      • 기저 세포암에서 P53 단백의 발현

        피부과 영역에서 가장 흔한 기저 세포암의 파라핀 블록을 대상으로 종양 억제 유전자로 알려진 P53 유전자 단백의 발현 양상을 면역 조직 화학 염색 방법으로 관찰하였다. 전체 기저 세포암 11예종 8예에서 양성을 보여 73% 양성율을 보였으며 태양 광선 노출부의 기저세포암은 6예중에 4예에서 비노출부위는 5예중 4예에서 양성을 보여 차이를 발견할 수 없었다. 이러한 사실은 P53 단백의 과발현과 기저 세포암의 생성은 밀접한 관련이 있으리라 생각되며 기저 세포암의 P53 단백의 과발현은 자외선 뿐 아니라 다른 발암원에 의해서도 유발되리라 시사해 준다. A total of 11 specimens of histologically determined, cutaneous basal cell epithelioma from 11 patients were investigated for the incidence of mutant P53 protein expression by LSAB(Labelled Streptavidine Biotin) technique using monoclonal antiserum NCL-P53-Do-7 on paraffin embedded formalin fixed tissue sections. 8 specimens(73%) showed unclear positivity and in 4 cases tumors arose on sun exposed skin, in 4 cases tumors arose in non sun exposed sikn. This study suggested that accumulation of P53 protein, suggestive in many cases of P53 gene mutation and hence loss of tumor suppressive function, may occur as an important step in the development of basal cell epithelioma.

      • 장상피화생과 Helicobactor pylori와의 상관 관계에 관한 연구

        In 1965 Lauren divided advanced gastric carcinoma into two main types-namely, "intestinal" and "diffuse" type carcinoma, which differ not only morphologically but also in their clinical and epidemiological characteristics. Moreover, a different histogenetic process has been postulated by many authors that intestinal type gastric carcinoma might arise from areas with intestinal metaplasia. Since Warren and Marshall reported the presence of Helicobacter pylori in the stomach in 1983, strong evidence has been obtained that there is a close relationship between Helicobacter pylori and chronic gastritis, atrophic gastritis, intestinal metaplasia, and intestinal type gastric carcinoma. To study the relationship between intestinal metaplasia and Helicobacter pylori infection, we performed gastroscopic biopsies and CLO tests taken from patients with epigastric pain. Only 87 patients with macroscopically suspected antral gastritis were included. The prevalence of intestinal metaplasia in gastric antral biopsies increased from 33.3% in the age group<30 years to 63.6% in the age group 60 years. When all patients were divided into two age groups-namely, (i)<50 years(n=41) and (ii)≥ 50 years(n-=46), intestinal metaplasia was found significantly more often in patients ≥ 50 years. ((i) 39.2%, (ii) 54.3%, p<0.0001). The prevalence of Helicobacter pylori in gastric antral biopsies decreased from 66.7% in the age group<30 years to 62.5%, in the age group in the age group 50 to 59 years. When all patients were divided into the same two age groups-namely(i)<50 years and (ii)≥50 years, the prevalence of Helicobacter pylori did not differ between two groups in a statistically significant way. The mean age of intestinal metaplasia positive patients who were also Helicobacter pylori positive was 48±11.0(mean±S.D.)years, whereas the mean age of intestinal metaplasia positive patients who were Helicobacter pylori negative was 52±9.7(mean±S.D.)years. In conclusion. although our data do not prove a causal relationship between Helicobacter pylori infection and the histogenesis of intestinal metaplasia, we suggest that Helicobacter pylori plays an important role in the development of intestinal metaplasia in the gastric mucosa. Whether Helicobacter pylori has to be present at all times during this process or is only necessary as a 'trigger', needs further research.

      • Epidemiological analysis of acne vulgaris according to the age group including childhood acne: a multicenter study in Korea

        Background: Despite the prevalence of acne is increasing gradually in the population of Korea, relatively little is known about its epidemiology. Also the incidence of childhood acne is unknown. Objectives: Investigation about the number and distribution of acne according to age group by analyzing epidemiological data from several general hospitals in Korea. Methods: Retrospective multi-center study of patients who are diagnosed with acne vulgaris through department of dermatology in Korea. Results: 1. Acne patients have been increased by 60% in number compared to 10 years ago. The number of childhood acne also increased 10-15% annually during the recent 4 years and the number of patients with age under 19 has been doubled during 10 years. 2. Age under 12 accounted for 11% of total acne patients and showed 20% increase in number during 4 years. Although upper grade of elementary school patients (age 10-12) have biggest portion among the patients under 12, the number of patients under the age of 9 has been also increased rapidly by 73% compared to 10 years ago. Conclusion: This was the first large scale, multi-center study and analysis done in Korea including 14 general hospitals with maximum 10 years of long term follow-up. We could find out that acne patients are increasing consistently and the proportion of group under 19 is getting larger. Additionally onset age of acne is getting younger. So it is important to raise the public awareness of childhood acne for early management

