        한 전자제품 연구소 남자 종사자들의 생활습관실천과 대사증후군의 관련성

        Objectives : The objectives of this study were to determine the relationship between lifestyle-implementation and metabolic syndrome in an electronics research and development company, and to provide a foundation for health providers of health management programs for setting priorities. Methods : From July 1 to July 16, 2008 we carried out a descriptive cross-sectional survey. Consecutive workers of one R & D company in Seoul, Korea (N=2,079) were enrolled in study. A checklist for lifestyle (from the National Health Insurance Corporation) consisted of questions regarding diet, drinking, smoking and exercise. After the survey, researchers obtained data from health profiles for metabolic syndrome(waist-circumference, triglycerides, HDL cholesterol, blood pressure and fasting blood sugar level). Lifestyle was recorded as good or not good. Statistical analysis of metabolic syndrome and the lifestyle of subjects was done using multiple logistic regression analysis. Results : The prevalence of metabolic syndrome in our study gropu was 13.3% (N=277). After adjustment for age, the adjusted odds ratios (odds ratio, 95% confidence intervals) for metabolic syndrome increased in proportion to the number of bad habits: two (1.72, 1.23-2.44), three (2.47, 1.73-3.56), and four (3.63, 2.03-6.34). Relative to subjects eating both vegetables and meat', the OR for 'meat' eaters was 1.66 (1.18-2.31). Compared with 'nonsmokers and ever-smoker', the OR for 'current-smoker' was 1.62 (1.25-2.10). Compared with 'Healthy drinker', the OR for 'unhealthy drinker' was 1.38 (1.05-1.83). Conclusions : Poor lifestyle was associated with an increased likelihood of metabolic syndrome. These findings suggest that lifestyle-based occupational health interventions for young employees should include a specific diet, smoking cessation, and healthy-drinking programs.

        Sand dust is a source of particulate matter (PM) in Korea. Recently, an attention has been focused on the health effects of PM and fine PM. Here we examine the possible mechanisms of PM disposition and review the literature on the health effects of PM. PM can enter and accumulate in the respiratory tract via impaction, gravitational settling, diffusion, and electrostatic attractions. PM may cause oxidative stress, inflammation, direct penetration, and increased blood viscosity. This literature review revealed that PM exposure is associated with several health risks, such as mortality and respiratory and cardiovascular diseases. Furthermore, PM exposure may be linked to lung cancer. Thus, a comprehensive approach is needed to manage PM and reduce its environmental exposure and related health effects.         소방 공무원에서 간접 외상이 정신 건강에 미치는 영향

        Objectives : Indirect contact with trauma may occur when a person empathically listens to detailed descriptions, exposing the person to intense emotional pain from trauma victims. Although less severe than direct trauma, indirectly traumatized persons may experience the same fear, rage, and despair as direct trauma victims. This phenomenon has been variously termed vicarious trauma, traumatic countertransference, burnout, compassion fatigue, and secondary traumatic stress. Using a psychiatric symptoms questionnaires, this study investigated the effects of indirect exposure to co-worker's trauma on the mental health of firefighters who had not directly experienced traumatic events in the previous year. Methods : We administered self-report questionnaires, such as the Impact of Event Scale-Revised (IESR) and Alcohol Use Disorder Identification Test (AUDIT), to firefighters working at two fire stations in a metropolitan city. We analyzed 188 of 232 collected surveys, after excluding falsely entered data (28 cases) and questionnaires by directly-traumatized victims (16 cases). Results : Alcohol consumption and posttraumatic stress disorder (PTSD) symptoms, as determined by the AUDIT and IES-R, were significantly higher in the indirectly-traumatized group. The ratio of high-risk members scoring higher than 23 in IES-R was larger in the indirectly traumatized group, although this was not statistically significant. Conclusion : Alcohol consumption was significantly higher in the indirectly traumatized group. This group also included more members at high risk for PTSD. Further research, with a larger group of indirectly traumatized firefighters and ongoing investigation of PTSD development and other psychiatric symptoms, is needed.

