유치도뇨관 감염예방 가이드라인에 관한 체계적 문헌검색 사례-Part II: Ovid MEDLINE 이용

Purpose: To implement evidence-based nursing, it is important to know where and how to find the best available evidence. This study was conducted to identify the results of a search from Ovid MEDLINE and to compare the results from Ovid MEDLINE with those from PubMed MEDLINE. Methods: Four different approaches via Ovid MEDLINE were used to search for guidelines on preventing catheter-associated urinary tract infections. Outcomes of this study were the number of records and relevant literature, and the sensitivity and precision of the search methods via Ovid MEDLINE. Results: The number of retrieved items ranged 23 to 6,005 and that of relevant studies, 5 to 8 of 8. Simple searches resulted in the highest sensitivity of 100.0%. When using MeSH terms and limits feature, the precision was highest (21.7%) among four approaches for literature searches. Simple searches in Ovid had higher sensitivity and lower precision than those in PubMed. Conclusion: Simple searches in Ovid may be inefficient for busy clinicians compared to PubMed. However, to ensure a comprehensive and systematic literature search, using Ovid MEDLINE in addition to PubMed is recommended.
정상교합자의 두부방사선 계측사진과 안면모형을 이용한 안모의 3 차원적 연구

The cephalometric radiograph has been widely used to evaluat the indiviudual craniofacial structure, but it gives us only 2-dimentional information. This study was done to evaluate the facial structure as 3-dimensional approach that will more effective to describe the spatial position of certain objectives. Cephalometric radiographs and casts of facial structure were taken on the adult samples who have relatively well balanced face and normal occusion. The prominence of every part of facial strucrure were estimated as numerical value and expressed as computerized graphic image and contour graphic. And also, the differences between male and female in the absolute size, prominence and asymmetry were evaluated. The following results were obtained ; 1. The absolute size of facial structure was greater in male than in female. 2. The prominence of facial structure was greater in male and the forehead was well developed in male generally. 3. In the prominence of facial structure, the most different area between male and female was ala of nose. 4. The faces were asymmetric only within ±5㎜ difference in prominence. 5. In male, the prominence of the right side of the face was greater when compared with the left side, but on the contrary in female.
흰쥐 내측 전정핵세포의 막전도도와 시냅스 전달에 대한 Orphan Opioid의 작용

Background: The medial vestibular nucleus (MVN) is involved in the reflex control of the head and eyes, and the recovery of vestibular function after vestibular injuries. This study was performed to investigate the actions of the orphan opioid (nociceptin) on the membrane conductances and synaptic transmission in rat MVN neurons. Methods: Whole cell patch clamp recordings were carried out in the brainstem slice of neonatal rats. Results: Nociceptin (2 µM) inhibited the spontaneous discharge in the majority (83%) of MVN neurons. This inhibition was insensitive to the non-specific opioid receptor antagonist naloxone (10 µM), but was effectively antagonized by the selective opioid receptor-like 1 (ORL1) receptor antagonist, [Nphe1] nociceptin(1-13)NH2 (3 µM). Nociceptin had no effect on the rate or amplitude of miniature inhibitory postsynaptic currents (mIPSCs). Nociceptin induced an outward current, and which was blocked by [Nphe1] nociceptin(1-13)NH2 in MVN neurons. Outward current reversed at -81±2 mV, which was close to the K+ equilibrium potential as calculated by the Nernst equation in 6 mM extracellular potassium solution. This indicates that the action of nociceptin involves postsynaptic receptors on the MVN neurons. Conclusions: These results suggest that nociceptin modulate neuronal excitability by activating a K+ conductance in postsynaptic neurons, not by modulation of synaptic transmission in MVN neurons.
원광 뇌졸중 등록 자료를 통한 허혈성 뇌졸중의 임상 양상과 위험인자

Background: To investigate clinical features and risk factors of stroke patients who lived in Korean rural area, we analysis Wonkwang Stroke Registry and compared with other studies. Methods: We filled out Stroke Registry in the consecutive patients with acute ischemic stroke from march 1997 to February 1999. We analyzed demographic data, risk factors and subtypes of stroke. All patients were divied into 3 groups according to their age. Stroke subtype was classified into large artery atherosclerosis(LAA),small vessel occlusion(SVO), cardioembolism(CE), stroke of other determined etiology(SOE), and stroke of undetermined etiology(SUE) by TOAST(Trial of Org 10172 in Actue Stroke Treatment). Results: Five hundred nine patients were included in Wonkwang Stroke Registry from march 1997 to February 1999. Mean age was 65.47±0.48 years old. The ratio of males to females was 1 : 0.78. SVO was the most frequent subtype(49%) of ischemic stroke, and then followed by LAA(27.5%), CE(7%), SOE(8.1%), and SUE(8.4%). The internal carotid artery(ICA) territory was involved in 59.3% patients. Vertebrobasilar artery territory involvement was noted in 37.5%, and 3.2% of patients showed multiple artery territory involvement. All patients had risk factor of ischemic stroke except for 14 patients. Mean number of risk factors was 1.7/patient. Hypertension(62.1%) was the most frequent risk factor, and follwed by smoking(26.3%), diabetes mellitus(25.9%), and hyperlipidemia(25.55%). Conclusions: These results showed that the proportion of ischemic stroke subtype was not concordant with other studies. It may be caused by different mode of living and ethnic difference. This study may afford further information on the characteristics of Korean rural area stroke.
