        Learning Curve of Capsule Endoscopy

        Background/Aims: Capsule endoscopy (CE) has become an important tool for the diagnosis of small bowel disease. Although CE does not require the skill of endoscope insertion, the images should be interpreted by a person with experience in assessing images of the gastrointestinal mucosa. This investigation aimed to document the number of cases needed by trainees to gain the necessary experience for CE competency. Methods: Fifteen cases were distributed to 12 trainees with no previous experience of CE during their gastroenterology training as clinical fellows. Twelve trainees and an expert were asked to read CE images from one patient each week for 15 weeks. The diagnosis was reported using five categories (no abnormalities detected, small bowel erosion or ulcer, small bowel tumor, Crohn disease, and active small bowel bleeding with no identifiable source). We then examined, using the κ coefficient, how the degree of mean agreements between the trainees and the expert changed as the training progressed each week. Results: The agreement rate of CE diagnosis increased as the frequencies of interpretation increased. Most of the mean κ coefficients were >0.60 and >0.80 after week 9 and 11, respectively. Conclusions: Experience with approximately 10 cases of CE is appropriate for trainees to attain CE competency.

        Multi-word Clusters, Keywords and Key Terms in English Car Reviews: a Preliminary Corpus Linguistic Study

        Classroom use of authentic texts used in everyday communication has not always been an inseparable part of foreign language teaching. According to Grigaliuniene (2013, p. 17), “the structures many current textbooks teach for certain functions are either never used or used infrequently while quite unexpected structures are the ones that actually occur”. Despite the awareness of the importance of authentic patterns of language use, students often learn structures (lexical or grammatical) which are rarely used in actual communication. Teaching authentic phrases is even more important in English for Specific Purposes (ESP) where learners’ specific needs impact the design of a course (Dudley-Evans & St John, 1998). What is more, ESP students need to acquire vocabulary of a target profession. Thus, in this preliminary study, conducted using corpus linguistic methodology, an attempt is made to identify and explore the use and discourse functions of frequent and typical patterns of language use, i.e. keywords, recurrent clusters and multi-word terms (nominalizations), found in a custom-designed corpus of car reviews written in English. As shown by examples of student tasks, the results have pedagogical potential in that they can be used as a useful resource when designing or supplementing teaching materials for secondary-school ESP students (automotive technicians and car mechanics).1)

      • 폐기물 가스화에 의한 합성가스 생산특성 및 수소생산량 예측에 관한연구

        국내 폐기물 소각장은 대부분 지역 분산형으로 구축되어 있으며, 일부 지자체의 경우에는 광역 처리되고 있는 소각시설은 있으나 NIMBY로 인해 광역 처리의 한계성은 있다. 기존 폐기물 소각시설은 사용연한이 완료되었거나 도래되고 있는 실정이며, 사회적 이슈로 인해 고형연료 열병합발전 시설들은 건설이 취소되는 사례가 않아져 국내 폐기물의 처리시설에 대한 인프라 확보가 부족한 실정 이 다. 수도권 매립지는 2026년부터 전국적으로는 2030년부터 가연성폐기물의 직매립 금지 정책이 계획되어 있다. 미세먼지저감 동 대기오염물질 배출부하 관리와 신기후체재이후 탄소중립 정책으로 온실가스감축이 가능하고 NJMBY현상을 궁정적으로 전환할 수 있는 보다 친환경적인 폐기물 처리기술의 도입이 필요하다. 또한 수소에너지 정책 에 따라 그린수소 또는 불루수소의 공급원 확보가 필요하며, 생산수소의 운송비용이 높음에 따른 지역 분산형으로 수소 공급원의 확보가 필요하다. 폐기물 처리의 특성인 지역분산 처리와 수소공급원의 이슈인 지역분산 수소공급은 이슈를 동시에 만족시킬 수 있는 공통요소이다. 이러한 사회적 정책적 이슈 해소에 기여할 수 있는 가스화 기술과 같은 폐기물 열화학적 재활용 기 술이 보급이 필요한 시점이다. 본 연구에서는 국내 에 서 발생하는 다양한 폐 기 물을 가스화 기술을 적용하여 열화학적으로 재활용이 가능한 합성가스의 생산특성을 고찰하였고, 합성가스를 이용하여 수소충전소 또는 연료발전에 사용이 가능한 고순도 수소 생산량을 예측하였다. 폐기물 가스화를 통해 생산된 합성가스는 메탄올, 암모니아 둥과 같은 화학원료로 전환하여 재활용할 수 있으며, 고순도 수소를 생산하여 수소 공급원으로서 폐기물이 기여할 수 있다.

