        The multistage wavelength division multiplexed passive optical network (WDM-PON) with multiple remote nodes (RN) can accommodate widely spread subscribers efficiently and it increases the channel usage. We implemented a multistage RN using band add-drop filters and it was applied to a wavelength-locked Fabry-Petor laser diode based gigabit WDM-PON.

        Antiviral Resistance in Human Cytomegalovirus Due to UL54 Mutations Without UL97 Mutations

        Background: The concurrent detection of human cytomegalovirus (CMV) with UL97 andUL54 mutations is crucial for prescribing adequate antiviral treatment when drug-resistantCMV infection is suspected. We investigated the frequency of resistance-conferring mutationsamong patients with persistent or recurrent CMV infection and further reviewed thesubgroup with UL54 mutations without UL97 mutations. Methods: Patients with persistent or recurrent CMV infection after 4 weeks of treatment withganciclovir or foscarnet were consecutively enrolled between November 2012 and May 2019. The direct sequencing of UL97 and UL54 was performed to detect resistance mutations in CMV. Results: A total of 101 sequencing datasets were obtained from 65 enrolled patients. CMV UL97 and UL54 mutations were detected in 15.4% (10/65) and 9% (6/65) of patients,respectively. The CMV retrieved from two patients (3%) had mutations in both genes. Fourpatients with CMV UL54 mutations alone had a history of haploidentical peripheral bloodstem cell transplantation, and foscarnet was administered for over 4 weeks to these patients;21.5% of patients had suspected resistant CMV infection with either UL97 or UL54 mutations. Conclusion: In this study, CMV UL54 mutations but not UL97 mutations were found inpatients subjected to prolonged foscarnet administration for CMV disease.

        Evaluation of the efficacy of three medical device detergents on bacteria and yeast derived biofilm: a comparative study

        Background: This study aimed to evaluate the efficacy of three medical detergents againstbacteria and yeast-derived biofilms. Methods: The biofilm removal efficacy of EmpowerTM (Metrex, USA), CidezymeTM (Johnsonand Johnson Medical Inc, USA), and Matrix mintTM (Whiteley Medical, Australia) werecompared to that of chlorine bleach. Biofilms were produced using Staphylococcus aureusRN9120, Escherichia coli ATCC35218, Pseudomonas aeruginosa ATCC27853, Candida albicansATCC14053, and clinical isolates of Enterococcus faecalis, E. coli, Klebsiella pneumoniae,Candida auris, and Trichosporon asahii. The organisms were suspended in tryptic soy broth(TSB) in 96-well microplates and cultured for 72 hours. They were treated with the detergents,and the residual biofilm mass was quantified using crystal violet staining followed by opticaldensity measurements at 620 nm (OD620). Results: EmpowerTM and CidezymeTM significantly reduced the biofilm mass derived from allspecies by > 50% of OD620 at 37ºC except those from E. faecalis, T. asahii, and C. auris. MatrixmintTM had no effect on the biofilms under any condition. Conclusion: The culture conditions and the species of the biofilm-producing organisminfluenced the effectiveness of the detergent. Biofilms produced by E. faecalis, C. auris, and T. asahii were resistant to all detergent treatments under all conditions.

        만성 분선충증 환자에서 재발한 과중감염 1예

        Strongyloides stercoralis is an intestinal nematode that often causes chronic diarrhea and may develop severe complicated form of hyperinfection or disseminated infection in immunocompromised patients. Here, we report a case of recurrent strongyloidiasis presenting with pulmonary and meningeal involvement. A 55-year-old male diagnosed with pancreatic cancer 4 months ago was admitted due to chronic diarrhea, abdominal pain, and weight loss for 2–3 months. He had been treated with albendazole for chronic recurrent strongyloidiasis 13 years ago and again 2 years ago. He developed sepsis of Klebsiella pneumoniae and Escherichia coli on Days 3 and 7, respectively, and then meningitis of E. coli on Day 42. Strongyloidiasis was diagnosed by detection of abundant filariform larvae in sputum specimens on Day 15. He was treated for disseminated strongyloidiasis with albendazole and ivermectin for five weeks until clearance of larvae was confirmed in sputum and stool specimens. Laboratory diagnosis is important to guide appropriate treatment and to prevent chronic and recurrent strongyloidiasis. 분선충은 소화기 계통을 주로 침범하는 선충류로 주로 만성 설사의 원인이 되고, 면역환자에서는 과중감염이나 파종성 감염을 일으킬 수 있다. 만성 분선충증 환자에서 호흡기와 신경계를 침범하여 재발하는 증례를 경험하여 보고하고자 한다. 4개월 전 췌장암을 진단받은 55세 남성이 2–3개월간의 복통, 만성 설사, 체중감소를 주소로 입원하였다. 13년 전과 2년 전 만성 재발성 분선충증으로 albendazole 치료를 받았다. 입원 3일째와 7일째 각각 Escherichia coli와 Klebsiella pneumoniae에 의한 균혈증이 진단되었고, 입원 42일째는 E. coli에 의한 뇌수막염이 발생하였다. 입원 15일째 객담 검체에서 다수의 사상유충이 검출되어 분선충증 재발로 진단하였고, 뇌수막염이 진단된 후 파종성 감염을 의심하여 albendazole과 ivermectin으로 치료하였다. 객담과 대변검체에서 유충을 추적하여 박멸되었음을 증명할 때까지 약 5주간 치료하였다. 분선충증은 검사실 진단과 추적 관찰을 통해 적절한 치료를 하는 것이 만성 또는 재발성 분선충증을 예방하는 데 중요함을 알 수 있었다.

