      • Overexpression of EFGR as Prognostic Factor and Effect of EGF in the Progression of Hypopharyngeal Cancer

        연구배경 및 목적 표피성장인자수용체(EGFR)는 HER2/neu(erbB2), HER3(erbB3), HER4(erbB4)를 포함하는 receptor tyrosine ki-nase의 erbB 그룹에 속하는 수용체이다. 표피성장인자수용체의 과발현은 다양한 종류의 암, 특히 두경부편평세포암에서 예후를 악화시킨다고 알려져 있다. 이에 저자들은 하인두편평세포암에서 표피성장인자수용체의 발현 및 분포를 확인하고, 하인두암에서 표피성장인자(EGF)가 암세포의 증식과 침습에 미치는 영향에 대해 알아보고자 하였다. 대상 및 방법 57명의 하인두편평세포암 환자의 조직에서 표피성장인자수용체의 발현을 면역화학적염색을 통해 확인하고, 이에 대해 임상병리학적 요인과 생존율에 대한 분석을 시행하고, 일부 환자의 정상 및 암조직에서 Western blot을 시행하였다. 하인두편평세포암 세포주인 FaDu에서 proliferative assay, colony dispersion, wound healing assay, invasion assay를 시행하여, 하인두암의 진행에서 표피성장인자의 역할에 대하여 분석하였다. 또한 RT-PCR과 Zymography를 통하여 Matrix metalloproteinase(MMP)-2, 9의 발현을 확인하였다. 결 과 63.2%의 하인두편평세포암 조직에서 표피성장인자수용체의 발현이 확인되었다. 표피성장인자수용체의 발현은 정상조직에서 비하여 하인두암 조직에서 유의하게 증가되어 있었으며, 병리학적 병기(p=0.022)가 올라갈수록 유의하게 증가하였으나, 증례수의 제한으로 생존율에서는 통계적 유의성을 얻지는 못했다(p=0.053). in vitro의 결과로 표피성장인자를 FaDu 세포주에 처리하였을 때, FaDu 세포주의 증식이 유의하게 증가되었으며(p<0.05), Transwell invasion chamber상 침습의 증가가 확인되었다(p<0.05). RT-PCR과 zymogram 실험상 표피성장인자처리시 FaDu 세포주의 MMP-2, 9이 발현이 증가되고 활성화되는 것이 확인하였다. 결 론 본 연구에서 표피성장인자수용체의 과발현이 하인두암의 예후 인자로서의 가능성을 확인하였고, 표피성장인자가 하인두편평세포암의 증식과 침습에 관여하는 것을 확인하였다.

        부식성 상부식도 협착증에서 하인두-장 문합에 의한 식도재건술

        대상 및 방법: 저자들은 1995년 8월부터 1997년 1월까지 상부 식도를 포함한 광범위한 부식성 협착증 환자 7례에서 하인두-결장문합에 의한 식도재건술을 시행하였다. 결과: 연령은 20세에서 63세까지 였으며 7례중 6례가 여성이었다. 부식제는 6례에서 강산이었고 1례는 알칼리 용액이었으며, 부식제 연하후 식도재건까지의 기간은 6개월에서 50년이었다. 식도재건 방법으로 4례에서는 하인두- 우결장-공장문합을 시행하였으며, 3례에서는 하인두-우결장-위문합을 시행하였다. 수술사망례는 없었고 7례중 1례에서 만 문합부 협착으로 수차례의 식도 확장술을 받았으며, 6례에서는 경구섭식에 문제가 없어 전례에서 관찰기간중에 4kg 내지 13kg의 체중증가 소견을 보였다. 결론: 결론적으로 상부식도 협착증에서의 하인두-결장 문합술은 유효하고 안전한 술식이라고 할 수 있다. Material and Method: Esophageal reconstruction by the hypopharyngointestinal anastomosis was done in 7 patients of corrosive upper esophageal stricture at St. Mary's Hospital from August 1995 to January 1997. Result: There were one male and six female patients ranging from 20 to 63 years of age. The causative agents were acid in 6 patients and alkali in 1 patient. The esophageal reconstruction was made by hypopharyngcolojejunostomy in 4 patients and hypopharyngocologastrostomy in 3 patients. There were no operative mortalities. One patient developed anastomotic stenosis but others were free from dysphagia. All gained 4 kg to 13 kg of body weight during the follow-up period. Conclusion: In this experience right colon and terminal ileum including ileocecal valve was revealed as a good substitute for the esophagus and the esophageal reconstruction by hypopharyngocologastro (jejuno)stomy seems to be a satisfactory method with acceptable morbidity and mortality in corrosive upper esophageal stricture patient.

