항문직장의 생리검사에는 항문내압 검사, 배변조영술, 대장통과시간 검사, 그리고 PNTML을 포함하는 항문근전도 검사 등이 있다. 이런 생리검사는 만성변비와 변실금의 원인을 진단하고 치료방침을 정하는데 여전히 필요한 검사이기는 하나 임상적 유용성을 좀 더 확보 하자면 검사의 방법과 판독의 기준을 표준화할 필요가 있다. 만성변비의 원인을 찾자면 선별적인 의미로 대장통과시간 검사를 먼저 시행하고 그 결과에 따라 배변조영술이나 항문 근전도 검사 혹은 항문내압 검사를 시행하여 출구폐쇄형의 변비를 가린다. 변실금은 우선 항문내압 검사를 시행하여 항문 괄약근의 기능적 상태를 먼저 평가하는데 평가를 위해서는 기본적인 압력척도들 보다는 휴식기 직장항문 압력경사나 외괄약근의 피로도 지수와 같은 좀 더 발전된 파생적 척도들을 적극 활용한다. 항문내압 검사에서 이상소견을 보이면 항문초음파 검사를 시행하여 괄약근의 기계적 손상 여부를 확인해 보고 이상이 없다면 음부신경의 기능을 검사하여 신경경의 변실금을 배제한다.

The cause of indirect recurrence after the repair of indirect inguinal hernia is failure to close the patulous internal ring. This failure stems from some of the modifications of Bassini's original technique, which have come along with the misconception that suturing one or more musculoaponeurosis to the inguinal ligament automatically closes the internal ring. With careful dissection of the cremasteric muscle and the internal spermatic fascia, the internal ring can be identified and sutured to be closed against such recurrence.

Hyperplastic polyps are common large-bowel tumors, are frequently detected in middle- to old- aged people, and usually are minuscule lesions in the distal colon and rectum. Most hyperplastic polyps have no malignant potential, but recent studies suggest that some hyperplastic polyps can progress to colorectal cancers, possibly by the so-called serrated pathway. Hyperplastic polyposis is a rare syndrome characterized by multiple hyperplastic polyps, primarily in the proximal colon. Different from sporadic hyperplastic polyps, hyperplastic polyposis is alleged to have high potential for malignancy because patients with this syndrome may frequently have conventional adenomas, serrated adenomas, and adenocarcinomas. We report the case of a patient with hyperplastic polyposis, who had two synchronous colon cancers, as well as sessile serrated adenomas and tubular adenomas. This paper aims at shedding light on the formative layer of Bangataryeong written by Sin Jaehyo. I argue that the direct formative layer on which Bangataryeong had been formed was a popular folk song, 'Bangataryeong', even though its formative base is a base level folk song, 'Banganorae'. In addition, I show that the song structure of 'Bangataryeong' is 'front burden - main content - middle burden - ending content' and it is used as a device to secure the theme oriented popularity of each version. 'Bgang[ae]taryeong(방이타령)' is a song with a theme of blessing and amusement for the completion party of Gyeongbokgung and it was a combination of 10 different versions. Through this, we can derive that 'Bangataryeong' by Sin Jaehyo is not created as one song at a time but was weaved as one song frame using 'Danjapga(단잡가)', which is collection of versions in a single unit prepared to be arranged. In the light of the cultural tendency in which boundary blurring between areas of song culture was progressing lively, the text 'Bangataryeong' is likely to be sung with the voice of Jeongga(正歌) after circulated to Gibangs[妓房] even though 'Bang[ae]taryeong' sung by Jin Chaeseon(陳彩仙) might have been sung with the voice of epic song. 'Bang[hwa]taryeong(訪花打令)', which is rearranged from 'Bangataryeong' removing the first and second version of it and recomposing it in the theme of love, is likely within the realm of the former. Sin Jaehyo seems to have elaborated the lyrics of epic songs in the direction towards making them stand to the situation and reason. Whatever the evaluation be, his lyrics seems to be a result of effort for narrowing the distance between performers and appreciators. In this respect, 'Bangataryeong', the main object of discussion in this paper, can be seen as located in the same layer since it was created on the purpose of performing at the completion party of Gyeongbokgung while it was based on popular folk songs.
