        Optimizing the Hospital Blood Bank Stock in Korea: A Comparative Analysis of the Uniform 5-Day Stock Index and a Novel Blood Stock Index

        Background: Maintaining optimal blood inventory levels in hospitals is important to prevent blood shortage and wastage. We aimed to provide an efficient blood inventory management strategy for hospital blood banks nation-wide by comparing the current use of 5-day issuable stock (IS) with Lim’s IS as a novel target IS. Methods: The average and CV of daily usage (DU) were calculated from information entered into Korea’s Blood Management System by 194 participating hospitals in 2019 and 2020. Using these data, Lim’s IS was calculated by determining the simulated annual average blood shortage day nearest to 1 for each blood group in each hospital. The 5-day IS (5IS) was estimated by multiplying the average DU in 2018 by five to count the shortage days in 2019. Results: The average DU (0.3–231.3 units) and corresponding CV (0.33–7.14) in the participating hospitals were inversely proportional (r=–0.699 to –0.695). The hypothetical averages of 5IS and Lim’s IS were 27.0±41.2 and 24.7±20.8, respectively (P=0.006). The shortage days for 5IS and Lim’s IS were 8.9±22.7 and 1.0±1.9, respectively (P<0.001). Conclusions: While 5IS was unacceptable for universal application, Lim’s IS remained near one shortage day and is considered more efficient than 5IS. Hospitals should implement indicators that consider DU and its variations. This is the first study to introduce Lim’s IS as an indicator of optimal blood inventory, and the data are expected to provide guidance for effective blood inventory management nationwide, particularly during blood shortages.

        Differential Manifestation of Ocular Phenotypes in TALEN-mediated p19arf Knockout FVB/N and C57BL/6J mouse lines

        Background p19arf, primarily known as a tumor suppressor, has also been reported to play an essential role in normal development of mouse eyes. Consistently, lack of p19arf has been associated with ocular defects, but the mixed background of the knockout (KO) mouse strain used raised a concern on the accuracy of the phenotypes observed in association with the targeted gene due to genetic heterogeneity. Object We carried out a study to investigate into the efect of genetic background on the manifestation of p19arf KO associated phenotypes. Methods We characterized the phenotypes of novel p19arf KO mouse lines generated in FVB/N and C57BL/6J using a transcription activator-like efector nuclease (TALEN) system in comparison to the reported phenotypes of three other p19arf-defcient mouse lines generated using homologous recombination. Results Ninety-fve percent of FVB/N-p19arf KO mice showed ocular opacity from week 4 after birth which worsened rapidly until week 6, while such abnormality was absent in C57BL/6J-p19arf KO mice up to the age of 26 weeks. Histopathological analysis revealed retrolental masses and dysplasia in the retinal layer in FVB/N-p19arf KO mice from week 4. Besides these, both strains developed normally from birth to week 26 without increased tumorigenesis except for a subcutaneous tumor found in a C57BL/6J-p19arf KO mouse. Conclusion Our fndings demonstrated surprisingly variable manifestation of p19arf-linked phenotypes between FVB/N and C57BL/6J mice, and furthermore between our mouse lines and the established lines, indicating a critical impact of genetic background on functional study of genes using gene targeting strategies in mice.

        심정지환자에서 epinephrine투여시 혈중 catecholamine의 변화

        Background : Cardiac arrest is a potent stimulus for the release of endogenous catecholamine and high plasma catecholamine concentration has been reported during cardiopulmonary resuscitation(CPR). Few data were present about the clinical effect of high plasma catecholamine and endogenous catecholamine response to exogenous epinephrine administration during CPR. Purpose : This study was designed to evaluate endogenous catecholamine response to cardiac arrest and exogenously administered epinephrine during CPR. Method : In 21 cardiac arrest victims, blood sample for epinephrine and norepinephrine was drawn before and 1 minute after each epinephrine administration during CPR. 1mg of epinephrine was administered immediately after intravenous access and 3 minute interval during CPR. Plasma catecholamines were quantitated by high-performance liquid chromatography. Result : Baseline plasma epinephrine concentraqtion was higher in patients with cardiac arrest than normal controls (985±1627 vs 194±173 pg/ml, p<0.05). Baseline plasma norepinephrine concentration was also elevated in patients with cardiac arrest than normal controls, although statistically insignificnat(1965±4915 vs 360±250 pg/ml). Plasma epinephrine concentration in cardiac arrest patients was significantly elevated after exogenous administration of epinephrine, but its response was blunted after the third does of epinephrine. Plasma norepinephrine concentration in cardiac arrest patients was also elevated, but its response was observed only after the first epinephrine administration. Plasma epinephrine concentration was persistently higher in patients with return of spontaneous circulation (ROSC) than patients without ROSC. there was no significant difference of plasma norepinephrine concentration whether spontaneous circulation was restored or not. Endogenous epinephrine response to exogenous epinephrine was not observed in patients having prolonged arrest time over 20 minutes. Conclusion : Results of this study suggests that significant amount of catecholamine is released by exogenous epinephrine administration in spite of high plasma catecholamine concentration, and degree of endogenous catecholamine response influences resuscitation outcome.

