        Background : In recent years, gonorrhea has been panedemic and remains one of the most commom STDs in the world, especially in developing countries. Objective & Methods: For the detection of a more effective therapeutic regimen and assessing the prevalence of PPNG, we have been trying to study the patients who have visited the VD Clinic of Choong-Ku Public Health Center in Seoul since 1980 by means of the chromogenic cephalosporin method. Results: In 1996, 139 strains of N. gonorrhoeae were isolated, among which 53(39.0%) were PPNG. Conclusion: Our results suggests that after a peak of 74.3% in 1993, the prevalence of PPNG in Seoul is gradually declining.

      • 유방암과 섬유선종의 방사선학적 소견 : 유방 단순촬영술과 유방 초음파검사 소견의 비교 연구 Studies on mammographic and ultrasonographic findings evaluation

        Purpose : To evaluate the utility of mammography and ultrasonography in study of morphologic differential diagnosis between malignant and benign mass. Materials and Methods : Average age of patients with breast cancer was 48.4 year-old, and that of fibroadenoma was 29.4 year-old. The equipments used for examination were MF-159 high frequency mammography(Bennet Co.) and Acoustic image 5200S. We evaluated mammographic findings about breast parenchymal pattern, calcification, margin and size of the mass associated tumor nature, and ultrasonographic findings about shape, height and width, internal homogenicity and echo pattern, wall contour, boundary echo, and posterior shadow in breast mass. Thirty-seven of mammographic findings and 34 cases of ultrasonogreaphic findings in pathologically proven breast cancer and 34 case of mammographic findings and 71 cases of ultrasonographic findings in breast fibroadenoma were reviewed retrospectively. Results : In mammographic study, the parenchymal patterns of breast cancer according to Wolfe classification were NI(16.2%), PI(24.3%), P@(37.8%), and DY(21.6%) and those in fibroadenoma were NI(3.1%), Pl(11.7%), P2(35.2%),and DY(50%). Mammographic findings about presence of calcification were as follows; only calcification(16.2%), only mass(51.3%), and mass with calcification(32.5%) in breast cancer, and only mass(91.2%), mass with calcification(8.8%) in fibroadenoma. The features of mass margins were classified as ill-defined margin(45.9%), well-defined(5.4%), and well-defined with partial spiculation(48.6%) in breast cancer and ill-defined(20.5%), well-defined(73.5%), and well-defined with partial spiculation(5.8%) in fibroadenoma. The mass were grouped by size; 1-2cm(37.8%), 2-4cm(45.9%), and 4-6cm(16.3%) in breast cancer and 1-2cm(79.4%), 2-4cm(17.6%), and 4-6cm (3.0%) in fibroadenoma. On ultrasonographic study, the shape of masses in breast cancer were lobulated(82.3%),oval(14.7%), and round(3.0%),and those in fibroadenoma were lobulated(14.0%), oval(57.7%), and round(18.3%). The cases that the height was longer than width were 32.3% in breast cancer and 0% in fibroadenoma. The findings about homogenecity and echo patterns were as follows; hypoechoic(52.9%) and mixed(47.1%) in breast cancer and hypoechoic(2.8%), hypoechoic(63.4%), and mixed(33.8%) in fibroadenoma. The margin of masses were smooth in 85.9% of fibroadenoma, and irregular in 73.6% of breast cancer. The boundary echoes of masses and decreased posterior echo shadow were seen only in 61.8% and 55.9% of breast cancer, respectively. Conclusion: The margin and calcifications are more clearly delineated on mammography but breast cancer and fibroadenoma, both are more commonly developed in dense parenchymal pattern(P2+DY),and there findings are easily obscured in mammography. So ultrasoography is very useful in these masses evaluation, especially amoung young patients.

