        조구등(釣鉤藤)의 4-VO로 유발한 흰쥐뇌허혈에 대한 신경방어효과

        Uncariae Ramulus et Uncus(UR, ???) has sweet in flavour and slightly cold in property, acting on the liver and pericardium channels. This drug was described in a medical classic as having the ability to remove "heat", check hyperfunction of the liver and relieve dizziness, tremors, and convulsion, and subdue "endogenous wind". So this study was planned to check the neuroprotective effect of UR on the global ischemia induced by 4-vessel occlusion in Wistar rats. and UR extract was lyophilized after extraction with 70% methanol. We induced 4-vessel occlusion for 10 minutes and reperfused again. The number of CA1 pyramidal neurons were counted after 7 days of reperfusion under the cresyl violet staining. In 4-VO ischemia model, UR showed significantly neuroprotective effects(1,000 and 500 ㎎/㎏ of UR extracts, p<0.05) compared with control group. Each neuroprotective ratio was about 23.0%, 19.0% respectively. Consequently, Uncariae Ramulus et Uncus has neuroprotective effects on the global ischermia induced by 4-vessel occlusion in Wistar rats. So we expect that Uncariae Ramulus et Uncus can be used as a drug for neurodegenerative disease.

        폐좌상시 폐단락률과 PEEP치료의 효과

        To evaluate the physiologic extent of pulmonary contusion and effect of PEEP therapy for pulmonary contusion, we studied 16 patients received PEEP therapy with pulmonary contusion from nonpenetrating chest trauma. Hemodynamic parameters including pulmonary vascular resistance index and intrapulmonary shunt fraction were calculated from standard measurement, and arterial oxygen tension was measured. Pulmonary vascular resistance index and intrapulmonary shunt fraction were increased in patient group. Arterial oxygen tension was decreased with increase of the intrapulmonary shunt fraction(R=0.75). Arter minimal PEEP therap(5-10 ㎝H₂O), pulmonary vascular resistance index was remained unchanged and intrapulmonary shunt fraction was decreased without significant changes of pulmonary hemodynamics. Increment of arterial tension was increased with decrease of intrapulmonary shunt fraction(R=0.43). Decreased stroke volume index suggested of cardiac injury such as cardiac contusion in patient group. These results of our study suggested that increased intrapulmonary shunt fraction caused arterial hypoxia in pulmonary contusion and arterial oxygen tension was increased as a result of reduced intrapulmonary shunt by PEEP therapy.

        자궁근종 환자에서 정맥과 우심방 혈전의 방사선학적 소견을 보인 정맥내 평활근종 1예

        저자는 과거 자궁근중으로 수술한 50세 여자 환자가 심부전 및 광범위한 혈전증의증으로 본원에 내원하여 수술후 정맥내 평활근종으로 밝혀진 증례를 경험하였기에 이를 문헌고찰과 함께 보고하는 바이다. A case of intravenous leiomyomatosis (IVL) invading the vena cava and extending to the right atrium, a rare benign smooth-muscle tumor, is described. Despite their histological benignity these lesions have a tendency to metastasize and are closely related to the condition called "benign metastasizing leiomyoma" and " intracaval mass and cardiac extension" a 50-year old woman was admitted to the hospital with dyspnea beginning 6 months ago and precious history of hysterectomy due to uterne myoma. Echocardiography revealed severe tricupid valve insufficiency and a mobile elongated mass which occupied the inferior vena cava and was extending into the right ventricular cavity. Pelvic ultrasonography and pelvic MRI identified suspicious metastasis to both ovaries and peritoneum. She underwent resection of the cardiac tumor, conomitant tricuspid annuloplasty with a closure of the foramen ovale and both salphyngo-oophorectomy with vaginal stump mass biopsy. All of them could be histologically identified as IVL.

      • Unsteady streaming flow based TENG using hydrophobic film tube with different charge affinity

        <P><B>Abstract</B></P> <P>This paper proposes a simple, eco-friendly and economical fluid-based triboelectric nanogenerator (Flu-TENG), which is based on polyvinylidene fluoride (PVDF) between elastomeric tubes (P-TENG) and biaxially oriented polypropylene (BOPP) between elastomeric tubes (B-TENG). We examined the interactions based on the relation between the hydrodynamics of pipe flow and electrostatic induction at the electrical double layer (EDL). This is the first study to focus on the hydrodynamics of the unsteady pipe flow. From the experimental results, it was verified that the peak to peak current from unsteady flow is 70 times higher than that of steady flow. We also investigated the effect of triboelectric series to generate higher output. When the PVDF and Ground/PVDF/BOPP are connected respectively, it is called as P-TENG/PP-TENG/PB-TENG. The measured short circuit current at 15.9 Hz for P-TENG/PP-TENG/PB-TENG are 0.29 μA, 1.59 μA and 2.40 μA, respectively. And the measured open circuit voltage of P-TENG/PP-TENG/PB-TENG are 3.71 V, 14.6 V and 29.7 V, respectively. Finally, we verified the maximum power reaches 11.5 μW in the case of PB-TENG.         When a contouring NC machine tool works continuoously at the corner, the corner is caused and the precision of products is deteriorated. In this paper the performance of a retrofitted NC milling machine tool is considered in the view of corner error. The steeper the corner angle changes or the greater the feed rate is, the greater the corner error becomes Approximating the serve-table system to a second order system, a system paramer C/K is estimated by measuring the steady-state error and the other system parameter M/K is found by fitting the real 90 degree corner path closely with the simulated theoretical 90 degree corner path.