        The solubilities of benzene in water and aqueous urea solution were determined by an ultraviolet method at various temperatures between 0°and 40℃. Solubility minimums, ΔH_sol _min corresponding to a zero heat of solution appeared in 1∼3 M aqueous urea solution as well as in water, and they were shifted to lower temperature side in accordance with the increase of urea concentration. Calculated transfer thermodynamic parameters of benzene from water to urea solution and solubility mininmums were interpreted in terms of iceberg formation in solution and also support the concept that urea acts like a structure breaker in water.

        Recently, a multi facility, multi product and multi period industrial problem has been widely investigated in Supply Chain Management (SCM). One of the key issues in the current SCM research area involves reducing both production and distribution costs. The purpose of this study is to determine the optimum quantity of production and transportation with minimum cost in the supply chain network. We have presented a mathematical model that deals with real world factors and constraints. Considering the complexity of solving such model, we have applied the genetic algorithm approach for solving this model computational experiments using a commercial genetic algorithm based optimizer. The results show that the real size problems we encountered can be solved in reasonable time.

      • 12-O-Tetradecanoylphorbol-13-Acetate에 의한 HL-60 세포 분화유도중 Vimentin 유전자 전사조절에 대한 AP-1의 역할

        Purpose: To gain insight on the role of AP-1 in transcriptional regulation of vimentin gene during differentiation of HL-60 cells by 12-O-tetradecanoylphorbol-13-acetate (TPA), the levels of vimentin mRNA and AP-1 have been investigated with Northern blot hybridization and DNA mobility shift assay. Materials and Methods: HL-60 cells were grown in RPMI 1640 medium supplemented with 10% heat-inactivated fetal calf serum and antibiotics in a humidified 5% CO_(2) at 37°C. Total RNA was prepared by a modification of the method of Karlinsey et al. Northern blot hybridization was performed by the method of Virca et al. EcoRI fragment of pVIM-GEM was used as probe for vimentin mRNA. DNA mobility shift assay was performed by the method of Lim et al. End labeled DNA probe(Upper strand, 5'-CGCTTGATGAGTCAGCCG- 3') for AP-1 binding activity was mixed with nuclear extracts in a 20 μl reaction volume containing 300 mM KC1, 60 mM HEPES, pH 7.9, 25 mM MgCl_(2), 1 mM EDTA, 1 mM DTT, 60% glycerol, and 2μg of poly[dI-dC]. Results: TPA increased vimentin mRNA levels, with maximal stimulation reached at 24 hr. The level of vimentin mRNA was induced in proportion to the concentration of TPA. TPA-induced vimentin mRNA was almost reduced by actinomycin-D pretreatment. TPA-induced stimulation of vimentin gene was completely reduced by staurosporin pretreatment. In DNA mobility shift assay, AP-1 newly appeared at 24 hr during TPA-induced differentiation and was almost not detected after the pretreatment of staurosporin. Conclusions: These results suggest that the induction of vimentin mRNA during TPA-dependent differentiation in HL-60 cells may be mediated by protein kinases C signal transduction and AP-1 is important to transcriptional regulation.

