        대한제국시기 원구단의 원형복원과 변화에 관한 연구

        Wongudan(Temple of heaven in Korea) was a historically important structure at the end of the Daehan Empire, but it was damaged in 1913 by Japan and has not been properly evaluated since then. Yet, following the example of Temple of heaven in China, Wongudan is an edifice located near to the main palaces(Kyeong-un Palace) downtown in Seoul and built in an appropriate manner to the topography and urban features on the topology. This structure was started to be built in 1897 and the repair was conducted over seven years or so. Nevertheless, the limited site of the structure incurred frequent conflicts with its neighboring private houses, and when the main gate of Wongudan was moved to Sogong Street, the passages south of the structure were so narrow that its reconstruction was finally reduced. Till 1900, Hwangkungwoo including initial Wongudan had been completed, and Wongudan was restored for the second step in 1901 and properly remodeled in 1903. In 1913, however, Japan built Joseon Hotel on the site of Wongudan and converted some of its affiliate structures like the shrine into the hotel subsidiaries. Such a state was continued till 1967 after the emancipation of Korea. And when the hotel was newly built, all the structures were demolished except for Hwangkungwoo, stone drum podium, and three archi-gate of the Hwangkungwoo. Lack in the recognition and investigation of cultural assets led to the destruction of Wongudan's affiliate structures. Currently, it is difficult to restore and investigate the cultural assets since the imperial palaces are connected to the plot of Joseon Hotel, which is a private property. The extant photos, maps, and documents show the Wongudan space, Hwangkungwoo spaces, and part of the shrine space. It calls for further research into the remaining spaces of the edifice including its eastern wing.

        Qualitative Analysis of Proteins in Two Snake Venoms, Gloydius Blomhoffii and Agkistrodon Acutus

        Objectives: Snake venom is a complex mixture of various pharmacologically active substances, such as small proteins, peptides, and organic and mineral components. This paper aims to identify and analyse the proteins in common venomous snakes, such as Gloydius blomhoffii (G. blomhoffii) and Agkistrodon acutus (A. acutus), in Korea. Methods: We used mass spectrometry, electrophoresis, N-terminal sequencing and in-gel digestion to analyse the proteins in these two snake venoms. Results: We identified eight proteins in G. blomhoffii venom and four proteins in A. acutus venom. The proteins detected in G. blomhoffii and A. acutus venoms were phospholipase A2, snake venom metalloproteinase and cysteine-rich secretory protein. Snake C-type lectin (snaclec) was unique to A. acutus venom. Conclusion: These data will contribute to the current knowledge of proteins present in the venoms of viper snakes and provide useful information for investigating their therapeutic potential.

        간호대학생의 정신간호학실습 시 치료적 의사소통 경험

        This study was conducted for the purpose of investigation nursing students’ experiences of therapeutic communication in psychiatric nursing practice. This study used a qualitative research approach applying a qualitative case study method. The study subjects included 10 third-year nursing students with practical psychiatric nursing experience at a university located in G metropolitan city. The researcher collected information through in-depth interviews with students, selected 13 topics based on the analyzed content, and classified similar topic into 4 categories. These 4 categories, as explored in the study, include change in perception of mental illness, change in therapeutic relationship, learning about therapeutic communication and therapeutic relationship, and advanced deep reflection on therapeutic communication. Through this study, the following aspects can be confirmed and considered. That is necessary to consider therapeutic communication learning methods in psychiatric nursing theory classes and psychiatric nursing practice, and efforts should be made to provide opportunities to meet, experience, and communicate with various subjects during psychiatric nursing practice. It will be an opportunity for nursing students to effectively learn and practice therapeutic communication, which can be useful in actual psychiatric nursing settings. Based on the results of this study, it is expected to serve as basic data for maximizing the effectiveness of psychiatric nursing practice and improving the quality of psychiatric nursing learning.

