        척수로 전이된 후두엽 다형성 교아세포종 : 증례보고 A Case Report

        The tendency of glioblastoma to metastasize to the cerebrospinal fluid(CSF) is well documented, reaching approximately 20% in autopsy series. Nevertheless, clinical symptoms from spinal seeding seem to be infrequent or are at least rarely diagnosed. The authors report a case of 41-year old female patient with a cerebral glioblastoma multiform who developed paraplegia due to a pathologically confirmed spinal matastasis.

        두개강내로 전이된 Adenoid Cystic Carcinoma 2례 : 증례보고 Report of Two Cases

        The authors describe two cases of unusual intracranial metastatic adenoid cystic carcinomas. A 42-year-old woman had a right pariental epidural metastatic mass. presumed hematogenous. spread from a primary tumor in the parotid gland. In second case, a 32-year-old man had a intracranial metastatic adenoid cystic carcinoma mimicking calcified psammomatous meningioma on CT scans. We present these unusual cases with a review of relevant literature and a discussion of possible pathogenesis.

      • 스캔 설계 법칙 검사에 관한 연구

        Since testing of digital circuit is becoming increasingly important, many methodologies have been introduced to enhance design for testability (DFT). DFT was taken into account in the automated test procedure and improved reliability of the design. Both tasks are well suited for scan rule checker. This paper presents a scan rule checker for gate level digital circuits. The scan rule checker deals with basic rule check, basic scan rule check, muxed scan rule check, clocked scan rule check and lssd. Scan design informations were represented in the configuration file and libraries were modeled for the scan rule checker. It is an efficient way for the design for testability methodology.

      • Lutembacher 증후군 환자 증례 보고

        Lutembacher's syndrome is rare combination of mitral stenosis (MS) and atrial septal defect (ASD). The hemodynamic interplay between the MS and ASD leads to wide variation of clinical presentation. Here we describe a 43-year-old female with Lutembacher's syndrome and tricuspid regurgitation with pulmonary hypertension, who underwent direct closure of ASD and tricuspid valvuloplasty successfully. We also reviewed other literatures in an effort to increase awareness of this condition.

        신장이식은 환자의 삶의 질을 높이고 거의 정상적인 사회생활을 가능하게 함으로써 말기신부전 환자의 3가지 신대체 요법(혈액투석, 복막투석, 신장이식) 중에 가장 효과적이고 각광받는 치료법이라 할 수 있다. 최근 들어 의료기술과 의학의 발달로 신장이식의 성공률은 과거에 비해 비약적으로 높아지고 있으며 국내에서도 점차 신장이식수술을 받는 환자가 증가하고 있으나 서울이나 다른 대도시에 비해 의료환경이 열악한 중소도시나 지방의 병원에서는 아직도 시행하지 못하고 있는 형편이다. 이에 저자들은 경북 최초로 자체 의료진에 의해 고혈압과 말기 신부전으로 동국대 포항병원에서 혈액투석을 받고 있던 32세의 남자환자에게 부인으로부터 신장을 제공받아 신장이식을 성공적으로 시행하였기에 문헌고찰과 함께 보고하고자 한다. Kidney transplantation is the preferred treatment modality for many patients with end-stage renal disease (ESRD), because it offers improved quality of life over both hemodialysis and peritoneal dialysis. Patients who do well after transplantation generally report improvement in vitality and freedom to return to the style of life that they experienced before their progression to ESRD. Recently, the success rate of kidney transplantation is greatly being increased as the surgical technique and immunosuppressive drugs are developed. Kidney transplantation is also popular in Korea which has good result. But, until now the general hospitals which are located at small city can't try the kidney transplantation because they don't have proper facilities and experts. We firstly experienced a case of kidney transplantation in a 32 years old man with hypertension and chronic renal failure at KyoungBuk province. So we report this case with references.

