전침 자극에 의한 오디 괄약근 운동성 변화에 관한 연구

Background/Aims: This study was designed to evaluated the effects of electroacupuncture on the sphincter of Oddi (SO) motility in humans and to correlate the manometric findings with plasma cholecystokinin (CCK) levels. Methods: Eleven patients (M: F= 5: 6) who had various kinds of biliary disorders were enrolled. SO motility was monitored with a conventional low-compliant continuously perfused technique using ERCP (n=9) or percutaneous transhepatic cholangioscope (n=2). After baseline monitoring for phasic wave contractions of SO, electroacupuncture was applied at a specific acupoint, GB 34, in these 11 patients. A nonspecific acupoint 5 cm away from GB 34 was selected as a control. Manometric parameters of SO were also checked in 6 subjects during stimulation of the control acupoint. CCK plasma levels were measured during the time course of electroacupuncture stimulation. Result: All the manometric parameters including the basal pressure of SO, amplitude, frequency and duration of phasic wave contraction of SO were significantly decreased (p< 0.05) during electroacupuncture stimulation. The inhibition of SO contractility was accompanied by increased CCK plasma levels. After removal of electroacupuncture stimulation, restoration of amplitude and duration to basal condition were noted. A tendency towards the return of SO contractility was also observed in basal pressure and frequency. Stimulation of the control acupoint, however, did not affect the SO contractility and plasma CCK levels. Conclusions: Electroacupuncture stimulation of acupoint GB 34 showed reversible inhibition of SO contraction in humans. We speculate that the response of SO to electroacupuncture stimulation may be mediated by some neurohormonal mechanisms including CCK release. K-식(式) 침전관(沈澱管)에 의한 간질충란(肝蛭蟲卵) 간이검사법(簡易檢査法)에 관한 연구(硏究)

피내반응법(皮內反應法)에 의(依)해 간질(肝蛭) 양성반응(陽性反應)으로 판정(判定)된 유우(乳牛)를 대상(對像)으로 분편검사(糞便檢査)를 실시(實施)하여 충란(蟲卵)을 확인(確認)한 후(後), 현재(現在) 사용(使用)되고 있는 간질(肝蛭) 충란검사법(蟲卵儉査法)들의 단점(短點)을 개선(改善)하기 위하여 잔재물(殘滓物)을 성력적(省力的)으로 제거(除去)하는 사(篩)의 조합법(組合法)과 K-식(式) 침전관(沈澱管)을 고안(考案)하여, 인위적(人爲的)으로 분즙(糞汁)에 충란(蟲卵)을 혼입(混入)하여 K-식(式) 침전관법(沈澱管法)에 따라 검사(檢査)한 결과(結果) 현재(現在)까지 보고(報告)된 타방법(他方法)들에 비(比)하여 조작(操作)이 간편(簡便)하고 검출율(檢出率)도 높아 이상적(理想的)인 방법(方法)으로 사료(思料)되었기에 그 개요(槪要)와 결과(結果)를 보고(報告)하는 바이다. 1. 간질충란(肝蛭蟲卵)을 여과(慮過)하고 잔재물(殘滓物)을 제거(除去)하기 사(篩)의 조합(組合)을 3단계(段階)(100mesh, 150mesh, 250mesh)로 조합(組合)하여 성력적(省力的)인 방법(方法)으로 개선(改善)하였다. 2. 3종(種)의 침전관중(沈澱管中) 제(第)III형(型)의 침전관(沈澱管)에 의(依)한 간질충란(肝蛭蟲卵) 회수율(回收率)이 가장 높았으므로 제(第)III형(型)을 K-식(式) 침전관(沈澱管)으로 지칭(指稱) 채택(採擇)하였다. 3. K-식(式) 침전관법(沈澱管法)에서 A-II, A-III, B-III, C-III형(型)의 1구(回) 경검성적(鏡檢成績)은 99~100%였으며 A-I, B-II, C-I 및 C-II형(型)은 71~91%였다. 4. A형(型)($5m{\ell}$, 5-10침전(沈澱)), B형(型)($10m{\ell}$, 10-10침전(沈澱)) 및 C형(型)($15m{\ell}$, 15-15침전(沈澱))은 각기(各其) 특징(特徵)을 가진 방법(方法)들로서 판정시간(判定時間)을 단축(短縮)하는데는 A형(型)이 가장 유용(有用)한 방법(方法)이었다. 5. K-식(式) 침전관법(沈澱管法)은 다른 검출법(檢出法)에 비(比)하여 가장 검출율(檢出率)이 높은 간편(簡便)한 방법(方法)이었으며 E. P. G가(價)도 동시(同時)에 얻을 수 있는 이상적(理想的)인 방법(方法)으로서 임상응용가치(臨床應用價値)가 매우 높은 것으로 사료(思料)되었다. The results obtained are summerized as follows: 1. Detection method of Fasciola eggs was improved by 3 meshes(100mesh, 150mesh, 250mesh) for filtration of fecal fluid contained Fasciola eggs. 2. III type of tube in the 3 kinds of sedimental tube had the highest recovery rate of Fasciola egg. Thus, this type of tube was selected and called as Kim's sedimental tube. 3. A-II, A-III, B-III and C-III type by Kim's sedimental tube method had the recovery rates of eggs in the 1st dropping as ranged 99 to 100% and A-I, B-II, C-I and C-II as 71 to 91%. 4. All types as themselves contained the special aspects. In the these type was useful to determine the results in the urgent cases add to shorten the time of the diagnostic procedures. 5. Kim's sedimental tube method high recovery rate and convenient procedures as compared with other detection methods reported. In addition, through this method can also obatained E. P. C. value. Therefore, it is desirable that Kim's sedimental tube method be recommended by clinicians.