        노인정신장애 평가척도(Psychogeriatric Assessment Scale)의 한국판 표준화 연구

        연구목적 : 본 연구는 치매와 우울증을 다차원적으로 평가할 수 있는 도구인 Psychogeriatric Assessment Scale(PAS)의 한국판(PAS-K)을 개발하기 위한 것이다. 방 법 : 번역, 역번역, 그리고 세차례의 예비조사를 통하여 PAS-K를 제작하였다. 60세 이상의 노인 291명(임상환자군 : 58명, 지역사회노인군 : 168명, 수용시설 거주자 : 67명)과 그들의 정보제공자들을 대상으로 PAS-K, Geriatric Depression Scale(GDS), Korean version of Mini-Mental State Examination(MMSE-K), Hamilton Rating Scale for Depression(HRSD), 그리고 Hachinski Ischemic Score 를 적용하고, Diagnostic Interview chedule(DIS-Ⅲ-R)을 사용하여 치매와 우울증을 진단하였다. PAS-K의 신뢰도와 타당도를 평가하고, 예민도와 특이도, Kappa값, 그리고 ROC커브 분석법을 이용하여 PAS-K의 6개의 소척도들(피검자면접 : 인지기능 장애척도, 우울척도, 뇌졸중척도, 정보제공자면접 : 인지기능 저하척도, 행동변화척도, 뇌졸중척도)의 최적 절단점을 구하였다. 결 과 : PAS-K는 높은 내적 일관성을 보였고, 검사자간 신뢰도도 만족할 만한 수준이었다. PAS-K의 소척도들을 다른 표준화된 검사도구들과 비교하였을 때 인지기능 장애척도와 인지기능 저하척도는 MMSE-K와, 우울척도는 GDS 및 HRSD와 유의한 상관관계를 보였으며, 뇌졸중척도는 Hachinski Ischemic Score와 통계적으로 의미있는 상관관계를 보였다. 최적절단점은 인지기능 장애척도 10점, 인지기능 저하척도 3점, 우울척도 5점, 피검자면접과 정보제공자 뇌졸중척도 모두 1점, 그리고 행동변화척도 2점으로 추정하였다. 결 론 : 본 연구를 통해 PAS-K의 신뢰도와 타당도가 검증되었고, 노인정신의학 역학연구나 보건분야에서 치매와 우울증의 임상경과관찰 및 일차 선별도구로서 유용하게 사용될 수 있게 되었다. Objectives : This study was designed to develop the Korean version of the Psychogeriatric Assessment Scale(PAS-K), a multidimensional screening tool for the dementia and depression. Methods : Through three times preliminary trials, the authors translated PAS into Korean. The PAS-K, Geriatric Depression Scale, Hamilton Depression Rating Scale, Hachinski Ischemic Scale and Mini-Mental State Examination-Korean version were administered to 291 subjects over the age of 60 and the same number of their informants(67 subjects were institutionalized, 168 subjects were in community, 58 subjects were psychiatric hospital patients). And the Diagnostic Interview Schedule(DSM-Ⅲ-R) was independently administered to exactly diagnose dementia and depression. The reliability and validity test, optimal cut-off point estimation for six each scale and ROC curve analyses were done to investigate the diagnostic validity of PAS-K. Results : Internal consistency and interrater reliability of the PAS-K were high. Concurrent validity of each scales of PAS-K was good in being measured with other standardized scales. The optimal cut-off points of each scale of the PAS-K were estimated as follows : 1) Stroke Scale of subject interview : 1, 2) Cognitive Impairment Scale of subject interview: 10, 3) Depression Scale of subject interview : 5, 4) Stroke Scale of informant interview : 1, 5) Cognitive Decline Scale of informant interview : 3, and 6) Behavioral Change Scale of informant : 2. conclusions : The PAS-K was valid and reliable screening tools for detecting dementia and depression. Therefore the PAS-K could be widely and extensively used in psychogeriatric epidemiological research or clinical setting of primary screening for dementia and depression.