        말매미충 (Cicadella viridis L.) 구기내의 미각감각기

        말매미충(Cicadella viridis L.)의 구기에 있는 미각감각기에 대한 주사전자현미경 관찰에서 전구강에 위치한 상인두의 D-sensilla는 10개이며, 상인두 함입부의 양쪽변에서 P-sensilla 8 개가, 하인두의 식도 중간부위에서 H -sensilla가 2개 관찰되었다. D- 및 P-sensilla는 형태적 으로 2군으로 분리되었다. 그리고 D- 및 P-sensilla로부터 유래된 신경다발 1쌍씩은 상인두 뒷면의 큐티클 안에서, H -sensilla로부터의 신경다발 1쌍은 하인두 뒷면의 큐티클 안에서 관찰되었다. The gustatory sense organs in the mouthparts of th adult Green Leafhopper (Cicadella viridis L.) were observed by scanning electron microscope. The gustatory sensilla were composed of ten D-sensilla on the precibarim, eight P-sensilla on the wall of both side of eipharyngeal basin, and two H-sensilla in the food canal of the hypopharynx. D- and P-sensilla were again subdivided into 2 subtypes as two types by their morphology. Beneath the cuticle of epipharynx, two paris of nerve bundles were seen originating from D- and P-sensilla, respectively. And a pair of nerve bundles from H-sensilla, beneath the cuticle of hypoharynx, was observed.

        하인두 내에 발생한 유경성 섬유지방종 1예

        Fibrolipomas are a rare microscopic variant of lipomas, characterized by mature adipose tissue interspersed with dense fibrous connective tissue. These lesions are rarely observed in the head and neck region and represent <0.6% of all benign tumors of the larynx and hypopharynx. Their clinical presentation depends on the location and size of lesions. Patients present with variable symptoms; however, fibrolipomas of the larynx and hypopharynx are clinically important because these tumors can cause unpredictable airway obstruction, particularly during general anesthesia induction. We present a case of a hypopharyngeal fibrolipoma in a 61-year-old patient with frequent dyspnea, dysphagia, and muffled voice. Laryngoscopy revealed a well-demarcated pedunculated ovoid mass involving the larynx. We performed laryngomicrosurgery using a carbon dioxide laser for surgical excision and subsequent evaluation confirmed the diagnosis of fibrolipoma. We report a case of hypopharyngeal fibrolipoma, together with a literature review.

        Fibromatosis is a broad group of benign fibroblastic proliferation that shows locally aggressive growth but never metastasize. Common anatomic sites include abdominal wall, extremity, and mesentery. Little is reported about clini-cal features and outcome of fibromatosis of the head and neck. The treatment of choice is wide excision, which is of-ten difficult. Postoperative recurrence rates are high. We recently confirmed a unique case of fibromatosis occurred on the hypopharynx of 44-year-old male patient and report this interesting case with review of literature.

