Productivity Loss of Rheumatoid Arthritis Patients according to the Their Stages of the Disease Activity Score

Objective. Productivity loss was compared by 3-stage of disease activity and associations between higher disease activity and high productivity loss were identified. Methods. Data were extracted from Rheumatoid Arthritis (RA) Patient-reported Outcomes Research, which enrolled 2,000 RA patients (>20-year) on disease-modifying-antirheumatic-drugs (DMARDs) (≥ 6-month) from December 2012 to June 2013. This included 1,457 RA patients with the disease activity score (DAS-28-ESR) in their medical charts. Productivity loss in time and indirect cost was estimated using The World Health Organization Health and Work Performance Questionnaire (HPQ). Baseline characteristics and productivity loss outcomes were compared according to DAS-28-ESR groups. Results. 84.4% were females, 54.2% had low DAS-28-ESR (<3.2), and 38.2% and 7.6% had moderate (3.2∼5.1) and high DAS-28-ESR (>5.1). Patients with moderate to high DAS-28-ESR had higher lost productivity time (LPT) and monthly costs of LPT than those with low DAS-28-ESR (time in hours: 110.0±58.4 vs. 132.4±57.2 vs. 71.5±52.0, p<0.0001; monthly costs of LPT in 1,000 Korean won: 1,097±607 vs. 1,302±554 vs. 741±531, p<0.0001). Multiple regression analyses revealed significant associations with high LPT in high (adjusted odds ratio [OR]=3.87, 95% confidence interval [CI]: 2.18∼6.87) and moderate DAS-28-ESR (adjusted OR=1.88, 95% CI: 1.41∼2.52) compared to low DAS-28-ESR. In addition, positive associations with high monthly costs of LPT were observed in high (adjusted OR=3.45, 95% CI: 1.98∼5.99) and moderate DAS-28-ESR (adjusted OR=1.93, 95% CI: 1.43∼2.54) compared to low DAS-28-ESR. Conclusion. Timely therapeutic strategies should be taken into consideration given that the RA patients with moderate to high DAS-28-ESR showed strong associations with high productivity loss for effective management of RA. The prevalence of PPNG among pretreated gonorrhea cases isolated at the STD clinic of Choong-Ku Public Health Center in Seoul has been studied and reported annually since 1981. In 1991, 123 strains of N.gonorrhoeae were isolated, among which 58(47.1%) were PPNG. In 1992, 98 starains of N.gonorrhoeae were isolated, among which 51(52.0%) were PPNG. In all, 109(49.3%) strains were found to be PPNG among 221 strains isolated between 1991-1992. The prevalence of PPNG in Seoul showed increased tendency till 1989, thereafter, it has been stationary or slightly decreasing.

Gastrointestinal stromal tumor (GIST) is the most common non-epithelial neoplasm arising in the gastrointestinal tract, but this tumor is rarely seen in association with type l neurofibromatosis (NFl) Generally, the majority of GISTs are isolated neoplasm and they are sporadically found in the stomach, but the GISTs in NFl patients are usually multiple and usually found in the small intestine. We report a case of multiple GISTs in the jejunum of a 63-year-old woman diagnosed as NFl accompanied by complication of gastrointestinal bleeding, In this case, Patient had multiple cafe-au lait spots and neurofibroma on skin and had freckling on axilla and groin, and then, we made a diagnosis of NFl. Gastrointestinal bleeding is controlled by resection of multiple GISTs. Generally, only 3-5% of all gastrointestinal bleeding comes from the small bowel. Causes of small intestinal bleeding are angiodysplasia, neoplasm, NSAIDs induced ulcer etc. but, If patients have NFl accompanied by complication of gastrointestinal bleeding, GISTs on small bowel must be considered.

