목적 : 국내 병원에서의 정주 ciprofloxacin 사용의 적정성을 평가하고자 하였다. 재료 및 방법 : 2003년 9월 25일부터 8개의 대학병원에서 정주 ciprofloxacin을 받았던 290명의 연속적으로 모집한 성인환자를 대상으로 약제사용 평가(DUE)를 전향적으로 수행하였다. 이 연구에 사용된 DUE 기준은 미국병원약사회의 기준을 국내 실정에 맡게 수정하여 사용하였으며 1) 사용의 정당성에 관한 항목, 2) 사용 시 준수되거나 시행되어야 할 사항에 대한 항목, 3) 부작용에 대한 항목, 4) 치료 효과에 대한 항목으로 나누어 조사하였다. 결과 : 환자의 평균 나이는 54세였다. 33예(26.9%)에서만 약제 사용의 적정성 측면에서 기준을 만족하였고 91예(73.1%)에서는 사용이 적절하지 않았다. 정주 cipro-floxacin의 부적절한 사용의 흔한 원인은 수술 전후의 예방약 사용, 비뇨기계 처치 전의 예방적 사용, 열성 질환에 대한 경험적 사용 등의 순이었다. 사용 시 준수사항 의 측면에서는 사용 전 배양검사(73.8%)와 사용 중 배양검사(68.6%)가 낮은 수행 정도를 보였으나 그 외는 비교적 높은 적정성(82.4-100%)을 보여주었다. 정주 ciprofloxacin사용의 합병증은 드물었다. 치료 효과의 면에서 치료반응은 93.3%였다. 결론 : 약제 사용의 정당성 기준에 따른 정주 cipro-floxacin의 부적절한 사용은 연구에 포함된 대학병원에서 매우 흔하였다(73.1%). 그러므로 적절한 항생제 사용에 대한 교육과 제도적 장치가 매우 필요한 것으로 판단되었다. Background : To evaluate the appropriateness of intravenous (IV) ciprofloxacin usage in university hospitals in Korea. Materials and Methods : We prospectively evaluated the appropriateness of drug usage in 290 consecutive adult patients who received IV ciprofloxacin in eight teaching hospitals during the period from September 25, 2003 to November 15, 2003. Drug use evaluation (DUE) in this study was based on the modified standard by the American Society of Hospital Pharmacy with regard to the justification of drug use, critical and process indications, complications, and outcome measures. Results : Average age of the patients was 54 years. The use of IV ciprofloxacin was appropriate only in 33 cases (26.9%). Common reasons for inappropriate use of IV ciprofloxacin include perioperative uses or prophylactic injection before urologic procedures, and empirical therapy for febrile episodes. Most of the "critical indications" and "process indications" showed high rate of appropriateness (82.4-100%). However, the rate of taking culture and sensitivity test before (73.8%) and during ciprofloxacin use (68.6%) were low. Complications with ciprofloxacin use were rare. In 93.3% of cases, ciprofloxacin treatment was clinically effective. Conclusion : Use of intravenous ciprofloxacin is frequently inappropriate in tertiary care hospitals in Korea. Implementation of education program and antibiotic control system is critical in improving the appropriateness of antimicrobial use.

For the development of a large-scale dialysis process for silk fibroin solution, a batch and continuous dialysis chamber was designed and built, which are to be used as a unit module for the process. A series of dialysis experiments were carried out to estimate the kinetic parameters for dialysis using the solutions of salt-solubilized silk fibroin. The solution was dialyzed by distilled water either batch-wise or continuously. Dialysis kinetics was monitored by measuring refractometery at high salt concentration and conductivity at lower salt concentration. The apparent dialysis coefficients were 5.4g/m2-hr and 1.8g/m2-hr, for well mixed batch and continuous dialysis, respectively. The alteration of molecular weight distribution of dialyzed silk fibroin solution was monitored.

<P>The important merit of Bi-2212/Ag high T<SUB>c</SUB> superconducting wire is to apply cable as round wire geometry. Bi-2212/Ag wires were fabricated in order to develop Rutherford cable for SMES near the future. Various Ag ratio and various numbers of filaments for PIT (powder-in-tube) process were applied to investigate the workability during drawing and to improve critical current property of the wires. In order to find proper working process, we investigated the variation of drawing stress during working. Post-annealing of Bi-2212/Ag wires at various atmospheres was carried out after main heat-treatment in order to increase critical current density. The results of post-annealing in nitrogen were better than that of post-annealing in oxygen. J<SUB>c</SUB> of Bi-2212/Ag wire was increased as increased maximum temperature,T<SUB>m</SUB> , for partial melting until 890degC, and highest J<SUB>c</SUB> was over than 2000 A/mm<SUP>2</SUP>. The shape of grain was observed by SEM to investigate Bi-2212 phase formation in filaments. The critical current density increased in proportion to the increase of Bi-2212/Ag wire's filaments number.</P>
잡음 영상에서 국부 영역의 반복적인 위상 상관도를 이용한 부화소 이동량 추정방법

