      • Gilbert 증후군에서 열량 제한 시험과 Phenobarbital 자극 시험의 의의(14예)

        Gilbert 증후군은 인구의 7%에서까지 나타날 수 있는 매우 흔한 증후군으로서 비진행성인 양성의 만성적 경과를 치하며, 간질환의 증상과 징후가 없는 경한 비포합형 고빌리루빈혈증이 특징인 일종의 체질적인 증상으로서 혈장 빌리루빈 농도에 대한 사춘기의 영향 때문에 10대와 20대에 자주 진단이 된다. 따라서 임상적인 중요성은 미약하지만 높은 빈도가 예상되는 점에 그 중요성이 부여되어야 할 것이다. 따라서 적정한 임상적 진단법으로 기왕에 소개된 열량제한 시험과 phenobarbital 유도 시험을 시행하고 이들의 진단적 가치를 알아보기 위하여 본 연구를 시행하였다. 1990년 7월부터 1999년 4월까지 충남대학교병원에 내원하여 HBsAg, IgG anti-HBc 및 anti-HCV가 음성이고, 간 초음파 스캔에서 이상이 없으며, 혈청 AST, ALT 및 AP가 정상인 비음주자에서 경한 비포합형 고빌리루빈혈증이 있는 14예의 환자들을 대상으로 ^(99m)Tc-DISID 스캔을 시행하였으며, 기저 치 총빌리루빈 및 포합형 빌리루빈 치를 측정한 다음에 하루에 400Kcal로 48시간동안 제한한 열량 제한 시험을 시행하였고, phenobarbital을 하루 60mg씩 5일간 투여한 후에도 각각 총빌리루빈과 포합형 빌리루빈 치를 검사하여 비포합형을 구하였다. 대상 환자들은 모두 14예로서 남자가 11예(78.6%)였고 여자가 3예(21.4%)여서 3.7:1로 남자에서 많았으며, 20대가 6예(42.9%), 30대가 역시 6예(42.9%) 및 40대가 2예(14.2%)로서 2,30대가 대부분(85.8%)이었다. 열량 제한 시험 후의 총빌리루빈 치, 비포합형 및 포합형 빌리루빈 치들은 평균 각각 5.5±2.7, 4.2±2.3 및 1.3±10mg/dL 로서, 시험 전 치들인 3.0±0.8, 2.2±0.8 및 0.7±0.4mg/dL 보다 유의하게(p=0.001, p=0.001, p=0.023) 상승하였다. 포합형 빌리루빈 치도 유의하게 상승하였으나 비포합형의 상승보다는 훨씬 낮아서 주로 비포합형이 증가하였다. phenobarbital 투여 중 설사가 발생하여 중단한 1예를 제외한 13예에서 열량 제한 시험 후에 상승하였던 총, 비포합형 및 포합형 빌리루빈 치가 phenobarbital 유도 시험후에는 2.0±1.1, 1.5±0.8 및 0.5±0.4mg/dL로서 열량 제한 시험 결과보다 유의하게 낮아졌고(p=0.00, p=0.000, p=0.001), 열량 제한 시험 전의 기초치들인 3.0±0.8, 2.2±0.8 및 0.7±0.4mg.dL 보다도 더욱 낮아졌으며 유의한 차이(p=0.001, p=0.02, p=0.005)를 나타내었다. 14예에서 시행한 ^(99m)-Tc DISIDA 스캔에서 9예(64.3%)가 정상이었고, 5예(35.7%)에서는 심장 및 신장으로의 간외 섭취가 3예였고, 60분까지 소장 배출이 없는 배설 지연 예와 담낭 수축 불량 예가 각각 1예 씩 발견되었다. Phenobarbital 투여시험에서 민감도가 열량제한시험에 비해 더 높았다(92.3%와 50.0%). Gilbert 증후군에서 1일 400 Kcal로 48시간의 열량제한 시험과 1일 60mg의 phenobarbital을 5일간 투여하는 유도 시험은 편리하고 유용한 임상적인 진단법으로 이용할 수 있다고 생각된다. 그러나 열량 제한 시험에서는 증가 기준의 통일이 필요하다고 유추되며 phenobarbital 유도 시험이 민감도가 더 높은 것으로 생각된다. Gilbert's syndrome is very frequent and benign chronic process characterized by mild, intermittent, unconjugated hyperbilirubinemia without any symptom and sign of liver disease. Previously intoduced caloric restriction test and phenobarbital stimulation test as two appropriate clinical tests had been examined and their diagnostic values were reevaluated. Fourteen patients with mild, persistent, unconjugated hyperbilirubinemia were included. Subsequently caloric restriction has been applicated by 400 Kcal/day for 48 hours and phenobarbital has been prescribed by 60 mg/day for 5 days. Therafter serum levels of total and direct bilirubin were measured. Most of the patients were third and fourth decade(85.8%) and male predominant. Each basal serum levels of total, indirect and direct bilirubin were 3.0±0.8, 2.2±0.8 and 0.7±0.4 mg/dL. After caloric restriction test, each levels were increased significantly to 5.5±2.7, 4.2±2.3 and 1.3±1.0 mg/dL(p=0.001, p=0.001, p=0.023). After phenobarbital stimulation test for 13 patients had been practiced, increased levels of each bilirubin after caloric restriction test were decreased significantly to 2.0±1.0, 1.5±0.8 and 0.5±0.4 mg/dL(p=0.000, p=0.000, p=0.001) and these levels were significantly lower than basal levels(p=0.001, p=0.02, p=0.005). The sensitivities of caloric restriction test were 85.7%, 50.0%, and 71.4%, 35.7%(1.0, 1.5 mg increase of total bilirubin and 1.0, 1.5 mg/dL increase of indirect bilirubin). The sensitivities of phenobarbital stimulation test were 93.2% and 92.3% at criteria of 1.5 mg/dL increase of total bilirubin and indirect bilirubin. On the diagnosis of Gilbert syndrome, caloric restriction test and phenobarbital stimulation test are convenient and useful diagnostic tools in clinical face. And also phenobarbital stimulation test has higher sensitivity than caloric restriction test. Furthermore, standardization of bilirubin increment would be necessary in caloric restriction test.