      • P255 : Relationship between the HLA-G 14bp insertion/ deletion polymorphism and susceptibility to autoimmune disease: a mata-analysis

        Background: Numerous studies have investigated the potentialrelationship of human leukocyte antigen (HLA)-G 14bp insertion/deletion (INS/DEL) polymorphism with autoimmune disease (AID). However, results from published data were inconclusive. Objectives: Our aim in this study was to determine whether the 14bp INS/DEL polymorphism in the HLA-G gene contributes to the risk of AID. Methods: A systemic literature search of PubMed and Embase databases was conducted to identify eligible studies investigating the association of HLA-G 14bp INS/DEL polymorphism with AID. The final analysis included 13 publications with a total of 6462 individuals. Results: Overall, no significant association between HLA-G 14-bp INS/DEL polymorphism and overall AID was detected in all comparison models. Further subgroup analyses based on AID types and ethnicity showed no significant association. Conclusion: Our results suggest that the HLA-G 14bp INS/DEL polymorphism might not be related to the development of AID. Further studies with larger sample size are warranted to validate our finding.

        Enhanced Antitumor Effect of the Combination of Bacille Calmette-Guérin and an Immune Checkpoint Inhibitor in Bladder Cancer-On-a-Chip

        We upgraded preexisting bladder cancer-on-a-chip (BCOC) by adding T cells and evaluated the antitumor eff ect of a combina- tion of intravesical Bacille Calmette-Guérin (BCG) and pembrolizumab. We fabricated bioprinted BCOC with microfl uids, incorporating HT1376, MRC-5, HUVEC, THP-1 and Jurkat cells. We evaluated the eff ector-to-target cytotoxicity, cytokine, and cell viability in 2D culture, live/dead assay, migration assay, and cytokine assay in BCOC. Additionally, we evaluated the antitumor effi cacy of the combination of BCG and pembrolizumab in an orthotopic mouse model. The combination group showed the most eff ective reduction compared to the control in 2D culture (100.0 ± 0.8% vs. 36.4 ± 0.8%, p < 0.001). In BCOC, cancer cell viabilities were decreased at 3 days in the BCG group (70.1 ± 9.8%, p = 0.013) and combination group (49.3 ± 8.1%, p < 0.001). The combination group showed the highest immune reaction in the cytokine assay (interferon-γ, p = 0.045; interleukin-6, p = 0.037) and migration assay (fold change 1.3 ± 0.1, p < 0.001), whereas in the in vivo model, it showed lower signal intensities from days 10 to 14 compared to that in the control group ( p = 0.031 and p = 0.014, respec- tively). No signifi cant weight changes were observed among the groups. We developed a 3D bioprinted BCOC via use of the monocytic THP-1 cells and Jurkat T cells to assess the effi cacy of immunotherapy. The combination of BCG and pem- brolizumab showed the best antitumor effi cacy in BCOC and animal models.

        Stenosis of central vein is a common complication arising after percutaneous subclavian vein catheter insertion performed for temporary vascular access in chronic renal failure patients undergoing hemodialysis. There are several treatment methods for the condition like percutaneous angioplasty(PTA), stent insertion, and surgery, but recent trend is toward PTA and stents. Among the patients diagnosed with chronic renal failure from March 1993 to May 2002 and undergoing hemodialysis through AV fistula, the 14 Patients in whom central vein stenosis arose were selected for the study. A total of 28 percutaneous interventions(5 PTA and 23 stent placement) were performed, and restenosis rate and the time taken till the restenosis in de novo lesions and instant lesions were compared. All 28 cases were operated successfully. The 14 cases that received both anigioplasty and stent placement initially. (de novo lesion : 14 cases), Among the 10 cases with de novo lesion that followed up more than 1 year, 3 cases are currently undergoing hemodialysis without restenosis, and the remaining 7 cases have recurred stenosis with the mean time to restenosis of 10.9 months. In the 7 cases in whom stenosis recurred, 11 interventions were done(instent lesion: 11 cases). 4 of these were using only ballon angioplasty with 100% restenosis rate and the mean time of 3 months until restenosis. The remaining 7 cases were using both balloon angioplasty and stent placement, also with 100% restenosis rate but with the mean time of 12 months until restenosis, which was later than the group receiving only balloon angioplasty. In treating the patients with central vein stenosis, stent placement seems to be more advantageous over PTA in terms of restenosis rate and the mean duration of patency. In the case of instent lesion, inserting the stent for the second time after stenosis recurred lengthened the duration of patency compared to performing balloon angioplasty alone.

