        심상성 루프스에서 Nested-Primer Gene Amplification Assay에 의한 결핵균 DNA의 검출

        Lupus vulgaris, which is a progressive form of postprimary tuberculosis in patients with a moderate to high degree of immunity and tuberculin sensitivity, is the most common, most serious, and most variable type of cutaneous and mucous membrane tuberculosis. Domonstration of M.tuberculosis directly or in culture in some of these eruptions can be difficult. It is essential to demonstrate the presence of M.tuberculosis in a clinical sample for a definitive diagnosis of tuberculosis. With the discovery of the polymerase chain reaction (PCR), the DNA diagnostic approach for infection has developed in a short period. With the dual purpose of definitive and rapid diagnosis of tuberculosis, this study examined the usefulness of a nested PCR for detection of M. turberculosis. Compared with a single-step PCR, the two-step PCR was able to enhance sensitivity approximately 1,000-fold on the basis of bacterial counts and DNA quantity. We concluded that two-step nested PCR appeared to be the most useful PCR protocol for early and sensitive diagnostic method detecting M.tuberculosis in acid-fast stain negative specimens.

      • 경주지역 여고생의 초경과 체성장간의 관계

        본 연구는 경주지역 1개 고등학교 3학년 여고생을 대상으로 초경 시작 전인 초등학교 4학년부터 고등학교 2학년까지의 키, 몸무게, 가슴둘레 수치와 초경 연령을 파악하여 여성의 초경이 빠를수록 체성장 속도의 변화가 있을 것이라는 가설을 기초로 두 요인간의 관련성을 파악하고자 시행되었다. 경주시 1개 여자고등학교 3학년생 총 317명에 대해 ㅊ경 나이, 초경시 심리 상태, 초경이 시작되었을 때의 상담 여부와 상담하였던 사람, 부모 신장과 초경전후의 식사 규칙성, 다이어트 경험, 수면시간, 스트레스, 음주/흡연 여부, 질병력 등에 대한 설문 조사를 시행하였고, 초등학교 4학년부터 고등학교 2학년까지의 키, 몸무게, 가슴둘레에 대한 정보는 건강기록부에서 확보하였다. 상기 대상자 중 건강기록부가 분실된 39명, 초경나이에 대해 응답하지 않았던 29명 및 결석 등으로 조사를 하지 못하였던 42명을 제외한 조사 대상자는 206명으로 선정하였다. 대상자들의 초경연령 평균값(Mean±SD)은 13.0±1.11이었고, 초등학교 4학년부터 고등학교 2학년까지의 초경 이후 신장 성장률은 감소하였으며, 초경연령이 빠른 군이 초등학교 때는 신장에 있어서 우위를 차지하나 고등학교에 오면서 신장은 비슷한 수준이 되었다. 초경후 1-2년 사이에 체중과 흉위의 증가율은 감소하였고, 초경연령이 빠른 군이 초등학교와 고등학교 사이에 체중과 흉위에서 모두 우위를 차지하였다. 본 연구의 결과는, 흔히 이차 성징의 시작 시점 혹은 사춘기의 시작 시점으로 보고있는 초경 연령이 청소년기의 성장과 밀접한 관련성이 있고, 특히 체중과 흉위의 성장 속도와 밀접한 관련성이 있음을 시사한다. To understand the onset of menarche in relation to changes in physical growth that take place during female adolescence, especially the changes in height and weight velocity. The 207 senior in one women high school of Kyongju were investigated in April 3 to 8, 2001. We collected the information, relation in the age at menarche, height & weight of parents, regularity of eating, and obesity diet, duration of sleeping, sickness & stress near age at menarche, by self-recording. The status of height, weight & chest circumference between the 4th grade of primary school and the junior of high school was collected by individual Health Record. The results were followed; The average age at menarche was 12.95±1.11 years old and the most frequent age of menarche is 13 years. After menarche, the velocity of height, weight and chest circumference were decreased. The most rapid group of menarche hold dominant position in height during elementary school. As time passed, they dont hold dominant position in height. The most rapid group of menarche get an advantage in the weight and the girth of chest after elementary school. The height of subjects is directly proportional to the height of their parents. These results suggest that the starting age at menestuation can be closely associated with physical growth and development.

      • 영상 합성 시스템 구현

        본 논문에서는 사진 및 영상을 편집, 합성할 수 있는 프로그램을 구현하였다. 구현한 프로그램은 영상 합성하는 과정에서 색체와 단색 배경을 분리하여 새로운 영상을 합성시키는 크로마 키(Chroma key)기법을 이용하여 영상을 합성하는 기능을 구현하였으며, 실시간 영상을 합성과 영상 편집 기능 등을 추가하여 사용하기 편리하게 구현하였다. 기존의 영상을 합성하는 프로그램들은 고가이며, 전문 사용자 위주로 제작되었다. 본 논문에서 구현한 프로그램은 일반 사용자를 위한 쉽고 간단하면서 다양한 기능을 제공하고 있다.

        비만치료제 의약품이상사례 실마리정보 검색 : 국내 의약품 이상사례보고시스템 자료를 활용하여

        The objective of this study was detection of signals corresponding to adverse drug reactions following obesitytreatments using the Korea Adverse Event Reporting System Database. Eight obesity drugs recommended in the obesitytreatment guidelines were analyzed for adverse events reported from January 2009 to December 2018. The ratio ofreporting, ratio of reporting odds, and information components were analyzed, and the case in which all three criteria weremet was detected as a signal. Signals that were not included in the label were classified as new signals. In the case ofphentermine, phendimetrazine, diethylpropion, lorcaserin, mazindol, liraglutide, or listat, and naltrexone/bupropion, therewere 2, 10, 5, 6, 1, 10, 7, and 5 confirmed cases of signal detection, respectively. No new signal was detected forphentermine and mazindol adverse events. For phendimetrazine, diethylpropion, lorcaserin, liraglutide, orlistat, andnaltrexone/bupropion, there were 5, 2, 1, 7, 4, and 1 cases of new signals, respectively. This study assessed signal detectionrelated to adverse events for eight obesity drugs. It is necessary to consider whether permission information should bereflected through continuous research on the detection of signals that are not reflected in the existing permits.