      • α-Sialon 세라믹스(X=0.15)의 기계적 성질에 미치는 소결분위기 및 ZrO₂의 첨가 영향

        4.82 wt% AIN and 2.95 wt% Y₂O₃were added to Si₃N₄as sintering agents in order to have α-Sialon composition of X=0.15 and monoclinic ZrO₂between 0 to 10 wt% was admixed with Si₃N₄. Si₃N₄based ceramics were fabricated by hot-pressing at 1750℃ for 90 min under 30 MPa in argon and nitrogen atmospheres Effects of sintering atmospheres and addition of ZrO₂on mechanical proporties of Si₃N₄based ceramics were investigated. As ZrO₂content increased, the fraction of α-Sialon tended to decrease and the amount of cubic ZrO₂increased because Y₂O₃acted as the stabilizer of ZrO₂. The sintering atmospheres didn't affect on the sintering behavior and the product phases in hot-pressing of Si₃N₄. Bending strength increased when ZrO₂was added up to 3 wt%. Hardness showed tendency to decrease with ZrO₂content. ZrO₂didn't contribute to the increase of fracture toughness because stabilized cubic ZrO₂was produced by the added Y₂O₃.

        치아 우식부의 기계화학적 제거 효과에 대한 연구

        Mechanical removals in decayed teeth have been performed using drill and sharp hand instruments. These methods have some disadvantages such as pain, local anesthesia and overextended cavities Therefore chemo-mechanical excavation of dentin carious lesions has been introduced. The purpose of this study was to evaluate the efficacy of traditional mechanical methods using burs and chemo-mechanical methods (Carisolv) of caries dentin. Mechanical caries removal was carried with low speed round bur Chemo-mechanical caries excavation was performed with Carisolv (Medi-team), using the Carisolv hand instruments. The mean time to remove caries with two different methods was evaluated and the data analyzed with SPSS software (ver 11.5) by t-test (p < 0.05). For histomorphometry of caries removal were also carried with mechanical or chemo-mechanical (Carisolv) methods from 20 extracted caries permanent molars. Complete caries removal was verified with a #23 sharp explorers, Caries Detector (Kuraray Co. Japan), and standard apical radiography. 1. Chemo-mechanical method was taken more times than mechanical method (1.5 fold) (p < 0.05) 2. Excavation for caries took more time for molar lesion than premolar lesion, and the least time was taken to remove the caries in incisor lesion (p < 0.05). 3. There were no significant differences to remove the caries between the maxilla and mandible (p > 0.05). 4. The remaining carious dentin was detected after the ckemo-mechanical removal of the carious dentin, and no smear layer were seen after the mechanical and chemo-mechanical removal of the carious dentin. 본 연구는 우식의 기계적 제거와 기계화학적 제거의 효과를 비교, 평가하고자 하였으며, 기계적 제거방법은 저속의 round bur로, 기계 화학적 제거 방법은 Carisolv로 우식을 제거한 후, 잔존 우식을 확인하여 소요시간을 측정하였고, 또 실험실에서 발치한 우식 대구치 20개를 위 두 가지 방법으로 우식을 제거 후, Villanueva bone stain액으로 염색하고 표본을 제작 후 관찰하여 다음의 결론을 얻었다; 1.우식제거에 평균소요시간은 기계화학적 제거가 기계적인 제거보다 1.5배 더 길게 나타났다 (P < 0.05). 2, 우식제거에 평균소요시간은 소구치보다 대구치에서 더 길었고, 전치에서 가장 적은것으로 나타났다 (P < 0.05) 3. 상 · 하악에서는 차이가 없었다 (P > 0.05) 4. 기계화학적 방법으로 우식 상아질을 제거 시 잔존 우식 상아질이 관찰되었으며 두 방법 모두 우식 상아질을 제거한 표면에서는 도말층을 관찰할 수 없었다.

        표준 셀 라이브러리 P&R 포팅과 테스트 칩의 설계

        본 논문에서는 최신의 미세공정인 0.18㎛ CMOS 공정을 이용한 표준 셀 라이브러리를 설계하고, 이를 P&R(Placement and Routing) CAD 틀에 사용할 수 있도록 포팅한다 제작결과를 검증하기 위하여 간단한 테스트칩을 제작하였으며 설계에 사용된 표준 셀 라이브러리는 0.18㎛ 아남반도체의 공정이다. 이러한 설계 및 제작과정을 통하여 최신의 미세공정을 이용하여 디지털 시스템의 자동설계가 가능함을 확인하였다. In this paper, we design standard cell libraries using the 0.18um deep submircom CMOS process, and port them into a P&R (Placement and Routing) CAD tool. A simple test ship has been designed in order to verify the functionalities of the 0.18um standard cell libraries whose technical process was provided by Anam semiconductor. Through these experiments, we have found that the new 0.18um CMOS process can be successfully applied to automatic digital system design.
