        From May to July 1993, we observed an epidemic of aseptic meningitis. We performed clinical and virological studies on 174 children with aseptic meningitis who had been admitted to the Department of Pediatrics, Asan Medical Center, from May to July, 1993 The results were as follows; 1) This epidemic of aseptic meningitis was most prevalent in June 1993. The most of the patients were 3 to 6 years old and male to female ratio was 1.6 : 1. 2) The most patients suffered from fever, vomiting, headache, and abdominal pain was noticed in 36% of patients, characteristically. 3) In 7 cases among 10 randomly sampled cases, enterovirus was cultured. Among them, we performed serotyping in 3 cases and Echovirus 9 was isolated.

        The Need for a Well-Organized, Video-Assisted Asthma Education Program at Korean Primary Care Clinics

        Background: The purpose of this study was to assess the effect of our new video-assisted asthma education program on patients' knowledge regarding asthma and asthma control. Methods: Adult asthmatics who were diagnosed by primary care physicians and followed for at least 1 year were educated via smart devices and pamphlets. The education sessions were carried out three times at 2-week intervals. Each education period lasted at most 5 minutes. The effectiveness was then evaluated using questionnaires and an asthma control test (ACT). Results: The study enrolled 144 patients (mean age, $56.7{\pm}16.7years$). Half of the patients had not been taught how to use their inhalers. After participating in the education program, the participants' understanding of asthma improved significantly across all six items of a questionnaire assessing their general knowledge of asthma. The proportion of patients who made errors while manipulating their inhalers was reduced to less than 10%. The ACT score increased from $16.6{\pm}4.6$ to $20.0{\pm}3.9$ (p<0.001). The number of asthmatics whose ACT score was at least 20 increased from 45 (33.3%) to 93 (65.3%) (p<0.001). The magnitude of improvement in the ACT score did not differ between patients who received an education session at least three times within 1 year and those who had not. The majority of patients agreed to the need for an education program (95.8%) and showed a willingness to pay an additional cost for the education (81.9%). Conclusion: This study indicated that our newly developed education program would become an effective component of asthma management in primary care clinics.

      • Phase II, multicentre, randomised trial of eribulin plus gemcitabine versus paclitaxel plus gemcitabine as first-line chemotherapy in patients with HER2-negative metastatic breast cancer

        <P><B>Abstract</B></P> <P><B>Background</B></P> <P>Paclitaxel plus gemcitabine (PG) combination chemotherapy is a preferred chemotherapeutic regimen for patients with metastatic breast cancer (MBC). Eribulin mesylate is a halichondrin non-taxane inhibitor of microtubule dynamics. A recent pooled analysis with eribulin showed improved overall survival (OS) in various MBC patient subgroups pretreated with anthracycline and taxane. Furthermore, eribulin may have less neurotoxicity than paclitaxel.</P> <P><B>Patients and methods</B></P> <P>This study was a prospective randomised phase II, open-label, two-arm, multicentre study comparing eribulin plus gemcitabine (EG) with PG chemotherapy as a first-line treatment for patients with human epidermal growth factor receptor 2-negative MBC. We hypothesised that EG chemotherapy would not be inferior to PG chemotherapy. The primary end-point was progression-free survival (PFS), which was estimated to be 70% at 6 months for each arm. The secondary end-points were as follows: OS, neuropathic scale, toxicity and clinical benefit rate.</P> <P><B>Results</B></P> <P>A total of 118 patients (median age: 50, 24–66) were enrolled between March 2015 and March 2016 and were randomly assigned to PG (<I>n</I> = <I>59</I>) or EG (<I>n</I> = <I>59</I>) chemotherapy. The mean number of metastatic sites was 3 (range 1–8). The 6-month PFS rates for both arms were 72% for EG and 73% for PG (<I>P</I> = <I>0.457</I>). There was no significant difference in OS between the two groups (not reached versus 21.2 months, <I>P</I> = <I>0.2234</I>). The median number of chemotherapy cycles for both groups was 10 for EG and 8 for PG (range 2–32). Clinical benefit rates were 44% for EG and 49% for PG. Major toxicities were neutropenia and neurotoxicity.         Case Report : Anesthetic experience of a patient with hereditary factor 11 deficiency (Hemophilia C)

        Factor XI deficiency (also called Hemophilia C) rarely occurs among ethnicities other than Ashkenazi Jews. A boy was scheduled for frontoethmoidectomy due to bilateral chronic rhinosinusitis. He was incidentally found to have factor XI deficiency due to prolonged aPTT on preoperative laboratory finding. His medical history reveals frequent epistaxis 2 or 3 times per day and his factor XI and XII activity were 17% (normal; 60-140%) and 34% (normal; 60-140%), respectively on furthermore laboratory evaluation. He was diagnosed as hereditary factor XI deficiency. He underwent the operation with administration of the fresh frozen plasma without complication. (Korean J Anesthesiol 2009; 56: 706~8)