      • 디지털콘텐츠를 위한 백업 시스템의 설계 및 구현

        첨단 IT기술의 발전으로 유무선 통신망을 통하여 다양한 환경에서 사용되는 대용량 디지털콘텐츠들이 급증하고 있다. 또한 기하급수적으로 늘어나고 있는 디지털콘텐츠를 한정된 저장 공간에 효율적으로 저장하기 위한 콘텐츠기반 저장기술에 대한 연구가 활발히 진행되고 있다. 이와 함께 불시의 사고에 의한 디지털콘텐츠의 손실을 방지하기 위해서 지속적인 데이터 백업의 중요성 또한 증가하고 있다. 본 논문에서는 디지털콘텐츠의 특성을 고려하여 데이터 입출력 I/O 크기 단위로 콘텐츠를 객체화하고 유일한 ID를 부여하여 백업 장치에 저장하는 시스템을 설계하고 구현한다. 제안하는 백업 시스템은 중복되어 발생하는 데이터는 하나의 사본만 백업하고, 이를 공유함으로써 백업되는 데이터의 양을 효과적으로 감소시킨다. With the development of IT technology, the amount of digital contents used in various environments of wired and wireless networks have been increased hugely and rapidly. Also, many researches have been done to process a contents-based storage technology that saves the digital contents in limited storage. To protect the loss of the digital contents by the sudden accident, continuous data backup is required. In this paper, we design and implement the backup system that stores digital contents in backup storage by objectifying the contents with a unit of I/O size and giving them the unique ID using the properties of digital contents. The backup system reduces the amount of backup data by backuping and sharing the only one copy of the backup data.

      • 합성 병렬 전단벽의 구조성능

        The cyclic response of steel coupling beams embedded into reinforced coupled shear walls was studied. Four two-third scale subassemblies representing a portion of a prototype structure were designed, constructed, and tested to failure. The main test variables were the distribution of axial stress on wall (eccentrical length of axial loading point), and presence of auxiliary stud bolt attached to the beam flanges for a smoother transfer of bearing stresses. The distribution of axial stress on coupled shear walls had an effect on the hysteretic behavior and failure mode of steel coupling beams. The steel coupling beams reinforced by auxiliary stud bolt and rebar had very stable hysteretic curve with little loss of strength.

      • 비만을 동반한 위암 환자에서 두 병을 같이 수술 치료한 경험(증례)

        Obesity is growing problem in Korea. We had a case of bariatric surgery during gastric cancer operation. Patient was 29 year old Korean lady with early gastric cancer located in the lesser curvature side of the middle 1/3 of the stomach. Her body weight was 89 kg, height 163 cm, and thus body mass index was 33.5 kg/m2. Preoperative blood pressure was 130/90 mmHg, hemoglobin 12.9 g/dL, total lymphocyte count 3,280/mL, serum albumin 4.3 g/dL, CEA 1.1 ng/mL, CA 19-9 9.1 ng/mL, and alpha fetoprotein 2 ng/mL. Another associated disease was right thyroid follicular neoplasm. The chest X-ray was normal. After IM injection of 2,500 units of heparin 30 minutes before the induction of general anesthesia, she received curative subtotal gastrectomy and Roux en Y gastrojejunostomy when a malabsorption loop of 120 cm jejunum was incorporated between the Treitz ligament and the end to side jejuno-jejunostmy site at May 29th 2006. Her postoperative course was uneventful except a minor wound seroma and the elevations of serum amylase(up to 4 folds) and lipase(up to 2 folds). She lost her body weight 9 kg in 5th, 21 kg in 7th, and 27 kg in 10th postoperative months to became 61.5 kg. On follow up exams in July 2008, she gained 4 kg to overcome her weakness and fatigue. From the above result, the incorporation of a malabsorption loop during reconstruction phase of gastric cancer operation was a good option for obese gastric cancer patients especially in cases of early gastric cancer.

