        제2형 당뇨병 환자에서 Voglibose와 Acarbose의 비교임상연구

        연구배경:아카보스와 보글리보스는 ­glucosidase inhibitors로써 비록 약리학적 작용이나 부작용에 있어서 두 약물간에 차이가 있다는 것은 잘 알려져 있으나 당뇨병 환자를 대상으로 아직 이에 대한 두 약물간에 직접적인 비교에 대해 연구된 바는 없었다. 이에 저자등은 국내 2형 당뇨병 환자에 대해 유효성과 부작용 발현에 대해 두 약제를 비교하고자 무작위법에 의한 위약 대조군의 이중 맹검법 연구를 시행하였다. 방법:시험 약제 투여 4주간의 관찰기를 설정하여 공복혈당의 변화가 30㎎/dL 이하이고, 식후 혈당이 200㎎/dL 이상인 환자로 기타 제외 기준에 해당하지 않은 환자 53명을 대상으로 하여 보글리보스 군(24명)과 아카보스 군(29명)으로 무작위로 나누었다. 치료기간은 총 8주로 하였으며, 4주간 간격으로 혈청학적 검사와 부작용을 분석하여 치료 효과가 부작용을 평가하였다. 결과:1)혈당 변화:보글리보스군은 식후 1시간 혈당이 치료 후 4주, 8주째 의미 있게 감소하였도, 아카보스군은 식후 1시간과 2시간 혈당이 치료 후 4주, 8주째 의미있게 감소하였다. 또한 관찰기 혈당에 대한 치료 4주째 감소량은 아카보스군에서 더 큰 경향을 보였으나, 치료 8주째에는 두 군 간의 강하정도에 의미있는 차이가 없었다(p=0.569). 2)인슐린 치의 변화:보글리스 군은 식후 1시간 인슐린 치가 치료 전에 비해 치료 4주, 8주째 감소하는 경향을 보였고, 공복 인슐린이나 식후 2시간 인슐린치는 치료전 후에 의미 있는 차이가 없었다. 아카보스군에서는 치료 전후로 공복 인슐린, 식후 1시간과 2시간 인슐린치에 의미 있는 차이를 보이지 않았다. 두약제 간에 치료 전과 치료 8주사이의 식후 2시간 인슐린의 감소량이 보글리보스 군에서 의미있게 높았다(p=0.040). 3)당화혈색소:보글리보스 군은 치료 전에 비해 치료후 당화혈색소가 감소하는 경향을 보였고, 아카보스군은 치료 전에 비해 의미있게 감소하였다. 당화혈색소 변화량은 두 군간에 의미 있는 차이는 없었다(p=0.412). 4)지질대사의 변화:중성지방, 콜레스테롤, 고밀도 진단백 콜레스테롤에 대해 두 군간에 의미 있는 차이는 없었다. 5)부작용:소화기계 부작용의 빈도는 치료 4주째 보글리보스 군에서 의미 있게 낮았으나(p=0.028), 치료 8주째 부작용의 빈도는 두 군간에 의미 있는 차이가 없었다(p=0.215). 결론:2형 당뇨병 환자에서 보글리보스와 아카보스의 두 약제의 임상적 유효성과 부작용발현에 대해 비교한 결과 치료 후 8 주 후 혈당강화효과는 두 약제간에 유사한 효과를 보였으나 보글리보스군에서 4주째의 초기 위장관 부작용이 적었다. Background : Acarbose and voglibose are alpha-glucosidase inhibitors. Although different pharmacological effects and adverse abdominal events associated with the two drugs have been reported, no study directly compared acarbose and voglibose in diabetes has been undertaken. To compare the pharmacological effects and gastrointestinal adverse events between two drugs, a randomized, placebo-controlled, double-bind study was performed in type 2 diabetes patients. Methods : The period of study was 12 weeks(observation period: 4 weeks; treatment period: 8 weeks). Fifty-three patients were randomized into two groups(the acarbose group: 24 patients; the voglibose group: 29 patients). The serum glucose, insulin, fructosamine, HbA_1c, cholesterol, triglyceride and the incidence of adverse events were measured. Results : 1) The reduction of glucose from before treatment to 4 weeks after treatment was significantly higher in the acarbose group, but the change before treatment and 8 weeks after treatment in the two groups was similar(p=0.569). 2) The insulin significantly decreased after voglibose treatment(p=0.040). 3) HbAa_1c level tended to decrease in voglibose group, and there was a significant decrease after acarbose treatment. However, the change in HbA_1c level before and after treatment was similar between the two groups(p=0.412). 4) The two drugs did not cause any other changes in the total, HDL-cholesterol and triglyceride. 5) The number of patients with gastrointestinal adverse events was significantly low 4 weeks after voglibose treatment (p=0.049), but the incidence in the two groups was similar after 8 weeks(p=0.215). Conclusions : Acarbose and voglibose significantly improved postprandial hyperglycemia in diabetes. The incidence of gastrointestinal adverse events was low 4 weeks after voglibose treatment(J Kor Diabetes 26:134~145, 2002).

