Background : In recent years, gonorrhea has been panedemic and remains one of the most commom STDs in the world, especially in developing countries. Objective & Methods: For the detection of a more effective therapeutic regimen and assessing the prevalence of PPNG, we have been trying to study the patients who have visited the VD Clinic of Choong-Ku Public Health Center in Seoul since 1980 by means of the chromogenic cephalosporin method. Results: In 1996, 139 strains of N. gonorrhoeae were isolated, among which 53(39.0%) were PPNG. Conclusion: Our results suggests that after a peak of 74.3% in 1993, the prevalence of PPNG in Seoul is gradually declining.

Talc has been used as mineral drugs in the oriental medicine to cure catharsis, dysentery, dry breast, poor micturition, chronic indigestion, etc. according to the 'Donguebogam' which had been published by Heo Jun in 1613. In the book there is a description of 'talc from Chungju is of good quality'. To investigate the description geologically and oriental medically, talc from Dongyang Talc Mine in the Chungju area was analysed and compared with that of Oriental Medical Hospital of Kyungsan University, which has MgO 31.65%, CaO 0.73%, SiO_(2) 60.04%, ignition loss 6.4% close to those of pure talc containing MgO 31.7%, SiO_(2) 63.5%, H_(2)O 4.8%. However, talc from Dongyang Mine with MgO 31.8%, CaO 0.51% and SiO_(2) 61.0% is closer to pure talc and contains less impurities (Fe_(2)O_(3) 0.8%). Ca and Fe may add medical effects to talc drug as in the case of actinolite. Formula (3) logα_(Mg)^(2+) = 11.89 - 2pH and (4) logα_(Mg^(2+)/α^(2)_(H)^(+)) = 6.6 - 1.33 logα_(H4SiO4) may be used as basic data to predict and examine soluble contents of talc drug for medical experiments.

치아의 본래 기능은 저작이다. 저작은 소화의 전계단이므로 치아의 결손은 소화에 까지 그 영향이 있을뿐만 아니라 특히 성장기에 있는 아동들이 영양상태에 까지 그 영향이 있다. 충치자면 물론 저작이 불량하고 불량하면 소화흡수에 큰영향이 있을것이고 이것은 곧 saliva에도 어떤 영향이 있을것이다. 현재까지 구체적으로 충치자의 saliva와 무충지자의 saliva에 대한 실험이 적으며 특히 생화학적 연구는 더욱 적다.
Multiple Spinal Metastases of Anaplastic Meningioma: A Case Report

Meningiomas are primary benign tumors that occur in intracranial and intraspinal regions. Rarely, atypical and anaplastic meningiomas exhibit malignant tendencies and can metastasize. A 56-year-old female patient visited the hospital complaining of a dull headache, mild dysarthria, sudden onset of blurred vision, and mild weakness in the left upper limbs. A homogeneously augmented mass was revealed in the right temporal lobe following magnetic resonance imaging (MRI) and was determined to be an anaplastic meningioma after surgical resection. During follow-up, revision surgery was performed due to the recurrence of the primary tumor. After the revision surgery, the patient complained of new symptoms, which included paresthesia and muscle weakness in the right lower extremity. MRI revealed a mass in the cervical and lumbar vertebrae suspected to have been metastasized. Another revision surgery was conducted on the intracranial primary tumor. The tumor was resected from the cervical and lumbar vertebrae. Histopathology revealed that they were all anaplastic meningiomas. Although anaplastic meningioma is rare, it can cause extracranial metastases. This case shows that multiple spinal metastases of anaplastic meningioma require considerable attention in diagnosis and treatment.

