        혈관질환 정보관리 시스템

        As medical technology progresses rapidly, there is a rise in the average age along with the Korean dietary lifestyle becoming more westernized, which leads to an increase in the number of vascular disease patients in Korea. Thus, we need to manage the medical information of a disease systematically in order to diagnose and treat constructively. However, since there has been no standardized method of man agement to date, a great deal of information could not be properly utilized nor studied. Therefore, the departments of Cardiology, Radiology and Neurology of Samsung Seoul Hospital recently got together to develop an information management system called the Vascular Data System. This program was developed to be run on win98 O/S, upper Pentium Ⅲ, and upper 128 MB Memory, and its source code is Dephi 4.0. It was configured for the user to set the configurations as well as do a variety of search and analysis. If this program were to be updated continuously, it may be used extensively as well as in various parts of clinical research activities.

        TGase를 사용하여 재구성돈육 제조시 결착시간과 압력조건이 품질에 미치는 영향

        본 연구는 효소제인 TGase를 사용하여 돈육을 재구성시킬 때 결착시간(2시간, 4시간, 6시간 및 16시간) 및 압력조건(1㎏, 3㎏, 5㎏ 및 10㎏)이 재구성 돈육의 품질에 미치는 영향을 시험하였다. 그 결과 결착시간별에 따른 육색은 L, a, b값 모두 유의적인 차이가 없었다. 압력별에서는 5㎏ 처리구 a값이 12.81로 높게 나타났다(p<0.05). 시간별에서 인장강도는 생육에서는 6시간 처리구가 202.1±32.3g, 가열육에서는 386.0±72.9㎏ 으로 높은 값을 보였지만 처리구간 유의적인 차이는 없었다. 압력별에서는 생육에서 5㎏ 처리구가 114.6±48.9g, 가열육에서는 303.7±64.7g으로 타 처리구에 비해 유의적으로 높게 나타났다(p<0.05). 결착시간별 조직감에서 hardness, springiness, cohesiveness 및 chewiness 모두 생육 및 가열육에서 처리구간 차이가 없었다. 관능검사 결과 시간별 생육에서는 4시간 처리구가 항목에서 가장 높게 나타났고(p<0.05) 가열육에서는 4시간과 6시간 처리구가 우수하게 평가되었다(p<0.05). 또한 압력별에서는 생육 및 가열육에서 5㎏ 처리구가 각 항목 모두에서 유의차를 나타내었다(p<0.05). 따라서 TGase를 사용하여 재구성돈육 제조시 결착시간은 4-6시간, 압력은 5㎏으로 결착시키는 육색, 인장강도 및 관능적 특징에서 가장 좋다고 할 수 있겠다. This study was performed to compare the effects of binding time(2hr, 4hr, 6hr and 16hr) and pressing condition (1㎏, 3㎏, 5㎏ and 10㎏) on manufacturing restructured pork. Meat color, in terms of L, a, b showed significant difference by treatment of binding time. Five ㎏ pressing resulted in the highest red value(P<0.05). In tensile strength, 6 hour treatment showed significant difference ranging in 202.1g for raw restructured pork and 389.0g for cooked restructured pork. Five ㎏ pressing showed the highest tensile strength, 114.6 for raw restructured pork and 303.3 for cooked restructured pork. In hardness, springiness, cohesiveness and chewiness, raw and cooked restructured pork showed insignificant difference regardless of pressing time. In sensory evaluation, four hour treated raw restructured pork showed the highest values of color, binding degree, shape, acceptance, while four or six hour treated restructured pork showed the highest values in cooked restructured pork. Therefore the optimal conditions for manufacturing restructure pork using TGase were four through six hour pressing with 5㎏ pressure.

