A functionless hand resulting from the sequalae of a thermal burn is very serious. It prevents restoration of socioeconomic behavior. Since the magnitude of this problem is rewards for the most deligent and competent primary care greater. It can not be emphasized too strongly the result of good primary care are always superior to what one can expect from secondary restoration. The pathogenesis of the functionless hand is caused by a triad of factors resulting from thermal injury that together lead to the rapid stiffening of joints and adhesion of tendons. This triad of factors is edema, inflammation, and immobilization. (Beasley, 1981) Thus, the principles for primary care of burned hands leading to rapid recovery and minimal complications are now established and many kinds of methods are proposed. But, despite much effort, many circumstances prevent ideal primary care and we are still confronted with the need for secondary repairs. Secondary repair of burned hands requires a careful, cojmprehensive and individualized plan for each patien, and with due respect to all the patient's problems. We reviewed 42 cases of the postburn hand deformities which were operated on in our Department of Plastic and Reconstructive Surgery from 1976 to 1981.
Purge & Trap-GC를 이용한 의약품 필름코팅 정제 중 잔류용제에 관한 연구

의약품은 약물을 생체에 적풋하기 위하여 유효성분의 효과가 언제나 일정하게 확보되고 사응에 편리하도록 만들어지는 것이므로 유효썽분 이외에 약효에 영향을 주지 않는 성분이 첨가되는 경운가 많다. 이 때 사용되는 용매들은 제피의 광택 및 건쪼시간의 단축 등을 위하여 휘발점이 낮을 용매들이 주로 사용되어진다. 본 연구는 의약품 필름코팅정제 중 잔류용매 4종(chlorofonr benzen, trichloro ethylen, 1,4-dioxane)에 대한 변형된 pirge & trap-GC 장치를 이용한 동시분석방법을 개발하였으며, 각 표준품의 RSD 값은 chloroform 3.03%, benzen 3.17%, trichloroethylen 3.69% and 1,4-dioxane 3.41%였다. 또한 시중 유통중인 의약품 50종에 대하여 잔류웅매 양을 측정하였으며, 검출되는 잔류용매는 한 건도 없었다. This study nras carried out to develope the analytical method for the mixture of chlorefonn, benzen, trichloroethylen and 1,4-dioxane simultaneously and determine the remainingorgauic solvents in coating tablets by Purge & Trap-GC. The results were as follouFs ; 1. Chloroform, benzen, trio:tloroethylen and 1,4-dioxane separated by tenax #5 trap by HP-624GC column by terrlperature programming. The peaks were separated completely at retentiontime of 6.88min for chloroform, 8.21min for benzen, 10.38miu for trichloroethylen and 11.95minfor 1,4-dioxane. 2. Standard RSD were individually chloroform 3.03%, benzen 3.17%, trichloroethylen 3.69%and 1,4-diorane 3.41%. 3. 60 samples were not detrcted chloroform, benzen, trichloroethylen and 1,4-dioxane.
Sympathetic Block for Treating Primary Erythromelalgia

Primary erythromelalgia is a rare condition that’s characterized by erythema, an increased skin temperature and burning pain in the extremities. The pain is often very severe, and treating erythromelalgia is frustrating and difficult. We report here on the case of a 12-year old girl with primary erythromelalgia in both lower extremities. The pain was refractory to medical treatment, but a bilateral sympathetic block with lidocaine and triamcinolone resulted in relief from the pain. Our experience with this disease demonstrates that sympathetic blocks are effective in improving the symptoms and they may be attempted on erythromelalgia patients who do not respond to other treatments, including medication and epidural blocks.

Macrophage-derived chemokine, C-C motif chemokine 22 (MDC/CCL22), is one of the inflammatory chemokines that controls the movement of monocytes, monocyte-derived dendritic cells, and natural killer cells. Serum and skin MDC/CCL22 levels are elevated in atopic dermatitis, which suggests that the chemokines produced from keratinocytes are responsible for attracting inflammatory lymphocytes to the skin. A major signaling pathway in the interferon-γ (IFN-γ)-stimulated inflammation response involves the signal transducers and activators of transcription 1 (STAT1). In the present study, we investigated the anti-inflammatory effect of dieckol and its possible action mechanisms in the category of skin inflammation including atopic dermatitis. Dieckol inhibited MDC/CCL22 production induced by IFN-γ (10 ng/mL) in a dose dependent manner. Dieckol (5 and 10 μM) suppressed the phosphorylation and the nuclear translocation of STAT1. These results suggest that dieckol exhibits anti-inflammatory effect via the down-regulation of STAT1 activation.

Epidemiological studies of monoclonal B-cell lymphocytosis (MBL) have been conducted in limited geographical regions. Little is known about the prevalence of MBL in Asia. We investigated the prevalence and immunophenotypic characteristics of MBL in Koreans who had idiopathic lymphocytosis (lymphocyte count >4.0×109/L) and were ≥40 years of age. A total of 105 leftover peripheral blood samples met these criteria among those from 73,727 healthy individuals who visited the Health Promotion Center, Samsung Medical Center, Korea, from June 2018 to August 2019. The samples were analyzed using eight-color flow cytometry with the following monoclonal antibodies: CD45, CD5, CD10, CD19, CD20, CD23, and kappa and lambda light chains. The overall prevalence of MBL in the study population was 2.9% (3/105); there was one case of chronic lymphocytic leukemia (CLL)-like MBL (CD5+CD23+), one case of atypical CLL-like MBL (CD5+CD23−), and one case of CD5−MBL with a lambda restriction pattern. This is the first study on the MBL prevalence in an East Asian population, and it reveals a relatively low prevalence of MBL in healthy Korean individuals with lymphocytosis.
Oleylamine-induced pustular contact dermatitis

A 23-year-old female presented with multiple yellowish pustules on face, neck and both forearms occurred after accidental contact with oleylamine. Although the patient irrigated the wound immediately, pustules occured several hours later. Laboratory findings revealed a C-reactive protein level of 2.34mg/dL and bacteria did not grow in bacterial culture from pustules. Histopathologic examination revealed epidermal necrolysis with neutrophils and eosinophils infiltration accompanied by pustule formation. Based on these clinical and histopathological findings, she was diagnosed with pustular contact dermatitis. Oral prednisolone 10mg, cephradine 1000 mg for 2 weeks and topical methylprednisolone was applied. The lesions improved with only slight pigmentation within several weeks. We believe that the chronological correlation and clinical aspect support the diagnosis of an oleylamine-induced contact dermatitis. The pustulation of the skin is believed to be caused by irritancy or delayed hypersensitivity. Oleylamine is organic compound mainly used as a surfactant, and known to cause skin corrosion and severe eye damage. Pustular allergic or irritant contact dermatitis has been reported previously due to croton oil, merbromine, fluorine and minoxidil. But there was no case that pustular contact dermatitis triggered by oleylamine. Herein, we report the first case of oleylamineinduced pustular contact dermatitis.
