Xanthogranulomatous Pancreatitis Presents as a Solid Tumor Mass: A Case Report

Xanthogranulomatous inflammation (XGI) is a rare, idiopathic process in which lipid-laden histiocytes are deposited at various locations in the body. Although XGI has been reported to occur in various organs such as the gallbladder, kidney, bone, stomach, colon,appendix, lymph nodes, urachus, and urinary bladder and in soft tissues,xanthogranulomatous pancreatitis (XGP) is extremely rare. Herein, we report a case of XGP occurring in a 70-yr-old woman, who presented with abdominal pain for several months. On physical examination, mild epigastric tenderness was noted. Abdomen CT scan revealed a low attenuated mass in uncinate process of pancreas, suggesting malignant lesion. Whipple’s operation was performed and the final pathologic diagnosis was XGP. The patient’s post-operative course was uneventful, and no recurrence was found within 7months of the operation. When a pancreatic mass does not show clinico-radiological features typical of common pancreatic neoplasms, XGP should be considered for a differential diagnosis.

Drunk driving defines a driver as unable to drive a vehicle safely due to drinking. To crack down on drunk driving, alcohol concentration evaluates through breathing and crack down on drinking using S-shaped courses. A method for assessing drunk driving without using BAC or BrAC is measurement via biosignal. Depending on the individual specificity of drinking, alcohol evaluation studies through various biosignals need to be conducted. In this study, we measure biosignals that are related to alcohol concentration, predict BrAC through SVM, and verify the effectiveness of the S-shaped course. Participants were 8 men who have a driving license. Subjects conducted a d2 test and a scenario evaluation of driving an S-shaped course when they attained BrAC’s certain criteria. We utilized SVR to predict BrAC via biosignals. Statistical analysis used a one-way Anova test. Depending on the amount of drinking, there was a tendency to increase pupil size, HR, normLF, skin conductivity, body temperature, SE, and speed, while normHF tended to decrease. There was no apparent change in the respiratory rate and TN-E. The result of the D2 test tended to increase from 0.03% and decrease from 0.08%. Measured biosignals have enabled BrAC predictions using SVR models to obtain high Figs in primary and secondary cross-validations. In this study, we were able to predict BrAC through changes in biosignals and SVMs depending on alcohol concentration and verified the effectiveness of the S-shaped course drinking control method.

Cytologic features of a case of mantle cell lymphoma is presented, which was obtained by fine needle aspiration cytoloby and confirmed by excisional biopsy of axillary lymph node. A 67-year-old female alleged palpable masses in both axillae for several months. Additional multiple lymphadenopathies were found in the both neck and inguinal areas. The main cytologic feature was carpeting on monotonous slightly atypical small lymphocytes without heterogeneous components. The nuclei of these lymphocytes are slightly larger than benign small lymphocyte and relatively round with some Indentation. Nucleolus was not prominent and no mitosis was found. Their cytoplasm was scanty and cyanophilic in Papanicolaou's stain. The histiocytic cells, which had bland-looking banded nuclei and abundant cytoplasm, corresponding to pink histiocytes were shown. Excisional blopsy of lymph nodes was diagnosed as mantle ceil lymphoma, diffuse type.
