        석면폐증 1례

        Asbestosis is the disease of pulmonary fibrosis caused by the inhaled asbestos fibers, and could be diagnosed clinically, in the case of exposure history to asbestos is proved, by clinical symptoms of dyspnea or dry cough, physical examonation findings, and the radiographic features. But many othder inorganic dusts would show similar findings in the chest radiogram and sometimes the exposure history is obscure, so for the exact diagnosis of asbestosis lung biopsy is needed. In Korea, there have been some reports of survey in the workplace where asbestos is handled or of asbestos related diseases. This is a case report of asbestosis with accompanying pleural plaques, who had the occupational exposure to asbestos for 30 years and the consistent clinical, radiographic and pathological findings in the lung tissue obtained by the videoscope assisted thoracoscopic biopsy(VATS).

      • 히스토아크릴을 이용한 십이지장 궤양 출혈의 지혈 후 발생한 급성췌장염

        Upper gastrointestinal bleeding is common and potentially life threatening medical emergency. Emergency endoscopy is the first choice of diagnostic and treatment measure for patients with active upper gastrointestinal bleeding. Histoacryl should be considered a second-line treatment modality when conventional treatments were failed. However, it might cause several complications. Herein, we report a case of acute pancreatitis developed after histoacryl injection therapy for active duodenal ulcer bleeding. A 71-year-old man was admitted with melena and hematemesis. On emergency endoscopy, a 2 cm sized active ulcer with bleeding from an exposedvessel was seen at the duodenal bulb. Attempts to arrest the bleeding with hemoclipping and submucosal epinephrine injection were tried, but failed. We changed the method to endoscopic histoacryl injection, and obtained hemostasis immediately. A few hourslater, after successful hemostasis, patient complained diffuse abdominal pain. Ultrasonography revealed hyperechoic heterogenous diffuse pancreatic enlargement and right pararenal space fluid collection, this ultrasonographic findings and elevated serum pancreatic enzymes are compatible with acute pancreatitis.

        고포도당이 백서 사구체 메산지움 배양세포에서 p38 MAPK 활성화 및 Fibronectin 생성에 미치는 영향

        목 적 : 고포도당으로 자극한 백서 메산지움 배양세포에서 p38 mitogen activated protein kinase (MAPK)의 활성화와 p38 MAPK의 상부와 하부인자로 알려져 있는 MAPK kinase 3/6 (MKK3/6)와 c-AMP responsive element binding protein (CREB)의 활성화, 그리고 fibronectin 합성을 자극 시간에 따라 관찰하고, p38 MAPK 경로와 fibronectin 합성 사이의 연관성을 규명함으로서 당뇨병성 신증의 병태생리를 규명하고자 하였다. 방 법 : 백서 메산지움 배양세포를 정상 포도당 (5.6 mM), 고포도당 (30 mM), 또는 정상 포도당+만니톨 (24.4 mM)로 자극한 후 첫째 p38 MAPK; 둘째 p38 MAPK의 상부 인자인 MKK3/6; 셋째 p38 MAPK의 하부인자인 CREB; 넷째 MAPK phosphatase-1 (MKP-1)의 변화를 자극 시간 (3분-48시간)에 따라 Western blot을 이용하여 관찰하였으며, 다섯째 fibronectin의 변화는 RT-PCR과 ELISA를 이용하여 확인하였다. 