Dieulafoy’s lesion is very rare cause of gastrointestinal bleeding, Deiulafoy’s lesion is exposure of non ulcerative, abnormally large tortuous, thick- walled artery in the muscularis mucosa through a small submucosal defect. It is usullay found on stomach antrum and lesser curvature side of the proximal body, and possibly found on any sites throughout whole gastrointestinal tract, but rarely on rectum. The principal of treating Dieulafoy’s lesion was the surgical resection of lesion, but recently remarkable advances in endoscopic technique make endoscopic approach more popular. We report a case of rectal Dieulafoy’s lesion successfully treated with endoscopic hemoclipping in an elderly female patient.

피부첩포시험을 이용한 실리콘겔 쉬트의 피부과민성에 대한 연구

Silicone gel sheeting is widely used to manage the hypertrophic or keloid scars. Since first reported in 1982 to be an effective treatment for burn scars and contractures, many authors reported its efficacy to treat scars. Chemically silicone gel sheet composed of cross-linked dimethyl and vinyl enblocked poly-dimethylsiloxane polymer. The exact mechanism of silicone gel sheet to treat hypertrophic scar was still unknown, but decreasing the water vapor transmission was supposed to level the scar. During our clinical experience, a few patients suffered from skin problems by silicone gel sheeting. So we designed a study to determine the severity of skin hypersensitivity of silicone gel sheeting. Four types of silicone gel sheets were applied to upper arms of 140 healthy volunteers. Resultant skin lesions were analysed 48 and 96 hours later to differentiate the irritation and the true hypersensitivity. About 30 percent of volunteers represented mild skin irritatility(48 hours later), but true skin hypersensitivity was not found(96 hours later). The site to be applied with silicone gel sheet is very critical area, so pretesting the iriitability of silicone gel sheeting to individuals is an important step to control the hypertrophic scar.

중등학교 미디어교육을 위한 문화기호학적 방법론

이 연구는 미디어 환경 변화에 따른 학교 미디어교육의 필요성을 인식하고, 중등학교 미디어교육을 위한 교수학습방법론을 제시 하는 데 목적을 두고 있다. 이를 위해 미디어교육의 패러다임 변화에 따른 미디어교육 개념의 변화에 대해 조사하고, 학교 미디어 교육으로서 특히 중등 국어과의 미디어교육적 요소를 검토하게 된다. 나아가 현행 국어과에서 적용되는 미디어교육의 방법론을 개괄하고 문제점을 도출하게 된다. 아직까지 미디어교육이 ‘미디어를 통한 교육’ 즉 미디어를 통해 특정 교육 요소를 이해시키는데 활용하는 도구적 입장을 취하고 있다. 이 연구에서는 이런 점을 비판적으로 성찰하고 ‘미디어에 대한 교육’이 중점적으로 이루어 질 수 있도록 교육적 대안을 마련하게 된다. 그것이 바로 문화기호학적 교수학습방법론이다. 일찍이 기호학은 구조주의를 바탕으로 한 질적 연구방법으로서 교육적 영역에 도입되었다. 이 연구에서는 그레마스에 의해 시도되었던 행동자 모형과 내러티브 분석 기법을 중등학교 미디어교육에 도입한다. 이 문화기호학적 방법론은 멀티 리더러시를 가능케 하는 데, 이는 미디어 수용자 혹은 잠재적인 미디어 학습자가 특정 미디어 텍스트를 통해 문화적 차원의 의미 해석에 기여한다. 뿐만 아니라 수용자나 학습자가 처한 사회문화적 콘텍스트를 비판적으로 성찰 하는데 도움을 줄 것이다. The research explores acceptance of media Education in Korean Language Education. The concept of media education extends to teach communication skills as 'new language'. Using internet is not only the technology but the process of communication. For the new generation, media education value the process of communication like the constructivism paradigm. Media education have to be get through via the idea of literacy. For going forward expansion of communication ability, the purpose and contents of media education need to be re-established. Results of this article were as follow: First, the idea of media education was reflected on the 7th Korean curriculum. And some Korean teachers group developed series of textbooks for media education in secondary schools. Second, the teaching methodologies based cultural semiotics, multi-literacy methodology and narrative scheme methodology play an important role for secondary Korean class.

