        원형무기폐의 전산화단층촬영 소견

        Rounded atelectasis is an unusual from of peripheral lober collapse. It appears as a mass-like lesion on chest radiography that often mimics a pulmonary neoplasm. Four cases of rounded atelectasis were diagnosed radiologically, and the CT findings were analyzed. In every case the CT showed a round mass, 3-5cm in diameter, abutting a thickened pleural surface on the lung periphery. The bronchovascular bundles near the mass appeared to converge on the mass in a curved course, forming a so-called "comet-tail" appearacne on the CT scan. Our experience suggests that the CT findings of rounded atelectasis are characteristic of the disease and that the diagnosis of the disease can be made radiologically without histological confirmation.firmation.

        구강설부 및 구강기저부 병변에서의 초음파검사의 유용성

        The evaluation of the tongue and mouth floor using the CT scan can be limited due to dental artifacts. Because the therapeutic plan depends on the nature and the cancer staging of lesions, it is essential to obtain accurate evaluation or lesions. In this study are explored the of Ultrasonography for the evaluation of tongue and month floor pathology. We have obtained ultrasonograms in 20 patients who had benign and malignant lesions in the tongue and mouth floor. Comparative analysis was made on 15 patients who underwnt both CT and ultrasonography. We used the bimanual compression technique and the phonation technique during real time scanning. Out of 15 cases, six had tongue cancer, and seven of them had cancer of the mouth floor. The other cases included a ranula, a dermoid cyst, a Ludwig's angina, an abscess of salivary gland, and one care with stones of the submandibular gland. All the benign and malignant masses appeared hypoechoic. Even though the lesions of tongue and mouth floor could not be evaluated by CT scan due to dental artifacts and extremely small size in 5 cses, ultrasonogram using the bimanual compression technique demonstrated good localization of the lesion, and also was greatly helpful in visualizing the invasion of the surrounding structures. Ultrasonograpy of the tongue and mouth floor proved to be superior to the CT scan not only in delneating the size and extent of the primary lesion, but also in visualizing invasion of surrounding sturctures.

        국내 감각통합치료실 구성을 위한 가이드라인 개발 : 델파이 연구

        목적 : 본 연구는 델파이 방법을 적용하여 국내 감각통합치료실 구성을 위한 가이드라인을 개발하고자 하였다. 연구방법 : 2020년 5월부터 6월까지 감각통합치료 관련된 전문가 패널 22명을 대상으로 델파이 방법을 적용 하여 전문가의 의견을 수집하였다. 델파이 조사는 2차에 걸쳐 진행 되었으며, 1차 델파이 설문에는 선행연 구를 근거로 하여 평가도구 40개와 치료도구 23개의 폐쇄형 질문과 안전을 위한 물리적 환경 10개의 항목 과 개방형 질문을 혼합하여 의견을 수렴하였다. 2차 델파이 설문은 1차 설문의 결과를 바탕으로 항목 삭제 및 수정과정을 거쳐 최종 수렴하였다. 결과 : 1차 델파이 결과에서는 총 59개 항목이 선정되었다. 최종 2차 델파이 결과에서는 59개의 모든 항목의 내용타당도 비율이 .42 이상으로 분석되어 삭제한 항목은 없었으며, 안정도 또한 모든 항목 0.5 이하로 분 석되어 추가적인 설문이 필요 없음으로 판단하였다. 최종 델파이 조사의 평균 내용타당도 비율은 .92, 안정 도 .15, 수렴도 .36, 합의도 .80으로 높은 일치도를 보였다. 결론 : 본 연구에서는 감각통합치료실의 물리적 환경과 치료 도구 및 평가 도구를 조사하고, 감각통합치료 자들에게 임상 현장에서 얻어진 감각통합치료실 환경에 대한 견해를 파악함으로써 앞으로 감각통합치료 실을 개설하기 원하는 기관들과 임상 치료사에게 도움이 될 것으로 기대된다. Objective : This study presents guidelines for setting up a sensory integration room by applying the Delphi method. Methods : The Delphi method was applied in a survey of 22 expert panels asked to review sensory integration therapy from May to June 2020 to collect expert opinions. The survey was conducted in two rounds. The first round involved the collection of opinions based on prior research that used a total of 40 assessment tools, 23 closed questions about treatment tools, and 10 questions about the physical environment. Based on the results of the first round, the second was conducted using a process of item deletion and modification. Results : A total of 59 items were selected in the first round of the survey. In the second the contents of these 59 items were analyzed: each item scored .42 or higher, so no items were deleted. The stability of the survey was also calculated and found to be less than 0.5, so no further testing was required. The average content feasibility ratio of the final Delphi survey was .92, stability .15, convergence .36, and consensus .80, showing high agreement. Conclusion : This study is expected to help institutions and clinical therapists who want to create a sensory integration therapy room in their facilities by examining the physical environment, treatment tools, and evaluation tools used in sensory integration therapy rooms and understanding the views on such environments obtained from the clinical site to sensory integration therapists.