        거리환경미화원의 근골격계 증상의 특징 및 인간공학적 평가

        The aim of this study was to evaluate the musculo-skeletal symptomatic features of municipal sanitation workers and to compare differences of the musculo-skeletal symptoms by work types. We conducted descriptive cross-sectional survey concerning the musculo-skeletal symptomatic features of 315 street cleaners in Seoul and GyeongGi Province, Korea, during 2 weeks of September 2006. Questionnaires were consisted of general characteristics, occupational characteristics and musculo-skeletal symptoms. And we observed their works and evaluated their movement and posture by REBA. For work types, tools and subjective physical work loading, there were statistical difference whether or not NIOSH symptom positive on upper limb. Musculo-skeletal symptoms on upper limb were claimed from 43.2% of street cleaners, more than in 32.4% of cleaners. Musculo-skeletal symptoms on upper limb were reported higher in the group who felt their subjective physical work loading severe than in the otherwise group. The REBA results were over 'high risk stage' in street cleaners. Street cleaners had more repetitive motions than solid waste collectors on upper limbs. Works according to the criteria of musculoskeletal burdened work by the Ministry of Labor results were over 'high risk stage' of No. 2 and No. 4 in street cleaners. We found that street cleaners complained musculo-skeletal symptoms on upper limb more than solid waste collectors. This study suggests that it is necessary for street cleaners to carry out the prevention program of musculo-skeletal diseases. For street cleaner, the measure plans such as job rotation, automatic street sweepers, reducing work hours are helpful in preventing musculoskeletal symptoms on the upper limb.

        한국 소아 청소년 남아에서 복부 지방 분포 측정의 임상적 의의

        목 적: 성인에서 내장 지방은 대사 이상의 위험 요소와 밀접한 상관이 있다고 알려져 있으나 소아 청소년 연령에서의 연구는 미흡하다. 소아 청소년에서의 복부지방의 분포와 대사 이상의 위험 요소간에 상관성을 연구하고자 하였다. 방 법: 78명의 남아와 23명의 여아를 포함한 101명의 소아 청소년(평균 연령 10.8${\pm}$2.4세)을 대상으로 연구를 시행하였다. 신체 계측과 대사 증후군의 위험 요소에 대한 분석을 개별 환자에서 시행하였고, 복부 전산화 단층 촬영을 통해 내장 지방과 피하 지방의 양을 측정하였다. 연령을 보정한 편 상관 분석을 통해 내장 지방, 피하 지방의 양, 신체 계측 지표와 대사 이상의 위험요소 간의 상관관계를 분석하였다. 결 과: 남아와 여아 모두에서 피하 지방이 나이가 들어감에 따라서 내장 지방보다 급격히 증가하는 것을 확인할 수 있었다. 남아에서 내장 지방과 피하 지방은 신체 계측 지표와는 유의한 상관성을 보였고, 내장 지방은 낮은 수치의 HDL-C과 피하 지방은 높은 이완기 혈압과 유의한 연관성을 보였으나. 남아에서 내장 지방과 피하 지방, 신체 계측 지표는 모두 대사 이상의 위험요소의 수와 밀접한 관계가 있는 것으로 나타났다. 결 론: 남아에서 내장 지방은 낮은 수치의 HDL-C과, 피하 지방은 높은 이완기 혈압과 유의한 상관성을 나타내었으나 성인에서의 기존 연구에 비해 본 연구는 다른 대사 이상의 위험 요소들에 대한 상관성이 떨어지는 것으로 나타났다. Purpose: Visceral adipose tissue may be strongly linked to increased metabolic risks in adults. However, because little is known regarding the effect of visceral adipose tissue in children and adolescents, we performed this study to determine the association between abdominal fat distribution and metabolic risk factors in this population. Methods: One hundred one children and adolescents (78 males and 23 females; mean age, 10.8${\pm}$2.4 years) were enrolled. The anthropometric data and metabolic risk factors were evaluated. Theabdominal fat distribution was assessed according to the CT measurement. Age-adjusted, partial correlations were performed among the visceral adipose fat area (VFA), subcutaneous adiposefat area (SFA), metabolic risk factors, and anthropometrics. Results: The SFA increased more rapidly than the VFA with advancing years in both genders. In males, the VFA and SFA were positively correlated with anthropometrics. The VFA was correlated with low HDL-cholesterol and the SFA was correlated with diastolic blood pressure (DBP). However, there was no statistical significance between the VFA, SFA, anthropometrics, and other metabolic risk factors. The VFA and SFA were strongly linked to a number of metabolic risk factors, such as other anthropometrics. Conclusion: This study investigated how a low HDL-C was correlated with VFA and how a high DBP was associated with SFA in Korean male children and adolescents. Our results suggest that the correlation between the VFA, SFA, and metabolic risk factors was relatively weak compared to that reported in previous adult studies.

