A 80 year-old man ingested hydrochloric acid as suicidal purpose and as a result developed an epigastric fullness and pain. Marked stricture of esophagogastric junction and gastric outlet were found by endoscopic examination after 20 days of admission. A total gastrectomy and Roux-en-Y esophagojejunostomy was performed. Grossly, resected stomach revealed redness and erosion especially over antrum and pylorus. On cross section, a flat elvated lesion measuring 0.7×0.5 cm was incidentally found on high body along lesser curvature. On microscopic examination, the mass consisted of atypical epithelial glands having increased cellularity and round hyperchromatic nuclei without invasive foci. The lesion was diagnosed with ‘epithelial dysplasia, high grade’. Corrosive gastritis and submucosal epithelial heterotopia were found in surrounding mucosa also.

Toxoplasmosis is a rare but fatal complication in hematopoietic stem cell transplant recipients, usually associated with allogeneic hematopoietic stem cell transplantation (HSCT). We report a case of cerebral toxoplasmosis in a patient with multiple myeloma, following tandem autologous stem cell transplantation. A 55-year-old Korean male presented with weakness in both legs that had progressed to both arms. A magnetic resonance imaging scan of the brain revealed multiple, variablesized ring-enhancing lesions with surrounding edema in the cerebral hemispheres and brain stem. Stereotactic biopsy revealed bradyzoites of Toxoplasma gondii in the brain tissue. The patient received trimethoprim-sulfamethoxazole, followed by pyrimethamine and sulfadiazine, accompanying treatment for progressive multiple myeloma. Cerebral toxoplasmosis should be considered as one of the differential diagnoses in patients with neurologic signs following autologous HSCT.

Diabetic foot infection is one of the important complications in patients with advanced diabetes mellitus. Limb threatening infections such as osteomyelitis, abscess, and necrotizing fasciitis are frequently accompanied by the disease. Non-tuberculous mycobacterium (NTM) is a rare causative organism of diabetic foot infection. Thus, if one is not suspicious or meticulous, infection due to NTM will be easily overlooked and this will result in delayed diagnose and treat. Therefore, it is necessary to consider NTM as the causative organism if the wound does not respond to the conventional antibiotic treatment and the culture from the adequately obtained specimen reveals atypical acid-fast bacilli. We present a case of diabetic foot infection with osteomyelitis and abscess due to Mycobacterium mageritense, one of the rapid growing mycobacteria, that was successfully treated with surgical debridement and appropriate antibiotic treatment.

Plasmodium vivax malaria is an endemic disease in Korea, which rarely causes severe complications including those occurring in the cerebrum. There are limited numbers of complicated cases that have been reported around the world. We experienced a case of vivax malaria with cerebral complication: cognitive impairment and ataxia. A 55-year-old female with diabetes mellitus presented to the emergency department with acute fever of two days’ duration. She did not have any history of travelling abroad or receiving blood transfusions. Peripheral blood smear revealed vivax malaria with parasitemia density of 0.53 percent. She demonstrated loss of orientation, especially regarding time and place, and ataxia. Although the initial hydroxychloroquine treatment for malaria was successful, cognitive impairment and ataxia persisted and were not recovered. Brain MRI showed no structural abnormality. Brain PET showed diffuse hypometabolism in right parieto-temporal lobe of the brain.

가상현실 서비스를 이용하는 일부 사용자들이 겪고 있는 사이버멀미는 멀미와 유사한 증상으로서 현기증, 구토감 등을 유발한다. 가상현실 모션 트래킹 과정에서 발생하는 오차는 실제 및 가상현실 간의 간극을 유발시킨다. 이는 사이버멀미의 원인 중 하나이다. 본 논문에서는 이 간극을 외이(外耳) 전정기관을 이용한 오차 측정 및 보정 과정을 통해 해소하는 방법을 제안한다. 이를 통해 가상현실 충실도를 향상시켜 사이버멀미를 저감시키는 기술을 개발하는 것이 본 연구의 목적이다. 아울러 사이버멀미 저감연구 외 다양한 운동 행위 보정 기술에도 활용될 수 있음을 기대하고 있다.
Analysis of Environments in Smart Greenhouses as Influenced by Cultivation Method of Cut Roses