결 과 : 고포도당으로 자극한 백서 사구체 메산지움 배양세포에서 p38 MAPK 경로가 활성화 되었다. Total p38 MAPK 단백은 변화가 없었으나, 활성화된 phospho-p38 MAPK 단백은 고포도당군에서 자극 10분 후부터 정상 포도당군에 비해 의의있게 증가되었으며 (평균 1.9배), phospho-CREB 단백 역시 고포도당군에서 자극 10분 후부터 의미있게 증가되었다 (평균 2.5배). 이에 비해 p38 MAPK의 상부인자인 phospho-MKK3/6는 고포도당군에서 자극 3분 후부터 의의있게 증가되었다 (평균 2.4배). 또한, MKP-1 단백 발현은 고포도당군에서 p38 MAPK의 활성이 증가하기 시작한 시기와 동일한 시기 (자극 10분 후)부터 증가하기 시작하였다 (평균 1.9배). Fibronectin mRNA와 세포 배양액내 fibronectin은 고포도당군에서 자극 48시간 후에 각각 1.7배와 1.5배 증가되었으며, 이러한 증가는 p38 MAPK 억제제인 SB203580 (1 μM)에 의해 각각 73%와 69% 억제되었다. 결 론 : 고포도당으로 자극한 백서 사구체 메산지움 배양세포에서 p38 MAPK 경로의 활성화를 확인하였으며, p38 MAPK 억제제가 고포도당에 의한 fibronectin의 합성 증가를 억제시키는 것으로 보아 당뇨병성 신증에서 관찰되어지는 fibronectin의 축적에 p38 MAPK 경로가 관여할 것으로 생각된다. Background : The p38 mitogen-activated protein kinase (MAPK) pathway is activated by several stress factors, potentially leading to cellular apoptosis and growth. Little is known about the pattern of p38 MAPK pathway activation in mesangial cells under high glucose conditions. We examined the activity and expression of p38 MAPK members, 1x78 MAPK, MAPK kinase 3/6 (MKK3/6), c-AMP responsive element binding protein (CREE), and MAPK phosphatase-l (MKP-1) in cultured rnesangial cells exposed to high glucose. Methods : Mesangial cells were subcultured from rat glomeruli isolated by sieving technique. After serum restriction for 48 hours rnesangial cells were exposed to 5.6 mM glucose (low glucose, LG), 5.6 mM glucose+24.4 mM mannitol (LG+M), or 30 mM glucose (high glucose, HG) for 3 minutes to 48 hours with or without SU203580. Western blot was performed to determine the activity and protein expression of p38 MAPK members. R`I-I`CR and ELISA were performed for fibronectin mRNA expression and fibronectin synthesis, respectively. Results : p38 MAPK and CREB activities were significantly increased in rnesangial cells exposed to FIG compared with LG or LG+M after 10 minutes and was sustained at higher levels to 48 hours (p<0.05), but total p38 MAPK and CREB protein expressions did not differ. MKP--1 showed a similar pattern as p 3 MAPK and CREE (p<O.O.i). MKK3/6 acitvity was significantly higher in HG cells after 3 minutes and remained at higher levels throught the study period (p<0.05). Fibronectin mRNA expression and fibronectin synthesis were significantly increased in mesangial cells exposed to HG after 48 hours (p<0.05). SB203580 (1 pM) pretreatment for 1 hour significantly reduced HG-induced fibronectin mRNA expression and fibronectin synthesis by 73% and 69%, respectively (p<0.05). Conclusion : p38 MAPK activity was increased in mesangial cells exposed to HG in parallel with increased MKK3/6 activity, resulting in CREB activation and increased fibronectin synthesis. This activated p38 MAPK may play a role in the pathogenesis of diabetic nephropathy.