Objective: As Korea has become a super-aging society, the number of elderly patients with degenerative spinal disease has steadily increased. This study aimed to evaluate the usefulness of oblique lumbar interbody fusion (OLIF) by comparing the results and complications between OLIF and open spinal fusion in elderly patients with degenerative spinal disease.Methods: Thirty-one patients underwent one-level posterior lumbar interbody fusion (PLIF), and 35 patients underwent one-level indirect decompression OLIF. The clinical outcomes of the two patient groups were analyzed using the visual analog scale (VAS) and Oswestry Disability Index (ODI). Through a comparative analysis of pre- and post-operative images, we measured changes in disc height (DH) and foraminal height (FH), and investigated the occurrence of subsidence in each patient group. The cross-sectional area was measured using magnetic resonance imaging pre- and post-operatively in OLIF.Results: The pre-operative VAS and ODI scores were similar between the two groups and tended to improve post-operatively. In the OLIF group, the estimated blood loss (EBL) was significantly lower, DH and FH gains were more frequent, DH and FH maintenance was better, and cage subsidence occurred less frequently. In OLIF, psoas paresis and approach-site hematoma occurred; however, all patients with psoas paresis recovered without further treatment before discharge. In the PLIF group, dural tears occurred in four patients who recovered without revision surgery. Conclusion: OLIF enables indirect decompression, reduced EBL, shorter hospitalization, and lesser subsidence. OLIF is considered a beneficial, minimally invasive surgical method for elderly patients.

Objective : Recently, robotic-assisted spine surgery (RASS) has been considered a minimally invasive and relatively accurate method. In total, 495 robotic-assisted pedicle screw fixation (RAPSF) procedures were attempted on 100 patients during a 14-month period. The current study aimed to analyze the accuracy, potential risk factors, and learning curve of RAPSF. Methods : This retrospective study evaluated the position of RAPSF using the Gertzbein and Robbins scale (GRS). The accuracy was analyzed using the ratio of the clinically acceptable group (GRS grades A and B), the dissatisfying group (GRS grades C, D, and E), and the Surgical Evaluation Assistant program. The RAPSF was divided into the no-breached group (GRS grade A) and breached group (GRS grades B, C, D, and E), and the potential risk factors of RAPSF were evaluated. The learning curve was analyzed by changes in robot-used time per screw and the occurrence tendency of breached and failed screws according to case accumulation. Results : The clinically acceptable group in RAPSF was 98.12%. In the analysis using the Surgical Evaluation Assistant program, the tip offset was 2.37±1.89 mm, the tail offset was 3.09±1.90 mm, and the angular offset was 3.72°±2.72°. In the analysis of potential risk factors, the difference in screw fixation level (p=0.009) and segmental distance between the tracker and the instrumented level (p=0.001) between the no-breached and breached group were statistically significant, but not for the other factors. The mean difference between the no-breach and breach groups was statistically significant in terms of pedicle width (p<0.001) and tail offset (p=0.042). In the learning curve analysis, the occurrence of breached and failed screws and the robot-used time per screw screws showed a significant decreasing trend. Conclusion : In the current study, RAPSF was highly accurate and the specific potential risk factors were not identified. However, pedicle width was presumed to be related to breached screw. Meanwhile, the robot-used time per screw and the incidence of breached and failed screws decreased with the learning curve.

Objective : Oblique lateral interbody fusion (OLIF) is becoming the preferred treatment for degenerative lumbar diseases. As beginners, we performed 143 surgeries over 19 months. In these consecutive cases, we analyzed the learning curve and reviewed the complications in our experience. Methods : This was a retrospective study; however, complications that were well known in the previous literature were strictly recorded prospectively. We followed up the changes in estimated blood loss (EBL), operation time, and transient psoas paresis according to case accumulation to analyze the learning curve. Results : Complication-free patients accounted for 43.6% (12.9%, early stage 70 patients and 74.3%, late stage 70 patients). The most common complication was transient psoas paresis (n=52). Most of these complications occurred in the early stages of learning. C-reactive protein normalization was delayed in seven patients (4.89%). The operation time showed a decreasing trend with the cases; however, EBL did not show any significant change. Notable operation-induced complications were cage malposition, vertebral body fracture, injury to the ureter, and injury to the lumbar vein. Conclusion : According to the learning curve, the operation time and psoas paresis decreased. It is important to select an appropriately sized cage along with clear dissection of the anterior border of the psoas muscle to prevent OLIF-specific complications.