Complication of Intraoperative Radiation Therapy (IORT) in Gastric Cancer

영남대학교 의료원 치료방사선과에서 1988년 6월 15일에 위함 환자의 수술중 방사선치료를 시작한 이래 1992년 8월 30일까지 총 58예에서 시도하여 그중 53예에서 IORT를 실시하였으며, 정기적인 추적검사에서 한명의 국소개발 환자도 보고되지 않고 있다. 출혈(3예), 장관폐쇄(3예), 폐혈증(2예), 골수기능저차(1예)를 포함한 총 9예(17%)의 합병증이 보고되었고, 이중 6예(13%)가 사망하였다. IORT(1500cGy), 외부 방사선치료 (-4500cGy)와 강한 항암제를 병합치료 하였음에도 불구하고 주등(수술과 항암제 치료)의 25.2%, 김등(수술 불가능한 환자에서 항암제 투여)의 18%, 리등(수술)의 18.5%, Kraming등(IORT 2800-3500cGy)의 35.3%에 비해 낮은 합병증을 보여 IORT가 위암의 치료에 공헌할 수 있음을 시사하였다. 그러나 비교적 높은 치사율(11.3%)은 더욱 세심한 수술수기 및 수술 후 환자의 치료가 필요하며 외부 방사선치료와 항암제치료의 적절한 시기 조절 및 치료선량의 가감이 필요할 것으로 생각된다. Local control is the important prognostic factor in cancer treatment because local control decrease the relative risk of metastatic spread and increase distant metastasis free survival. IORT is the modality which could increase local control without increasing complication, combined with curative operation. Eventhough we could achieve significant deacreased local failure by IORT and curative resection, it should not be committed as a main treatment modality without proving acceptable complications. Therapeutic Radiology Department of Yeungnam University Medical Center have tried 58 IORT from June 15, 1988 and performed 53 IORT in patients with gastric cancer. No local failure had been reported including interstinal obstrution, hemorrhage, sepsis, and bone marrow depression. These complications could be comparable to Jo's 25.2% (chemotherapy + operation), Kim's 18% (chemotherapy only in inoperable patients), because our treatment regimen is consisted of IORT (1500cGy), external irradiation (--4500cGy) and extensive chemotherapy (FAM, 5FU + MMC, BACOP). Our data encouraged us to re-inforce further IORT in stomach cancer treatment.

구강내 악성흑색종에 대한 임상연구

The prognosis of oral malignant melanoma is poor compared with cutaneous melanoma. It may be related to the difficulty of wide enough resection, the early hematogenous matastases, higher stage at initial diagnosis, tendency to growth vertically. In the view of histological differences between oral mucosa and skin, it is impossible use Clark’s and Breslow’s classifications for prognosis. The great problem is that there is still no consensus on the treatment due to rarity. Because data collection from case reports is considered to be the best source of information and should be pooled to anlalyze key determinants of outcome, We reported on 6 cases of primary malignant melanoma of the oral cavity which were diagnosed and treated in Pusan National University Hospital recently and reviewed the literature. Immunohistochemical study on S100 Protein, GP 100 (HMB-45) with biopsy was usable to confirm the melanoma. Neck dissection including wide excision is recommended if lymph node involvement is suspected. Additionally, adjuvant chemotherapy could be supporting therapy.

淨水場 單位工程의 最適化 自動制御 시스템에 관한 硏究

본 연구는 상수도 공급의 합리적 방안 연구의 일환으로서 수질개선과 효율적인 공급관리 체계확립을 연구하였다. 먼저 상수도 체계도를 각각 단계별로 분류하였으며, 정수장의 수처리 공정에서 발생하는 근원적인 문제점과 실태를 조사ㆍ분석하여 개선책과 대안들을 제시하고자 하였다. 또한 상수도 관리공정의 자동화 및 계측제어 system 방식의 도입을 통하여 비효율적인 기존의 인위적 생산방식을 탈피하고, 제한된 용수자원을 최대한 호라용하여 능률적인 상수원 공급체계를 확립하고자 하였다. Water Supply in Water Treatment Plants has been accomplished at a great extent for last decade in Anyang City. Some problems to be solved were disclosed in the survey which dealt with operation and the installation procedure. Optimum Auto-Control System is a kind of preventive maintenance equipment which monitors continuously the operating condition of equipment and alarms to the operator in case of abnormal condition to prevent equipment from transition to big trouble. This system monitoring the efficiency of water treatment plants. By suggesting standard model of Optimizing Control System after completion, it will be possible to adapt this model to the renewaland/or new construction of other Water Supply System. Also it will contribute to the effective equipment operation by maintenancing cost cut-down and remote monitoring and control.