      • 차지법-드라이 클리닝에 의한 견직물의 세척성

        The effects of charge system on the removal of water soluble and oily soils from silk fabrics were studied. The charge systems used in this experiments were Light mate DX in petroleum solvent and Light Park MK in perchloroethylene. The detergency was examind with water/detergent ratio and concentration of dry soaps and solvent, respectively. 1. In the case of condition charged with detergents without water, as the concentration of Light Mate DX in petroleum solvent was increased, the detergency of water soluble soils was improved. 2. In the case of condition charged with detergents with water, as water/detergent ratio was increased, the detergency of water soluble soils was improved. Petroleum solvent was superior to perchloroethylene in the detergency as water/detergent ratio was low. 3. In the case of condition charged with detergents without water, the detergency of oily soils of Light Park MK in perchloroethylene was higher than that Light Mate DX in petroleum solvent. The detergency was influenced by characteristics of solvents. The order of the detergency in dry soap concentration was 1.5>1.0>2.0>0.5(%) 4. In the case of condition charged with detergents with water, as water/detergent ratio was increased, the detergency of oily soils in petroleum solvent was decreased. In perchloroethylene, the detergency was not so much affected by water/detergent ratio.

      • 복합레진의 조건등색에 관한 연구 : D_(65), A, C, FCW and TL84

        There were problems in selecting exact colors of composite resins currently used in clinic by examining with naked eyes. This study was done to present a criterion in selection of the most proper light sources and materials by measuring metamerism index of the light curing composite resins with spectrocolorimeter. Metamerism is present when two objects appear to be the same color in one illuminant but different in another. This is due to the fact that they have different spectral curves that fail to match under the second illuminant. In this study, five light polymerized composite resins; Esthet-X, Filteck Z250, Filteck A110, Charisma, Vitalescence which are currently used in clinic were chosen based on Vita shade. Five samples were made for shade of each product with Teflon mold(diameter: 15mm, thickness: 2mm). Compressed resins with glass slab were light polymerized with a visible light curing unit(Optilux 500, 13mm tip diameter, Demetron, USA) for 60 seconds. Polymerized samples were removed and those were grinded to 2.0mm in thickness with sand papers(#600 & #1000). Samples were carefully prepared without fingerprints and impurities in grinding procedure and kept in distilled water for a week at room temperature. Metamerism index of each samples on a Barium sulfate plate(L^(*)=96.54, a^(*)=0.19, b^(*)=0.01) prepared for sample fixation were measured with spectrocolorimeter(Miniscan XE plus. Model 4000s. Hunter Lab, USA) by applying standard light source D65, C, Fcw, TL84' and A. Standardization was done with Reference standard(X=80.8, Y=85.7, Z=90.8) and Light trap. Samples were kept in distilled water at room temperature in all procedures. The results are as follows. 1. Different resins using same Vita shade showed recognizable color difference(ΔE^(*)>2). 2. All composites had Ml below accepted value 0.5 between standard illuminant(D65, C, & A) and below 1.5 under fluorescent condition (Fcw & TL84). 3. Ml value between D65 and A showed higher value than Ml value between other source of light(p<0.01). 4. All resins except Z25O showed Ml value that A3 is higher than Al between D65 and A(p<0.05).