저자들은 부갑상선절제술 이후에도 교정되지 않는 고칼슘혈증과 계속된 수양성 설사로 인한 하지마비 및 전신쇠약을 주소로 내원한 70세 남자환자에서 일반 화학검사, 호르몬 건사, 방사선 검사 등으로 VIPoma와 동반된 MEN Ⅰ으로 진단하여 외과적 절제술 후 수양성설사, 저칼슘혈증 및 고칼슘혈증이 호전된 1례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Multiple endocrine neoplasia I (MEN Ⅰ) is a genetic disorder that consists of neoplasia of neuroendocrine type in the parathyroid glands, in the islets of Langerhans in the pancreas, and in the anterior pituitary gland. Primary hyperparathyroidism is most common feature of the MEN I and occurs is approximately 95% of patients. Pancreatic islet cell tumors occur in 40% of MEN I patients. Most of these tumors produce excessive amounts of hormone, such as gastrin, insulin, glucagon and vasoactive intestinal polypeptide (VIP). VIP-producing pancreatic tumors (VIPoma) associated with MEN I are rare and there was only one case report in Korea so far. Recently we experienced a case of MEN I, especially VIPoma presented in a 70 year old man. He got subtotal parathyroidectomy for hyperparathyroidism 5 months ago, but his symptoms of watery diarrhea, severe general weakness and paralysis of lower limbs were not improved and hypercalcemia, hypokalemia were persisted. On computerized tomography scan of abdomen, demonstrated a mass of 3㎝ in diameter at the tail of pancreas. Serum level of VIP hormone was 250pg/㎖ (normal range : 0∼23pg/㎖). After partial pancreatectomy, his symptoms were improved dramatically and biochemical parameters such as, serum potassium, calcium, and VIP returned to the normal ranges.

선천성 거대결장증 진단에 필요한 조직화학염색 : Rapid Acetylcholinesterase Staining

Hirschsprung's disease is a congenital absence of ganglion cells in the Meissner's and Auerbach's plexuses of the colon. The definitive diagnosis of the disease can only be based on histologic examination of bowel wall biopsies demonstrating aganglionosis. To distinguish Hirschsprung's disease from other causes of contipation and megacolon, it is helpful to use histologic examination with routine hematoxylin and eosin (H&E) staining and acetylcholinesterase (AChE) special histochemical staining techniques. With H&E staining, the difficulties may arise in the hypoganglionic area of the transition zone, or in hypoganglionosis or neuronal intestinal dyspiasia. The AChE technique provides reliable identification of ganglion cells by staining nerve filaments in close proximity to the ganglion cells. However, the conventional AChE technique takes about 2 hours for the examination of specimens, so it is not suitable for intraoperative examination. Using a rapid AChE histochemical staining technique, it is possible to produce staining of cholinergic fibers in 20 minutes. It should enable the surgeon to use it selectively during operation and to support or refute the H&E findings.

Die vorliegende Arbeit befasst sich mit den Eigenschaften des Zeichensystems, das den Diskursmedien zugrunde liegt. Zuerst wurden die klassische Zeichenkonzepte im Peirceschen Sinne diskutiert und jedoch fu¨r die heutige Zeichenwelt als nicht ausreichend befunden. Dazu wurden in dieser Arbeit zwei neue Zeichtentypen. d. h. hinweisende Zeichen und registrative Zeichen. eingefu¨hrt. Die bisher entstandenen Diskussionspunkte werden im folgenden zusammengefasst: Erstens. alle Diskursmedien enthalten die Diskursfunktion. tragen die Bedeutungen, existieren nur durch menschliche Sender sowie Empfanger und sind physisch wahrnehmbar. Zweitens. alle Diskursmedien mu¨ssen von einem System, das aus drei Komponenten besteht. aufgebaut werden: Eine materiell-sensorische Komponente ermo¨glicht die menschliche Wahrnehmung. Eine kognitivsemiotische ermo¨glicht den gedanklichen Bezug. Und eine soziale bildet die Grundlage des medialen Prozesses. Drittens. im Zeichensystem der Diskursmedien gibt es folgende notwendigen Prozesse: Die Verknupfung von Materie und Bedeutung ( = Zeichenprozess) . die U¨bertragung von Bedeutungen( = Signalprozess) und die Zuwendung zu Zeichen und Signalen (= Kontakprozess). Viertens. fur die modernen Diskursmedien ist die Einstellung des hinweisenden und registrativen Zeichens sehr notig. Daher sind das Filmzeichen und das Gestikzeichen analysierbar.

