임상연구 : Desflurane 마취 후 지속 정주된 Remifentanil이 각성 시 기침에 미치는 영향

Background: Coughing during emergence from general anesthesia is a common clinical problem and results in a number of undesirable side effects. Remifentanil stimulate μ-opioid peptide receptor known to be related to antitussive effect. The goal of this study was to evaluate the effect of remifentanil on coughing after general anesthesia with desflurane. Methods: Fifty one ASA physical status I and II patients undergoing elective oral and maxillofacial surgery were randomly assigned to receive either remifentanil with 1 ng/ml effect site concentration or normal saline until extubation. The number and intensity of coughs were monitored during emergence and the recovery time was recorded. Results: The incidence and number of coughing during emergence was significant less frequent in the remifentanil group (P < 0.05). The intensity of coughing was significant milder in the remifentanil group (P < 0.05). There was no significant difference between two groups in the recovery time. Conclusions: Continuous remifentanil infusion with 1 ng/ml effect site concentration during emergence from general anesthesia with desflurane decrease the incidence of coughing without delaying the recovery time. 다양한 직경의 속이 빈 탄소구체의 제조 및 리튬 저장 특성

The carbonaceous materials have attracted much attention for utilization of anode materials for lithium-ion batteries. Among them, hollow carbon spheres have great advantages (high specific capacity and good rate capability) to replace currently used graphite anode materials, due to their unique features such as high surface areas, high electrical conductivities, and outstanding chemical and thermal stability. Herein, we have synthesized various sizes of hollow carbon spheres by a facile hardtemplate method and investigated the anode properties for lithium-ion batteries. The obtained hollow carbon spheres have uniform diameters of 350 ~ 600 nm by varying the template condition, and they do not have any cracks after the optimization of the process. Increasing the diameter of hollow carbon spheres decreases their specific capacities, since the larger hollow carbon spheres have more useless spaces inside that could have a disadvantage for lithium storage. The hollow carbon spheres have outstanding rate and cyclic performance, which is originated from the high surface area and high electrical properties of the hollow carbon spheres. Therefore, hollow carbon spheres with smaller diameters are expected to have higher specific capacities, and the noble channel structures through various doping approaches can give the great possibility of high lithium storage properties.
Recent Development of Transition Metal Oxide Based Aqueous Supercapacitor Electrode Materials

With the development of electric devices such as smart phones or electric vehicles, energy storage systems with high energy/power density and stable operation need to be developed together. The supercapacitors using aqueous electrolyte have great potential for electrochemical energy storage system due to their high power densities and long cyclic performances. The state-of-the-art materials for supercapacitors are carbon based porous materials having high specific surface areas and electrical conductivities. However, they have relatively low energy density that still need to enhance specific capacitance by using the nanostructure of transition metal oxide (TMO) materials. The TMOs store charge through redox reactions like battery systems and their rates are comparable to those of conventional electrochemical double-layer capacitors. In this review, we describe the fundamental principle of TMO based supercapacitors and the recent progress for realizing high performance of TMO based supercapacitors. In addition, we categorize the TMO system as single cation and mixed cations and suggest the prospects for electrochemical energy storage fields.
Peripheral T-Cell Lymphoma Presenting as a Scalp Mass

Peripheral scalp T-cell lymphoma is a very rare disease. We report a case of a 22-year-old man who presented an indolent large scalp mass in the right frontal scalp region. The patient’s physical examina- tion demonstrated no palpable mass in the chest, abdomen, and extremities. The brain CT revealed a high-density large scalp mass of the subgaleal layer in the right frontal and a small scalp mass of the subgaleal layer in the left frontal. The brain MRI showed multifocal enhancing masses in the bilateral dura, the subgaleal layer of the scalp, and the skull. The patient underwent removal of the tumor found in the right frontal scalp. The histologic diagnosis was peripheral T-cell lymphoma. Bone marrow aspi- ration showed the involvement of T-cell lymphoma. The patient received chemotherapy with cyclo- phosphamide, vincristine, doxorubicin, and prednisolone (CHOP protocol) for 3 cycles. The patient was discharged without neurological deficit. The patient showed no evidence of recurrence 15 months after surgery. We report a rare case of peripheral T-cell lymphoma mimicking benign scalp tumors.

Nontraumatic spontaneous spinal epidural hematoma is a rare entity, which can be associated with a severe neurologic deficit. The presenting symptoms are usually back or neck pain, either local or radicular, followed by progressive bilateral weakness, and sensory loss hours and even days later. In the absence of precipitating factors such as severe trauma or known coagulopathy, the diagnosis may be delayed. Imaging, including cervical CT or MRI, may play an essential role for the prompt diagnosis and differentiation from other cervical pathologies. With a full review of the literature, we report and discuss a case of nontraumatic cervical epidural hematoma in which a 67-year-old female visited the emergency center for neck and right shoulder pain. She experienced a complete recovery after conservative treatment only.

27-Hydroxycholesterol induces differentiation of monocytic cells into mature dendritic cells, mDCs. In the current study we sought to determine roles of the PI3K and the ERK pathways in the 27OHChol-induced differentiation. Up-regulation of mDC-specific markers like CD80, CD83 and CD88 induced by stimulation with 27OHChol was significantly reduced in the presence of LY294002, an inhibitor of PI3K, and U0126, an inhibitor of ERK. Surface expression of MHC class I and II molecules elevated by 27OHChol was decreased to basal levels in the presence of the inhibitors. Treatment with LY294002 or U0126 resulted in recovery of endocytic activity which was reduced by 27OHChol. CD197 expression and cell adherence enhanced by 27OHChol were attenuated in the presence of the inhibitors. Transcription and surface expression of CD molecules involved in atherosclerosis such as CD105, CD137 and CD166 were also significantly decreased by treatment with LY294002 and U0126. These results mean that the PI3K and the ERK signaling pathways are necessary for differentiation of monocytic cells into mDCs and involved in over-expression of atherosclerosis-associated molecules in response to 27OHChol.