        백서에서 Depulpin®과 Formocresol에 대한 치수와 치근단 조직의 반응

        One fifth dilution of formocresol is usually for pulpotomy of the primary teeth and emergency pulpotomy of the permanent teeth. However, the use of formaldehyde has been subjected to criticism because it may be absorbed into the blood stream and become distributed systemically, it may also alter the pulp tissue rendering it immumologically active, and have carcinogenic potential. Recently Depulpin®(VoCo., Germany) gains popularity as a devitalizing agent during root canal therapy in spite of high concentration of 49% paraformaldehyde because it facilitate devitalization of pulp and make root canal therapy easier. But there have been not enough publications about the reaction of pulp and periapical tissue caused by Depulpin. This study was performed to evaluate the histological changes in pulp and periapical tissue of rats after pulpotomy using formocresol and Depulpin and to elucidate the toxic effects of these agents. Thirty six Sprague-Dawley rats were anesthetized by intraperitoneal injection of ketamine. Maxillary first molar teeth were used for pulpotomy with formocresol and Depulpin. Rats were sacrificed after 2 days, 4 days, 1 week, 2 weeks, 3 weeks and 4 weeks respectively. Specimens were histologically observed by light microscope changes in pulp and periapical tissue. The obtained results were as follows. 1. Formocresol group A zone of fixed tissue, in which odontoblasts could clearly be defined, was present directly underneath the pulpotomy dressing in almost all teeth of this group. This was followed by an area of necrotic tissue which resembled dried out fibrous tissue with no cellular detail except some pyknotic nuclei. In the specimens of after 2 days, 4 days, 1 week, 2 weeks in which vital tissue was present. it was separated from the fibrous area by a zone of inflammation. In the specimens of after 3 weeks and after 4 weeks, inflammatory infiltrate was in the periodontal ligament adjacent to the apical foramina of the teeth. 2. Depulpin® group The area of necrotic tissue which had no cells and fibers , was present adjacent to the dressing. This was followed by dried out fibrous tissue with no cellular details except some pyknotic nucleli. A short stump of vital pulp with odontoblasts was present at the end of the canal after 2 days. Inflammatory infiltrate was in the periodontal ligament after 4 days and after 1week. Severe root resorption and necrosis of periapical tissue opposite the root resorption site were defined after 2 weeks and after 3 weeks. Periapical lesion which consist of necrotic tissue surrounded by a fibrous connective wall. was found after 4 weeks. The results indicated that Depulpin can cause more adverse reaction to the dental pulp and periapical tissue than formocresol, and further studies are needed for its clinical use with safety.

      • 부산시 고등학생들의 환경에 대한 인식정도와 영향요인 분석

        The purpose of this study was to identify the degree of recognition of the high school students about environmental and influential factors on it, selecting two regular schools and three vocational schools among the high schools in Busan area, questionnairing one class in the 10th, 11th, 12th grades of each school and analyzing the result. The result of analyzing the degree of knowledge, practice and recognition of seriousness concerning environment according to grade and gender is that the significant difference has been found in the degree of practice. The result of analyzing them according to maternal academic background is that the significant difference has been found in the degree of knowledge and a living standard and the style of housing have no effect on the environmental education, In the survey according to family style, it is shown that the students 'who live with their parents' recognize the seriousness of environment more. In the survey according to the experience of having the subject 'environment' in their school curriculum or not, it is shown that the significant difference has been found in the degree of knowledge. As the source of information on environment, mass media such as TV, magazines and newspapers have a great impact on the students. As the significant difference has been found in the degree of knowledge, practice, the recognition of seriousness from the viewpoint of the analysis on the survey according to the degree of the recognition on the necessity of the environmental preservation, it is shown that the more the students feel the necessity of environmental preservation, the higher the degree of knowledge, practice and recognition of seriousness on environment becomes, In the survey according to the kind of school, the significant difference has been found in the degree of knowledge, In the survey according to the degree of maternal concern on environment it is shown that the more concerns the mothers have, the more the students put what they learn into practice, In the survey according to the kinds of school, the significant difference has been found in the degree of knowledge. As the result of multiple regression analysis is summarized as below. 1. The variables influencing the environmental knowledge are the experience of having the class, 'environment', the recognition of the necessity of the environmental preservation and maternal academic background. 2. The elements influencing the degree of recognition of seriousness about environmental contamination are gender, the experience of having a class, 'environment' or not and the necessity of the environmental education. 3. The elements influencing the degree of practice are grade, information on environment, and maternal concern.

        기아시 버들치 Rhynchocypris oxycephalus(Sauvage and Dabry)의 일부 영양조건에서의 효과 : 2. 체 절단면 계측형질의 변화 2. Characteristics of the Morphometric Changes in the Sectioned Body

        버들치 Rhynchocypris oxycephalus(Sauvage and Dabry) 기아시의 영양 상태 평가를 위하여 비만도, 내장괴지수, dreessing 비를 비롯한 체 횡단 절단면에서의 형태적 변화를 조사였다. 9주간의 기아시 비만도, 내장괴지수 및 대부분의 체 횡단 절단면 계측 형질들에서의 감소를 보였다(P<0.05). 본 연구 결과, 실험에 적용된 영양성 parameter는 본 종의 영양 상태 파악에 유용한 지표임을 시사한다. 연구 결과를 자세히 해석하여, 버들치에서의 주요 체 절단 형질의 산업성을 논의하였다. The effects of starvation on the morphometrical changes in sectioned body traits, condition factor, viscera index and dressing percentage were determined for evaluating nutritional conditions of Rhynchocypris oxycephalus(Sauvage and Dabry). Starvation for nine weeks resulted in a decrease in most sectioned traits as well as in condition factor and viscera index(P<0.05). These findings suggest that nutritional parameters used in this study appear to be a useful index for nutritional status in this species. The data has been interpreted in detail and some important body sectioned values of interest to commercial growers discussed.