        The alignment and dynamic response of carbon nanotubes (CNTs) in nematic liquid crystal (NLC) medium induced by strong electric field have been observed through polarizing optical microscope. Density-functional calculations suggest that LC molecule anchors helically to the CNT wall to enhance π-stacking with a binding energy of nearly -2.0 eV due to a considerable amount of charge transfer from LC molecule to CNT, resulting in the formation of excess charges and permanent dipole moment in CNTs. Under strong electric field, the motion of CNTs distorted the director of adjacent LC molecules. Our detailed analysis of dynamics revealed that the four-lobe textures in vertical cell and two vertical stripes in in-plane switching cell were strongly correlated, i.e., the side view of textures by the vertical motion of CNTs in vertical cell was similar to the textures in in-plane switching cell. Interestingly, the magnitude of textures in microscope was strongly dependent on the size of CNTs and theapplied field strength. The statistical size distribution of textures similar to that of CNTs provided information for the degree of dispersion of CNTs.

        Developing Core Elements and Checklist Items for Implementing Antimicrobial Stewardship Programs in Korean General Hospitals: A Modified Delphi Survey

        Background: Antimicrobial stewardship programs (ASPs) aim to optimize antimicrobial use by minimizing the spread of antimicrobial resistance. The core elements for implementing ASPs in healthcare facilities have been developed by the World Health Organization, international research group and government agencies of various countries. However, to date, there is no documented core elements for implementation of ASP in Korea. This survey aimed to establish a national consensus on a set of core elements and their related checklist items for the implementation of ASPs in Korean general hospitals. Materials and Methods: The survey was conducted from July 2022 to August 2022 by the Korean Society for Antimicrobial Therapy with support from the Korea Disease Control and Prevention Agency. A literature review was conducted by searching Medline and relevant websites to retrieve a list of core elements and checklist items. These core elements and checklist items were evaluated by a multidisciplinary panel of experts using a structured modified Delphi consensus procedure, using two-step survey included online in-depth questionnaires and in-person meeting. Results: The literature review identified 6 core elements (Leadership commitment, Operating system, Action, Tracking, Reporting, and Education) and 37 related checklist items. Fifteen experts participated in the consensus procedures. Ultimately, all 6 core elements were retained, and 28 checklist items were proposed, all with ≥80% agreement; in addition 9 items were merged into 2 items, 2 items were deleted, and 15 items were rephrased. Conclusion: This Delphi survey provides useful indicators for the implementation of ASP in Korea and suggests national policy improvement about the barriers (e.g., shortage of staffing and financial support) existing in Korea for optimal implementation of ASPs.

        Low serum bilirubin level predicts the develop-ment of chronic kidney disease in patients with type 2 diabetes mellitus

        Background/Aims: We evaluated whether serum bilirubin levels can predict the development of chronic kidney disease (CKD) in patients with type 2 diabetes mellitus (T<sub>2</sub>DM). Methods: This was a retrospective observational longitudinal study of patients presenting at the Pusan National University Hospital. A total of<sub>349</sub> patients with T<sub>2</sub>DM and preserved kidney function (estimated glomerular filtration rate ≥ 60 mL/min/1.73 m<sup>2</sup>) were enrolled. The main outcome was the development of CKD stage 3 or greater. The patients were divided into four groups according to the quartiles of the total serum bilirubin levels at baseline. Results: The group with the lowest range of total serum bilirubin level (Q<sub>1</sub>) showed the highest cumulative incidence of CKD stage 3 or greater than that of the other lower quartiles (Q<sub>1</sub> vs. Q<sub>4</sub>; hazard ratio [HR], 6.75; 95% confidence in-terval [CI], 1.54 to 29.47; p = 0.011). In multivariate analysis, the risk of developing CKD stage 3 or greater was higher in the second lowest quartile of the serum bili-rubin level than that in the highest quartile of the serum bilirubin level (Q<sub>2</sub> vs. Q<sub>4</sub>; HR, 9.36; 95% CI, 1.33 to 65.73; p = 0.024). In the normoalbuminuria subgroup (n = 236), multivariate analysis showed that the risk of developing CKD stage 3 or greater was higher in the lowest quartile of the serum bilirubin level than that in the highest quartile of the serum bilirubin level (Q1 vs. Q4; HR, 7.36; 95% CI, 1.24 to 35.82; p = 0.019). Conclusions: Serum bilirubin might be an early clinical marker for predicting the progression of CKD in patients with T<sub>2</sub>DM and preserved renal function.
