Primary Shewanella algae Bacteremia Mimicking Vibrio Septicemia

Shewanella algae infections are rare in humans. Previously reported cases of S. algae have mainly been associated with direct contact with seawater. We report a case of primary S. algae bacteremia occurring after the ingestion of raw seafood in a patient with liver cirrhosis that presented a fulminent course of necrotizing fasciitis.

The human intestinal microbiota is a community of 1013-1014 microorganisms that harbor in the intestine and normally participate in a symbiotic relationship with human. Technical and conceptual advances have enabled rapid progress in characterizing the taxonomic composition, metabolic capacity and immunomodulatory activity of the human intestinal microbiota. Their collective genome, defined as microbiome, is estimated to contain ≥150 times as many genes as 2.85 billion base pair human genome. The intestinal microbiota and its microbiome form a diverse and complex ecological community that profoundly impact intestinal homeostasis and disease states. It is becoming increasingly evident that the large and complex bacterial population of the large intestine plays an important role in colorectal carcinogenesis. Numerous studies show that gut immunity and inflammation have impact on the development of colorectal cancer. Additionally, bacteria have been linked to colorectal cancer by the production of toxic and genotoxic bacterial metabolite. In this review, we discuss the multifactorial role of intestinal microbiota in colorectal cancer and role for probiotics in the prevention of colorectal cancer. An angiomyolipoma is a benign mesenchymal neoplasm that typically occurs in the kidney of patients with tuberous sclerosis. Extrarenal angiomyolipomas are uncommon, and the adrenal gland is an extremely rare site for the tumor. An incidental adrenal mass is the usual presentation of an adrenal angiomyolipoma, as most of the tumors are hormonally inactive. Recently we experienced one case of a right adrenal angiomyolipoma that presented with an adrenal incidentaloma. To the best of our knowledge, this is the first case of an adrenal angiomyolipoma described in the Korean medical literature. We report the case with a special emphasis on the differential imaging findings of fat-containing adrenal tumors. (J Kor Endocrine Soc 22:371~375, 2007) 부신의 혈관근육지방종은 매우 드물며 국내에 보고된 바 없다. 지방 조직, 평활근 및 혈관으로 구성된 중간엽 기원의 양성 종양이며 호르몬 검사상 이상 소견을 보이지 않으므로 대부분 영상진단 검사과정에서 우연히 발견된다. 영상진단 소견상 지방육종과 같은 악성종양과 감별이 어려워 대부분 수술적 절제가 필요하며, 완전히 절제한 경우 양성종양으로 재발의 가능성은 매우 낮다. 저자들은 우측 부신에 발생한 혈관근육지방종을 경험하였기에 그 특징적인 영상진단소견 및 임상상을 보고하는 바이다.
Parathyroid Cyst Presenting as Acute Pancreatitis: Report of a Case

We report the first case of hypercalcemia-induced acute pancreatitis caused by a functioning parathyroid cyst in a 67-year-old man. Laboratory investigation revealed increased serum amylase and lipase, increased serum ionized calcium and parathyroid hormone (PTH) levels, and decreased serum phosphate, indicating pancreatitis and primary hyperparathyroidism (PHPT). Abdominal computed tomography (CT) revealed mild swelling of the pancreatic head with peri-pancreatic fat infiltration and fluid collection around the pancreatic tail. Ultrasonography and CT of the neck showed a cystic lesion at the inferior portion of the left thyroid gland, suggesting a parathyroid cyst. There was no evidence of parathyroid adenoma by 99mTc sestamibi scintigraphy. PHPT caused by a functioning parathyroid cyst was suspected. The patient underwent surgical resection of the functioning parathyroid cyst owing to his prolonged hypercalcemia. At 3 weeks after the operation, his serum levels of PTH, total calcium, ionized calcium, inorganic phosphate, amylase, and lipase were normalized. At the follow-up examinations, he has remained asymptomatic.