        작업장 누적소음 노출과 혈압과의 관련성

        목적 : 작업장에서의 누적소음노출값과 혈압의 변화 사이의 관계를 관찰하기 위한 단면 연구를 시행하였다. 방법 : 소음발생 제조업체의 남성근로자 중 건강진단 자료, 설문지 자료, 인사기록이 모두 갖추어진 852명을 연구대상으로 하였다. 작업장 소음측정값은 34개 지점에서 측정된 작업환경 측정 보고서 값을 사용하였고, 혈압은 건강진단시 측정한 수축기 혈압과 이완기 혈압 각각의 평균을 이용하였다. 가능한 교란변수들을 건강진단시 설문조사를 통하여 조사하였다. 근로자의 근무기간과 근무했던 작업부서의 소음노출값을 이용하여 누적소음노출값을 추정하였고, 이에 따라 전체 연구대상 근로자를 저소음노출군, 중등도소음노출군, 고소음노출군, 과다소음노출군으로 구분하였다. 누적소음노출값으로 구분한 소음노출군 사이에 평균 수축기 혈압 및 이완기 혈압의 차이가 있는 지를 나이, 비만도지수, 혈중 콜레스테롤값, 고혈압의 가족력, 흡연력, 음주력 등의 가능한 교란변수들의 영향을 통제한 상태에서 비교하였다. 결과 : 가능한 교란변수들을 보정한 상태에서 저소음노출군에서 과다소음노출군으로 갈수록 수축기 혈압 및 이완기 혈압의 증가가 있는지를 관찰하기 위해 일반선형모델을 이용하여 분석하였을 때, 수축기 혈압 및 이완기 혈압 모두 소음노출군이 혈압 변화의 유의한 설명변수였고, 수축기 혈압 및 이완기 혈압 모두 저소음노출군에 비해 과다소음노출군이 각각 2.1 mmHg, 2.7 mmHg 만큼 높았다. 결론 : 이는 만성적으로 누적된 고소음에의 노출로 인하여 혈압 상승의 가능성이 있음을 시사한다. Objectives : The association of workplace cumulative noise exposure and blood pressure was investigated in this study using cross-sectional design. Methods : The study population comprised 852 manufacturing male workers of whom occupational health examination data, questionnaire and personnel records were available. Workplace ambient noise monitoring data was used for calculating individual cumulative noise exposure level. Mean of each systolic and diastolic blood pressure of occupational health examination data was used for individual systolic and diastolic blood pressure level. Possible confounding variables including family history of hypertension, smoking habit, alcohol drinking habit were collected by questionnaire. On the basis of job location and duration of work at the location, a cumulatlve time-weighted average noise level was calculated for each workers. According to this cumulative noise exposure level, each study subject was categorized as low noise exposure group, moderate noise exposure group, high noise exposure group, very high noise exposure group. Among noise exposure groups, mean of systolic and diastolic blood pressure was compared after adjusting possible confounding variables such as age, body mass index, serum cholesterol level, family history of hypertension, smoking history, alcohol drinking habit. Results : After adjusting possible confounding variables, noise exposure group was signnificant explanatory variables for both systolic and diastolic blood pressure, and mean of both systolic and diastolic blood pressure of very high exposure group was higher than that of low exposure group. Conclusions : These findings suggested that the high cumulative noise exposure might elevate the blood pressure.

        관로 내의 염소농도 변화특성의 모의를 위한 수질모형의 개발과 비교

        Considering the dynamic conditions in the pipe network, the inverse chlorine models have been integrated to the hydraulic transient model on the platform of the method of characteristics. EPANET2 is compared with the developed models to express the advantages of the unsteady model to the steady state approach. The developed models show realistic prediction of chlorine concentration with the impact of friction loss. However, the stability of numerical scheme need further research to overcome the numerical errors in the rapidly varied flow condition.

        환경영향평가에 있어서 보건영향평가의 방법, 절차 및 적용에 관한 연구

        Most environmental impact assessment(EIA) programs around the world require the consideration of human health impacts. Yet relatively few EM documents adequately address those impacts. This article examines how, why, and to what extent health impacts are analyzed in environmental impact assessments. This article investigates these problems and provides recommendations to improve human health impact assessment(HIA), using methods, procedures and case study. Also, a comprehensive approach for the evaluation of possible health effects in an EIA is described, illustrated with the example of Amsterdam Airport Schiphol. Unlike many EIAs, we estimated quantitatively the impact of aircraft-related pollution in terms of the number of affected people for aircraft noise annoyance, odour annoyance and hypertension. In addition, an analysis of health registry data on cardiovascular and respiratory diseases and a short survey on annoyance and risk perception were carried out.