본 논문에서는 원 영상에서 특정한 국부 영역을 선택하고 선택된 국부 영역에 반복적인 위상 상관도를 수행함으로써 잡음을 가지고 있는 영상에서 정확한 부화소(subpixel) 이동량을 추정하는 방법을 제안한다. 기존의 방법은 영상 전체를 이용해 위상 상관도를 수행 한 후 포물선, sinc 같은 함수 보간을 통해 간접적으로 부화소 이동량을 추정하였다. 이 같은 추정방법은 정확하나 백색 가우시안 잡음(additive white gaussian noise)이나 에일리어싱(aliasing)과 같은 잡음이 영상에 존재하면 정확한 부화소 이동량 추정이 어렵다. 그래서 영상 전체를 이용하기 보다는 잡음이 적은 국부 영역을 선택하여 이를 이용해 위상 상관도를 수행하고 부화소 이동량을 계산할 때 기존의 함수의 보간을 사용하기 않고 반복적으로 위상 상관도를 수행함으로써 잡음에 강한 부화소 이동량을 추정 방법을 제안하였다. 또한 위상 상관도 함수의 분석을 통해 위상 상관도 반복횟수를 줄이는 방법도 제안하였다. 실험을 통해 제안한 방법과 기존의 방법으로 추정한 부화소 이동량의 오차를 비교하고 초해상도 영상을 만들어 봄으로써 제안한 방법이 잡음이 있는 조건에서 더 정확한 부화소 이동량 추정성능을 보여줌을 알 수 있었다. In this paper, we propose a subpixel shift estimation method using phase correlation with a local region for the registration of noisy images. Phase correlation is commonly used to estimate the subpixel shift between images, which is derived from analyzing shifted and downsampled images. However, when the images are affected by additive white Gaussian noise and aliasing artifacts, the estimation error is increased. Thus, instead of using the whole image, the proposed method uses a specific local region that is less affect by noises. In addition, to improve the estimation accuracy, iterative phase correlation is applied between selected local regions rather than using a fitting function. the restricted range is determined by analyzing the maximum peak and the two adjacent values of the inverse Fourier transform of the normalized cross power spectrum. In the experiments, the proposed method shows higher accuracy in registering noisy images than the other methods. Thus, the edge-sharpness and clearness in the super-resolved image is also improved.

Congenital melanocytic nevus is defined as a lesion present at birth and containing nevus cells. It is characterized by a diffuse infiltration of nevus cells in the deepest reticular dermis to subcutaneous layer and presence of nevus cells around skin appendages. Cutaneous mucin deposition can be observed in variety of cutaneous disorders. However, it is rare in association with congenital melanocytic nevus. A 9-year-old girl presented with a black colored nodule on the right areolar area. The lesion existed from birth and the size of the lesion increased proportionally as the patient grows up. No subjective symptom such as itching or pain was noted. There was no trauma or medication for nevus lesion on her past history. Skin biopsy was done from the black colored nodule on the right areolar area showed diffuse infiltrated cell nests composed of nevus cells with abundant mucin deposit in papillary and reticular dermis. Mucin deposition in congenital melanocytic nevus has been rarely reported. Aging process, trauma and medication have been suspected as trigger factors. Furthermore, the interaction between melanocytes and the extracellular matrix via various cytokines might be a possible mechanism, but the exact pathway remains to be identified. We report a rare case of congenital melanocytic nevus with mucin deposition, which is especially scarce in children.
Epidermal cysts on buttocks: a commonly misdiagnosed condition

Background: Epidermal cysts are the most common epithelial cysts of the skin. They may occur anywhere on the body. However, there have been few studies to determine the characteristics of epidermal cysts on buttocks. Objectives: The purpose of this study was to investigate the clinicohistopathologic features of epidermal cysts on buttocks. Methods: We retrospectively reviewed the medical records of 43 patients with histologically proved epidermal cysts on buttocks (buttock group) and 1279 patients with confirmed epidermal cysts on non-buttock area (non-buttock group) from March 2007 to August 2016. Results: A total of 48 lesions from 43 patients were included. Mean age of patients was significantly older in buttock group (52.3±22.6 years) than non-buttock group (45.8±18.7 years). Size of the lesion was significantly larger in buttock group, and also inflammatory change of cysts more frequently occurred in buttock group (52.1%), compared to non-buttock group (28.9%). Punctum was less frequently found in buttock group (19%), compared to non-buttock group (52.1%). In addition, 44.2% of epidermal cysts on buttocks were confused with other dermatoses (e.g., lipoma, carbuncle) at the first visit. Conclusion: Epidermal cysts on buttocks have some different clinical features such as age, size, and coexistent inflammation or punctum, compared to typical epidermal cysts on non-buttock area. Our study may be informative for clinicians to prevent misdiagnosis of epidermal cysts on buttocks.

Background: In Korea, traditional medicine called Korean oriental medicine (KOM) is widely used. However, especially in dermatologic field, there have never been reported a study of investigating the usage status of KOM. Objectives: The purpose of this study was to investigate the usage status of KOM in patients with dermatologic problems. Methods: A face-to-face questionnaire-based survey was performed to 203 patients who had ever used KOM at least one time before visiting department of dermatology at Pusan National University from July 2015 to August 2016. Results: The ratio of male to female patients was 0.73:1. Mean age was 37.9 years (range, 2-87). The mean duration of skin symptom was 54 months (range, 10 days to 40 years). The most common motive of using KOM was ‘Unsatisfied results of modern medicine’ (62.6%), followed by ‘Desire to try everything’ (28.2%). The most common response for subjective satisfaction with KOM was ‘Not exactly knowing’ (32.4%), followed by ‘Disappointed’ (30.2%). The most common cause of quit using KOM was ‘No effects’ (68.3%), followed by ‘Side effects’ (25.5%), and ‘High cost’ (19.3%). The patients most frequently complained with aggravation of skin symptom as a side effect of KOM (35.7%). Conclusion: Our study sheds light on current usage status of KOM in dermatology field, especially in a tertiary hospital. The use of KOM seems not to be very effective and also not to be more cost-effective than dermatology based on modern scientific evidence.