Liver-Fluke의 寄生이 山羊의 繁殖障害와 乳肉生産에 미치는 影響에 관한 硏究

These studies were carried out to investigate the effects of liver-fluke infection on the reproductive disorder and milk and meat production in Korea Native Goat. A survey of infected goat in Chungnam district was conducted with 474 head by interdermal reaction and also a clinical observation was founded. A experiment was carried out to determine the effects of liver fluke extract on the reproductive disorder by subcutaneous injection and milk and meat yield were measured from treated goats. The results obtained were summarized as follows. 1. The rate of positive goat was 71.3% among the surveyed goat and the highest rate was appeared at 3∼4 years old goats as 81.4%. 2. The treated goats lost condition and failed to thrive, and became progressive weaker. 3. In hemogram observation, the rate of eosinophil leucocyte was increased significantly by extract treated. 4. It was appeared the pathogenic signs such symptoms poisoning liver function disturbance and reproductive disorder from treated goats with liver fluke extract. 5. In milking goat the milk yield droped significantly and fattening goat did not fatten. 6. The gross income value per capita was lower from infected goat and it was analysed more sensitivly in net income value.

Sandblast 처리한 상아질면의 접착에 관한 연구

Dentin adhesion was studied on sandblasted dentin with 4-MET/MMA-TBB resin. The effect of change of air pressure, with or without pretreatment and primer on tensile bond strength, was mainly investigated. In the effect of air pressure at sandblast treatment, the higher pressure showed the increase of tensile bond strength. The pretreatment with 10-3 solution after sandblast treatment increased tensile bond strength in the almost conditions. Application of primer also increased the tensile bond strength, confirmed the effect of primer application after sandblast treatment. Alumina particle was not found on the dentin surface in the SEM and EDAX examination.

우리나라 政府出捐 硏究機關의 外國技術 導入 經路選擇의 決定要因에 관한 硏究

우리나라는 `90년대 산업의 질적 고도화와 지속적인 경제성장을 뒷받침하기 위하여 科學技術의 중요성이 요구되고 있다. 이를 실질적으로 뒷받침하기 위한 방법의 하나로서 政府出捐 硏究機關들이 외국으로부터 산업기술 및 공공기술에 필요한 기술도입계약과 함께 정부 특정연구과제로서 國際共同硏究와 아울러 國際技術協力 등을 중심으로 한 公共技術協力을 추진하고 있다. 민간제조기업의 기술도입경로가 海外直, 合作投資 및 技術導入契約 등인 데 비해 정부출연연구기관의 기술도입경로는 기술도입계약외에 公共技術協力 (이는 국제공동연구, 국제기술협력 등으로 대별됨)이라는 상이한 경로를 밟고 있다. 그러나 국제간 기술이전에 대한 연구의 대부분이 민간제조기업에 의해 수행되는 현상을 연구해온 데 대해 본 연구에서는 이미 중요한 기술도입주체로 부각되어 향후 우리나라를 기술대국으로서의 기반을 짊어져야 하는 주체로서의 정부출연연구기관을 연구대상으로 하여 기술도입경로선택에 영향을 미치는 결정요인을 규명해 보자는 데 그 의의가 있다. 이를 위해서는 연구기관이 갖는 特有性(uniqueness)을 smoothe시키고 普遍性(universality)을 전제로 하기 위해 새로이 구성하였다. 한편 기술도입경로도 민간기업과는 달리 기술도입계약, 공공기술협력(국제공동연구, 국제기술협력)등으로 재편하였다. 보다 보편적인 決定要因을 추출하기 위해 독립변수군을 기술특성군, 기술제공특성군, 기술도입자특성군, 정부특성군 등으로 4대분류하여, 기술도입경로선택의 결정요인을 檢證하였다. 결론적으로 정부출연연구기관의 外國技術導入經路의 選澤 및 說明變數로는 다수 (예: 기술수명 주기상의 위치, 기술연령, 기술도입담당부서, 국제협정지원 등) 가 선정될 수 있으나 그 중에서 技術의 垂直的 位階分類가 가장 중요한 결정요인임을 밝혔다. 즉 정부출연연구기관의 개별성 및 특유성에 관계없이 도입기술이 기술위계상 基礎基盤技術일 경우에는 國際共同硏究, 應用技術일 경우에는 國際技術協力, 常用化技術일 경우에는 技術導入契約이라는 經路를 선택하는 성향을 검증했다는 점이다. 이 연구결과는 정부출연연구기관의 경우 기술위계상 특성에 따라 외국으로부터 기술도입경로를 선택할 수 있는 기준을 제시할 수 있다는 데 그 의의가 있다고 본다.