      • 창상치유에 있어서 OpSite 의 효과에 대한 임상적 연구

        The purpose of our study is to evaluate the effects of occlusive dressing by using the synthetic skin substitute, OpSite in the management of skin graft donor site, superficial secondary degree burn and abrasion wound. OpSite, made of polyurethane form, is a surgical adhesive drape which is permeable to water vapour but not to bacteria. Eighty patients(40 patients with skin graft donor site, 20 patients with secondary degree burn and 20 patients with abrasion) were divided into two groups, OpSite treated group and Sofratulle treated group. We investigated pain, comfort, time for complete healing, numbers of dressing change, and wound infection between OpSite treated group and Sofratulle treated group. OpSite treated group had less pain, less limitation of activity, less frequent change of dressings than the control group using Sofratulle. We concluded that the occlusive dressing with OpSite? was an effective alternative to conventional dressing for optimal wound healing in painful donor site on skin graft, small-sized superficial secondary degree burn and abrasion wound.

        다발성 손상 환자에서 Abdominal CT Scan을 이용한 흉부손상의 진단

        Injuries to the thorax was evaluated among 224 patients admitted to the emergency room with multiple trauma who had undergone abdominal CT scan for possible intraabdominal injuries. In order to detect possible intrathoracic injury, abdominal CT scan were sliced 1cm apart starting from a point 2cm superior to the diaphragm. Soft tissue and lung window were performed on cross sectional images 2cm, 1cm above the diaphragm and to the diaphragm level. Among the 224 patients with multiple trauma who had undergone abdominal CT scan, thoracic injuries previously undetected on plain chest X-ray were detected in 42 patients(18.8%) on the abdominal CT scan. 123 different thoracic lesions were found to present on the 42 patients diagnosed to have thoracic injuries on the abdominal CT scan, among which there were 67 cases of hemothorax, 24 cases of lung contusion, 24 SQ emphysematous lesions, 6 cases of pneumothorax, and chest wall hematoma in 1 patient. Among the 67 patients with hemothorax 21 patient(31.3%) and 4 patients(66.7%) among the 6 with pneumothorax eventually received closed thoracostomy, and 4 patients among the 24 with lung contusion received ventilator care following admission. In conclusion, when abdominal CT scan is performed for evaluation of suspected abdominal injuries with absence of thoracic injuries on plain chest X-ray, possible thoracic as well as abdominal lesions should be closely observed.

        상안검하수(거근 기능이 2~4mm)환자에서 거근 절제술과 전두근의 기능을 이용한 방법의 원격추적 결과

        The choice of operative procedures was determined by many factors, but the levator function is considered as the most important factor. Fox recommended that when the levator function was 2 to 9 mm, the patient should be treated by the levator resection and when the levator function was below 2mm, the patient should be treated by frontalis suspension. But Collin recommended that when the levator function was below 4mm, the brow suspension procedure got the better result. Beside that, many other authors recommended various opinions. Therefore, when levator function of the patients was between 2 and 4 mm, the choice of operative procedure was much confusing us. The aim of this study is to clarify which is better procedures between levator resection and the frontalis transfer or orbicularis oculi muscle transfer among the patients with 2 to 4 mm of levator action. From Jan. of 1991 to Dec. of 1994, among 26 patients with 2 to 4 mm levator function, 13 cases of 10 patients were operated by levator resection, 21 cases of 16 patients were treated by frontalis transfer or orbicularis oculi muscle flap. The results were evaluated with the average 27 months of follow-up and we compared the result of the levator resection procedure with that of frontalis muscle transfer or orbicularis oculi muscle transfer. The preoperative average amount of ptosis is about 2.7mm in cases with levator resection, about 4.0mm in cases with frontalis muscle transfer or orbicularis oculi muscle flap transfer. The postoperative average amount of ptosis is about 1.7mm in cases with levator resection, about 2.14 mm in cases with frontalis muscle transfer or orbicularis oculi muscle flap transfer and so the postoperative improvement of amount of ptosis is about 1.0mm in cases with levator resection, about 1.86 mm in cases with frontalis mucsle transfer or orbicularis oculi muscle flap transfer. The major complication of levator resection method is undercorrection. However, the eyelid deformity due to excessive upward traction was more frequent in frontalis muscle flap technique.
