      • 위 MALT 림프종에서 다발성 림프절 전이로 오인된 동시성 림프절 결핵 1예

        28세 남자가 내시경적 점막절제술을 통해 저등급 위MALT 림프종으로 진단되었다. 병기설정을 위해 18F-FDG PET 검사를 시행하였고 우측 쇄골상부, 우측상/하부 기관주위, AP window 부위에 강한 섭취가 관찰되어 전이성 림프절로 판단하였다. 그러나, 우측 쇄골상부 림프절에서 시행한 절제생검에서 만성육아종성염증소견이 관찰되었고 조직의 결핵 중합효소연쇄반응에서도 양성 반응을 보였다. 2개월간 항결핵제의 사용 후 시행한 추적 컴퓨터단층촬영 검사에서 림프절 크기의 감소를 보여 최종적으로 위 MALT 림프종에서 광범위 림프절 전이로 오인된 동시성 림프절 결핵으로 진단되었다. A 28-year-old man was diagnosed as gastric MALT lymphoma in diagnostic EMR (endoscopic mucosal resection). He subsequently underwent an 18F-FDG PET. 18F-FDG PET showed intense multifocal uptake in right supraclavicular, right upper and lower paratracheal, and AP window areas that was considered radiologically to represent widespread metastatic lymphadenopathy. Excisional biopsy of a supraclavicular lymph node, however, revealed chronic granulomatous lymphadenitis with a positive reaction of Tb-nested PCR. After antituberculosis treatment for 2 months, a follow-up CT scan showed complete resolution of the lesions. Intense 18F-FDG uptake could be due to an infectious process such as synchronous tuberculous lymphadenitis. Therefore, this could mimic a malignant condition such as lymphoma with extensive lymph node metastasis due to a false-positive finding, which may lead to a misdiagnosis.

      • Seyferth시약의 발암성과 반응메카니즘 연구

        The reaction mechanism and molecular carcinogenetic property for the reaction of Seyferth's reagents such as phenyl(bromodichloromethyl)mercury and phenyl(tribromomethyl) mercury with food additives as dehydroacetic acid and coumarin has been studied under the conditions fo UV/vis photolysis and laser flash photolysis. 1. The reaction reactivities of Seyferth's regents as phenyl(bromodichloromethy1)mercury and phenyl(tribromomethyl) mercury are compared in all reaction conditions. The reactivity of phenyl(bromodichloromethy1) mercury is higher than the reactivity of phenyl(tribomomethy1) mercury at any reaction conditions in this research. This means that the leaving effect of : CCl₂carbene is higher than that of CBr₂carbene from mercury atom in Seyferth's reagent. 2. The yield of a highly toxic carcinogenetic carbene intermediate in the condition of UV/vis photolysis is less than those of laser flash photolysis. It is explainable that the reactions of Seyferth's reagent with the food additives are inhibited compared with the dimerization reaction of dihalogen carbenes each other. 3. The reactions of Seyferth' s reagents with dehydroacetic acid and coumarin are accelerated increasing with the negative(-) values of the bond energy of HgCCX₂and σ MO energy.

      • 위 유문동 평활근 세포의 전기적 활동에 미치는 Vanadate의 효과

        Effects of vanadate on the resting membrane potential and slow waves of the circular smooth muscle were observed using glass capillary microelectrodes. The obtained results were as follows: 1. Vanadate did not show any effects on; the slow waves of spontaneously beating preparations. 2. Variation of the resting membrane potential due to the change of outside K^+ was 20-28m V/decade which suggested the significant contribution of Na^+ to maintain the resting membrane potential. 3. Vanadate did not show any further effects on the depolarized state about l0mV by outside 20mM K^+. 4. Vanadate induced the depolarization in the low outside K^+ condition and the effect was exaggerated when Cal^(2+) was depleted. 5. Vanadate induced the depolarization in the low Na^+ and K^+ condition. It was conjectured that the effects of vanadate were enhancing the voltage-dependent calcium.

        Background : The Outcome of Infants weighing less than 1,500 g(very low birth weight infants : VLBWI) reflect recent progress in neonatal intensive care. In this study, we evaluated mortality and morbidity of very low birth weight (VLBW) infants during the past eleven years. Methods : A retrospective review of medical record was analyzed for VLBWIs admitted to the neonatal intensive care unit of Kosin University Gospel hospital between January 1, 1996 and March 31 2007. Results : The incidence of VLBWI was 3.6%. Overall survival rate of VLBWI was 65.9%. The survival rate increased significantly over time(period Ⅰ: 49.4%, period Ⅱ : 82.4%, P<0.05). The survival rate increased as birth weight increased. The incidence of respiratory distress syndrome was 70.3%(102/171), bronchopulmonary dysplasia 32.1% (36/171), intraventricular hemorrhage 19.4%(33/171), sepsis 24.4%(41/171). The main causes of death were respiratory distress syndrome and sepsis. Conclusion : The survival rate of VLBWI during recent 11 years was 65.9%. Survival rate increased as birth weight and gestational age increased. More efforts are needed to improve the long term outcome by reducing common morbidities.