        복부 대동맥 가성동맥류를 동반한 Behcet 씨 병 : 증례 보고

        Behcet's disease is a multisystem disorder presenting with recurrent oral and genital ulcerations as well as ocular involvement. The etiology and pathogenesis of this disease remain obscure; it is considered an autoimmune disease since vasculitis is the main pathologic lesion and circulating autoantibodies to human oral mucous membrane are found in approximately 50 percent of the cases. Vascular lesions with Behcet's disease may include deep venous thrombosis, superficial thrombophlebitis, arterial obstruction, or arterial aneurysms. Although arterial aneurysmal lesions are uncommon in patients with Behcet's disease, they may pose serious, life-threatening therapeutic problems due to the risk of rupture. Here, we report a case of Behcet' s disease, involving abdominal aorta with review of literature.

        서울시 아동을 대상으로 한 주의력결핍과잉행동장애의 선별기준에 대한 연구

        Objectives : The purpose of this study is to investigate a cut-off value for screening children with ADHD in community based sample using the Korean Child Behavior Checklist (K-CBCL) and the Korean ADHD Rating Scale (K-ARS). Methods : Children aged 6 to 8 (n=661) participated in this study. The K-CBCL and the K-ARS were used as the screening instruments. Diagnoses were confirmed through the Diagnostic Interview Schedule for Children Version Ⅳ (DISC-Ⅳ). Results : Of the 189 subjects who underwent the DISC-Ⅳ, 26 subjects were diagnosed as ADHD. In the K-CBCL, scores in the attention problems, externalizing problems and total behavioral problems were positively correlated with the total score of the K-ARS. The K-ARS-Parent version (K-ARS-P) and above three scales in the K-CBCL showed significant screening validity in the ROC curve analysis. The reasonable level of sensitivity, specificity, and negative predictive value were obtained in the total scores ≥90^(th) percentile in the K-ARS-Parent and ≥60T scores in the K-CBCL attention problems. The optimal level of specificity, positive predictive value and negative predictive value were obtained when combined the K-CBCL (≥60T scores in the attention problems and ≥63T scores in the total problems) and the total scores ≥90^(th) percentile in the K-ARS-P. Conclusion : The combined use of the K-CBCL and the K-ARS-P may serve as useful screening methods for ADHD children.

        자녀의 문제행동에 대한 도움추구 방식과 정신과 진료에 대한 만족도 : 다기관 시점 역학조사

        Objectives : This study was done to evaluate present status of child and adolescent psychiatric practice in the view of medical consumers. Help seeking behaviour process after recognizing their children's problem, satisfaction with psychiatric service, and difficult problems during practice were evaluated, and variable factors affecting them were analysed. Methods : Through 10 multi-center point epidemiologic survey, 540 parents and their psychiatrists were involved in this study by structured questionnaire. Results : Above 70% of subjects reported positive response to psychiatric service. The barrier to psychiatric service were different by subjects' age. In children, Parents' inability to identify severity of children's problem and proper management place were major reason. However in adolescent, children's lack of desire to receive help and parents' worry about stigma related to receiving help were major barrier to psychiatric service. Conclusion : These findings suggest the importance of interagency collaboration and education services. Researchers and policymakers should consider strategies to reduce barrier related to perceptions of mental health problems and services, especially in adolescents.

      • 충청지방에서 발생한 한국형출혈열 54예의 임상적 고찰

        In Dae Jeon Eul-Ji General Hospital, activated since 1581, 54 patients with Korean Hemorrhagic Fever Who were all positive for Korea antigen were observed from January, 1981 to December, 1984. Epidemiology, clinical features, laboratory findings, complications and clinical course in Korean Hemorrhagic Fever were analyzed. The results were as follows: 1. The most prevalent area was OK-Chun Kun. 2. The peak incidence of age was fifth decade and male to female ratio was 2:1. 3. The peak incidence was November to December. 4. The most frequent symptomes were epigastric pain, nausea and vomiting and followed by hemsturia. 5. Conservative treatment was done and hemodialysis was performed in 19 patients without death.