        Immunomodulatory Effect of a New Herbal Preparation (HemoHIM) in Cyclophosphamide-treated Mice

        The immunomodulatory effect of a new herbal preparation, HemoHIM, on the recovery from leukopenia induced by cyclophosphamide treatment was investigated. The HemoHIM was made up with an addition of the ethanol-insoluble fraction to the total water extract of Angelica Radix, Cnidii Rhizoma and Paeonia Radix. Daily oral administration of 100 mg/kg BW or 500 mg/kg BW HemoHIM accelerated the recovery from cyclophosphamide-induced leukopenia. HemoHIM increased the number of leukocytes and lymphocytes in the peripheral blood when compared with the cyclophosphamide-treated control. Moreover, the suppressed natural killer (NK) cell activity and interferon $(IFN)-{\gamma}$ secretion in the cyclophosphamide-treated mice were restored by the administration of HemoHIM. HemoHIM significantly reduced the abnormally heightened ratio of interleukin $(IL)-4/IFN-{\gamma}$ and immunoglobulin (Ig)E/IgG2a in the cyclophosphamide-treated mice. These results suggest that HemoHIM accelerates the recovery from leukopenia and alleviates the imbalanced T helper (Th)l/Th2 responses in the cyclophosphamide-treated mice. Additionally, HemoHIM was found to stimulate normal splenocytes to secrete not only Thl type cytokines such as $IFN-{\gamma}$ and IL-2, but also Th2 type cytokine IL-4. In conclusion, our results show that HemoHIM certainly has an influence on the balanced recovery of immune cells and the activation of their activities in the cyclophosphamide-treated mice.

      • Dye-Ligand Affinity Chromatography를 이용한 Staphylococcus aureus의 장독소 B 분리

        Staphylococcal enterotoxins, which cause staphylococcal food poisoning, have been purified by several methods. These procedures have usually included various combinations of ion-exchange chromatogram and gel filtration; such procedures can be time-consuming and may also result in low recoveries of enterotoxins. Although high recoveries of staphylococcal enterotoxins have been reported with chromatofocusing, this technique is expensive and cannot easily be adapted for large-scale purification. In this study we applied a simple, single-step procedure for the purification of Staphylococcal enterotoxin B (SEB) from culture supernatant fluids, namely, dye ligand affinity chromatography in which dye is coupled to an agarose support matrix. Dye ligand affinity chromatography has been used to purify a wide range of enzymes and some blood proteins. We used ten kinds of dye ligand affinity columns. The results showed that SEB had the binding ability to Red A, Reactive Green 19, Reactive Green 5, Reactive Brown 10 and Cibacron Blue 3GA-Agarose. We had the highest resolution by using the Reactive Green 19 dye affinity column. The purified SEB produced a single band when subjected to sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS PAGE) and western blot using anti-SEB. We could also confirm that the purified SEB produced superantigenic effect. Therefore, it could be concluded that SEB was produced purely in single step process by dye-ligand affinity chromatography. Such a method will form the basis of a high-yielding, rapid means of purifying the staphylococcal enterotoxins that can easily be adapted to large-scale production.

      • 충청지방에서 발생한 한국형출혈열 54예의 임상적 고찰

        In Dae Jeon Eul-Ji General Hospital, activated since 1581, 54 patients with Korean Hemorrhagic Fever Who were all positive for Korea antigen were observed from January, 1981 to December, 1984. Epidemiology, clinical features, laboratory findings, complications and clinical course in Korean Hemorrhagic Fever were analyzed. The results were as follows: 1. The most prevalent area was OK-Chun Kun. 2. The peak incidence of age was fifth decade and male to female ratio was 2:1. 3. The peak incidence was November to December. 4. The most frequent symptomes were epigastric pain, nausea and vomiting and followed by hemsturia. 5. Conservative treatment was done and hemodialysis was performed in 19 patients without death.

        단심실증의 수술치험 2례

        A modified Fontan procedure was performe on two patients with univentricular heart. The first patient had UVH of right ventricular type with trabecular pouch and had various associated anomalies, such as common atrium, common atrioventricular valve and combined pulmonary stenosis. The second patient had UVH of left ventricular type with ourlet chamber and the associated anomalies were atrial septal defect, tricuspid stenosis and combined pulmonary stenosis. Postoperative hemodynamic insufficiency, fluid retention and renal insufficiency were occured in the first patient, but relieved with the aid of inotropics and vasodilators. We thought that the good postoperative course and surgical result were gained from the widely patent atriopulmonary anastomosis.