건강한 성인에서 발병한 칸디다와 연관된 위궤양 1례

Fungus such as Candida albicans is a normal flora that is frequently found in the oral cavity, gastrointestinal tract, genitourinary tract, vaginal mucosa in a normal person. However, candida can cause opportunistic infection in an immune compromised host. Candidiasis has broad spectrum of disease from mucocutaneous infection to invasive or disseminated infection. But, it is rarely reported that candida is associaed with gastrointestinal tract disease in a healthy adult. The case of gastric ulcer associated with candida particularly in a health adult is reported with relevant literature.

Afferent loop syndrome is an uncommon complication of a gastrectomy and Billroth Ⅱ reconstruction. It may cause symtoms at any time from the first postoperative day to many years after the gastrectomy. Afferent loop syndrome is characterized by abdominal pain, vomiting and elevation of serum amylase. Thus, it is difficult to differentiate afferent loop syndrome from other cause of acute pancreatitis. However, the history of gastrectomy can be an important clue for diagnosing afferent loop syndrome. We experienced one case of chronic afferent loop syndrome with acute pancreatitis. After appropriate management, the abdominal pain disappeared and serum amylase level decreased. We report this case with a review of relevant literatures.

진해 일대 조류의 종 다양성

본 연구은 진해항 일대의 조류상을 조사ㆍ분석한 것이다. 조사는 2001년 12월부터 2002년 4월까지 28회를 조사하였다. 센서스 기간 중 총 61종의 조류를 조사 지역내에서 관찰할 수 있었으며, 연 55,115개체가 관찰되었다. 종수의 합계와 총 개체수 그리고 종별ㆍ개체별 출현 빈도에서 유도된 종 중요도는 불은부리갈매기[0.58], 괭이갈매기[0.14], 흰죽지[0.12], 고방오리[0.07], 민물도요[0.07], 참새[0.06], 재갈매기[0.06]가 중요한 종으로 나타났다. 월별 최대 관찰 개체수는 2001년 12월에 46종 5,993개체를 관찰하였으며, 2002년 1월에 37종 7,105개체, 2월에 37종 8,837개체, 3월에 38종 4,106개체, 그리고 4월에 32종 2,819개체를 관찰하였다. 종 풍부도는 4월에 가장 높게 나타났으며, 종 다양성과 균등도는 1월이 가장 높게 나타났다. 월별 종의 유ㆍ무와 개체수를 비교하여 양방향 유사도를 분석한 결과 2월과 3월의 조류 군집이 유사한(0.69: Pearson Correlation Index) 것으로 나타났고, 다음으로 1월(0.617), 12월(0.522)로 12, 1, 2, 3월이 하나의 cluster를 형성하고 있었으며, 4월이 틀린 cluster를 형성하고 있다. 수금류, 섭금류, 갈매기류, 참새류 등 4가지 중요 군집으로 구분하여 개체수를 비교한 결과 66%가 붉은부리갈매기 외 3종의 갈매기류였고, 21%가 혹부리오리 외 13종의 수금류, 7%가 붉은머리오목눈이 외 21종의 참새목, 5%가 흰물떼새 외 5종의 섭금류, 1%미만의 맹금류, 논병아리류, 가마우지류의 조류로 나타났다. We investigated the abundance and local distribution of birds, in four areas of the jinhae. Data were collected from four areas in the jinhae from 2001 to 2002. In this study, 55115 individual birds of 80 species were censused. We evaluated important species with abundance of birds were as follows: Black-headed Gull[0.58], Black-tailed Gull[0.14], Pochard[0.12], Pintail[0.07], Dunlin[0.07], Tree Sparrow[0.06], and Herring Gull[0.06]. The number of species and individuals in each month were recorded as follows: 5993 individual birds of 46 species in December(2001), 7105 individual birds of 37 species in January(2002), 8837 individual birds of 37 species in February, 4106 individual birds of 38 species in March, 2819 individual birds of 32 species in April. In study period, the april was the highset richness(R' =0.60) and the January was the highest diversity(H'=2.08) and evenness(E5=0.52) with an expected species number of E[S_(818)]=29. Pairwise similarity declined with increasing distance between recording month and recording month from two different clustered separately in a UPGMA cluster tree. Comparison of population variable at 4 sites of 4 classes was compared as follows: 1.Gull(66%), 2. Waterfowl(21%), 3. Passerine(7%), and 4. Wader(5%).