      • 식품가공공장 에너지관리시스템 개발에 관한 연구

        국내 에너지소비량 중 산업부문이 137,444천toe(2019) 61 .9%로 대부분을 차지하고 있으며 제조업 이 127,431천 toe(2019)로 산업부문의 92 . 7%가 소비하고 있다. 제조업종 중 음식류 제조업이 1,929 천toe(2019)로 전체 제조업종의 1. 5%를 소비하고 있다. 제조업의 설비별 에너 지 사용은 보일러용 에너지 소비량이 29.5%와 열사용/열이송 설비가 19 . 4%로 48.9%를 사용하고 있다. 제조업의 열 에너 지 사용량 절감이 보일러용 및 열사용에 대한 에너지 관리물 통해 에너지 철감 및 온실가스 감축이 필요하다. 에너지 다소비 업종 중 매우 다양한 제품을 생산하는 식품가공업종의 공통적인 에너지 사용패턴 분석과 관리물 통해 공통적 인 에너지 절감 요소를 도출하여 업종 맞춤형 에너지 관리시스템의 개발이 필요하다. 이러한 식품가공업종의 열에너지의 소비패턴을 분석하여 에너지 절감을 위해 식품공장 맞춤형 에너지 소비 분석, 예측 및 절감 기술개발이 필요하다. 본 연구에서는 식품공장의 열에너지 생산/공급 공정인 보일러와 열에너지 사용공정인 식품가공공정의 에너지 소비구조 및 소비패턴 분석, 에너지 소비량 예측, 생산량 연계한 필수 인프라 선정. 에 너지 소비 베이스라인 산정방법 개발 및 데이터분석을 통한 에너지 소비 공정제어 기술을 포함한 식품공장 맞춤형 에너지관리시스템 개발을 수행하고 있으며 이에 대한 연구내 용을 정리하였다. 식품공장 맞춤형 에너지관리시스템은 유사한 식품공장 또는 유사한 에너지 소비패턴의 제조업에 확대보급이 가능한 기술을 개발·보급하여 제조업의 에너지 소비량 절감과 이에 따른 온실가스몰 감축하여 탄소중립 목표 달성에 기여하고자 한다.

        Guidelines for Bowel Preparation before Video Capsule Endoscopy

        The preparation for video capsule endoscopy (VCE) of the bowel suggested by manufacturers of capsule endoscopy systems consists only of a clear liquid diet and an 8-hour fast. While there is evidence for a benefit from bowel preparation for VCE, so far there is no domestic consensus on the preparation regimen in Korea. Therefore, we performed this study to recommend guidelines for bowel preparation before VCE. The guidelines on VCE were developed by the Korean Gut Image Study Group, part of the Korean Society of Gastrointestinal Endoscopy. Four key questions were selected. According to our guidelines, bowel preparation with polyethylene glycol (PEG) solution enhances small bowel visualization quality (SBVQ) and diagnostic yield (DY), but it has no effect on cecal completion rate (CR). Bowel preparation with 2 L of PEG solution is similar to that with 4 L of PEG in terms of the SBVQ, DY, and CR of VCE. Bowel preparation with fasting or PEG solution combined with simethicone enhances the SBVQ, but it does not affect the CR of VCE. Bowel preparation with prokinetics does not enhance the SBVQ, DY, or CR of VCE.

