으로 종양표지물질로의 사용보다 다른 요소와 병행하여 사용 가능성이 연구되어야 할 것으로 생각된다. Interleukin-6 (IL-6) is a multifuctional cytokine and deregulated IL-6 gene expession has been implicated as being involved in the pathogenesis of a number of disease. Tissue-plasminogen acgivator (t-PA) is serine protease which secreted in most tumor cell line and participates in extracellular tissue degradation of tumor cell. So we measured the blood levels of Il-6 & t-PA to elucidate the relationship between IL-6, t-PA and stage of cervical carcinoma and evaluate the value of IL-6 and t-PA as new tumor marker in cervical carcinoma. The blood levels of IL-6 & t-PA were measured in 60 patients admitted to the Severance hospital, from August, 1992 to February, 1993, of whom 30 patients were diagnosed as invasive cancer of the cervix, 20 as CIN, and 10 controls. The average serum level of IL-6 & t-PA in invasive cervical cancer, CIN and control person were respectively 30.33 pg/ml, 29.20 pg/ml, 12.45 pg/ml and 6.15 ng/ml, 4.61 ng/ml and 5.00 ng/ml. In 20% of invasive cervical cancer patients (6/30), and 15% of CIN patients (3/20) studied blood levels of IL-6 were detected whereas only 20% of control persons showed the same results. But the level of t-PA was detected in all groups. These results suggest the solitary tumor markers of IL-6 and t-PA is not justified and need more factors to use as new tumor markers.

        난소의 성숙기형종에서 발생한 주 유암종 1 례

        Carcinoid tumors, producing biogenic amines and various polypeptides, arise from the diffuse peripheral endocrine system. They are most frequently localized in different parts of the gastrointestinal tract and less frequently in bronchi, biliary tract and ovaries. Carcinoid tumors of the ovary are classified into two categories: primary and metastatic. The primary carcinoid tumors are classified as follows: insular or islet carcinoid(carcinoid tumors of the midgut type); trabecular carcinoid (carcinoid tumors of the foregut or hindgut type); struma carcinoid (struma ovarii and carcinoid or carcinoid tumor combined with thyroid tissue). Primary trabecular carcicnoid of the ovary is an uncommon ovarian tumor, begaving like a tumor of low malignant potential. In the younger patient the treatment is unilateral salpingo-oophorectomy, while in the postmenopausal paient bilateral salpingo-oophorectomy and hysterectomy is the treatment of choice. Long term follow up is advisable. Authors have experienced a case of primary trabecular carcinoid tumor in mature ovarian teratoma and reported with a brief literature review.

        임신성 고혈압이 합병된 산모에서 독립적 주산기 예후인자로서의 도플러 제대동맥 혈류속도파형

        임신성 고혈압이 합병되어 서울대학교병원 산부인과에 내원한 산모 68명을 대상으로 제대동맥 혈류속도파형 분석을 시행하여 제대동맥 혈류속도파형 이상을 보인 군과 제대동맥 혈류속도파형이 정상이었던 군에서 주산기예후를 비교분석한 결과 다음과 같은 결론을 얻었다. 1. 조산(p$lt;0.01), 제왕절개를 요하는 태아곤란증(p$lt;0.0001), 분만 1분 및 5분후 Apgar 점수가 7이하인 경우(p$lt;0.001), 신생아 중환자실 입원(p$lt;0.0001), 주산기 사망(p$lt;0.0001), 주산기예후 불량(p$lt;0.0001), 그리고 유의한 신생아 이환 및 사망(p$lt;0.0001)은 제대동맥 혈류속도파형 이상을 보인환자군에서 더 많았고 출생시 신생아체중(p$lt;0.0001)은 제대동맥 혈류속도파형 이상을 보인 군에서 적었다. 2. 제대동맥 혈류속도파형 이상소견은 분만시 임신주수에 비해 주산기예후 불량(odds ratio, 27.1 : p$lt;0.001)과 유의한 주산기 이환 및 사망(odds ratio, 38.3 ; p$lt;0.01)에 미치는 영향이 월등하였다. 3. 제대동맥 혈류속도파형 이상소견은 분만시 임신주수를 고려하더라도 독립적인 주산기예후 예측인자로 사용될 수 있을 것으로 사료된다. An abnormal umbilical artery Doppler waveform is a risk factor for adverse perinatal outcome. However, it has not been established whether this is due to earlier gestational age at delivery of fetuses with abnormal Doppler findings or if Doppler findings are an independent predictor of perinatal outcome. This study was designed to determine if an abnormal Doppler umbilical arery waveform is associated with adverse perinatal outcome even after controlling the gestational age at delivery as a confounding variable in patients with preeclampsia. Umbilical artery velocimetry studies were performed within 2 weeks of delivery in 68 consecutive patients with preeclampsia who admitted to our unit. Adverse perinatal outcome was defined as: fetal distress requiring cesarean delivery, Apgar score$lt;7 at 5 min, significant neonatal morbidity, or perinatal death. Significant neonatal morbidity was definds as: neonatal sepsis, intraventricular hemorrhage(grade 2-4), respiratory distress syndrom, pneumonia, or acute renal failure. Stepwise multiple logistic regression was used for statistical analysis. Patients with abnormal umbilical artery velocimetry had a significantly higher rate of complications, including cesarean section for fetal distress, preterm delivery, low Apgar scores, significant neonatal morbidity, and perinatal death than patients with normal waveform. Stepwise logistic regression analysis indicated that an abnormal umbilical artery waveform was a significant independent predictor of the delvelopment of adverse perinatal outcom(odds ratio, 27.1;p$lt;0.0001) and significant neonatal morbidity/mortality(odds ratio, 38.3;p$lt;0.01) after adjustment of other confounding variables. An abnormal Doppler umbilical artery waveform is a strong and independent predictor of adverse perinatal outcome in patients with preeclampsia.