        수리된 복합레진 수복물의 전달결합강도 연구

        This study was performed to evaluate the interfacial shear bond strength of base (direct and indirect) and repair composites with aging and surface treatment methods. Direct composites resin specimens (Charisma®, Heraeus Kulzer, Germany) were aged for 5 min, 1 hour, 24 hours, and 1 week in 37℃ distilled water before surface treatment, and then divided into five groups: Group 1, grinding; Group 2, grinding and application of bonding agent; Group 3, grinding, etching with 37% phosphoric acid for 30sec, and application of bonding agent; Group 4, grinding, etching with 37% phosphoric acid for 30sec, silane treatment, and application of bonding agent; Group 5, grinding, etching with 4% hydrofluoric acid for 30sec, silane treatment, and application of bonding agent. Indirect composite resin specimens (Artglass®, Heraeus Kulzer, Germany) were aged for 1 week in 37℃ distilled water and divided into seven groups: Group 1 - Group 5, equal to Charisma specimens; Group 6, grinding, etching 37% phosphoric acid for 60sec, silane treatment, and application of bonding agent; Group 7, grinding, etching with 4% hydrofluoric acid for 60 sec, silane treatment, and application of bonding agent. The repair material(Charisma®) was then added on the center of the surface (5mm in diameter, 5mm in height). The shear bond strength was tested and the data was analyzed using one-way ANOVA and the Student-Newman-Keuls test. The following conclusions were drawn. 1. The shear bond strength of Charisma® specimens aged for 1 hour was significantly higher in Group 2 and Group 5 than in Group 1 (p<0.05), and that of Charisma® specimens aged for 1 week was significantly higher in Group 3 and Group 5 than in Group 1 (p<0.05). No significant difference was found in the bond strength of specimens aged for 5 min and 24 hours. 2. In Group 2 of the Charisma® specimens, there was significant difference between the bond strength of 24 hours and that of 1 week (p<0.05). 3. In Group 4 of the Charisma® specimens, the shear bond strength of specimens aged for 24 hours was significantly higher than the others(p<0.05). 4. There was no significant diiference between the shear bond strength of the Artglass® specimens. 5. Most of the Charisma® specimens showed cohesive fractures. Artglass® specimens that were etched with acid(phosphoric or hydrofluoric) for 30 sec showed more cohesive fractures.

        저 Pb Sn-5%Pb-1.5%Ag-x%In계 솔도 합금의 특성에 관한 연구

        Pb의 환경오염 문제를 발생하지 않는 저농도 Pb 솔도합금을 개발하기 위하여, 새로운 Sn-5%Pb-1.5%Ag-x%In계 합금 조성을 설계하고, 이 합금의 융점, 젖음성, 상분석, 경도, 인장강도, 드로스성을 평가하여, Sn-37%Pb 솔더오 대체 가능성을 타진하였다. Sn-37%Pb 솔도 합금의 Pbdldhs 용출농도는 국제규제치인 3ppm보다 훨씬 적은 0.46ppm이었고, 환경문제를 유발하지 않는 것으로 확인되었다. 이 합금계의 융점은 $183-192^{\circ}C$이고, 응고온도범위도 $5^{\circ}C$내외로 매우 좁았다. 젖음성은 In의 첨가양에 따라 큰 차이가 거의 없었으며, Sn-375Pb와 비슷하였다. 융점 및 젖음성 측면에서 Sn-37%Pb와 대체 가능한 것으로 판단되었다. 경도는 Sn-37%Pb의 약 1.5배이고, 인장강도는 Sn-37%Pb의 것보다 높고, In의 첨가량에 따라 증가하였지만, 연신율은 감소하였다. In이 1% 첨가된 합금에서는 수지 상정 경계에 Ag3Sn과 Pb가 정출되고, 3% 이상에서는 $Ag_3Sn$과 $Ag_3In$ 및 Pb가 정출되었다. 드로스 생성속도는 Sn-37%Pb 합금이 Sn-5%Pb-1.5%Ag 합금보다 빠르고, In을 첨가할수록 느리고 2%의 In을 첨가한 합금은 180분에서도 거의 드로스가 발생하지 않았다. This work designed Sn-5%Pb-1.5%Ag-x%In solder alloy to develop the solder alloy with low Pb content. This solder alloy doesn't cause environmental pollution. and this study reviewed the probability of replacement of Sn-37%Pb solder as evaluation of melting range, wettability. microstructure, microhardne'ss, tensile strength, drossability of this new solder alloys. The level of international regulation in dissolution amount of Pb ion was 3ppm. But dissolution amount of Pb ion in Sn-5%Pb solder alloy confirmed not to threat the global environmental is 0.46ppm. The melting range of this solder alloy was $183-192^{\circ}C$. Also the range of solidification was very narrow within $5^{\circ}C$. The wettability was similar to Sn-37%Pb solder, and the effect of amount of In addition of wettability couldn't be founded. The probability of replacement in the melting range and wettability is very high. And microhardness of this solder alloy was 1.5 times of conventional type solder. Tensile strength of new solder alloys was a little high than that of conventional type solder. With increasing amount of In% addition, tensile strength was increased, but elongation was decreased. The solder alloy of l%In addition revealed AgSn and Pb on dendrite microstructure boundary, and $Ag_3Sn$, $Ag_3In$ and Pb were revealed on it at the solder alloy of 3% In addition. The drossability was superior to Sn-37%Pb solder alloy and the solder alloys of 2% In addition was not generated for 3hrs.