        Guidelines for Video Capsule Endoscopy: Emphasis on Crohn’s Disease

        Video capsule endoscopy (VCE) is an ingestible video camera that transmits high-quality images of the small intestinal mucosa. This makes the small intestine more readily accessible to physicians investigating the presence of small bowel disorders, such as Crohn’s disease (CD). Although VCE is frequently performed in Korea, there are no evidence-based guidelines on the appropriate use of VCE in the diagnosis of CD. To provide accurate information and suggest correct testing approaches for small bowel diseases, the Korean Gut Image Study Group, part of the Korean Society of Gastrointestinal Endoscopy, developed guidelines on VCE. Teams were set up to develop guidelines on VCE. Four areas were selected: diagnosis of obscure gastrointestinal bleeding, small bowel preparation for VCE, diagnosis of CD, and diagnosis of small bowel tumors. Three key questions were selected regarding the role of VCE in CD. In preparing these guidelines, a systematic literature search, evaluation, selection, and meta-analysis were performed. After writing a draft of the guidelines, the opinions of various experts were solicited before producing the final document. These guidelines are expected to play a role in the diagnosis of CD. They will need to be updated as new data and evidence become available.

      • Polycrystalline Ceramic Fibers by Extrusion

        titanate and a silicon carbide/zirconium diboride particulate composite have each been blended with thermoplastic of aqueous binders and extruded. The green extrudates have diameters ranging between 50 and 150 ㎛ and polyethylene-base 150 ${\mu}m$ diameter fibers can be drawn down at elevated temperature to approximately 40 ${\mu}m$ diameter. Hollow fibers with 150${\mu}m$ outer and 90 ${\mu}m$ inner diameter can also be fabricated. Green fibers have been processed into chopped fiber felts for use as gas distributors/current collectors in an experimental solid oxide fuel cell (SOFC) and the first attempts at producing simple textile structures have been successful. The fibers, tubes and felts have been successfully debound and sintered and characterization of the sintered PSZ fibers, for example, has revealed a density in excess of 99% and tensile failure stresses up to 1.0 GPa for 78 ${\mu}m$ diameter fibers.

        Comparative response of two seasonal spotted wing drosophila (Drosophila suzukii) morphs to different classes of insecticides

        Drosophila suzukii Matsumura (Drosophilidae) is a devasting invasive pest affecting berry crops and cherry production throughout North America, South America, and Europe. This species has two different morph phenotypes, winter and summer morphs, which are temperature dependent. Chemical control is the most widely used management approach for managing the spotted wing drosophila, D. suzukii. Little is known regarding the differential response of the two seasonal morphs to insecticides. In this study, we identified a high number of differentially expressed genes likely involved in phase I, II, and III of detoxification pathways and other cuticular proteins in winter morphs as compared to summer morphs. Specifically, several detoxification genes (phase I: Cyp4e3, Cyp4s3, Cyp6d5, Cyp49a1, Cyp318a1; phase II: GstD10, Ugt35Bb, Ugt37b1, Ugt58Fa; phase III: Mdr65) were overexpressed more than two-fold in winter morph. Additionally, we determined the median lethal concentration, LC50, values of Malathion, Cyantraniliprole, Imidan, Zeta- cypermethrin, and Spinetoram insecticides to compare the insecticide susceptibility against two seasonal morphs of D. suzukii. For most of the pesticides tested, there were no differences between the LC50 values, between summer and winter morphs, however, we found that winter morph exhibited an LC50 value of Spinetoram 3.7- fold, significantly higher than that of the summer morph. Overall, we demonstrated that seasonally induced different morphological phenotypes may result in different transcriptional response of phases I, II, and III of the detoxification pathways and other cuticular proteins. However, we found different responses to at least one insecticide, Spinetoram. It remains to be determined what are underlying physiological differences that lead to these changes in response to Spinetoram.