이 연구는 스마트 온실의 온습도 데이터를 수집하고, 그 환 경 요인들 간의 상관관계를 분석하여 스마트 온실 환경관리의 기초자료를 제공하고자 수행되었다. 스마트팜 시스템이 도입된 토경 방식의 OS 온실과 암면 배지경 방식의 KB 온실을 대 상으로 기온, 근권온도, 상대습도의 항목에서 매일 24시간의 데이터를 수집하였고, 이중 최고와 최저, 일평균, 주간 평균, 야간 평균, 일교차의 여섯 가지로 세분하여 정리한 후 두 온실 간의 차이와 기온과 근권온도의 상관관계를 분석하였다. 장미의 생육적온은 주간, 야간, 일교차는 이며, 근권온도는, 습도 는 95% 미만을 유지하는 것이 좋다. 두 온실의 실제 환경과 장미의 적정 환경 조건을 비교했을 때, 주간 기온은 OS 온실 에서 5개월 간, KB 온실에서 7개월 간 적정 수준(24~27˚C)을 유지하였다. 야간 기온은 두 온실 모두 2개월간 적정 수준 (15~18˚C)을 유지하였고, 일교차는 OS 온실에서 6개월간, B 온실은 9개월 간 적정 수준(약 10˚C)에 근접하였다. 근권온도 는 OS 온실에서 10개월, KB 온실에서 5개월간 적정 수준 (20~25˚C)을 유지한 반면, 상대습도는 두 온실 모두 연중 적정 범위 내에 들었다. 기온과 근권온도의 상관관계를 분석한 결 과, OS 온실은 최고 온도, 평균 온도, 주간 온도, 야간온도에 서는 뚜렷한 상관관계를, 최저온도와 일교차에서는 약한 상관 관계를 보였으며, KB 온실에서는 전 영역에서 매우 강한 직선 적 비례 관계를 보였다. 결론적으로 토경재배를 실시하는 OS 온실이 고설식 암면배지경을 실시하는 KB 온실에 비해 근권 온도가 기온의 영향을 덜 받았으며, 완충능력이 높은 토경재 배에서는 기온의 영향을 덜 받는다고 볼 수 있다. This study was conducted to collect data on the environmental factors in smart greenhouses, analyze the correlation between the factors, and provide basic data for smart farm environment management. Two smart greenhouses of cut roses were selected as a soil culture-type greenhouse OS located in Dalseong-gun, Daegu City, and a rock wool culture-type greenhouse KB located in Gimhae-si, Gyeongsangnam-do. Collected data were divided into six categories and analyzed after organizing: daily maximum, daily minimum, daily average, daytime average, nighttime average, and daily difference for air and media temperatures, and relative humidity. When comparing the optimal environmental conditions with those of the two greenhouses, the daytime temperatures were maintained at the optimal levels (24 - 27°C) for 5 months at greenhouse OS and for 7 months at greenhouse KB. The nighttime temperatures were m aintained at t he o ptimal l evels (15 - 18°C ) for 2 months at both greenhouses. The daily temperature difference was close to the optimal level (about 10°C) for 6 months at greenhouse OS and 9 months at greenhouse KB. The media temperatures were maintained at the optimal levels (20 - 25°C) in greenhouse OS for 10 months for 5 months in greenhouse KB. In relative humidity, both greenhouses were kept within the proper range throughout the year. As a result of analyzing the correlation between air and media temperatures, OS greenhouse showed a clear correlation in the maximum, average, daytime, and nighttime temperatures, and a weak correlation in the minimum and daily difference, while KB greenhouse showed a very strong linear proportional correlation in all areas. In other words, the temperature of soil with high buffering capacity was less affected by air temperature than soilless culture using rock wool.

<P><B>Abstract</B></P> <P>We propose silicone implants capable of the local, controlled release of a glucocorticoid drug, triamcinolone acetonide (TA), for the prevention of fibrosis. The shells of these silicone implants were coated with two different loading amounts of TA, which could release the drug in a sustained manner for 12 weeks. The drug-loaded implants were inserted into the subcutaneous space in living rats, and the tissues were biopsied at scheduled times during 12 weeks. For the drug-coated implants, the capsule thickness and collagen density decreased compared with those of the non-coated implant. Because of the effect of TA, inflammation and the expression of pro-inflammatory cytokines were downregulated, thereby decreasing the number of monocytes during acute inflammation. This effect in turn decreased the number of macrophages at the later stage of inflammation, leading to the expression of less TGF-β and consequently fewer fibroblasts and myofibroblasts. Our findings also revealed that with an appropriate dose control, skin and muscle atrophy, major side effects of TA, could be avoided while still effectively reducing fibrosis. Therefore, we conclude that the local, sustained release of an appropriate dose of a glucocorticoid drug can be a promising strategy for safely preventing fibrosis around silicone implants.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

Upper extremity deep vein thrombosis (DVT) is an unusual condition compared to lower extremity DVT, and it represents about 10% of all DVTs. We report a case of upper extremity DVT after clavicle fracture and immobilization.