        ISO 9000 Quality System has been proved it's effectiveness in the advanced countries for years. And many companies in the world has been accepting the Quality System resulting in increasing of sales, decreasing of poor quality goods or complaints and elevating sense of quality, etc. Still, however, in the construction industrial fields, there are several problems to solve in the process of accepting the Quality System such as surveys for speciality of constructing site. If government organization in charge of operating the System, Accreditation Body, Certification Body, research institute and certificated construction companies are searching for the improving devices organically, ISO 9000 Quality System will be settled reasonably in the construction industry and become a good system to guarantee the construction quality and working system.

        혈압 조절 후 신기능의 호전을 보였던 중증 고혈압 환자들의 임상 특징 분석

        배 경 : 본태성 고혈압 환자의 10-40%에서 경도의 신기능 저하를 동반하는 것으로 보고되어 있으며, 고혈압 환자에서 신부전이 동반된 경우 심혈관 사망률이 높은 것으로 알려져 있다. 본 연구에서는 내원 당시 중증 고혈압과 동반된 신부전 소견을 보였으나, 적극적인 혈압 조절 후 신기능의 호진을 보인 환자들의 임상 특징을 분석해 보고자 하였다. 방 법 : 1993년 1월부터 2002년 12월까지 10년간 가톨릭의대 강남성모병원 및 성바오로병원에서 중증 고혈압 및 신부전증 (혈청 크레아티닌치 2.0 ㎎/dL 이상)으로 진단된 환자들 중에서 혈압 조절 후 신기능의 호전 (혈청 크레아티닌의 감소)을 보인 환자들을 대상으로 이들의 평균 동맥압 및 혈청 생화학 검사치 변화를 분석하고, 신생검 소견 및 투어 혈압 약제를 분석하였다. 결 과 : 전체 14명의 환자 중 남자는 8명 여자는 6명이었고, 이들의 평균 연령은 38±8세 였다. 평균 동맥압의 유의한 감소와 더불어 혈청 크레아티닌치의 감소 (p=0.001)와 헤마토크리트치의 유의한 증가 (p=0.001) 소견이 관찰되었다. 그러나 혈청 요산, 총콜레스테롤, 중성지방 농도의 유의한 변화는 없었다. 신초음파 검사상 정상 신장 크기를 보인 경우는 12명 (80%)이었고, 대상 환자군은 대부분 3가지 이상의 항고혈압제를 병합 투여하여 적극적인 혈압 조절을 하였으며, 전환효소차단제 및 안지오텐신 Ⅱ 수용체 길항제 (22%), 칼슘통로차단제 (21%), 베타차단제 (21%)를 주로 투여하였다. 신생검을 시행한 6예를 분석한 결과, 4예는 고혈압성 신경화증, 2예는 IgA 신증으로 진단되었다. 6예 모두 신세동맥의 초자질 침착과 혈관내피의 점액양 변성을 동반한 신세동맥의 비후 소견이 존재하였고, 경도에서 중등도의 세뇨관 위축 소견과 간질성 섬유화 소견이 관찰되었다. 결 론 : 신부전을 동반한 중증 고혈압 환자들 중, 신장 크기가 정상이고 나이가 젊은 환자의 경우, 혈압 조절을 통해 신기능을 호전시킬 수 있으므로, 이들에 대한 적극적인 혈압 조절이 필수적이라 하겠다. Background : Mild renal dysfunction is relatively common in patients with long standing primary hypertension, ranging from 10% to 40% in various studies. The presence of renal dysfunction is associated with high cardiovascular mortality and morbidity rates of patients with primary hypertension. The purpose of this study is to analyze the clinical characteristics of patients with severe hypertensin and reversible renal dysfunction after blood pressure control. Methods : This retrospective study enrolled 14 patients with severe hypertension and reversible renal dysfunction after blood pressure control, between January 1993 and December 2002 at Kangnam St. Mary`s Hospital and St. Paul`s Hospital. We investigated the laboratory data using Wilcoxon signed rank test, and analysed renal biopsy findings and antihypertensive drugs. Results : The mean age of the patients was 38±9 years and the number of male patients was 8. During 33.5±28.8 months of mean follow-up period, there was a significant decrease in mean arterial pressure and serum creatinine level, and significnat increase in hematocrit level. But there was no significant changes in the level of uric acid, total cholesterol, and triglyceride. Each patient took more than 3 antihypertensive drugs consisting angiotensin converting enzyme inhibitor or angiotensin Ⅱ recep색 blocker (22%), calcium channel blocker (21%), and beta blocker (21%). Renal biopsy was done in 6 cases, and histologic diagnosis resulted in 4 cases of benign hypertensive nephrosclerosis and 2 cases of IgA nephropathy. The typical morphological features of hypertensive nephrosclerosis were seen in all cases, and there were varying degrees of glomerular sclerosis from 0% to 92%. But the percent of glomerular sclerosis was not related to the level of initial serum creatinine, mean arterial pressure, and amount of proteinuria per day. Conclusion : Careful monitoring of renal function and effective treatment of blood pressure are therefore mandatory in treating young patient with severe gypertension with renal dysfunction.         Physicochemical and Antioxidant Properties of Red Ginseng Starch, a By-Product of the Red Ginseng Industry

        Red ginseng starch (RGS) is one of the by-products of the manufacturing process of red ginseng concentrate. In this study, the total ginsenoside, phenol, and flavonoid contents in RGS were measured to be 2.9, 7.5, and 0.4 mg/g, respectively, while the 1,1- diphenyl-2-picrylhydrazyl and 2,2'-azinobis (3-ethylbenzothiazoline- 6-sulfonic acid, ABTS) levels were increased by 50% at 471 and 270 μg/mL of RGS. This by-product of the red ginseng industry may thereby prove to be a functional cosmetic and food material.

        Bouncing과 Sliding에 의한 55 kW급 전기 트랙터의 조향 불안정성 수치해석

        Tractors are used for farming in harsh terrain such as slopes with slippery fields and steep passages. In these potentially dangerous terrain, steering instability caused by bouncing and sliding can lead to tractor rollover accidents. The center of gravity changes as parts such as engines and transmissions used in existing internal combustion engine tractors are replaced by motors and batteries, and the risk of conduction must be newly considered accordingly. The purpose of this study was to derive the center of gravity of a 55 kW class electric tractor using an electric platform from an existing internal combustion engine tractor, and to numerically investigate the tractor steering instability due to bouncing and sliding. The analysis provides a strong analysis by integrating an elaborate combination of a bouncing model and a sliding model based on Coulombs theory of friction. Various parameters such as tractor speed, static coefficient of friction, bump length and radius of rotation are carefully analyzed through a series of detailed designs. In particular, the simulation results show that the cornering force is significantly reduced, resulting in unintended trajectory deviations. By considering such complexity, this study aims to secure optimal performance and safety in the challenging terrain within the agricultural landscape by providing valuable insights to improve tractor safety measures.