        장애아동에게 적용된 The Listening Program(TLP) 중재법에 관한 체계적 고찰

        본 연구는 체계적 고찰을 통해 The Listening Program(TLP) 중재의 주요대상, 적용한 총 회기수, 평가영역, 측정도구 등과 그 연구수준을 분석하여 임상적으로 유용한 자료를 제시하고자 하였다. 아동에게 TLP 중재를 적용한 논문을 검색하기 위하여 Scholar of Google, ProQuest, EBSCOhost, RISS 데이터베이스를 사용하였다. 대상 논문의 범위는 2009년 1월부터 2019년 9월까지로 하였으며, 주요 용어는 “the listening program” OR “sound-based intervention” AND “child”를 해외논문 검색에 사용하였고, 국내논문은 “청지각 훈련” OR “the listening program” OR “치료적 듣기” OR “청각 인식 훈련”으로 검색하였다. 검색된 논문 445편 중 본 연구에 적합한 임상실험논문 총 10편을 최종 선택하여 분석하였다. 분석대상 실험논문은 개별실험연구 9편, 비 무작위 두 집단 연구가 1편으로 근거수준이 낮은 것으로 나타났다. TLP 중재의 적용대상을 분석한 결과 자폐 스펙트럼 장애가 가장 많았으며 다운증후군, 불특정 전반적 발달장애, 발달지연, 중증도 복합적 학습장애, 감각처리장애 등 다양하게 나타났다. TLP 중재의 결과를 측정하는 영역은 말하기 및 언어영역이 가장 많았으며, 감각처리기능, 주의집중력, 작업수행도, 운동기술, 시지각 기술, 발달 수준, 학업 성취도 등 다양하게 평가되었다. 또한 효과를 측정하는 도구로는 관찰법을 가장 많이 사용하였다. TLP 중재 시 적용한 총 회기 수는 20주 동안 100회기, 10주 동안 50회기가 주를 이루었다. 본 TLP 중재법의 체계적 고찰 연구는 구체적인 근거수준 및 적합한 적용대상 및 활동, 결과측정도구를 제시함으로써 임상적 활용 가능성을 제시한데 의의가 있다.

        건강검진 수진자의 복부 초음파 검사 시 선별 검사용 흉부 저선량 전산화단층촬영의 추가적인 가치

        Purpose: The purpose of this study was to assess the added value of screening low-dose computed tomography of the chest (LDCT) covering the abdomen in evaluating abdominal solid organs to the screening abdominal ultrasonography (US). Materials and Methods: We prospectively enrolled a consecutive series of 393physical check-up patients, who underwent screening abdominal US and LDCT of the chest from January to February, 2008. LDCT covered the lungs, liver, pancreas, gallbladder,spleen, and both kidneys. The 1st screening abdominal US were performed without information from the LDCT, and then abdominal images covered by LDCT were immediately evaluated. Then a 2nd US session was done with additional information from LDCT and US examination was focused to the findings of LDCT. Perpatient and per-lesion analyses were performed. Results: In per-patient analysis, additional focal lesions were found in 20 patients (5.1%) for liver and 9 patients (2.3%) for kidneys in the 2nd US sessions. In per-lesion analysis, 154 and 73 focal lesions were found in liver and kidneys, respectively, in the 1st US sessions. On the 2nd US session, 186 and 86 lesions were found in liver and kidneys, respectively. 20.8% and 17.8% of focal lesions were additionally found on 2nd US session in liver and kidneys, respectively. Most (62.5%) of the additional lesions detected in liver were located in segment 7 and 8, the hepatic dome. Conclusion: Previewing LDCT of the chest and abdominal solid organs before performing screening abdominal US can enhance the diagnostic performance of US in physical check-up patients. 목적: 본 연구의 목적은 복부 장기를 포함하는 흉부 저선량전산화단층촬영이, 복부 초음파 검사의 진단 능력 향상에 도움이 되는 지 알아보는 것이다. 재료 및 방법: 2008년 1월에서 2월 사이에 복부 초음파검사와 흉부 저선량전산화단층촬영을 포함한 건강 검진을시행한 393명의 수진자(남자 227명, 여자 166명, 평균연령 50.0세)에서 전향적인 연구가 시행되었다. 저선량전산화단층촬영은 간, 췌장, 담낭, 비장, 신장 등의 복부 장기를포함하여 촬영이 이루어졌다. 먼저 저선량전산화단층촬영을 시행 후, 그 영상을 보지 않은 상태에서 복부 초음파 검사가 1차로 시행되었으며, 이후 저선량전산화단층촬영 영상 정보를 참고하여 2차 초음파 검사가 시행되었다. 1차및 2차 초음파 검사에서 각 복부 장기에서 발견된 병변들을 수진자별, 병변별로 분석하였다. 결과: 수진자별 분석에서, 간의 경우 20명(5.1%), 신장의 경우 9명(2.3%)에서 추가적인 국소 병변이 2차 초음파검사에서 발견되었다. 병변별 분석에서 간과 신장에서는 1차 초음파 검사에서 각각 154개, 73개, 2차 초음파 검사에서 각각 186개와 86개의 국소 병변이 발견되었으며 각각20.8%와 17.8%의 국소 병변이 추가적으로 발견되었다. 간에서 추가로 발견된 병변의 62.5%는 간지붕부인 7번 및8번 분절에서 발견되었다. 결론: 건강검진 수검자에서, 흉부 저선량전산화단층촬영에 포함된 복부 장기의 소견을 복부 초음파 검사 전에 미리 확인하는 것은 초음파 검사의 진단 능력 향상에 도움이될 것으로 생각된다.