        Fit Test를 이용한 국내산 N95 마스크의 교육 후 밀착도 비교

        Objectives: A number of medical institutions have been conducting fit tests to perform seal checks on masks. This study aimed to compare the differences fit factor before and after domestic N95 mask-wearing training through a fit-test. Methods: A survey of 59 healthcare workers was conducted regarding whether they had undergone a fit test or received training on mask-wearing. Further, the fit of two types of domestic N95 masks was measured before and after the training using a QNFT(Quantitative Fit test). The average fit factors before and after training were compared using a paired t-test. Additionally the differences in the fit test pass rate were analyzed using a McNemar test. Results: A statistically significant difference was seen between the fit factors in the fit tests conducted before and after the training (p=0.0015), as well as in the fit of the two types of masks tested (p<0.01). Thus, an improvement in mask fitting was seen after the training, even with differently fitted masks. Conclusions: Upon using a QNFT, a significant increase in the fit factors for N95 masks was observed after training compared with masks that were fitted as usual. This highlights the importance of training in mask-wearing, with the conclusion that training healthcare providers will improve the fit of masks.

        어린이용 황사 및 미세먼지 마스크 개발 연구

        Objectives: No 3D anthropometric analyses have been conducted for Korean children’s faces for the purpose of designing respiratory protective devices. The aim of this study was to develop masks against yellow dust and fine particulates, particularly for children in Korea. Methods: This study utilized a 3D scanning method to obtain 16 facial anthropometric data from children, ages of 5 to 13 years old. A total of 144 boys and girls were recruited from the kindergarten, elementary schools and middle schools in Seoul. With facial dimensions obtained, cluster analysis was performed to categorize them into similar facial groups. For each cluster, an optimal mask was designed and manufactured using a 3D printer. In addition, lung function data were obtained from 62 subjects and compared with those of normal adults. The pulmonary physiological results were subsequently used to suggest a test method for mask certification. Results: Facial shapes were classified into tree clusters: small, medium, and large. The face width and length for the first group were small with high nosal protrusion. The face width and length for the second group were the largest among the three clusters. The third group had the largest angle of nose root - gnathion(n-prn-gn). Age was the most significant variable in the facial dimensions. Children’s pulmonary physiological capacity was about 60% of adults’ capacity. The results of fit test using the prototype masks developed showed very good fits for children. Conclusions: For Korean children, three mask sizes will be sufficient and practical for providing protection against yellow dust and fine particulates. Anthropometric data obtained using digitalized 3D face analysis can be very effective for designing respiratory devices. 3D images can be accurate and easily measured for multiple dimensions, particularly for curved areas of the face. It is imperative to adopt different test methods for certifying respiratory protective devices for children, since their pulmonary physiological capacity is inferior compared with that of adults.

        착용자 환경 및 시간에 대한 마스크 미생물 오염 연구

        Objectives: This study aims to investigate differences in microbial contamination according to the duration and environment of mask wearing. Methods: Forty-five participants were recruited from workers in an offices, multi-purpose facilities, and a schools. After wearing of KF94 mask for two. four, and six hours, the microorganisms adsorbed on the outer and inner layers of the mask were inoculated on BAP(Blood Agar Plate), Chocolate agar, and SDA plates. The bacterial count (CFUs: colony-forming units) cultured in each plate was measured and analyzed for changes in filtration efficiency. Results: The microbial contamination of masks worn in classrooms, offices, and multi-purpose facilities showed a significant difference depending on the environment (p<0.000). The measured CFUs increased significantly according to the time wearing the mask. The difference between the inner and outer layers of the mask was also significant (p<0.05). However, there was no statistical difference in the filtration efficiency of the masks by duration time (p=0.515). Conclusions: Masks worn by workers in the offices, multi-purpose facilities, and schools showed an increase of microbial contamination with the amount of time wearing the mask. The results indicate that the masks used in daily life may have adverse health effects if they are worn for a long time or reused over several days without the realizing that the masks can be contaminated with biological hazards. Guidelines on the safe threshold time for mask use should be established through further research.
