        정상면역을 가진 성인에서 발생한 식도 방선균증

        Hyun Soo Kim, Jong Woon Cheon, Min Su Kim, Chang Kil Jung, Kyung Rok Kim, Jae Won Choi, Dong Woo Kang and Sun Young Kim1 Departments of Internal Medicine and Pathology1, DongKang Medical Center, Ulsan, Korea Actinomycosis is a chronic suppurative disease and caused by Actinomycosis species, principally Actinomyces israelii, which are part of the normal inhabitant on the mucous membrane of the oropharynx, gastrointestinal tract, and urogenital tract. It usually affects cervicofacial, thoracic and abdominal tissue. Cervicofacial type has the highest percentage of occurrence with 50%. Actinomycosis frequently occurs following dental extraction, jaw surgery, chronic infection or poor oral hygiene. It may also be considered as an opportunistic infection in immunocompromised patients such as malignancy, human immunodeficiency virus infection, diabetes mellitus, steroid usage or alcoholism. But, actinomycosis rarely occurs in adults with normal immunity and rare in the esophagus. We report an unusual case of esophageal actinomycosis which was developed in a patient with normal immunity and improved by therapy with intravenous penicillin G followed oral amoxicillin, and we also reviewed the associated literature.

        강변여과에서 파일럿규모 수평집수관의 수두분포 연구

        강변여과용 수평집수관을 모사하는 파일럿 규모의 모래통 실험을 수행하였으며, 다양한 출구유속에 대한 수평집수관에서의 수두분포를 구하였다. Kim24)이 수행한 실험실 규모의 수평집수관에서의 손실수두에 Kim25)이 개발한 이론 즉, 작은 규모의 수평집수관에서의 손실수두로부터 큰 집수관의 손실수두를 계산하는 방법을 적용하여 본 연구의 관측치와 비교함으로써 Kim25)의 이론이 타당함을 알 수 있었다. 또한, 이들의 이론을 적용하여 본 연구에서의 관측치로부터 실규모 집수관에서의 수두분포를 구하는 방안을 예를 들어 설명함으로써 강변여과의 수평집수관 설계방법을 제시하였다. A pilot-scale sand-box experiment simulating a lateral of collector well used in riverbank filtration was performed, and hydraulic head distributions along the lateral were measured according to the various drawdowns at the well. The results of this experiment were compared to the predicted values obtained using a theory previously developed by Kim25) in order for its validation. This theory predicts the head loss in a large-scale lateral given the loss in a small-scale one, and to have actual values for comparison, the results from a lab-scale experiment previously performed by Kim24) were employed. Comparing the experimental values to the extrapolated value indeed confirmed the validity of the theory. A procedure to determine the hydraulic head distribution of a practical-scale lateral was also presented applying the theory to the experimental result of this study in an effort to show the process of lateral design for riverbank filtration.

        씨름선수에서 체중 감량과 회복기의 Leptin과 PAI-1 변화

        Obesity which is defined as accumulation of excess body fat, is central factor of insulin resistance syndrome. Recently, it is revealed tat adipose tissue is not simply an energy storage organ but it also secretes a variety of molecules which affect the metabolism of the whole body, those are leptin and plasminogen activator inhibitor-1(PAI-1). Therefore, leptin and PAI-1 are increased in the obese state. Leptin regulates energy homeostasis and satiety and PAI-1 regulates fibrinolytic system. For these reasons, elevated levels of leptin and PAI-1 are considered as link factors between obesity and insulin resistance syndrome. However, the exact regulating mechanism for serum levels of leptin and PAI-1 is not fully understood yet. In this study, to evaluate the regulating mechanisms of serum levels of leptin and PAI-1 according to the weight changes, we measured leptin, PAI-1, physical, metabolic, and endocrine parameters during 10 days of weight reduction and 10 days of regain period in 7 young athletes. The mean body weight change was -4.7 kg(5.0%) in the weight reduction period and -2.4 kg(2.5%) in the weight regain period compared to baseline value. Baseline level of leptin in athletes was 1.7±0.66 ng/ml, which was significantly correlated with body weight, BMI, percent body fat, body fat mass, triglyceride, insulin, and PAI-1. Baseline level of PAI-1 in athletes was 16.6±5.26 ng/ml, which was significantly correlated with body weight, BMI, triglyceride, insulin, and leptin. Leptin was decreased to 0.7±0.39(44% of the basaline value) in the weight reduction period, and increased to 1.9±0.64(119% of the baseline value) in the regain period. PAI-1 was decreased to 7.4±2.72(44% of the basaline value) in the weight reduction period, and increased to 22.8±7.33(138% of the baseline value) in the regain period. The changes of leptin during weight reduction period were significantly correlated with the changes of insulin(r=0.890, P<0.01) and triglycerides(r=0.874, P<0.01). The changes of PAI-1 during weight reduction period were significantly correlated with the changes of FFA(r=0.889, P<0.01) and triglycerides(r=0.869, P<0.05). The changes of both leptin and AAI-1 during weight regain period were significantly correlated with the changes of insulin(r=0.755 and 0.849, P<0.05, respectively). In summary, these results suggest that serum levels of leptin and PAI-1 were affected by weight cycling, the percentages of change were more greater than that of weight change, and rebound phenomena were occurred during weight regain period.