      • 준설수심에 따른 사천만의 해수유동 특성에 관한 연구

        This study on the characteristics of the tidal current at the half closed bay selected Sachoˇn Bay located in the western Kyung-nam area as the object location of the study. Then, I analyzed the current with the varying dredging depth after the depth integral of the three Dimensional continuous equation and Navier-stokes equation. The study would get the following results, and the results could be used meamingfully as the analysis material about the dredging depth's influnce on the surrounding ocean when developing the coastal port. 1. When measuring the tidal current, there is a bit dispersion in the current direction of the observed object, but it is the wind effect at the bay. Considering that the calculated result is the shallow-water equation integranted in depth, the results can be concluded to in accored with my expectancy. 2. In case of reclaiming and dredging, the flow velocity and direciton change constantly, compared with the present condition. 3. At the wide bay dredge depth does not arrect the level of the ocean, but in case of inflow from the flood, the increase of the inflow of the river water affects the level of the ocean. 4. As the dredge depth grows deep, the maximum flood tidal current and obb tidal current at the front coast of the reclaimed decreases a little. However, the wide area Pc-7~Pc-9 doesn't show any change. As we saw in the above result, feacturcs of the tidal current as the level of the ocean and the current velocity don't change in the aspect of the ocean engineering even if we increase the degree of the dredge depth up to 6m in Sachoˇn Bay

        모터구동 밸브 주기적 안전성 확인을 위한 중요도 분류

        Safety-related motor operated valve(MOV) safety significance for Ulchin Unit 3 was categorized. The safety evaluation of MOV of domestic power plants affects the generic data used for the quantification of MOV common cause failure(CCF) events in Ulchin Units 3&4 PSA> Therefore, in this paper, MGL(multiple greek letter) parameter β, used for the evaluation of MOV CCF probabilities in Ulchin Units 3&4 probabilistic safety assessment(PSA), was re-estimated and the MOV safety significance was categorized. The re-estimation results of MGL parameter show that the value of(is decreased by 30% compared with the current value used in Ulchin Unit 3&4 PSA. The categorization results of MOV safety significance using the changed value of MGL parameter(show that the number of HSSCs(high safety significant components) is decreased by 54.5% compared with those using the current value of it used in Ulchin Units 3&4 PSA>

        H9c2 심근 세포주에서 외인성 nitric oxide가 허혈에 의한 세포 독성에 미치는 영향

        Background: Nitric oxide(NO) is known to have protective effects on an ischemic heart and to exert triggering effects on ischemic preconditioning. However, the effects of NO during the ischemic period have not been investigated. To investigate the role of exogenous nitric oxide in a model of ischemic heart cell death, we studied the effects of ischemic preconditioning and ischemia in a normal and an ischemic buffer. Methods: Rat cardiac myoblast cells(H9c2) were cultured in a normal and an ischemic buffered medium. For the ischemic culture of heart cells, the cells were cultured in a dessicator with GasPak for 5 hrs. In ischemic preconditioning, the cells were pretreated with ischemic buffer for 5 min and then perfused with normal medium for 30 min. For the measurement of the cytotoxicity, a MTT(3-4-Sdimethylthiazol-2-yl-2,5-diphenyl tetrazolium bromide) assay was performed. A DAPI(4',6-diamidino-2-phenylindole dihydrochloride) staining procedure and a flow cytometry analysis were performed to confirm apoptotic cell death by ischemia. Results: Cell viability, as determined by using a MTT assay, showed that the preconditioned group treated with NO showed more cell death than with the not-preconditioned groups in both normal and ischemic buffers. But, In normal medium and not-preconditioned groups, NO showed protective effect according to the concentrations(100,1000μM) . No treatment with NO produced the different results. In normal medium, the protective effect of ischemic preconditioning was demonstrated, but no protective effect of ischemic preconditioning could be seen in the case of the ischemic buffer. The DAPI staining and flow cytometry analysis of heart cells showed characteristic apoptotic features. Conclusion: NO added in the ischemic phase had deterious effects on heart cells. Ischemic preconditioning was more harmful than ischemia alone. The toxicity of the cells was characteristic apoptosis.

      • Flavobacterium meningosepticum에 의한 균혈증 1예

        1996년 6월 카톨릭대학교 부천성가병원에 뇌경색으로 입원했던 61세 남자 환자의 혈액에서 Flavobacterium meningosepticum을 분리하였다. 환자는 폐렴을 동반한 패혈증의 예로 ceftriaxone과 metronidazole을 투여하였으나 입원 48시간만에 사망하였다. Flabovacterium meningosepticum is a rae cause of nosocomial infection which shows multi-drug resistance. It mainly invades patients with impaired immunity. Recently, we experienced a case of F. meningosepticum bacteremia in a patient with chronic debilitated state owing to stroke. The clinical progress was so rapid that he died within 48 hours. Blood culture revealed F. meningosepticum which showed characteristic yellow colony in blood agar plate.

        Decision making problem is the process of selecting a possible course of action from all the available alternatives. Recently, the above problem has been considerable interest in the treatment of multiple objective in the decision making process. This paper deals with an interactive method for solving the multiple objective mathematical programming. The interactive multiple objective decision making depend on the progressive definition of the decision maker's preferences among with the exploration of the criterion space. An interactive algorithm which is efficient and useful to assist a decision maker in finding his most preferred efficient solution to the multiple objective mathematical programming is presented. To compare several technique with regard to criteria and an illustration of numerical example is included.