        Tooth survival and periodontal tissues healing of allogenic-transplanted teeth in the mice

        <P>Objective: </P><P>For the successful organ transplantation, immune rejection has to be considered. Autogenic transplantation of human teeth is generally carried out free of clinical difficulty because of the lack of immune reaction, whereas allogenic tooth transplantations easily induce host immune rejection to donor tissues. The aim of the present study was to evaluate the replacement of donor tissue by host cells after allogenic tooth transplantation</P><P>Materials and methods: </P><P>First molars extracted from <I>lacZ</I> transgenic ROSA26 mice were transplanted into the alveolar socket and the tongue of host wildtype mice, where the first molar had existed.</P><P>Results: </P><P>Donor cells from <I>lacZ</I> transgenic mice were not detected in the periodontal ligament space, but rather in the pulp chamber of the donor tooth. Furthermore, if the pulp chamber was widely open to an affluent blood supply, odontoblasts and fibroblasts in the donor tissue survived in the dental pulp.</P><P>Conclusions: </P><P>Our experimental models using <I>lacZ</I> transgenic ROSA26 mice clearly demonstrate that donor periodontal tissue cells are replaced by host cells and that periodontal tissue can regenerate after allogenic tooth transplantation. Furthermore, our models suggest that donor pulpal cells can survive if the vascular supply into the pulp chamber is sufficient.</P>

        Resolvin D1 attenuates activation of sensory transient receptor potential channels leading to multiple anti-nociception : Resolvin D1 inhibits sensory TRPs

        <P>BACKGROUND AND PURPOSE Temperature-sensitive transient receptor potential ion channels (thermoTRPs) expressed in primary sensory neurons and skin keratinocytes play a crucial role as peripheral pain detectors. Many natural and synthetic ligands have been found to act on thermoTRPs, but little is known about endogenous compounds that inhibit these TRPs. Here, we asked whether resolvin D1 (RvD1), a naturally occurring anti-inflammatory and pro-resolving lipid molecule is able to affect the TRP channel activation. EXPERIMENTAL APPROACH We examined the effect of RvD1 on the six thermoTRPs using Ca(2+) imaging and whole cell electrophysiology experiments using the HEK cell heterologous expression system, cultured sensory neurons and HaCaT keratinocytes. We also checked changes in agonist-specific acute licking/flicking or flinching behaviours and TRP-related mechanical and thermal pain behaviours using Hargreaves, Randall-Selitto and von Frey assay systems with or without inflammation. KEY RESULTS RvD1 inhibited the activities of TRPA1, TRPV3 and TRPV4 at nanomolar and micromolar levels. Consistent attenuations in agonist-specific acute pain behaviours by immediate peripheral administration with RvD1 were also observed. Furthermore, local pretreatment with RvD1 significantly reversed mechanical and thermal hypersensitivity in inflamed tissues. CONCLUSIONS AND IMPLICATIONS RvD1 was a novel endogenous inhibitor for several sensory TRPs. The results of our behavioural studies suggest that RvD1 has an analgesic potential via these TRP-related mechanisms.</P>
