        정신분열병에 대한 리스페리돈의 효과 및 안정성

        연구목적 : 본 시험의 목적은 임상시험 시작전에 연구자들을 대상으로 PANSS Workshop을 통하여 PANSS, ESRS에 대한 국내에서의 표준화 작업을 구축하고 새로운 정신병 치료제인 리스페리돈의 효과와 안정성을 재확인하여 리스페리돈 사용에 대한 적정화를 이루는데 있다. 연구방법 : 1996년 4월부터 1996년 9월까지 국내 39개 대학병원 정신과에 입원중인 혹은 증상이 악화되어 입원하는 정신분열병 환자 377명을 대상으로 다시설 개방 연구를 시행하였다. 1주일간의 약물 배설기간을 가진후, 리스페리돈을 8주간 투여하였고, 기준점, 1주, 2주, 4주, 그리고 8주후에 평가되었다. 용량은 제1일에는 리스페리돈 1mg씩 1일 2회, 제2일에는 2mg씩 1일 2회, 제3∼7일에는 3mg씩 1일 2회 투여하였다. 이후 환자의 임상상태에 따라 임의로 증량할 수 있으며, 최대 일일 16mg을 초과하지 않도록 하였다. 추체외로 증상을 조절하기 위한 투약을 허용하였다. 임상증상 및 부작용의 평가는 PANSS(Positive and Negative Syndrome Scale), CGI(Clinical Global Impression) 그리고 ESRS(Extrapyramidal Symptom Rating Scale)을 사용하였다. 연구결과 : 377명중 343명(91%)이 8주간의 연구를 완결하였다. 치료 종결시점인 8주후 PANSS 총점수가 20% 이상 호전된 경우를 약물 반응군으로 정의할때, 약물반응군은 81.3%였다. 리스페리돈에 반응하는 예측인자로는 발병연령, 이전의 입원 횟수, 유병기간이 관련 있었다. 리스페리돈은 1주후부터 PANSS양성, 음성, 및 일반정신병리 점수상에 유의한 호전을 보여 효과가 빨랐다. CGI의 경우도 기준점에 비해 1주후부터 유의한 감소를 나타내었다. ESRS의 경우, 파킨슨 평가점수는 기준점과 비교해 투여 1주, 2주, 4주후 유의하게 증가되었다가 8주후 기준점과 차이가 없었다. Dystonia 평가점수는 1주후만 유의한 증가를 보였으며, dyskinesia 평가점수는 유의한 차이가 없었다. 혈압, 맥박수의 생명징후 및 일반 혈액학 검사, 생화학적 검사, 심전도 검사에서 유의한 변화는 없었다. 결 론 : 이상의 다시설 개방 임상 연구를 통해 리스페리돈은 정신분열병 환자에서 양성증상뿐만 아니라 음성증상 및 전반적인 증상에도 효과적인 것으로 사료된다. 보다 명확한 평가를 위해서는 다른 항정신병약물과의 이중맹검 연구가 필요할 것으로 생각되며, 또한 장기적 치료에 대한 평가도 함께 이루어져야 하겠다. Objective : The purpose of this study was to investigate the efficacy and safety of risperidone in the treatment of Korean schizophrenic patients. Method : This multicenter open study included 377 schizophrenic patients drawn from 39 university hospitals. After a wash-out period of 1 week, the schizophrenic patients were treated with risperidone for 8 weeks and evaluated at 5 points ; at baseline, and 1, 2, 4 and 8 weeks of treatment. The dose was increased from 2mg/day(1mg twice daily) to 6mg/day(3mg twice daily) during the first week and adjusted to a maximum of 16mg/day over the next 7 weeks according to the patient's clinical response. Medication to control extrapyramidal symptoms was permitted. The psychiatric and neurological status of the patients was assessed by PANSS, CGI, and ESRS scales. Results : 343(91%) of 377 patients completed the 8-week trial period. Clinical improvement, as defined by a 20% or more reduction in total PANSS score at end point, was shown by 81.3% of patients. The predictors of response to risperidone were associated older age, shorter duration of illness, fewer previous hospitalization. Risperidone had rapid onset of action ; a significant decrease of the total PANSS and three PANSS factor(positive, negative, general), and CGI was already noticed at the end of first week. For the ESRS, parkinsonism rating scores were significantly increased until week 4 comparing with baseline. Dystonia rating scores were significantly increased until week 1, and dyskinesia rating scores were not significantly changed during the study. Laboratory parameters including vital sign, EKG, hematological, and biochemical values showed no significant changes during the trial. Conclusions : This study suggests that risperidone is generally safe and effective against both the positive and negative symptoms in our group of patients.