Many of the growth-promoting properties of growth hormone(GH) are mediated by insulin-like growth factor-I(IGF-I), a highly conserved circulating 70-amino acid peptide. Recent studies have shown that multiple mechanisms influence IGF-I gene expression, including transcription from two promoters, alternative RNA splicing, and variable polyadenylation. In thyroid tissue, thyroid stimulating hormone(TSH) and IGF-I are the most possible candidates for follicular cell proliferation and hypertrophy. Actually IGF-I had autocrine and paracrine effect for tissue growing. We prepared thyroid tumor tissue mRNAs using single step method for detecting IGF-I levels according to different tissues, i.e., thyroid adenoma or papillary thyroid carcinoma. We used Northern blot analysis for IGF-I mRNA and RNase protection assay (RPA) for IGF-I transcription start sites. For Northern blot, we used whole human IGF-I cDNA as a DNA probe and for RPA, we used IGF-I exon 1 containing noncoding promoter 1 as a riboprobe. We got good RNA bands from Northern blot analysis around 1 kb (IGF-IA) and 7.5 kb (IGF-IB) region. To clarify the amount of both IGF-IA and IB mRNAs, we measured autoradiographied signal of IGF-I mRNAs bands using densitometer. In IGF-IA signals, there's no change among liver and thyroid tissues, but in case of IGF-IB mRNA bands, the signal was markedly increased in thyroid carcinoma tissues than that of normal thyroid tissue (85% vs 14%). In the study of RPA, all thyroid tissues used the same transcription start sites as those of liver's. We concluded that that this different regulation of IGF-I mRNA was originated from tissue specificity. That meant some tissue specific transcription factor/s were related to tissue IGF-I expression.

The purpose of this study was to compare the characterists and change in physique and physical function by college student of physical education major(287males and 40 females of age groups ranging from 18 to 26). The results were as following; 1) The characteristics of physique and physical function by sex and age. ① The male group of 18yrs old showed higher values in Height, Weight, Upper Limb Length, Strength, Flexibility, and Vital Capacity than any other groups, but female group showed lower values in Height, Weight, Chest Girth, Lower Limb Length, Thigh, Girth, Calf Girth and all of the Physical function. ② The male group of 19yrs old showed high values in power and Agility, then female group showed in Strength and Flexibility. ③ The male group of 20yrs old showed the lower values in Weight, Waist Girth, Upper Limb Length, Fore Arm Girth, Thigh Girh.Calf Girth, Vital Capacity, and female group are very lowed in Upper Arm Lenght, Fore Arm Length, Skin Fold Thickness and 5 minute Run. ④ The male group of 23yrs old showed the high values in Upper Arm Girth, Fore Arm Girth and Balance, and then female group higher in Weight, Chest Girth, Waist Girth, Upper Arm Girth, Fore Arm Girth, Thigh Girth, Power, Shuttle Run, Balance and vital capacity than any other age Groups. ⑤ In male group, Chest Girth, and Blood Pressure were significantly related, and Side Step and Trunk Extension were nagatively related to the age at 0.1% level, and then in female group, Sargent Jump, Standing Long Jump and Vital Capacity were showed a significantly related at 0.5% level. 2) The difference of physique and physical function between male and female by age. ① Everage difference in whole physique items except skinfold thickness was 9.8%, and that 21.1% in all of physical function except Trunk Flextion between male and female group subjects. ② About the ages, the 18yrs old male group showed the 22.8% higher values of 26 items, but 22yrs old male group were about 12.2% in 23 items, thus it is very lower than any other groups. 3) The difference of physique and physical function between General group and physical education group in age-matched. ① In the physique male physical education groups showed about 7% higher in Weight, Chest Girth, Upper Limb Length, Upper Arm Girth, Thigh Girth, and female group showed about 7% higher in Weight and Upper Limb Length. ② In the physical function, Trunk Flextion, 5 minute Run, and Blood Pressure(Diastolic) were about 8% high values in whole group, and female group showed about 8% higher in Standing Long Jump, Shuttle Run, and Trunk Extension. ③ In 22yrs old male and female physical education group were very dominated in physique and physical function than the other age group against the general group.

As time goes up, structural capacity of members decreases in reinforced concrete structures. The deterioration of the capacity is mainly due to the corrosion of materials, poor construction and environmental pollution, etc. This deterioration is unavoidable, thus it is necessary to develop the method for repair and retrofit of structures in order to ensure structural safety of members. The purpose of this paper is to investigate the load-deflection, ultimate strength, failure mode, and energy absorption effect. For the test, the specimen size of column is 20cm×20cm×90cm reinforced with 4 number of main bars of 13mm diameter, tied bars of 6mm diameter. The test results show that the increasement of bolt and bolt tension's quantity improve the ultimate strength and energy absorption capacity.
