Objective: Occupational medicine specialists in the Busan, Ulsan and Kyung-Nam areas established an area-based occupational disease surveillance system and used this system to collected case information for the purpose of preventing occupational diseases Methods: l l hospital participated in this system. The authors selected five main diseases, which were hand-arm vibration syndrome (HAVS), work-related musculoskeletal disorder (WRMSD), occupational asthma, occupational skin disease and occupational and occupational lung cancer and established their case definitions. All cases were reported on the web, and real time analysis was conducted. Results: Between April 2001 and April 2003, 192 cases of HAVS, 118 cases of WRMSD, 33 cases of occupational asthma, 17 cases of occupational skin disease and 17 cases of occupational lung cancer (for a total of 377 cases of these five main diseases) were reported. most of the HAVS cases came from shipbuilding, and 172 of them (92.7%) were associated with grinding. Of the four main types of WRMSD, carpal tunnel syndrome (CTS) was the most prevalent with 46 cases and shipbuilding was also the main industry involved (83.9%). The ergonomic risk factors involved mainly associated with the hand. In 19 (57.6%) and 4 (12.1%) cases of occupational asthma, the agents involved. The causative agents of occupational lung cancer included Cr, welding fume, PAH, Ni, etc. 10(58.8%) of the cases were diagnosed as squamous cell carcinoma and 5(29.4%) as adenoma. Conclusion: This result showed that an area-based occupational disease surveillance system might provide an effective method of evaluating the prevalence of such diseases, however the Busan, Ulsan and Kyung-Nam provinces are too large to be treated as individual areas. Therefore, the authors suggest that each province should establish its own surveillance system.

Objective: Recent evidence shows that the quantitative value of amyloid-beta (Aβ) deposition below the threshold of Aβ positivity carries biological and clinical significance regarding future cognitive decline. We evaluated whether the quantitative value of subthreshold Aβ deposition had a significant correlation with neuropsychological test scores in cognitively normal older adults without the APOE ε4 allele. Methods: Sixty cognitively normal APOE ε4 allele non-carriers with negative Aβ retention aged 60 to 85 years were included in this study. We assessed neuropsychological performance with the Korean version of the Consortium to Establish a Registry for Alzheimer’s Disease (CERAD-K) and obtained standardized [18F] flutemetamol uptake values in the pons as a reference (SUVRPONS), evaluated with PET. Multiple regression analyses were conducted to assess the effect of global and regional Aβ load on cognitive performance, adjusting for age, sex, years of education, and volumes of white matter hyperintensities. Results: We found that Aβ deposition in the precuneus, posterior cingulate cortex, and parietal lobe had a significant association with the total CERAD-K scores. There was also a significant correlation between the SUVRPONS in the precuneus and the CERAD-K total score after Bonferroni correction. Conclusion: Subthreshold Aβ retention in the core brain regions of the default mode network could affect cognitive functions in the cognitively normal APOE ε4 non-carriers, considered to be the lowest risk group for Alzheimer’s disease (AD).

Erdheim-Chester disease (ECD) is a proliferative non-Langerhans cell histiocytosis of multiple organs. This is a rare disease of unknown etiology with a high mortality. We present the case report of a 26-year-old man diagnosed with ECD. He was referred to our hospital with elevated levels of aminotransferases. Although the diagnosis was uncertain, the patient was lost to follow up at that time. One year later, the patient returned to the hospital with generalized edema. Although a specific bone lesion was not found, the patient was experiencing the following: glomerulonephritis, aplastic anemia, hepatitis, and lung involvement. A lung biopsy was performed: the immunohistochemical stain were positive for CD68 and negative for S-100 protein and CD1a. We diagnosed as the patient as havinf ECD. Approximately 50% of ECD cases present with extraskeletal involvement. ECD should be considered as part of the differential diagnosis when multiple organs are involved.