        Regulation of Inhibitors of Differentiation Family Proteins by Thyroid-Stimulating Hormone in FRTL-5 Thyroid Cells

        <P>Members of the inhibitors of differentiation (Id) family of helix-loop-helix (HLH) proteins are known to play important roles in the proliferation and differentiation of many cell types. Thyroid-stimulating hormone (TSH) regulates proliferation and differentiation by activating TSH receptor (TSHR) in thyrocytes. In this study, we found that Id2, one of the Id family proteins, is a major target for regulation by TSH in FRTL-5 thyroid cells. TSH rapidly increases the Id2 mRNA level in FRTL-5 thyroid cells but the Id2 protein showed biphasic regulatory patterns, being transiently reduced and subsequently induced by TSH treatment. Transient reduction of Id2 protein was noted within 2 hr of TSH treatment and was mediated by proteasomal degradation. Moreover, reduced Id2 expression correlated with the activity of the phosphatidylinositol 3 kinase pathway, which is activated by TSH. Although TSH increases the activity of the Id2 promoter, TSH-induced activation of this promoter was independent of c-Myc. Id2 did not alter TTF-1- and Pax-8-mediated effects on the regulation of the Tg promoter. Thus, in summary, we found that TSH regulates Id2 expression, but that Id2 does not alter the expression of thyroid-specific genes, such as Tg, in FRTL-5 thyroid cells.</P>

        Relaxation Patterns of Human Gastric Corporal Smooth Muscle by Cyclic Nucleotides Producing Agents

        To elucidate the mechanism of cyclic nucleotides, such as adenosine 3',5'-cyclic monophosphate (cAMP) and guanosine 3',5'-cyclic monophosphate (cGMP), in the regulation of human gastric motility, we examined the effects of forskolin (FSK), isoproterenol (ISO) and sodium nitroprusside (SNP) on the spontaneous, high $K^+$ and acetylcholine (ACh)-induced contractions of corporal circular smooth muscle in human stomach. Gastric circular smooth muscle showed regular spontaneous contraction, and FSK, ISO and SNP inhibited its phasic contraction and basal tone in a concentration-dependent manner. High $K^+$ (50 mM) produced sustained tonic contraction, and ACh $(10\;{\mu}M)$ produced initial transient contraction followed by later sustained tonic contraction with superimposed phasic contractions. FSK, ISO and SNP inhibited high $K^+$-induced tonic contraction and also ACh-induced phasic and tonic contraction in a reversible manner. Nifedipine $(1\;{\mu}M)$, inhibitor of voltage-dependent L-type calcium current $(VDCC_L)$, almost abolished ACh-induced phasic contractions. These findings suggest that FSK, ISO and SNP, which are known cyclic nucleotide stimulators, inhibit smooth muscle contraction in human stomach partly via inhibition of $VDCC_L$.

      • Clinical outcomes of adult patients with plaque psoriasis treated with ustekinumab under real-world practice in Korea: a prospective, observational, multi-center, postmarketing surveillance study

        Background: Postmarketing surveillance is conducted to establish drug safety and effectiveness under real practice. Objectives: To validate ustekinumab’s effectiveness and safety in the treatment of adult Korean patients with plaque psoriasis under real practice. Methods: This was a prospective, observational, and multi-center study. The study subjects were aged 18 years and above, who were administered ustekinumab for treating plaque psoriasis. Results: The study included 977 patients; 66.9% were male, with a mean±standard deviation of BSA(%) of 27.0±18.3 and mean PASI of 18.1±9.7. The effectiveness analysis included 581 patients who had at least one follow-up assessment and met treatment criteria per local label and reimbursement guidelines; of these patients, 287 had effectiveness data for visit 6. The timing of visit 6 occurred at 53.7±2.1, weeks; at visit 6 91.6%, 51.2%, and 9.4% achieved PASI75, PASI90, and PASI100 responses, respectively. Adverse events (AEs) occurred in 11.5% yielding an incidence rate of 21.5/100 patient-years (PYs). Serious AEs occurred in 0.8% with an incidence rate of 1.2/100 PYs. The estimated 1-year drug retention rate was 87.7%. Multiple logistic regression analyses showed that higher baseline PASI score and no prior biologic exposure were significant predictors for PASI90 response at visit 6. Conclusion: Ustekinumab is effective and safe, with high drug retention in the treatment of adult Korean patients with plaque psoriasis in real-world practice.