      • 石榴나무의 揷木에 關한 硏究 : 1. IBA가 發根에 미치는 影響 1. Effects on the Rooting of IBA in Punicaceae Tree

        石榴나무의 栽培面積 急增에 따른 揷木繁殖의 大量 生産을 爲한 基礎資料를 얻기 위하여, 植物生長調節制인 IBA가 石榴나무의 發根에 미치는 影響에 關한 試驗結果는 다음과 같다. 1. 發根率은 IBA의 處理濃度間에 有意性이 인정되었으며, 1500ppm에서 70.33%로 가장 높고, 2000ppm 區, 對照區, 1000ppm 順으로 나타났다. 2. 新梢生長의 길이는 各 處理間에 有意性이 인정되었으며, 1000ppm 區에서 가장 길고, 2000ppm 區, 對照區, 1500ppm 區 順이었다. 3. 根部무게는 1000ppm 區가 13.02g으로 가장 무거웠고, 그 밖의 各 處理區間에는 비슷한 傾向을 보였다. 4. 根部直徑은 全 處理區에서 비슷한 傾向을 보였다. 5. 1500ppm 區가 他區에 비해 대체로 良苗 狀態였다. Recently the pomegranates are grown wide, making it necessary to produce the cutting massively. The study is to examine the effects fo IBA hormone upon the rooting of pomegranates. The results of experiments are as follows; 1. The rooting percentage showed significant difference between each groups of different IBA concentrations. The highest was 70.33% in the 1500ppm group. The 2000ppm group showed the next highest percentage followed by the control and 1000ppm groups. 2. The length of new shoots also showed significant difference. The 1000ppm IBA concentration group produced the longest shoots, followed by the 2000ppm, control, and 1500ppm group in order. 3. The root weight was heaviest in the 1000ppm group (13.02g). The weights of the other groups were similar to each other. 4. The root diameter was similar in each group. 5. The 1500ppm group showed the best results.

      • SMEDDS를 이용한 난용성 약물의 용출율 향상

        ABSTRACT-A self-microemulsifying drug delivery system (SMEDDS) was developed to enhance the solubility and dissolution rate of poorly water soluble drug, biphenyl dimethyl dicarboxylate, DDB. The system was optimized by evaluating the solubility of DDB and the microemulsion existence range after the preparation of microemulsions with varying compositions of triacetin and surfactant-cosurfactant mixtures (Labrasol as surfactant (S) and the combination of Transcutol, Cremophor RH 40 and Plurol oleique as cosurfactant (CoS)). SMEDDS in this study markedly improved the solubility of DDB in water up to 10 mg/ml and the size of the o/w microemulsion droplets measured by dynamic light scattering showed a narrow monodisperse size distribution with an average diameter less than 50nm. The microemulsion existing range is increased proportional to the ration of S/CoS, however, it decreased remarkably as the oil content was more than 20%. In vitro dissolution study of SMEDDS showed a significantly increased dissolution rate of DDB in water (>12 fold over DDB powder), and SMEDDS also had significantly greater permeability of DDB in Caco-2 cell compared to powders.

      • Rat의 포유조절에 따른 난소와 유선의 조직학적 성상

        This experiment was conducted to investigate how the lactation regulation such as restricted-lactation, early weaning during the suckling period influences on ovarian functions in primiparous rats, on the bases of change in histological characteristics of ovary and mammary gland. 1. The ovaries of rats in middle lactation stage maintained the functional corpora lutea full of cytoplasm, and those of rats in late lactation stage contained matured ovarian follicles. 2. The milk was found full in the mammary gland tissue of NL rats, but short in the tissue of RL rats, and all the alveolar tissues were degraded in mammary gland of weaned rats. The alveoli structures of mammary gland of NL rats were uniformed on Day 10, but such uniformity was lost greatly on Day 20.

      • IBA處理가 감나무 初期生育에 미치는 影響

        One year old seedings of persimmon trees treated with IBA, growth regulator, in different levels were subjected to test. After one year of cultivation, the effect of IBA on the plant height, the root length, and the average weight per leaf were determined. The treatment 500 and 1000 ppm of IBA trended positive result of growth.

        도료산업의 자율안전관리를 위한 HPMA 시스템 개발

        We developed a JHPMA Information System to establish the voluntary safety management for the Paint Industry. IIPMA System means that manager, supervisor and worker on the process-line find out the hazardous and/or unsafe potentials, make improvement of the field safety by the cooperation with top manager, safety staff and eventually put in the practice. HI'MA Information System consists of functions to suggest and evaluate the safety concerns hierarchically from top manager to workers on mtemet or Intranet. We establish the safety management and sharing of the safety information and are able to find the unsafe potentials by the HPMA Information System. Also We are able to mine new safety information and establish accident prevention model by the Safety DATABASE.
