        신성 복증의 응급 초음파 검사

        Background. Intravenous urography(IVU) has long been accepted as the primary radiologic study for the diagnosis of renal colic. But the IVU does pose risks of contrast material reactions and exposure to radiation. Recently, IVU has been challenged as a first line modality and ultrasonography proposed as a replacement. We, therefore, felt obliged to re-evaluate the IVU, KUB and ultrasonography as diagnostic tools. Methods. We evaluate the sensitivity and specificity of ultrasound, IVU and KUB in detecting urinary calculi in suspected acute renal colic a prospective study of 69 patients, both sides of kidney and ureter, who visited urban university hospital emergency center from September 1997 to January 1998. Our sonographic criteria for a positive examination consisted of visualization of urinary tract calculus and/or urinoma. Results. In Ultrasonography, the accuracy, seusitivity and specificity were 94,94 and 94%. In IVU and KUB, they 90, 81, 98% and 71, 43, 96% Conclusion, Ultrasonography is very useful diagnostic tool in initial evaluation of acute renal colic.

      • 육안적 혈뇨로 내원한 환자에서 발견된 방광 유전분증 : 증례보고

        Amyloidosis is a disease resulting in deposition of amyloid in the body. The disease can be either primary or secondary. The amyloidosis of urinary bladder is more usually affected by primary amyloidosis and also very rare. The patient with primary amyloidosis of the bladder usually presents with gross hematuria. The amyloidosis resembles bladder cancer in cystoscopy. It can be confirmed by biopsy. The treatment consists of medication (e.g., steroid, antitumor agent, cholchicine, DMSO) and/or transurethral resection. The standard treatment is mephalan and prednisolone. A 49-year-old man complained of recently developed gross hematuria without abdominal pain. We examined urinary bladder cystoscope for gross hematuria. It revealed small bulging mass on trigone are a of bladder. Therefore we did bladder biopsy. Finally, it revealed bladder amyloidosis. The patient was improved by medication (mephalan 2mg and prednisolone 1 mg/kg po daily). We report an unusual case of amyloidosis involving bladder.

        낙상으로 인한 좌심실 파열 후 생존한 환자 1례

        Blunt or nonpenetrating chest injuries are common after motor vehicle accidents or falls. Blunt injury to the heart ranges from contusion to rupture. Cardiac rupture, however, is un-common and associated with a very high mortality rate ; left ventricular rupture due to falling is even more rare. We experienced a case of left ventricular rupture of the heart caused by a fall, which the patient survived. A 63-year-old man fell off a ladder and presented with hypotension from left hemothorax and pericardial tamponade. The hemothorax was relieved via an emergency closed thoracostomy but dyspnea and hypotension continued. And so, echocardio-graphy was done and pericardial tamponade was detected. Emergency left thoracotomy was done to repair the rupture site. We believe that patients with cardiac rupture who reach the hospital alive can often be saved by prompt diagnosis and immediate surgical treatment.