The need for organic soil amendments is increasing in the Republic of Korea against the backdrop of increased soil acidification and nutrient losses. The pyrolysis of biomass produces biochar which not only increases soil productivity but also provides environmental benefits through carbon sequestration. The portion of the brewer’s spent grain (BSG) recycled is by far less than the amount generated, but pyrolysis can help to reverse this trend by turning BSG waste into a valuable soil amendment. The current study, therefore, evaluated the effects of brewer’s spent grain biochar ( BBXXX) produced at three different temperatures of 300 °C, 500 °C and 700 °C on the yield and quality characteristics of the leaf lettuce as well as the effects on soil chemical properties through a pot experiment. Each of the BBXXX and BSG were added to the soil at two rates of 2% and 5% by weight. The pH and carbon content of the BBxxx increased with increasing pyrolysis temperatures and the trend was replicated in the soil upon biochar application i.e. the soil pH and carbon content increased alongside temperatures at which biochar was pyrolyzed. On the other hand, however, the soil electrical conductivity (EC) diminished with the increasing pyrolysis temperatures of the biochar applied. With regards to crop growth, the BB500 5% amendment produced the highest marketable yield of the leaf lettuce and while the lettuce grown on the control produced leaf lettuce with the lowest content of nitrate nitrogen, BB500 5% amendment generally produced the highest quality lettuce. The results indicate that BB500 performed agronomically better than the rest of the amendments and is thus recommended as an effective BSG recycling measure.

Emphysematous pyelonephritis (EPN) is a life-threatening infection characterizedby the formation of gas. Complications of EPN include septic shock, acuterenal failure, and disseminated intravascular coagulation. Spontaneous subcapsularhematoma (SCH) has also been reported as a rare complication of EPN, althoughthere have been no reports to date of this occurring prior to the presentation of EPN. We report a case of EPN that initially presented as spontaneous SCH. The patientwas admitted for left flank pain, and initial computed tomography revealed SCHwithout any air shadows. Laboratory findings and clinical symptoms suggested thepresence of urinary tract infection and the patient was started on antibiotics. Feverdeveloped 24 hours after admission. On follow-up computed tomography 7 dayslater, EPN was newly observed, and a percutaneous drain was inserted. Blood, urine,and drainage fluid cultures all revealed growth of extended-spectrum β-lactamasenegativeEscherichia coli.

<P>Inflammation-mediated oncogenesis has been implicated in a variety of cancer types. Rheumatoid synovial tissues can be viewed as a tumor-like mass, consisting of hyperplastic fibroblast-like synoviocytes (FLSs). FLSs of rheumatoid arthritis (RA) patients have promigratory and invasive characteristics, which may be caused by chronic exposure to genotoxic stimuli, including hypoxia and growth factors. We tested whether a transformed phenotype of RA-FLSs is associated with placental growth factor (PlGF), a representative angiogenic growth factor induced by hypoxia. In this study, we identified <I>PlGF-1</I> and <I>PlGF-2</I> as the major <I>PlGF</I> isoforms in RA-FLSs. Global gene expression profiling revealed that cell proliferation, apoptosis, angiogenesis, and cell migration were mainly represented by differentially expressed genes in RA-FLSs transfected with small interfering RNA for <I>PlGF</I>. Indeed, <I>PlGF</I>-deficient RA-FLSs showed a decrease in cell proliferation, migration, and invasion, but an increase in apoptotic death in vitro. <I>PlGF</I> gene overexpression resulted in the opposite effects. Moreover, exogeneous PlGF-1 and PlGF-2 increased survival, migration, and invasiveness of RA-FLSs by binding their receptors, Flt-1 and neuropilin-1, and upregulating the expression of antiapoptotic molecules, pErk and Bcl2. Knockdown of <I>PlGF</I> transcripts reduced RA-FLS proliferation in a xenotransplantation model. Collectively, in addition to their role for neovascularization, PlGF-1 and -2 promote proliferation, survival, migration, and invasion of RA-FLSs in an autocrine and paracrine manner. These results demonstrated how primary cells of mesenchymal origin acquired an aggressive and transformed phenotype. PlGF and its receptors thus offer new targets for anti-FLS therapy.</P>
