Purpose Bevacizumab±irinotecan is effective for treatment of recurrent malignant gliomas. However, the optimal duration of treatment has not been established. Materials and Methods Ninety-four consecutive patients with recurrent malignant glioma who were treated with bevacizumab at our institutions were identified. Patients who continued bevacizumab until tumor progression were enrolled in a late discontinuation (LD) group, while those who stopped bevacizumab before tumor progression were enrolled in an early discontinuation (ED) group. Landmark analyses were performed at weeks 9, 18, and 26 for comparison of patient survival between the two groups. Results Among 89 assessable patients, 62 (69.7%) and 27 (30.3%) patients were categorized as the LD and ED groups, respectively. According to landmark analysis, survival times from weeks 9, 18, and 26 were not significantly different between the two groups in the overall population. However, the LD group showed a trend toward increased survival compared to the ED group among responders. In the ED group, the median time from discontinuation to disease progression was 11.4 weeks, and none of the patients showed a definite rebound phenomenon. Similar median survival times after disease progression were observed between groups (14.4 weeks vs. 15.7 weeks, p=0.251). Of 83 patients, 38 (45.8%) received further therapy at progression, and those who received further therapy showed longer survival in both the LD and ED groups. Conclusion In recurrent malignant glioma, duration of bevacizumab was not associated with survival time in the overall population. However, ED of bevacizumab in responding patients might be associated with decreased survival.
편마비환자의 팔꿉관절 굽힘각도에 따른 지팡이 보행시의 마비측 하지 및 비마비측 상지의 근활성도 비교

본 연구의 목적은 성인 편마비환자를 대상으로 지팡이 사용측 팔꿉관절의 굽힘각도에 따른 보행 시의 마비측 하지 및 비마비측 상지근육의 근활성도를 비교하여 성인 편마비환자만을 위한 효율적인 지팡이 길이를 제시하는데 있다. 한발 지팡이를 사용하여 보행 중에 있는 성인 편마비환자 16명을 대상으로 하였고, 모든 대상자들은 10m의 실내의 고른바닥을 지팡이를 사용하지 않은 보행과 팔꿉관절 굽힘각도 0º, 25º, 50º에 맞춘 지팡이 길이를 사용한 보행을 하였다. 지팡이를 사용하지 않은 보행 시의 근활성도를 기준으로 하여 각각의 팔꿉관절 굽힘각도의 지팡이 길이에 따른 마비측 하지의 넙다리뼈곧은근, 넙다리뼈두갈래근, 앞정강근, 장딴지근과 비마비측 상지근육인 위팔세갈래근, 넓은등근의 근활성도를 비교하기 위해 프리드만 검정을 실시하였다. 연구결과, 팔꿉관절 굽힘각도 50º에 해당하는 지팡이의 길이는 0º와 25º의 지팡이 길이에 비해 마비측 하지의 넙다리뼈두갈래근과 장딴지근의 근활성도가 높게 나타났으며, 팔꿉관절 굽힘각도 50º 지팡이 길이에서의 비마비측 위팔세갈근의 근활성도는 25º에 비해 낮게 나타났다. 따라서 팔꿉관절 굽힘각도 50°에 맞춘 지팡이의 길이에서의 보행은 편마비환자의 마비측 하지근육의 근수축 정상화와 지팡이 사용측 근육의 과한 근수축 감소에 가장 긍정적인 영향을 미칠 수 있는 길이로 본다. The purpose of this study was to propose appropriate cane length for stroke patients by comparing electromyography on the muscles of affected lower extremity and unaffected upper extremity according to the cane length fitted at different elbow flexion degrees during walking with cane. Sixteen adult stroke patients that have been walking with mono-cane were enrolled in this study. All the patients walked on the 10m indoor flat surfaces without cane, with cane fitted at 0º, 25º, and 50º elbow flexion, espectively. Friedman test was performed to compare the extent of electromyography on the rectus femoris, biceps femoris, tibilalis anterior, gastrocnemius of affected side, and triceps brachii, latissimus dorsi of unaffected side according to the cane length fitted at different elbow flexion degree. The results showed that the extent of electromyography on the biceps femoris and gastrocnemius of affected side in the walking with cane length fitted at 50º elbow flexion were higher than at 0º and 25º elbow flexion, and the extent of electromyography on the triceps brachii of unaffected side at 50º elbow flexion was lower than at 25º elbow flexion. Therefore, the cane length fitted at 50º elbow flexion seems to have a best effect on normalizing muscle strength of affected side and minimizing excessive muscle contraction of cane side.

까치복의 呈味成分에 관한 資料를 얻고자 核酸關聯物質, 유리아미노산, TMAO, TMA, betaine 및 총 creatinine을 分析하였다. 核酸關聯物質 중 IMP가 1.9μmole/g으로서 가장 많았고, 全核酸關聯物質에 대해 39.6%를 차지하였다. 유리아미노산 중 함량이 많은 것은 taurine, lysine, alanine 및 glycine 이고, 이들 네 가지 아미노산이 전유리아미노산의 68.2%를 차지하였다. 총 creatinine�량은 194.5mg/100g으로서 엑스分窒素에 대해 63.4%를 차지하였으며, TMAO 및 betaine 함량은 微量이었다. Omission test 結果 까치복의 呈味成分은 5'-mononucleotides 및 유리아미노산이 주된 구실을 한다는 것을 알았다. Yellowfin puffer, Fugu xanthopterus(Temminck et Schlegel), is one of the most palatable fishes in Korea. However, relatively little is known about the characteristics of its taste. In this study, the taste compounds including nucleotides and their related compounds, free amino acids, trimethylamine oxide, trimethylamine, betaine and total creatinine were analyzed, and their roles in tasting activity were investigated. The amounts of IMP was 1.9 μmole/g, and the ratio of IMP to the total nucleotides and their related compounds was 39.6%. The great portion of free amino acids in the extractives of yellowfin puffer muscle was occupied by taurine, lysine, alanine and glycine in order, and their content was 79.1% of the total free amino acids. Among the organic bases, total creatinine was abundant, and its nitrogen content was 63.4% of total extractive nitrogen. According to the results of the omission test, the main constituents of the characteristic taste of yellowfin puffer could be assumed as free amino acids and 5'-mononucleotides.

This study was conducted to identify irradiation-derived volatile components, having positive-correlation with irradiation doses, which may play roles as maker materials for detecting post-irradiation in pork meat. The volatile components of irradiated(0, 1, 3, 5 and 10 kGy doses) pork(Belly) meat was analyzed by liquid liquid continuous extraction(LLCE)/GC/MSD methods. By the results of linear regression analysis between irradiation doses and volatile component amounts in fresh pork, 3 compounds including γ-octalactone(r=0.82), 1-octen-3-ol(r=0.90) and butanoic acid(r=0.81) had high correlations with irradiation doses.