      • 시선제어 시스템에서의 비선형 토크보상 적용

        Line of sight(LOS) control systems are precision electro-optical-mechanical gimbal assemblies designed to control the angular position of the LOS of a sensor precisely, therefore they are isolated from the base vehicle dynamics and LOS is pointed toward the target in a desired direction. Nonlinear torque disturbances in their systems cause LOS jitter which limit system performance. Major sources of nonlinear disturbance are friction torque by rotation and gimbal imbalance torque by nonlinear environment. This paper describes the compensation technique for reducing LOS jitter due to nonlinear friction coupling of the host vibration. Proposed compensator controls the feedforward torque command of plant adaptively to converge the error between reference model without friction and plant output to zero. Simulation results show that proposed compensator is effective in cancelling the nonlinear torque and improving the stabilization performance.

        시호의 수용액층으로부터 항고지혈 활성 성분의 분리

        In our previous report, the water extract of Bupleuri radix showed the lowering effect of total cholesterol level in the experimentally induced hyperlipidemic rats. In order to elucidate the active ingredient(s), the water extract were dialyzed, ad two fractions, high molecule fraction and low molecule fraction, were obtained. These two fractions were tested on the experimentally induced hyperlipidemia in rats for lowering effect of serum lipoprotein contents. Hyperlipidemia was induced on male Wistar rats by feeding high cholesterol diet for 7 days. After oral administration of above samples for 4 weeks, serum lipids profile was monitored by measuring total cholesterol, triglyceride, high density lipoprotein cholesterol and low density lipoprotein cholesterol level. Low molecule fraction decreased total cholesterol and triglyceride levels significantly (p<0.05) at 4 week point when they were compared with the control group.

        Shock 환자에서 혈청 Myoglobin 치의 변화

        Serum myoglobin levels were measured by radioimmunoassay in 11 healthy subjects and 15 patients with shock, of which 11 cases were hypovolemic shock and 4 cases were septic shock. The follow-up measure of serum myoglobin levels was performed in 9 patients at intervals of 8 hours. Also the serum creatinine, PaO2, blood hydragen ion concentration, blood pressure, and urine output were measured simultaneously. The results were as follows: 1) The mean value of serum myoglobin in 11 healthy subjects was 20,5±3.1ng/ml. 2) The serum myoglobin level was increased in the patient with hypovolemic shock (552.0±188.6ng/ml) compared to the control group (p$lt;0.001). In patients with septic shock, the serum myoglobin level was markedly increased (4366.7±348.0ng/ml) compared to the control group (p$lt;0.001) and patients with hypovolemic shock (p$lt;0.001). 3) The serum myoglobin level shows a decreasing tendency after hydration and was normalized 16 hours after recovery of low blood pressure and law urine output (71.3±11.9ng/ml). 4) There was a significant relationship between serum myoglobin and blood hydrogen ion concentration (r=0.5, p$lt;0.05). 5) There was a significant negative correlation between serum myoglobin and PaO2 (r=0.7, p$lt;0.01). 6) There was a significant relationship between se- rum myoglobin and serum creatinine (r=0.8, p$lt;0.001). The above results suggest that in shock patients, the rapid recovery of blood pressure and urine output by hydration is important to protect the deterioration of renal function by myoglobin molecules.

        통신기기 및 부품산업 핵심체화기술요소의 분석

        본 연구는 우선 국내 통신기기 및 부품산업에서 사용하는 순수기술요소(하드웨어 체화기술요소)와 이들을 상업화하는데 필요한 관리기술요소(소프트웨어 체화기술요소)중 국내산업이 경쟁력을 갖도록 하는데 중요한 역할을 한 기술 요소가 무엇이었는지를 설문조사를 통해 밝혀내었다. 다음에는 요인분석을 통해 두 유형의 기술요소들을 몇 개의 기술군으로 묶은 다음, 이 기술군들이 상업화 성공에 미치는 영향력의 정도를 공분산분석을 통하여 살펴보았다. 분석결과 하드웨어 체화기술군들이 기술개발성과에 미치는 순수효과는 가술군에 따라 차이가 많았다. 반면에 소프트웨어 체화기술군의 순수 효과는 상대적으로 차이가 적었으며, 하드웨어 체화기술군과 연계되었을 때의 시너지 효과는 모든 경우에 많은 긍정적인 효과를 보였다. 마지막으로 현재 국내산업 하드웨어 기술 요소들의 절대적 수준을 선진국의 것과 비교하여 봄으로써 기술개발 전략수립에 참고가 되도록 하였다.