        악하부에 발생한 결핵성 경부 임파선염

        Tuberculosis, a chronic infectious granulomatous disease, is preumed to be quite a rare entity. Because of the development of chemotherapy and the improvement of nutritional conditions, incidence of tuberculosis have reduced. For these reasons, tuberculosis may be overlooked in the differential diagnosis whenever dealing with a submandibular swelling. Diagnosis of tuberculous lymphadenitis consists of historical data, physical findings, laboratory tests and histologic examination. The treatment of choice seems to be surgical excision and long term antituberculosis chemotherapy. Surgery provides a rapid tissue diagnosis, because the histological examination of the excisional biopsy is the most reliable diagnostic test. This is the report of a case of tuberculous cervical lymphadenitis on left submandibular area with no evidence of the involvement of lung.

      • 茵蔯淸肝場의 B型 및 C型 간염 바이러스에 대한 抗 바이러스 效果

        우리나라와 같이 B형 간염 바이러스의 유병율이 높은 지역에서는 B형 간염 바이러스 뿐만 아니라 HCV에 대한 항 바이러스 효과도 동시에 시행함이 유용할 것으로 사료되었다. 본 연구에서는 茵蔯淸肝湯을 이용하여 B형 간염 바이러스에 대한 抗 바이러스 효과를 PCR을 이용하여 HBV의 DNA를 검색하였다. 茵蔯淸肝湯이 처리된 처리군의 경우 대조군에 비하여 농도 의존적으로 B형 간염 바이러스의 표면항원 생산을 억제하였고, PCR을 이용하여 증폭한 경우에도 증폭된 DNA의 양은 감소하였다. 또한 C형 간염 바이러스에 대한 抗 바이러스효과를 검색하기 위하여 복제에 필수적인 단백질인 HCV helicase의 ATPase에 대한 활성 억제 효과를 조사하였다. 茵蔯淸肝湯이 HCV helicase의 ATPase 활성을 억제하는 효과가 있음을 확인한 후 dose-respond에 따른 실험을 수행하여 HCV helicase의 ATPase 활성을 억제함으로써 탈인산화 되지 않은 ATP의 양을 이용하여 PSL로 분석한 결과 茵蔯淸肝湯이 HCV helicase의 ATPase 기능에 대한 억제효과가 우수함이 확인되었다. 이상의 결과로 보아 茵蔯淸肝湯이 B형 및 C형 간염 치료에 동시에 응용될 수 있을것으로 사료된다. For the purpose of investigate the inhibition effects of Injinchunggantang on Hepatitis and C Virus. The inhibition experiments were as follows : 1. The inhibition effects of Hepatitis B surface antigen production was remarkably increased in the pretreated group of Injinchunggantang compared with control group. 2. Amplified PCR products of HBV-DNA in culture media was considerably decreased in the pretreated group of Injinchunggantang compared with control group. 3. Results of ATP quantitation and ATPase inhibition percent was increased pretreated group of Injinchunggantang. Also ATPase activity of HCV helicase inhibition assay was increased with a dose dependent manner. These results may suggest that Injinchunggantang will have the inhibition effects of Hepatitis B and C Virus.