      • 신경인성방광에서 요역동학적 고찰

        Background and objectives : A disparity between clinical and urodynamic findings is commonly found and its application to the clinical management of neurogenic bladder is pratically as well embrassing as it reading. We investigated urodynamic study for the diagnosis and management of neurogenic bladder. Materials and methods : From July 1997 to June 2001, total of 83 patients of neurogenic bladder were evaluated by urodynamic study. Results : urodynamically, there were 20 cases(24%) showing detrusor hyperreflexia with detrusor external sphincter dyssynergia(DESD), 12 cases(15%) detrusor areflexia with low compliance. A disparity between the clinical and urodynamic assessment was noted in 26 cases(31%), clinically severe symptom with complication was particularily common in detrusor hyperreflexia with DESD, detrusor areflexia with low compliance and catheter indwelling patients. Conclusion : In neurogenic bladder, clinically symptom with complication was particularly common in detrusor hyperreflexia with DESD, detrusor areflexia with low compliance and catheter indwelling patients.

      • 전립선대비증 평가에서 이행대 용적지수와 증상점수 및 요류속도와의 상관관계

        Purpose : otal prostate volume has been known to be poorly correlated with other parameters used to assess benign prostatic hyperplasia(BPH), including international prostatic symptom score(IPSS), peak flow rate(Qmax). This study was aimed to investigate the possible correlation between transition zone index(TZI), IPSS and Qmax in patients with clinical benign prostatic hyperplasia(BPH). Materials and methods : From September 1999 to December 2000, A total of 95patients with clinical BPH were evaluated total prostate volume(TV), transition zone volume(TZV), TZI, IPSS and Qmax. TZI was determined as the ratio between transition zone volume(TZV) and total prostate volume(TV). Results : The relationships between IPSS and TV, TZV, and TZI were not statistically significant. A statistically significant relationship was observed between TZV, TZI and Qmax and between TV, TZV, TZI and age. Conclusion : These results suggest that TZV, TZI may not be a useful method to assess symptom severity in BPH patients but may be limited clinical parameters.

      • Effects of Transgenic Cabbage Expressing Cry1Ac1 Protein on the Wolf Spider, Pardosa astrigera L. Koch (Araneae: Lycosidae)

        Transgenic lines of insect resistant cabbage (Brassica oleracea var. capitata) expressing Cry1Ac1 protein has been developed to control diamondback moth (Plutella xylostella). The potential adverse effects of Bt crops on non-target arthropod herbivores and predators are major concerns. We conducted a tritrophic bioassay to study the ecological impacts of insecticidal transgenic cabbage on the wolf spider (Pardosa astrigera), a non-target generalist predator. First, we measured the levels of Cry1Ac1 proteins in fruit flies that were fed with the transgenic cabbage as well as those levels in the wolf spiders preying on the Bt cabbage-fed fruit flies using enzyme linked immunosorbent assay (ELISA). Cry1Ac1 proteins were detected in the Bt cabbage fed fruit flies and also in the wolf spiders after preying on Bt cabbage-fed fruit flies. Second, we compared the life history characteristics of the wolf spiders preying on the Bt or non-Bt cabbage. Growth, development time and survival of the wolf spiders were not significantly different between Bt and non-Bt cabbage. Although the wolf spiders were exposed to Cry1Ac1 protein via feeding on the preys containing Cry1Ac1 proteins, their growth and survival was not significantly affected.

      • Effects of Transgenic Cabbage Expressing Cry1Ac1 Protein on Target Species and the Non-Target Arthropod Community

        Under field conditions, we investigated how transgenic Bt cabbage expressing the insecticidal Cry1Ac1 protein affects two target Lepidoptera species — Plutella xylostella (Plutellidae) and Pieris rapae (Pieridae) — as well as the structure of the local non-target arthropod community. When exposed to Bt cabbage line C30, Plutella xylostella and Pieris rapae were significantly less abundant than when in the presence of the non-transgenic control. Multivariate analyses (PerMANOVA and NMDS) showed that composition of the non-target arthropod community was affected by sampling date but not by cabbage genotype. These results suggest that transgenic cabbage expressing Cry1Ac1 protein can be effective in controlling Plutella xylostella and Pieris rapae in the field and that cultivation of this cabbage may not adversely affect such non-target arthropod communities.