        청지각 훈련(The Listening Program)이 주의력결핍 과잉행동장애 아동의 감각처리 및 주의집중력에 미치는 영향

        The purpose of this study was to investigate the changes in the children's attention and sensory processing level after applying auditory perception training to children with attention deficit/hyperactivity disorder. The subjects of this study were three children with attention deficit hyperactivity disorder, and the study design applied the ABA design of a single subject experimental design. The research process was conducted in a total of 23 sessions,and consisted of 4 sessions during the baseline period, 15 sessions during the intervention period, and 4 sessions during the re-baseline period. Auditory perception training was provided 15 times during the intervention period, 5 times a week, and 2 modules were listened to. To measure the change in the level of attention, the FAIR attention test was used. As a result of the study, after applying the auditory perception training, 2 out of 3 subjects had an effect on improving attentional ability, and the improvement in auditory processing ability was found in all subjects, but statistical significance was found only in 2 subjects. As a result of the SSP test, overall sensory processing ability was improved in all subjects, and subject 2 was able to confirm the tendency to change from problem category to suspicious category after intervention and maintain. Through the results of this study, it is significant in that auditory perception training is an effective intervention method for attention and sensory processing ability of children with attention deficit hyperactivity disorder.

        The Effectiveness of Ferritin as a Contrast Agent for Cell Tracking MRI in Mouse Cancer Models

        Purpose: We aimed to investigate the effectiveness of ferritin as a contrast agent and a potential reporter gene for tracking tumor cells or macrophages in mouse cancer models. Materials and Methods: Adenoviral human ferritin heavy chain (Ad-hFTH) was administrated to orthotopic glioma models and subcutaneous colon cancer mouse models using U87MG and HCT116 cells, respectively. Brain MR images were acquired before and daily for up to 6 days after the intracranial injection of Ad-hFTH. In the HCT116 tumor model, MR examinations were performedbefore and at 6, 24, and 48 h after intratumoral injection of Ad-hFTH, as well as before and every two days after intravenousinjection of ferritin-labeled macrophages. The contrast effect of ferritin in vitro was measured by MR imaging of cell pellets. MRI examinations using a 7T MR scanner comprised a T1-weighted (T1w) spin-echo sequence, T2-weighted (T2w) relaxation enhancement sequence, and T2*-weighted (T2*w) fast low angle shot sequence. Results: Cell pellet imaging of Ad-hFTH in vitro showed a strong negatively enhanced contrast in T2w and T2*w images, presentingwith darker signal intensity in high concentrations of Fe. T2w images of glioma and subcutaneous HCT116 tumor models showed a dark signal intensity around or within the Ad-hFTH tumor, which was distinct with time and apparent in T2*w images. After injection of ferritin-labeled macrophages, negative contrast enhancement was identified within the tumor. Conclusion: Ferritin could be a good candidate as an endogenous MR contrast agent and a potential reporter gene that is capable of maintaining cell labeling stability and cellular safety.

        불확실한 동역학을 갖는 차량의 군집 주행을 위한 외란 관측기 기반의 강인 제어 기법

        Vehicle platooning is a technology that drives a group of vehicles over very close distances between vehicles. By maintaining short inter-vehicle distances, platooning reduces fuel consumption and increases road capacity. However, the string stability, a characteristic that does not amplify the inter-vehicle distance errors toward the tail of the platoon, cannot be guaranteed for platoons with uncertain dynamics. In this paper, a disturbance observer based robust control approach is proposed to improve the string stability of the platoon when dynamics of its members are uncertain. Since an uncertain system behaves like a disturbance-free nominal system by the disturbance observer, this approach allows all vehicles in the platoon to operate like the same nominal model so that one can ensure the string stability by applying conventional cooperative adaptive cruse control techniques.

        We present an estimation scheme using deadbeat observers for discrete-time switched linear systems. Based on the finite settling time property of deadbeat response, the proposed scheme detects the switching time and estimates the active mode exactly within 2n steps where n is the order of subsystems. Once the switching signal is recovered, the deadbeat observer gives precise state estimates.

        In this paper, we estimate a switching signal for discrete-time switched linear systems where the output measurements are corrupted by noises. To solve the switching signal problem, we first propose a method to estimate the active mode of the plant. Then we present an implementable on-line algorithm to detect the switching time within a predetermined time interval after the actual switching occurred.