        신기능과 혈색소의 관계

        목적: 빈혈은 만성신질환 환자들에서 좌심실비대 등 건강에 여러 가지 악영향을 미치는 중요한 합병증 중의 하나이다. 기저질환에 관계없이 신기능의 악화에 따른 빈혈의 발생빈도, 혈색소 농도와 적혈구용적률의 변화 및 성별에 따른 차이를 알아보기 위해 이 연구를 시행하였다. 방법: 전남대학교병원 내과에서 치료하고 있는 환자들 중 남성 157명, 여성 132명의 의무기록을 후향적으로 분석하였다. MDRD 등식에 따라 추정 사구체여과율을 측정하고, 사구체여과율(GFR)이 90 mL/min/1.73 m2 이상을 기준(정상)으로 하여, 추정 사구체여과율이 10 mL/min/1.73 m2씩 감소함에 따라 9개 군으로 분류하여 기준과 비교하여 평균 혈색소 농도와 적혈구용적률이 통계학적으로 유의하게 감소하는 신기능의 범위를 찾아서 남성과 여성을 각각 비교하였다. 결과: 모든 대상 환자들은 기준치(추정 사구체여과율≥90 mL/min/1.73 m2)에 비해 추정 사구체여과율 50≤GFR<60 mL/min/1.73 m2에서 평균 혈색소 농도는 0.8 g/dL(p=0.021), 적혈구용적률은 2.6%(p=0.011)가 감소하였고, 신기능이 감소함에 따라 평균 혈색소 농도와 적혈구용적률은 더욱 유의하게 감소하였다(Hgb.: r=0.635, Hct.: r=0.640, p<0.001). 남성은 기준치에 비해 추정 사구체여과율 40≤GFR<50 mL/min/1.73 m2에서 평균 혈색소 농도는 1.7 g/dL(p<0.01), 적혈구용적률은 4.4%(p<0.01)가 감소하였고, 신기능이 감소함에 따라 평균 혈색소 농도와 적혈구용적률은 더욱 유의하게 감소하였다(Hgb.: r=0.69 8, Hct.: r=0.689, p<0.001). 여성은 기준치에 비해 추정 사구체여과율 60≤GFR<70 mL/min/1.73 m2에서 평균 혈색소 농도는 0.9 g/dL(p<0.01), 적혈구용적률은 2.8%(p<0.01)가 감소하였고, 신기능이 감소함에 따라 평균 혈색소 농도와 적혈구용적률은 더욱 유의하게 감소하였다(Hgb.: r=0.672, Hct.: r=0.687, p<0.001). 결론: 만성신질환 환자들에서 혈색소 농도와 적혈구용적률은 신기능이 감소함에 따라 점차 감소하였고 이것은 경증 내지 중등도 신기능 감소부터 관찰되었다. Background: The goal of this study is to define the relationship between the decreased renal function and anemia, and also to determine whether this relationship is different in male and female patients. Methods: We conducted a retrospective study of 289 patients (male:female=157:132) who were followed at the department of internal medicine at Chonnam National University Hospital. General linear models were used to analyze the relationship between the hemoglobin concentration and Modification of Diet in the Renal Disease formula estimated Glomerular Filtration Rate (mL/min/1.73 m2). Results: Among all patients, the mean hemoglobin concentration and hematocrit of the men with a Glomerular Filtration Rate of 50~59 mL/min/1.73 m2 was an absolute change of 0.8 g/dL (p=0.021) and it was 2.6% (p=0.011) lower than those of the patients with a Glomerular Filtration Rate≥90 mL/min/1.73 m2 and continued to decrease further as the Glomerular Filtration Rate decreased, respectively (Hgb.: r=0.635, Hct.: r=0.640, all p<0.001). Among the male patients, the mean hemoglobin concentration and the hematocrit of men with a Glomerular Filtration Rate of 40~49 mL/min/1.73 m2 was an absolute change of 1.7 g/dL (p<0.01) and it was 4.4% (p<0.01) lower than those of the male patients with a Glomerular Filtration Rate≥90 mL/min/1.73 m2 and continued decrease as the Glomerular Filtration Rate decreased, respectively (Hgb.: r=0.698, Hct.: r=0.689, all p<0.001). Among the female patients, the mean hemoglobin concentration with a Glomerular Filtration Rate of 60~69 mL/min/1.73 m2 was an absolute change of 0.9 g/dL (p<0.01) and the mean hematocrit was 2.8% (p<0.01) lower than those of the female patients with a Glomerular Filtration Rate ≥90 mL/min/1.73 m2 and continued to decrease further as the Glomerular Filtration Rate decreased, respectively (Hgb.: r=0.672, Hct.: r=0.687, all p<0.001). Conclusions: A decrease in the hemoglobin concentration was statistically significant in the patients of both genders, along with a moderately decreased Glomerular Filtration Rate (≤60 mL/min/1.73 m2).         Osthole enhances TRAIL-mediated apoptosis through downregulation of c-FLIP expression in renal carcinoma Caki cells

        <P>Osthole, an active constituent isolated from the fruit of Cnidium monnieri (L.) Cusson, has been shown to induce various beneficial biochemical effects such as anti-inflammatory and antitumor. In the present study, we examined whether osthole could sensitize TNF-related apoptosis-inducing ligand (TRAIL)-induced apoptosis in human renal carcinoma Caki cells. We found that osthole and TRAIL alone, had no effect on apoptosis, but combined treatment with osthole and TRAIL markedly induced apoptosis in Caki (renal carcinoma), U251MG (glioma) and MDA-MB-231 (breast carcinoma) cells. In contrast, combined treatment with osthole and TRAIL did not induce apoptosis in normal human skin fibroblast cells. Osthole induced downregulation of cellular FLICE-like inhibitory protein (c-FLIP) expression, and overexpression of c-FLIP markedly blocked apoptosis induced by the combined treatment with osthole and TRAIL. In addition, osthole markedly reduced mitochondrial membrane potential levels, and increased cytosolic cytochrome c release in combined treatment with osthole and TRAIL. Therefore, these data suggest that osthole may be an efficient TRAIL sensitizer.</P>

      • Extended Model of OGC Simple Feature Geometry For Spatial Analysis

        Recently, component-based software has become main trends in designing and developing computer software products. This component-based software. has advantage of the interoperability on distributed computing environment and the reusability of pre-developed components. Also, GIS is designed and implemented with this component-based methodology, called Open GIS Component. OGC(OpenGIS Consortium) have announced various implementation and design specification and topic in GIS. The simple feature geometry specification is announced by QGC to increase the full interoperability of various spatial data. This specification includes just geometry spatial data model. However, in GIS which manages spatial data, not only geometric data but also topological data and various analysis functions have been used. The performance of GIS depends on how this geometric and topological data is managed well and how various spatial analysis are executed efficiently. So it requires integrated spatial data model between geometry and topology and extended data model of topology for spatial analysis, in case network analysis and TIN analysis in open GIS component. In this paper, to manage topological information for spatial analysis in open GIS component, we design extended data model of simple feature geometry for spatial analysis. In addition to, we design the overall system architecture of open GIS component contained this topology model far spatial analysis.