        Chronic lymphocytic leukemia (CLL) is the most common type of leukemia occurring in Western nations. In CLL it is well known that the risk of a secondary malignancy is higher than in the normal population. But in Korea, CLL is a rare type of leukemia, so there have been only a few reported cases with a secondary malignancy. CLL is characterized by progressive defects in both cell-mediated and humoral immunity. It is known that defects in the immune system of patients with CLL contribute to the development of a secondary malignancy. We experienced a case of a 71-year-old man who suffered from a chronic cough and was diagnosed with small cell lung cancer coexisting with CLL. Until this case, there was no reported case in Korea of small cell lung cancer coexisting with CLL. We now report a case of small cell lung cancer coexisting with CLL and present a literature review.

      • Development of Electromagnet Brake Robot Finger for Highly Dexterous Motion through a Single Motor

        In this paper, a novel braking mechanism is proposed to enhance the dexterity of the robot finger through a single motor. The proposed braking mechanism has very simple structure, which consists of a brake shoe and an electromagnet switch per each joint. The electromagnet switch gives a brake shoe a trigger signal for brake, the brake shoe plays an important role to induce a large braking force/torque for a joint. In order to provide the design criteria of the brake shoe, we derive the brake condition based on the static analysis of the proposed mechanism, and it is numerically analyzed to determine the geometric shape of the brake shoe. Through the experiment, the effectiveness of the proposed mechanism is also verified.

        Development of a New Personal Magnetic Field Exposure Estimation Method for Use in Epidemiological EMF Surveys among Children under 17 Years of Age

        A number of scientific researches are currently being conducted on the potential health hazards of power frequency electric and magnetic field (EMF). There exists a non-objective and psychological belief that they are harmful, although no scientific and objective proof of such exists. This possible health risk from ELF magnetic field (MF) exposure, especially for children under 17 years of age, is currently one of Korea’s most highly contested social issues. Therefore, to assess the magnetic field exposure levels of those children in their general living environments, the personal MF exposure levels of 436 subjects were measured for about 6 years using government funding. Using the measured database, estimation formulas were developed to predict personal MF exposure levels. These formulas can serve as valuable tools in estimating 24-hour personal MF exposure levels without directly measuring the exposure. Three types of estimation formulas were developed by applying evolutionary computation methods such as genetic algorithm (GA) and genetic programming (GP). After tuning the database, the final three formulas with the smallest estimation error were selected, where the target estimation error was approximately 0.03 μT. The seven parameters of each of these three formulas are gender (G), age (A), house type (H), house size (HS), distance between the subject’s residence and a power line (RD), power line voltage class (KV), and the usage conditions of electric appliances (RULE).

        <P>Chronic hyperglycemia is toxic to pancreatic beta-cells, impairing cellular functioning as observed in type 2 diabetes; however, the mechanism underlying beta-cell dysfunction and the resulting apoptosis via glucose toxicity are not fully characterized. Here, using MIN6N8 cells, a mouse pancreatic beta-cell line, we show that chronic exposure to high glucose increases cell death mediated by Bax oligomerization, cytochrome C release, and caspase-3 activation. During apoptosis, glucokinase (GCK) expression decreases in high-glucose-treated cells, concomitant with a decrease in cellular ATP production and insulin secretion. Moreover, exposure to a chronically high dose of glucose decreases interactions between GCK and mitochondria with an increase in Bax binding to mitochondria and cytochrome C release. These events are prevented by GCK overexpression, and phosphorylation of proapoptotic Bad proteins in GCK-overexpressing cells is prolonged compared with Neo-transfected cells. Similar results are obtained using primary islet cells. Collectively, these data demonstrate that beta-cell apoptosis from exposure to chronic high glucose occurs in relation to lowered GCK expression and reduced association with mitochondria. Our results show that this may be one mechanism by which glucose is toxic to beta-cells and suggests a novel approach to prevent and treat diabetes by manipulating Bax- and GCK-controlled signaling to promote apoptosis or proliferation.</P>

        류마티스 관절염 환자에서 Adalimumab 사용 후 발생한 사르코이드증

        Adalimumab is a full human monoclonal antibody that inhibits tumor necrosis factor-alpha (TNF-α). This has recently been shown to be effective in the treatment of rheumatoid arthritis (RA), ankylosing spondylitis, and other conditions. Sacoidosis is known to be the target for adalimumab but we describe a patient who has developed sarcoidosis with lung involvement during adalimumab therapy for RA. A 48-year-old woman, who was treated with adalimumab for 5 months, was admitted because of chronic cough and both hilar lymphadenopathy on chest radiography. Chest computed tomography revealed the enlargement of multiple lymph nodes in the right supraclavicular, subcarinal, both hilar and right axillary area. She was diagnosed with sarcoidosis based on the biopsy of supraclavicular lymph node, skin and lung through video-associated thoracoscopic surgery, which was non-caseating epitheloid cell granuloma and excluded from a similar disease. She was treated for sarcoidosis with prednisolone and methotrexate instead of adalimumab.
