외과계 집중치료실에서 입원한 환자에서 APACHE II score의 의의

This study attempts to evaluate the prognostic value of the APACHE II scoring system in patients of surgical intensive care unit(SICU) and to examine its usefullness in stratifying these patients according to their surgical risk and as a potential aid in making rational and objective treatment decision. The records of 161 patients admitted to the Department of Surgery, Gyeong-Sang National University Hospital between Jan. 1991 and Dec. l995 were reviewed. 1) Of a total of 161 patients, 87 were male, 74 were female, and male to female ratio was 1.71: 1. 2) This group was seen most commonly in the age group of 55-64 which occupied 30.3% of entire group. 3) The distribution of patients according to the etiology were 74 patients(46%) in trauma, 61 patients(37%) iin postop-complication, 26 patients(9%) in others. 4) The distribution of patients according to the APACHE II scores was 9 to 29 point in adm. Day and the most patients was located between 10 and 14 of 62 patient(39%). 5) The length of hospitalization was 9 days in survived patients and 8 days in non-survived patients. The length of hospitalization of 77% of patients in this study was below 10 days. 6) The death rate was 45.45%(73 patient), this patients according to APACHE II scores was 13 to 29, median 19.2 in adm. Day, the most patients was located 20 to 24(20%). Non of the survived patients had the APACHE II score of above 20 points, but all recovered patients were below 13 points. 7) The median APACHE II score in survived patients was 10.52 point at adm. Day and 6.55 point at 5 days after admission. And non-survived patients was 20.25 point at adm. Day and 30.48 point 5 days after admission(p$lt;0.001). 8) Among the patients with low APACHE II score (below 14 points), there was no mortality. But, there was mortality with APACHE II score of above l5 points. We conclude that APACHE II scoring system is useful in measuring the severity of acute disease and predicting the outcome.
Multi-Dimensional Suppression of Normal Hematopoietic Cells by MN1-Induced Leukemic Cells

Leukemogenesis comprises engraftment and self-renewal of leukemic stem cells, but also causes insufficiency in normal hematopoietic cells. Here we have investigated the influence of leukemia on various stages of normal hematopoietic functions using meningioma 1 (MN1)-induced acute myelogeneous leukemia model. When normal hematopoietic stem cells (HSCs) were co-transplanted with MN1 leukemia cells, the ability of normal HSCs to execute self-renewal was profoundly suppressed, as demonstrated by lower numbers of competitive repopulating units (CRUs) in the reconstituted bone marrows. The inhibitory effects become more prominent in the competition of leukemic cells for bone marrow niche. Moreover, when normal bone marrow cells were co-cultured with MN1 cells in vitro, the apoptosis of normal bone marrow cells (BMCs) were >2 fold increased, as compared to the culture of normal BMCs alone (p<0.05), whereas MN1 cells in the culture were not affected. The apoptosis of normal BMCs occurred more frequently in the mature myeloid cells than in lineage-negative populations in a manner dependent on direct contact with leukemic cells. Interestingly, the mean survival of leukemia mice was more closely correlated to the extent of bone marrow ablation caused by different dose of radiation to mice than the numbers of leukemic cells transplanted into the irradiated mice. Taken together, our results show that leukemia cells exert inhibitory effects on the multiple stages of normal hematopoiesis, and that such inhibitory influence should play as a key factor in the morbidity of leukemia during the course of illness.

There are diverse ring-like narrowings in the lower esophagus: mucosal ring, muscular ring, ring-like annular peptic stricture. Esophageal muscular ring is extremely rare and generally asymptomatic. It occurs at the proximal border of the esophageal vestibule that corresponds to the upper end of the lower esophageal sphincter. We experienced a case of lower esophageal muscular ring with a symptom of intermittent mild dysphagia for 40 years and report the findings with a review of the literature.(Korean J Gastroenterol 2003;42:539-543)

Malignant eccrine poroma is a rare skin appendage tumor, originating from the intraepidermal and upper dermal eccrine ducts. It is a locally aggressive neoplasm with a high propensity to metastasize. A 47-year-old woman was admitted to the hospital because of dyspnea. One and half years before admission, she had been diagnosed porocarcinoma of left sole and treated with wide local excision. One months after excision, examination had revealed an inguinal lymph node metastasis and she had been treated with lymph node resection and radiotherapy. During the two weeks before admission, the patient had increasing exertional dyspnea, which was accompanied by Rt-sided chest wall pain. On admission right pleural effusion and multiple nodular densities were noted at initial chest radiograph. A computed tomographic (CT) scan of the chest showed lymphatic and hematogenous lung metastasis with pleural seeding of unknown primary tumor. Fine-needle aspiration biopsy of the pleura revealed typical cytologic findings of porocarcinoma. She was treated with therapeutic thoracentesis and pleurodesis with bleomycin. Because porocarcinoma is resistant to chemotherapy and radiotherapy, she has been treated with conservative treatment.

Background : The sodium-iodide symporter (NIS) is a key plasma membrane glycoprotein that mediates active iodide transport in the thyroid gland. Whereas relatively little is known about the mechanisms by which iodide regulates NIS activity, post-transcrip