        Uzbekistan 공화국의 농업현황

        1. Uzbekistan은 전통적인 농업국가로서 농업이 경제의 축이며 농촌인구가 60%,농업부문 종사자가 전 산업에서 차지하는 비율이 44%이고 농업생산이 GNP에서 차지하는 비중이 33%로서 비교적 높은 편이다. 2. 이 나라의 농업은 강우량이 적고 건조한 기후조건 때문에 주로 관개에 의존하고 있으며 안공수로와 저수지의 건설로 관개가능 면적은 430만㏊ 이고 충분한 수량만 확보된다면 700만㏊ 까지 경작이 가능하다. 농업생산은 목화, 밀, 벼, 포도, 채소, 멜론재배가 주종을 이루고 있으며 특히 일조시수가 3,000시간으로 매우 길고 일사량도 많아 목화, 고품질의 포도와 멜론 생산이 유명하다. 3. 축산업도 농업에서 중요한 부문으로 주로 유목에 의존하고 있으며 염소, 고기소, 젖소, 면양이 주된 가축이고 양잠업도 중요한 위치를 차지하고 있다. 4. Uzbekistan은 맥류, 목화, 포도, 사과, 양파 등의 원산지 일 뿐만 아니라 박과 작물의 2차원산지로서 유전적 다양성이 풍부하고 특히 멜론은 많은 재래종 품종들이 분화되어 재배되고 있다. 5. Uzbekistan는 1995년 한·우즈벡 농업기술협력 가능성 조사 이후 1996년부터 양국간 유전자원 공동연구가 활발히 추진되고 있다 금후 인근 중앙아시아 각국과의 유전자원 또는 농업관련 공동연구를 수행함에 있어 우즈베키스탄을 전진기지로 활용하는 방안이 적극 모색되어야 할 것으로 사료된다. The Republic of Uzbekistan is located in the central part of the middle Asia. The area of the Republic makes up 447.4 thousands ㎢ and about 60% of the country is semi-desert or desert with only 4.5 million hectares of the area cropped. The average temperature in July on the plains' territory varies from 26℃ in the North to 30℃ in the South, and the average temperature in January falls to as low as 0℃ in the South and to -8℃ in the North. Precipitation primarily occurs during the winter-spring period. Annual precipitation amounts to 80-200 ㎜ on the plains, 300-400 ㎜ in the foothills area and 600-800 ㎜ on the eastern and south-eastern slopes of the mountain ridges. Natural pastures occupy 50.1% of the total area of Uzbekistan, and 9.7%(4.3 Min. ha) of irrigated lands. Irrigated land is mainly cultivated for cotton, spiked cereals, rice and potato. Cotton plants occupy 36.5% of the cultivated areas and grain crops 39.5%. Given the abundance of solar radiation and heat, the major factor limiting the use of agroclimatic and land resources is a deficit of water. Agriculture in Uzbekistan was and still is the largest sector in Uzbekistan's economy. Agriculture and it's related industries account for approximately 33% of GDP, and 55% of the hard currency revenues of the country is related to Agriculture. Approximately 60% of the population resides in rural areas, and 44% of the work force is engaged in agricultural production. Uzbekistan is the world's fifth largest cotton producer and the second largest exporter. Cotton has been center to the economy because it accounts for about 50% of the country's export earnings. At the same time, however, cotton production has depended on vast amounts of irrigation, and this has had a possibly significant irreversible negative impact on the Aral Sea. The development strategy in the agricultural sector in the country is targeted at ensuring food and environmental security, while increasing the efficiency and export-potential of domestic production. According to the macroeconomic forecast, the agrarian sector will maintain its leading role in the economy (at 25% of GDP). To meet the strategic development goals by 2010, annual growth in agricultural output must be at least 5-6%.

        우리 나라 재해 의료의 문제점 : 수원시 재해 대응 훈련의 평가를 통하여 본 ANALYSIS OF EMERGENCY RESPONSE EXERCISE IN THE MOCK DISASTER IN SUWON

        Natural or man-made disasters are serious enough to paralyse the functions of the nation or regional distinct and affect the property and lives of numerous citizens. The first two hours after occurrence of the disaster are critical for determining death or permanent disabilities of the casualties. The current Disaster Medical Service(DMS) System in our country adopts mainly civil defense model which focuses the cause of the accident, compensation of the victims, or reconstruction. So in the initial phase of disaster, the appropriate rescue and emergency treatment is not provided effectively. In order to assess and reorganize the current status of DMS System in Korea, the Department of Emergency Medicine, Ajou University School of Medicine, planned and conducted an emergency response exercise in Suwon city on April 28, 1995. The exercise, which took place near the Kyunggido Culture and Art Center, coordinated the efforts of the Suwon Fire Department and other related agencies. As well as providing training on emergency response, the exercise was valuable for identifying areas of weakness which will be address in the future.