        대기오염과 천식증상에 의한 응급실내원과의 연관성에 관한 환자교차연구

        Objectives : This study, using case-crossover design, search a relationship between changing levels of air pollutants and asthma attack in asthma patients. Methods : We searched the daily total number of asthmatic patients at hospital's emergency room in Seoul, Korea by investigating total 49 general hospital's emergency room records. Otherwise, We investigated medical records of asthmatic patients at one of the 49 hospitals we have searched. We investigated addresses that they are living, diagnosis, smoking history, whether respiratory infection or not, medications. We analyzed the data by 1:m conditional logistic regression used when matched case-control study is analyzed. Control periods were chosen by bidirectional paired matching technique 7, 14 days before and after case periods. Results : the relative risk of asthmatic attack by 100 ppm ozone increase between June and September were 1.348(95 % CI = 1.010∼1.619). Between November and February, the relative risk by 100 ㎍/㎥ TSP increase, 100ppb sulfur dioxide and nitrogen dioxide were respectively 1,278(1.010∼1.619), 1.237(1.019∼1.502), 1.285(1.006∼1.640). In stratified analysis, the relative risk were significant when the asthmatic patients were children aged 15 years or below, when there were respiratory infection, and when the patients took medications irregularly Conclusions : There was statistically significant association between ambient levels of air pollutants and asthmatic attack. So, we have to exert our efforts to minimize the air pollution effectively, to protect public health from air pollution.

      • 일방향 응고시킨 Al-Fe 2원 공정합금의 응고조직과 기계적성질에 미치는 Zr 첨가의영향

        Effects of Zr addition on structure and mechanical properties in unidirectionally solidifed Al-Fe eutectic alloy have been carried out by means of scanning electron micrography and the measurement of hardness and tensile strength. Especially, effects of Zr addition on ageing behaviours and precipitate distributions have been also conducted by the observation of transmission electron microscope. At first, by the addition of Zr, the eutectic fibre produced were very fine but, as the growth rate increase, the mechanical properties were somewhat deteriorated at room temperature by misalignment of fibre due to constitutional supersooling. On the other hand, Al_3Zr phases were precipitated in interface of matrix/fibre by the ageing at 365℃ for 50 hrs and these phase contributed to increase the mechanical properties remarkablely from RT to 500℃. This results deduced that the shear slip in interface was suppressed by Al_sZr precipitates which mitigated the difference of heat expansion coefficient between matrix and fibre.

      • 2년간의 융모막융모생검 성적

        Chonionic villus sampling for the first trimester prenatal diagnosis has the potential of becoming an attractive alternative to midtrimester amniocentesis. Prior to becoming a standard, however, considerable caution and monitoring is necessary to establish the accuracy and safety of the procedure. In this pursuit, we wish to present our experience and trial in the technical aspects and safety of chorionic villus sampling. Chorionic villi were obtained by transcervical catheter(Trophocan and S-1) or transabdominal 19 gauge spinal needle under the real-time ultrasound guidance. Both direct and cultivation method of chorionic villus for chromosome preparation were performed. The patients consisted of 135 pregnant women who did genetic chorionic villus sampling in 8 to 14 weeks gestation with genetic indications The results obtained were as follows: 1) The chromosomal abnormality was found in 7 cases(5.2%): 1 in 47, XX, +21, 2 in 46, XY, inv(9)(p11q 21), 1 in 15q partial trisomy, 2 in 47, XXY and 1 in 45, X/47, XXX. 2) The fetal loss was found in 4 cases(3.0%). These results suggest that chorionic villus sampling now be considered a vialbe and quite possibly an equal safe alternative to second trimester amniocentesis.
