Background and Purposes: Over the past three decades, numerous studies performed in Korea have reported that job stress is a determinant risk factor for chronic diseases and work disability. Every society has its own culture and occupational climate particular to their organizations, and hence experiences different occupational stress. An occupational stress measurement tool therefore needs to be developed to estimate it objectively. The purpose of this study is to develop and standardize the Korean Occupational Stress Scale (KOSS) which is considered to be unique and specific occupational stressors in Korean employees. Subjects and Methods: Data were obtained from the National Study for Development and Standardization of Occupational Stress (NSDSOS Project: 2002-2004). A total of 12,631 employees from a nationwide sample proportional to the Korean Standard Industrial Classification and the Korean Standard Occupational Classification were administered. The KOSS was developed for 2 years (2002-2004). In the first year, we collected 255 items from the most popular job stress measurement tools such as JCQ, ERI, NIOSH and OSI, and 44 items derived from the a qualitative study (depth interview). Forty-three items of KOSS, in the second year, were retained for use in the final version of the KOSS by using Delphi and factor analysis. Items were scored using conventional 1-2-3-4 Likert scores for the response categories. Results: We developed eight subscales by using factor analysis and validation process: physical environment (3 items), job demand (8 items), insufficient job control (5 items), interpersonal conflict (4 items), job insecurity (6 items), organizational system (7 items), lack of reward (6 items), and occupational climate (4 items). Together they explained 50.0% of total variance. Internal consistency alpha scores were ranged from 0.51 to 0.82. Twenty-four items of the short form of the KOSS (KOSS-SF) were also developed to estimate job stress in the work setting. Because the levels of the subscales of occupational stress were gender dependent, gender-specific standard norms for both the 43-item full version and the 24-item short form using a quartile for the subscales of KOSS were presented. Conclusion: The results of this study suggest that KOSS might be an appropriate measurement scale to estimate occupational stress of Korean employees. Further and more detailed study needs to be conducted to improve the validity of this scale.

Chasmagnathus convexus (De Haan, 1835) is a monotypic species belonging to the family Varunidae. Chasmagnathus convexus has been designated as a marine protected species and endangered species by the Wildlife Protection and Management Act (2005) of Korea due to its declining population in the wild. This declining population is a result of habitat loss and environmental change. This study is the first to research the mitochondrial cytochrome c oxidase subunit I (COI) in Korean C. convexus. The maximum intra-specific genetic variation among all C. convexus individuals was 1.8%, while the inter-genetic variation among the five varunid species was in the range of 16.0-23.7%. The COI barcodes will be used as a reference for restoration and conservation studies of Korean C. convexus.

Background and Objectives : Epistaxis is a common disease from which approximately 10% of the normal population have suffered in their life. Most bleeding sites of epistaxis are located anteriorly and easily controlled. However posterior epistaxis presents significantly great problems. A number of different treatment modalities are used to control the posterior epistaxis. Although not widely practiced, the nasal endoscopic electrocautery has also been effective. The purpose of this article is to report on the efficacy of nasal endoscopic electrocautery as an alternative and adjuvant to the standard approach for control of posterior epistaxis. Materials and Methods: We conducted retrospective chart review about 19 cases of posterior epistaxis treated from January 1998 to December 1999. Group I patients were managed by nasal packing only and group II patients were managed by nasal packing and endoscopic electrocautery. Results: Hospital days were 5.9 days in the group I and 3.6 days in the group II. The duration of nasal packing was 4.1 days in the group I and 1.8 days in the group II. The results were significant statistically Conclusion: Nasal endoscopic electrocautery was an effective treatment modality for control of posterior epistaxis.

Central diabetes insipidus (DI) is a syndrome characterized by thirst, polydipsia and polyuria. Langerhans cell histiocytosis is one of the etiologies of DI. Recently we experienced a central DI associated with Langerhans cell histiocytosis. The 44 years old female patient complained right hip pain polydipsia and polyuria. We carried out water deprivation test. After vasopressin injection, urine osmotic pressure was increased form 109mOsmol/Kg to 327mOsmol/Kg (300%). Brain MRI showed a thickened pituitary stalk and at hot bone CT.CT guided biopsy revealed abnormal histiocytes proliferation and abundant lymphocytes, The final diagnosis was central DI associated with systemic Langerhans cell histiocytosis invading hip bone, L-spine and pituitary stalk. Desmopressin and etoposide chemotherapy were performed to the patient.
Changes of depression and job stress in workers after merger without downsizing

Background: Since the 1980s, restructuring, which includes downsizing, closures, mergers, and privatization, has expanded worldwide, and various studies have investigated its effect on health. However, previous studies have mainly focused on restructuring accompanied by massive lay-offs, and the effect of a merger on workers’ health is still controversial. This study aims to investigate changes in worker depression and job stress after a merger without downsizing, which is unusual in Korea. Methods: Repeated surveys were done in April 2014, April 2015, and April 2016 involving the participation of 209 subjects. Participants were divided into two groups, which were comprised of blue-collar workers (104) and whitecollar workers (105). Sociodemographic characteristics, including age, education level, job tenure, gender, marital status, smoking status, and alcohol consumption, were measured via a survey. To determine the level of depression, the Korean version of the Center for Epidemiologic Studies Depression Scale (CES-D) was employed, and to investigate job stress, the Korean Occupational Stress Scale-Short Form (KOSS-SF) was used. For statistical analyses, Pearson’s chi-square test, the Student’s t-test, and repeated measure analysis of variance (ANOVA) were performed. Results: The results showed that depression (CES-D, F[2, 400] = 0.466, p = 0.628) was changed but without significance and job stress (KOSS-SF, F[1.899, 379.831] = 3.192, p = 0.045) were significantly different. The betweengroup difference in the CES-D score between the blue- and white-collar workers by survey administration time was not statistically significant (F = 0.316, p = 0.574). The interaction between the survey time and occupational group was also not statistically significant (F = 0.967, p = 0.381). The between-group difference in the KOSS-SF total score was not statistically significant (F = 1.132, p = 0.289), and the interaction between the survey administration time and occupational group was also not significant (F = 0.817, p = 0.437). In the job stress subgroup analyses Job insecurity and Lack of reward showed a significant difference by survey administration time. Conclusion: This study showed that a merger without massive downsizing can cause negative health effects such as an changes in depression and increase in job stress. To improve the health of workers, both the immediate negative effects on health, and the long-term effects or their resolution over time should be considered prior to the merger.

Primary cutaneous marginal zone lymphoma is an indolent B-cell lymphoma of the skin, occasionally showing significant T-cell infiltration with some extent of cytologic atypia. However, as there have been several reports of progression of T-cell lymphoma within the background of primary cutaneous marginal zone lymphoma, it is important to clarify whether the T-cell infiltrates present in T-cell-rich primary cutaneous marginal zone lymphoma are malignant or not. We report a case of cutaneous marginal zone lymphoma with the coexistence of T-cell-rich angiomatoid polypoid pseudolymphoma on the face of a Korean girl.

We report a case of successful heart transplantation after 67 days of support with venoarterial extracorporeal membrane oxygenation (ECMO) in a patient who underwent surgery for type A aortic dissection and myocardial infarction complicated by irreversible myocardial damage and a deep sternal wound infection. During ECMO support, left heart vent and distal limb perfusion were performed. Mediastinitis was treated with mediastinal washout and irrigation. Multiple complications from peripheral ECMO were successfully managed.