        건물내장재(석고보드,합판)의 화재성능평가

        본 연구는 국내와 외국의 건물 내장재 화재성능평가방법을 고찰하고 국내에는 도입되지 않은 실대 화재 실험인 Room Corner Test를 국내 내장재중 석고보드와 합판을 대상으로 그 적용성을 위한 실험을 진행하였다. 현재 국내 내장재의 평가규정은 Bench-Scale Test에 의존하고 있으나, 미국, 일본 유럽등에서는 Room Corner Test를 적용한 실질적인 재료의 화재성능평가를 시행하고 있다. 실험 결과, NFPA 265에 의한 석고보드 및 ISO 9705에 의한 합판의 성능은 양호한 것으로 판단되었다. 현재 국내 내장재에 적용되는 등급분류의 평가방법은 재료의 실제 화재성능 평가에 한계가 있으므로 향후 Room Corner Test의 적용과 더불어 Bench-Scale Test 결과를 토대로 화재성능을 예측하는 화재모델을 이용하는 평가 방법의 도입이 필요하다. The fire performance evaluation methods in Korea and overseas for interior finish materials were analysed and tested with gypsum board and plywood by using room corner test not adopted by domestic code until now. The results of gypsum board (thickness: 8 mm) and plywood (thickness: 4 mm) applying NFPA 265 and ISO 9705 test respectively are satisfied the assessment criteria. To assess a actual fire performance and classify fore hazard levels for interior finish materials, room-corner test and flame spread models should be adopted in building code and fire code to overcome limitations of current bench-scale test method.       • 진주8경 사진에 나타난 경관자원에 관한 연구 : 제1회 진주8경 제정기념 사진공모전에 출품된 사진을 중심으로

        This study focused on the points of viewing scenic resources in order to see local scenic resources in new light in developing the scene improvement plan. For the purpose of this study, the methods of viewing scenic resources and their contents were analyzed from the pictures displayed on the picture contest organized by Jinju City to publicize its newly designated Palkyong. 1) In terms of scenic resources used in the Palkyo˘ng, the pictures of the old Palkyo˘ng included many abstract contents as well as concrete elements, whereas those of the new Palkyo˘ng were mostly based on concrete and clear objects. 2) In terms of viewing types, both horizontal viewing type and overlooking viewing type were mostly used. It could be found, therefore, that Jinju's Palkyo˘ng are mostly viewed horizontally or downward from the viewing points. 3) In addition to seasonal factors, these characteristics of viewing play an important role in making scenic resources become beauty spots. This means that viewing angle and seasonal factors can have a significant influence on the scenic value of beauty spots. 4) Scenic resources were also analyzed in terms of viewing distances in order to determine structural elements of the Palkyo˘ng. It have been recognized that for Jinju Castle and Chokseoknu Pavilion, the downtown area is an obstructive or inharmonic element.

      • 가변 부하를 위한 RF 전원 공급기 설계

        In the RF gas discharge system the load varies frequently by the operational environment. And it is often necessary to change the matching network to provide the maximum possible power transfer from the oscillator to the RF load. An Rf oscillator whose variable operating frequency is between 20 and 30 MHz and whose output RF power is about 200 watts was designed to solve this problem. The conversion efficiency from the DC power was about 80% It is found that the output oscillating frequency which gives minimum RF reflections from the loads follows automatically to the changes of the load or of the operating conditions. It is also found that the output frequency decreases with the larger load shunt capacitance.

      • 2,4,6-Collidinium Dichromate에 의한 알코올의 산화 특성 연구

        2, 4, 6-Collidinium dichromate(CDC) is more, stable. nonhygroscopic, and milder than other chromium(VI)oxidants. The new oxidant was characterized via rate of oxidation. CDC with pyridinium P-toluenesulfonate(PPTS) in dichloromethane was effective in oxidation of allylic, benzylic, and secondary alcohols to the corresponding carbonyl compounds at room temperature. while CDC alone did not oxidize above alcohols. Oxidation of ally lie and benzylic alcohols progressed more repididly than that of secondary alcohols. When molecular sieve(MS)was added to the CDC-PPTS system, rate of oxidation and yields increased. Saturated aliphatic primary alcohols were decomposed in two system, CDC-PPTS and CDC-PPTS system added MS. The oxidation selectivity of CDC-PPTS system was increased about 1.75 times than that of PDC-PPTS system.