        The Modeling of the Differential Measurement of Air Pressure for Non-intrusive Sleep Monitoring Sensor System

        The respiratory and heart beat signals are the fundamental physiological signals for sleep monitoring in the home. Using the air mattress sensor system, the respiration and heart beat movements can be measured without any harness or sensor on the subject's body which makes long term measurement difficult and troublesome. The differential measurement technique between two air cells is adopted to enhance the sensitivity. The concept of the balancing tube between two air cells is suggested to increase the robustness against postural changes during the measurement period. With this balancing tube, the meaningful frequency range could be selected by the pneumatic filter method. The mathematical model for the air mattress and balancing tube was suggested and the validation experiments were performed for step and sinusoidal input. The results show that the balancing tube can eliminate the low frequency component between two cells effectively. This technique was applied to measure the respiration and heart beat on the bed, which shows the potential applications for sleep monitoring device in home. With the analysis of the waveform, respiration intervals and heart beat intervals were calculated and compared with the signal from conventional methods. The results show that the measurement from air mattress with balancing tube can be used for monitoring respiration and heart beat in various situations.

        <P>Digital hearing aid users often complain of difficulty in understanding speech in the presence of background noise. To improve speech perception in a noisy environment, various speech enhancement algorithms have been applied in digital hearing aids. In this study, a speech enhancement algorithm using modified spectral subtraction and companding is proposed for digital hearing aids. We adjusted the biases of the estimated noise spectrum, based on a subtraction factor, to decrease the residual noise. Companding was applied to the channel of the formant frequency based on the speech presence indicator to enhance the formant. Noise suppression was achieved while retaining weak speech components and avoiding the residual noise phenomena. Objective and subjective evaluation under various environmental conditions confirmed the improvement due to the proposed algorithm. We tested segmental SNR and Log Likelihood Ratio (LLR), which have higher correlation with subjective measures. Segmental SNR has the highest and LLR the lowest correlation of the methods tested. In addition, we confirmed by spectrogram that the proposed method significantly reduced the residual noise and enhanced the formants. A mean opinion score that represented the global perception score was tested; this produced the highest quality speech using the proposed method. The results show that the proposed speech enhancement algorithm is beneficial for hearing aid users in noisy environments.</P>

        The inverse correlation of appendectomy and ulcerative colitis is well known, but the effect of appendectomy on the clinical course of ulcerative colitis remains unclear. Although the majority of opinions were negative for the therapeutic advantage of appendectomy in patients with ulcerative colitis, advocates for positive effect of appendectomy have been proposed in a few case reports and a prospective study from Western countries. We herein report a case of ulcerative colitis who experienced clinical remission after appendectomy for acute appendicitis. The patient was 35 year old male and had been managed with immunosuppressant before appendectomy. The very acute change in clinical course and a long period of remission after appendectomy highly suggest the therapeutic effect of appendectomy for ulcerative colitis.         B-stage 레진 필름의 카본나노튜브 분산도 평가 및 제조공정 최적화

        B-stage 레진 필름에 탄소나노튜브(CNT) 등을 균일하게 분포시킨 뒤에 기타의 보강섬유 층과 함께 여러 겹으로 적층하여 하이브리드 형태의 복합재료를 만드는 방법은 유용하다. 본 연구에서는 CNT가 포함된 에폭시 레진으로부터 shear mixing 및 Three-roll mill 공정을 이용하여 B-stage 레진 필름을 제작하였다. 두 공정을 통해 형 성된 CNT/레진 복합재 필름의 분산도를 파단면의 SEM 관찰을 통해 분석하였다. 보다 효율적인 공정을 위해 Calendering pass 횟수에 따른 분산도를 평가하였다. Pass의 횟수에 따른 샘플을 제조하고 CNT 분산도는 SEM 이미지를 통해 확인하고, 전기 전도도 측정을 통해 분석하였다. 추가적으로 gap mode, force mode를 통해 제작한 각각의 샘플의 전기 전도도를 측정하여 CNT 분산도를 분석하였고 이를 통해 최적공정을 도출하였다. An appropriate way to fabricate a hybrid composite containing evenly dispersed carbon nano tubes(CNTs) is to stacking B-stage resin films that contain evenly dispersed CNTs and various reinforcing fiber layers alternatively. In the present study, B-stage resin films are manufactured via shear mixing and three-roll milling. CNT dispersion in resin via these two processes are evaluated by SEM on their fracture surfaces. For more efficient process, the dispersivities are evaluated according to the number of calendering passes. Samples are made for different number of passes during calendering, and their dispersions are evaluated via SEM fractographs as well as by measuring their electrical conductivities. Additionally, the optimal process conditions are obtained by measuring the electrical conductivity and evaluating their dispersivity of the samples prepared by gap mode and force mode.