      • Ginseng berry improves hyperglycemia by downregulating hepatic gluconeogenesis and steatosis in mice with diet-induced type 2 diabetes

        <P><B>Abstract</B></P> <P>This study investigated the effect and the underlying mechanism of ginseng berry (GB) in a mouse model of type 2 diabetes, supplied with 0.05% or 0.1% dietary GB for 12weeks. GB significantly improved hyperglycemia and insulin resistance, as demonstrated by the blood glucose and insulin levels, HOMA-IR, and GTT. Moreover, the expression of gluconeogenic genes such as PEPCK and G6Pase and the hepatic metabolites involved in the pathway of gluconeogenesis, such as glucose-6-phosphate and dihydroxyacetone phosphate, were significantly reduced by GB. Hepatic steatosis was also significantly ameliorated; TG content and expression of lipogenic enzymes and transcriptional factors such as FAS, ACC, and SREBP1 were reduced by GB. Simultaneously, AMPK phosphorylation was increased both in fatty liver and in lipid-accumulated HepG2 cells by GB. In conclusion, GB improved hyperglycemia by downregulating hepatic glucose production and hepatic steatosis, and AMPK appeared to be an important regulator of these effects.</P> <P><B>Highlights</B></P> <P> <UL> <LI> GB significantly improves obesity-induced hyperglycemia and insulin resistance. </LI> <LI> Anti-diabetic effects of ginseng berry is mediated by downregulating hepatic glucose production and hepatic steatosis. </LI> <LI> AMPK may be an important contributing regulator in the GB-mediated suppression of hepatic glucose production. </LI> </UL> </P>

        A Study of the Relationship Analysis of Power Conversion and Changed Capacitance in the Depletion Region of Silicon Solar Cell

        In this paper, silicon solar cells are analyzed regarding power conversion efficiency by changed capacitance in the depletion region. For the capacitance control in the depletion region of silicon solar cell was applied for 10, 20, 40, 80, 160 and 320 Hz frequency band character and alternating current(AC) voltage with square wave of 0.2~1.4 V. Academically, symmetry formation of positive and negative change of the p-n junction is similar to the physical effect of capacitance. According to the experiment result, because input of square wave with alternating current(AC) voltage could be observed to changed capacitance effect by indirectly method through non-linear power conversion (Voltage-Current) output. In addition, when input alternating current(AC) voltage in the silicon solar cell, changed capacitance of depletion region with the forward bias condition and reverse bias condition gave a direct effect to the charge mobility.

        간외담관암과 이형성 조직에서 p53 , K - ras , Ki - 67 발현의 임상적 의의

        Background/Aims: We examined the expression of p53, K-ras, and Ki-67 in extrahepatic bile duct (EHBD) carcinoma, dysplasia, hyperplasia, and normal tissues. Then, their clinical significance was determined. We also investigated if dysplasia and hyperplasia are the precancerous lesion in EHBD carcinoma. Methods: Specimens of 23 patients with surgically resected EHBD carcinoma were classified into normal, hyperplasia, dysplasia, and carcinoma. Then, they were immunostained to examine the expression of p53, K-ras, and Ki-67 antibody. Results: p53 was detected in 46.7% of dysplasia and 66.7% of carcinoma but not in hyperplasia. K-ras was positive in 46.7% of dysplasia and 55.6% of carcinoma but not in hyperplasia. Ki-67 was positive in 18.1% of hyperplasia, 73.3% of dysplasia, and all the carcinoma. None of them was detected in normal tissue. There was no significant correlation between p53 immunoreactivity and gender, histological grade, stage of EHBD carcinoma, and survival. K-ras and Ki-67 immunoreactivity was not associated with gender, age, histological grade, stage of EHBD carcinoma, and survival. Conclusions: These data suggest that p53 and K-ras mutations may have a role in the carcinogenesis of EHBD carcinoma and that dysplasia may be a precancerous lesion of EHBD carcinoma. Ki-67 may be useful in differentiation of carcinoma from dysplasia. There is no prognostic relationship significantly between p53, K-ras, Ki-67 expression, and the clinicopathologic parameters.