        The distributions of neuropeptide Y(NPY) in the diencephalon of the adult rabbit was investigated using indirect immunohistochemistry. The method showed that neuropeptide Y had a widespread distributions and was present in extremely high concentration in the hypothalamic nuclei of the rabbit. A few number of NPY-containing perikarya was found in the arcuate nucleus, paraventricular nucleus. medial to the fornix. Neuropeptide Y-immunoreactive axons and terminals in the hypothalamus were found in most hypothalamic nuclei. The highest concentrations of neuropeptide Y-containing fibers were found in the arcuate nucleus. paraventricular nucleus. and alao the supraoptic nucleus. dorsomedial hypothalamic nucleus. preoptic area showed relatively high concentration of neuropeptide Y-immunoreactive fibers. The extremely high concentations of neuropeptide Y in the bypothalamus indicate important role in many brain functions

        적외선열화상장치를 이용한 Buchanan plugger 표면의 온도상승 분석

        This study was performed to evaluate the temperature rise on various position of the Buchanan plugger, the peak temperature of plugger's type and the temperature change by its touching time of heat control spring. The heat carrier system 'System B' (Model 1005. Analytic Technologies, USA) and the Buchanan's pluggers of F, FM, M and ML sizes are used for this study. The temperature was set to 200℃ which Dr. Buchanan's "continuous wave of condensation" technique recommended on digital display and the power level on it was set to 10. In order to apply heat on the Buchanan's pluggers, the heat control spring was touched for 1, 2, 3, 4 and 5 seconds respectively. The temperature rise on the surface of the pluggers were measured at 0.5 mm intervals from tip to 20 mm length of shank using the infraed thermography (Radiation Thermometer-IR temper, NEC San-ei Instruments, Ltd, Japan) and TH31-702 Data capture software program (NEC San-ei Instruments, Ltd, Japan). Data were analyzed using a one way ANOVA followed by Duncan's multiple range test and linear regression test. The results as follows. 1. The position at which temperature peaked was approximately at 0.5 mm to 1.5 mm far from the tip of Buchanan's pluggers (p<0.001). The temperature was constantly decreased toward the shank from the tip of it(p<0.001). 2. When the pluggers were heated over 5 seconds, the peak temperature by time of measurement revealed from 253.3±10.5℃ to 192.1±3.3℃ in a touch for 1 sec, from 218.6±5.0℃ to 179.5±4.2℃ in a touch for 2 sec, from 197.5±3.0℃ to 167.6±3.7℃ in a touch for 3 sec, from 183.7±2.5℃ to 159.8±3.6℃ in a touch for 4 sec and from 164.9±2.0℃ to 158.4±1.8℃ in a touch for 5 sec. A touch for 1 sec showed the highest peak temperature, followed by, in descending order, 2 sec, 3 sec, 4 sec. A touch for 5 sec showed the lowest peak temperature (p<0.001). 3. A each type of pluggers showed different peak temperatures. The peak temperature was the highest in F type and followed by, in descending order, M type, ML type. FM type revealed the lowest peak temerature (p<0.001). The results of this study indicated that pluggers are designed to concentrate heat at around its tip, its actual temperature dose not correlate well with the temperature which Buchanan's "continuous wave of condensation" technique recommended, and finally a quick touch of heat control spring for 1 sec reveals the highest temperature rise.