        The Impact of Social Distancing on the Transmission of Acute Respiratory Viruses during the COVID-19 Pandemic

        To the Editor, Since the first confirmed case of coronavirus disease 2019 (COVID-19) on January 20, 2020, the Korean government reported a total of 8,799 COVID-19 patients until March 21, 2020, which were mainly attributed to community outbreaks [1]. This novel respiratory viral infection is so feared that the number of individuals using public transport has dropped dramatically. Moreover, enhanced social distancing measures, including refraining from going out and following personal hygiene practices such as hand washing and wearing a mask, were announced on March 21. These strategies successfully attenuated COVID-19 transmission in South Korea [2]. Similar to pharmaceutical inventions such as the use of antivirals and vaccination, physical interventions like hand-washing and wearing masks have been documented as effective measures against the transmission of respiratory viruses like influenza [3]. The effectiveness of physical interventions on influenza and COVID-19 has been reported in Hong Kong and South Korea [4,5]. Here, using nationwide surveillance reports and a retrospective review of three-year records from a single institution, we measured the impact of social distancing on respiratory virus transmission. This study was reviewed and approved by the Institutional Review Board (IRB) of Asan Medical Center, Seoul, Korea (IRB No. 2020-0983). The IRB waived the requirement for written informed consent. The results of patients tested for the respiratory virus panel were collected from a university-affiliated hospital in South Korea from May 2017 to April 2020. Respiratory virus panel testing was performed using a multiplex real-time PCR (Anyplex II RV16; Seegene, Seoul, Korea). The national surveillance data for acute respiratory viruses (for the same period) were retrieved from the Infectious Disease Portal (http://www.cdc.go.kr/npt/biz/npp/iss/ariStatisticsMain.do) operated by the Korea Centers for Disease Control & Prevention. In addition to influenza, adenovirus, bocavirus, coronavirus, metapneumovirus, parainfluenza virus, respiratory syncytial virus, and rhinovirus were also included as acute respiratory viruses for statistical analysis. The chi-square test was performed using MedCalc (version 19.2; MedCalc 103 Software, Ostend, Belgium). From March to April 2020, the reported number of cases with acute respiratory viruses dramatically dropped compared to the same time period in the previous two years at this hospital and nationwide. Positive results from the respiratory panel testing and the patient numbers from the national surveillance data are shown in Fig. 1. A total of 2,941 samples were tested from March to April 2020, which is a marginal increase from 2018 and 2019 (2,829 and 2,501 samples, repsectively) for the same period. In 2020 , 164 cases of acute respiratory viral infection were reported, which is a significant decrease compared to the 781 (P < 0.001) and 687 (P < 0.001) cases reported in 2018 and 2019, respectively, for the same period. To the best of our knowledge, this is the first retrospective study on the impact of social distancing–introduced to stem COVID-19 spread–on the incidence of acute respiratory viral diseases besides influenza. Social distancing and improvements in personal hygiene seem to effectively prevent droplet or airborne transmission of acute respiratory viral diseases. Considering the increased risk of such infections in aging populations, education regarding hand hygiene and mask use should be encouraged in the post-COVID-19 era.

      • Hybrid Single-Slope ADC를 사용한 고성능 CMOS Image Sensor 설계

        In this paper, CMOS image sensor(CIS) with hybrid single slope ADC is presented. To realize small size and high precision CIS, subranging technique is applied with no holding capacitor to reduce area. Proposed hybrid ADC operates subranging ADC(coarse) and Single Slope ADC(fine). Based on 1-Poly 5-Metal 90nm back side illuminated(BSI) CIS process, the chip satisfies 1920 × 1440 pixel resolution whose pitch is 1.4um and 1.75-Tr active pixel sensor(APS).