        하인두의 초기 작품세계 연구

        Ha In-doo was among the young artists who pioneered the 1950s informel movement in Korean contemporary art. Most studies of Ha as an artist, by contrast, begin with works in his series that address religious and other themes, such as “germination” and “humans-love/hatred,” and which date from the early 1970s. This is because Ha"s independent oeuvre is believed to have taken shape from around the latter period onwards. Until now, studies of Ha have tended to regard the time between his informel years and the period in which he produced works embodying Buddhist themes as a kind of vacuum or cut-off. This study is an attempt to reveal the unique, independent sense of form that Ha consistently sought to achieve in the process of embracing various Western artistic trends such as cubism and informalism, during a period that stretched from his early work namely, the 1950sto the 1960s. This study attempts to highlight Ha"s pursuit of “monumentality” as demonstrated by his acceptance of geometric abstraction, as developed from Cezanne to Picasso, in the mid-1950s, and the fascination he showed with the subconsciousness while participating in the informel movement from 1958 onwards, as important factors leading to the search for an inner world that runs through his entire oeuvre. Ha In-doo managed to reach his own unique interpretation of Cezanne"s paintings and cubism. In his view, Cezanne and cubism were a strong and immutable global symbol that had to be built on the ruins left by the Korean War in the 1950s. Having absorbed a nihilistic, tragic worldview from poet Oh Sang-soon in the 1950s, Ha understood the fundamental desire of sculpture to be the placing of monumental structures on meaningless, fickle reality. The works Ha submitted to the “Cheongmaek Exhibition” in Busan in 1957 well illustrate his pursuit of monumentality. Ha In-doo jumped straight in the middle of Informal movement in 1958. His work has received a reputation among members that was especially ideological which was due to a deep interest in Surrealism and the automatism. Ha evaluated automatism as a universal language of contemporary art expression to reveal the hidden subconscious. His fascination for automatism has two significant factors. One is exchanges with Korea surrealist poets and the other one is the effect of Japan Informal movement, Gutai. Gutai recognized automatism as a way to Happening. Ha differently saw automatism as a means of recording the inner consciousness. This is the same recognition that understood Cubism in a spirit of trying to establish a Monumental structure.

      • 하인두에 발생한 활막육종(Synovial Sarcoma) 1예

        Synovial sarcoma is commonly found in the extremities of mesenchymal origin, but rare in the head and neck area. Histopathology is diagnostic and it shows a biphasic pattern with two neoplastic elements; spindle cell, sarcoma like stroma and gland like clefts lined by epitheloid cells. Synovial sarcoma is high-grade neoplasm that expresses epithelial as well as supporting features. This paper presents a case of synovial sarcoma of hypopharynx on 25 years old male. He was treated by surgical excision and postoperative chemoradiotherapy. There were no evidences of local recurrence & distant metastasis for 19 months postoperatively.

        종교예술로서의 청화(靑華) 하인두(河麟斗)의 회화

        Ha, In-doo (1930-1989), also known as Chunghwa, is one of the leading Korean abstract painters whose art can be characterized by a coherent inquiry into the subject matter of religious themes through his works. Not only he lived his daily life as a religious person but he also projected his religious views into his artistic endeavors. The distinctiveness of his abstract religious art was achieved as the result of his persistent efforts to give form to the entities symbolic of the ideas of different religions such as Cheondoism, Buddhism, and Christianity. Throughout his life, Ha was jostled by the oscillation between despair and hope amidst dramatic events, and this forced him to become keenly aware of the presence of human limitations. The inner anxiety caused by this realization in his mind compelled him to ask himself such questions as follows: Who am I?; Why are people born, suffer and die? And it was through religions that he sought to discover the answer to these questions―that is, his ‘real self.’ For Ha, artistic activities were tantamount to the religious practices through which he could attain ‘selfawakening.’ It is at this very point that one is allowed to define his works as ‘religious.’ The religiosity embodied in the paintings of Ha is of ‘the religious’ as the common faith of mankind rather than of specific “religions” as systems of belief in a god or gods. A religion is subject to the attribute of the obedience to or worship of a being or beings whose power is higher than that of man. On the contrary, the notion of “the religious” encompasses all human activities to delve into the question of eternal life through the organic experience of the everyday life. Furthermore, an experience of this “the religious” is directly linked to the fundamental principles underlying ecosystems where man and nature constitute a whole. A certain similarity is, therefore, found between one’s experience of this kind and his or her expression of his or her awe to nature. Then, it can be safely said that the process through which the self of the artist dispelled his anxieties and sufferings and fed and expanded his life by becoming one with cosmic nature is, too, one example of “artistic experiences as the religious.” His real self given birth to by this experience and the paintings as its symbolic embodiments are indeed what substantiates the truth of the religious art of Ha In-doo.