        <P>The MOCEH study is a prospective hospital- and community-based cohort study designed to collect information related to environmental exposures (chemical, biological, nutritional, physical, and psychosocial) during pregnancy and childhood and to examine how exposure to environmental pollutants affects growth, development, and disease. The MOCEH network includes one coordinating center, four local centers responsible for recruiting pregnant women, and four evaluation centers (a nutrition center, bio-repository center, neurocognitive development center, and environment assessment center). At the local centers, trained nurses interview the participants to gather information regarding their demographic and socioeconomic characteristics, complications related to the current gestation period, health behaviors and environmental factors. These centers also collect samples of blood, placenta, urine, and breast milk. Environmental hygienists measure each participant’s level of exposure to indoor and outdoor pollutants during the pre- and postnatal periods. The participants are followed up through delivery and until the child is 5 years of age. The MOCEH study plans to recruit 1,500 pregnant women between 2006 and 2010 and to perform follow-up studies on their children. We expect this study to provide evidence to support the hypothesis that the gestational environment has an effect on the development of diseases during adulthood. We also expect the study results to enable evaluation of latency and age-specific susceptibility to exposure to hazardous environmental pollutants, evaluation of growth retardation focused on environmental and genetic risk factors, selection of target environmental diseases in children, development of an environmental health index, and establishment of a national policy for improving the health of pregnant women and their children.</P>

        Preferential diffusion effects in downstream interactions between premixed H<sub>2</sub>–air and CO–air flames

        <P><B>Abstract</B></P> <P>The effects of preferential diffusion of hydrogen in downstream interacting counterflow H<SUB>2</SUB>–air and CO-flames were investigated using numerical simulations. The global strain rate was varied in the range 30–5917s<SUP>−1</SUP>, where the upper bound of this range corresponds to the flame-stretch limit. Preferential diffusion of hydrogen was studied by comparing flame structures with a mixed average diffusivity with those where the diffusivities of H, H<SUB>2</SUB> and N<SUB>2</SUB> were assumed to be equal. Flame stability diagrams are presented, which show the mapping of the limits of the concentrations of H<SUB>2</SUB> and CO as a function of the strain rate. The main oxidation route for CO is CO+O<SUB>2</SUB> →CO<SUB>2</SUB> +O, which is characterized by relatively slow chemical kinetics; however, a much faster route, namely CO+OH→CO<SUB>2</SUB> +H, can be made available, provided that hydrogen from the H<SUB>2</SUB>–air flame can penetrate the CO–air flame. This modifies the flame characteristics in the downstream interaction between the H<SUB>2</SUB>–air and CO–air flames, and can cause the interaction characteristics at the rich and lean extinction boundaries not to depend on the Lewis number of the deficient reactant, but rather to exhibit anomalous behavior with a much stronger dependence on the interaction between the two flames. Such anomalous behavior includes a partial opening of the upper lean extinction boundary in the interaction between a lean H<SUB>2</SUB>–air flame and a lean CO–air flame, as well as the formation of two islands of flame sustainability in a partially premixed configuration with a rich H<SUB>2</SUB>–air flame and a lean CO–air flame. At large strain rates, there are two conditions where the flame can survive, depending on the nature of the interaction between the two flames.         Effects of Dietary Fiber Extracted from Pumpkin (Cucurbita maxima Duch.) on the Physico-Chemical and Sensory Characteristics of Reduced-Fat Frankfurters

        In this study, we investigated the effects of reducing fat levels from 30% to 25, 20, and 15% by substituting pork fat with water and pumpkin fiber (2%) on the quality of frankfurters compared with control. Decreasing the fat concentration from 30% to 15% significantly increased moisture content, redness of meat batter and frankfurter, cooking loss, and water exudation, and decreased fat content, energy value, pH, and lightness of meat batter and frankfurter, hardness, cohesiveness, gumminess, chewiness, and apparent viscosity. The addition of 2% pumpkin fiber was significantly increased moisture content, yellowness of meat batter and frankfurter, hardness, cohesiveness, gumminess, chewiness, and apparent viscosity, whereas reduced cooking loss and emulsion stability. The treatment of reduced-fat frankfurters formulated with 20 and 25% fat levels and with pumpkin fiber had sensory properties similar to the high-fat control frankfurters. The results demonstrate that when the reduced-fat frankfurter with 2% added pumpkin fiber and water replaces fat levels can be readily made with high quality and acceptable sensory properties.
