소방응급의료서비스 향상을 위한 평가요인의 중요도에 관한 실증적 연구

This study is performed to determine the priorites of the evaluation factors for the improvement of fire emergency medical service by increasing it’s efficiency to protect the people’s lives effectively, and by recognizing it’s mid-long term development. Based on the output of the analysis, the critical evaluation factors of the fire emergency medical services to be improved shall be identified, and shall be intended to use strategically for increasing the efficiency of all the activity in the EMT service, and also for providing the basic date to product and manage the good quality services. To calculate the priority of the evaluation factor for each level as shown on this study, AHP(anaysis hierarchy process) with pairwise comparison method is applied. According to the priority analysis for the evaluation factors in main category (evaluation zone), ‘medical service professional aspect’ is recognized as highest one, and ‘service infrastructure aspect, ‘service management aspect’ are indicated in order. According to the priority analysis for the evaluation factors in evaluation index, ‘professional personal arrangement’ in the service infrastructure aspect, ‘appropriateness and timeliness of emergency medical care’ in the medical service professional aspect, and ‘clinical training’ in the service management aspect are respectively recognized as most important factors.
폐동맥 고혈압으로 발현한 선천성 간문맥 형성부전의 증례 보고

Congenital absence of the portal vein (CAPV) is a rare malformation in which intestinal and splenic venous flow bypasses the liver and drains directly into the systemic circulation via a congenital portosystemic shunt. We describe two cases of CAPV presenting as pulmonary hypertension that were initially suspected as primary pulmonary hypertension. However, subsequent ultrasonography and CT detected the absence of a portal vein and the presence of a portosystemic shunt. Pulmonary hypertension is a recognized complication of liver disease and portal hypertension. However, these two cases illustrate that CAPV may result in pulmonary hypertension without liver disease or portal hypertension. 선천성 간문맥형성부전은 문맥혈류가 간을 우회하여 문맥-체정맥 단락을통해 대정맥으로 직접 배출되는 드문 기형이다. 저자들은 일차성 폐동맥고혈압으로 진단되었다가 초음파 및 전산화단층촬영을 통해 문맥의 형성부전 및 선천성 문맥-체정맥 단락을 비침습적으로 발견해 냄으로써, 이로 인한 이차성 폐동맥 고혈압이 발생하였음을 밝힐 수 있었던 증례를 2예 보고하고자 한다.

Cre recombinase is widely used to manipulate DNA sequences for both in vitro and in vivo research. Attachment of a trans-activator of transcription (TAT) sequence to Cre allows TATCre to penetrate the cell membrane, and the addition of a nuclear localization signal (NLS) helps the enzyme to translocate into the nucleus. Since the yield of recombinant TAT-Cre is limited by formation of inclusion bodies, we hypothesized that the positively charged arginine-rich TAT sequence causes the inclusion body formation, whereas its neutralization by the addition of a negatively charged sequence improves solubility of the protein. To prove this, we neutralized the positively charged TAT sequence by proximally attaching a negatively charged poly-glutamate (E12) sequence. We found that the E12 tag improved the solubility and yield of E12-TAT-NLS-Cre (E12-TAT-Cre) compared with those of TAT-NLS-Cre (TATCre) when expressed in E. coli. Furthermore, the growth of cells expressing E12-TAT-Cre was increased compared with that of the cells expressing TAT-Cre. Efficacy of the purified TATCre was confirmed by a recombination test on a floxed plasmid in a cell-free system and 293 FT cells. Taken together, our results suggest that attachment of the E12 sequence to TAT-Cre improves its solubility during expression in E. coli (possibly by neutralizing the ionic-charge effects of the TAT sequence) and consequently increases the yield. This method can be applied to the production of transducible proteins for research and therapeutic purposes.
