      • FC 1-4 : A survey of attitudes, knowledge, and behavior regarding sun exposure and sunscreen use

        Background: Although sunscreen is used as a primary strategy to prevent sunburn, photoaging and skin cancer, only a few people regularly use sunscreen. Objectives: To investigate the awareness of effects of sunlight, the extent of sun exposure, and the behaviors about sunscreen uses in Korean. Methods: A questionnaire was administered 255 adult patients visited Dermatology Department at Seoul National University Boramae Hospital. It included questions about the awareness of benefit or harm of sunlight, perceived and actual extent of sun exposure, the use of sun-protective methods including sunscreen. Results: Sun exposure was chosen as the major cause of dyschromia (61.2%), skin cancer (62.8%) and wrinkle (28.6%). Respondents were likely to underestimate the extent of sun exposure. On average, quite a few respondents stated that they were exposed to sunlight more than one hour per day (23.1% on weekdays, 53.4% on weekends). But, only 8.7% thought that their sun exposures were problematic and 62.2% of respondents considered moderate sunlight exposure good for health. People`s sun protective behaviors were inadequate: only 30.8% used sunscreen regularly; 18.4% have never used sunscreen. SPF was the most important factor choosing sunscreen and 86.0% used sunscreen labeled SPF over 30. In contrast, only 40.3% used PA+++ sunscreen. Conclusion: Despite a fairly good knowledge about harmful effects of sunlight, people underestimated the risks of their sun exposure and behaviors were suboptimal.

        Heme oxygenase-1 induced by desoxo-narchinol-A attenuated the severity of acute pancreatitis via blockade of neutrophil infiltration

        <P><B>Abstract</B></P> <P>Heme oxygenase-1 (HO-1) has an anti-inflammatory action in acute pancreatitis (AP). However, its mechanism of action and natural compounds/drugs to induce HO-1 in pancreas are not well understood. In this study, we investigated the regulatory mechanisms of HO-1 during AP using desoxo-narchinol-A (DN), the natural compound inducing HO-1 in the pancreas. Female C57/BL6 Mice were intraperitoneally injected with supramaximal concentrations of cerulein (50 μg/kg) hourly for 6 h to induce AP. DMSO or DN was administered intraperitoneally, then mice were sacrificed 6 h after the final cerulein injection. Administration of DN increased pancreatic HO-1 expression through activation of activating protein-1, mediated by mitogen-activated protein kinases. Furthermore, DN treatment reduced the pancreatic weight-to-body weight ratio as well as production of digestive enzymes and pro-inflammatory cytokines. Inhibition of HO-1 by tin protoporphyrin IX abolished the protective effects of DN on pancreatic damage. Additionally, DN treatment inhibited neutrophil infiltration into the pancreas via regulation of chemokine (C-X-C motif) ligand 2 (CXCL2) by HO-1.         PGA2 induces the expression of HO-1 by activating p53 in HCT116 cells

        Prostaglandin (PG) A2 which is a cytotoxic PG, was reported to induce the expression of heme oxygenase (HO)-1 via activation of p38MAPK to keep U2OS cells from cell cycle arrest in G2M phase. The expression of HO-1 is primarily regulated at the level of transcription. But the transcription factors that are responsible for PGA2-induced HO-1 expression were not clarified yet. Here, we report that PGA2-induced transcription of HO-1 is mediated by p53, a tumor suppressive transcription factor. In HCT116 cells, PGA2 treatment led to the phosphorylation of p53 and an increase of p21WAF1 transcription as well as the activation of HO-1 transcription. Knocking p53 down via RNA interference or inhibiting the p53’s transcriptional activity by pifithrin-α treatment led to suppression of the increase in the level of both HO-1 expression and activity of HO-1 promoter. Pretreatment of NU- 7441, a chemical inhibitor of DNA-activated protein kinase (DNA-PK), prevented both the PGA2-induced phosphorylation of p53 and an increase of HO-1 transcription. In addition, N-acetyl-l-cysteine, a scavenger of reactive oxygen species (ROS), also mimicked the effect of NU-7441 on the PGA2-induced activation of p53 and HO-1 transcription. Collectively, these results suggest that PGA2 induces the expression of HO-1 via activation of p53, which is mediated by the ROSDNA- PK pathway.

        한국인에서 일광 노출의 유해성에 대한 인지도와 일광 차단제의 이용 행태에 대한 연구

        Background: Although deleterious effects of sunlight have been increased recently, the surveillance of the attitudes toward sun-exposure and the behavioral aspect of using sunscreens in Koreans has hot been carried out. Objective: Our purpose was to evaluate the attitude toward the harmfulness of sun-exposure and behaviors of using sunscreens in Korea. Method: Five hundred and fifty-two subjects were surveyed to assess the attitude toward sun-exposure, as well as the subjects knowledge about, and the use of sunscreens. Underlying data including demographic data, skin colors, skin types, occupation, and sun-exposure, were obtained. Results: Fifty-four percent of subjects believed that sun-exposure is bad for their skins, but, on the contrary, 12.5% believed it to be beneficial. Fifty-two percent of subjects used sunscreens. Almost all subjects(93.8%) knew why to use sunscreens but 62.7% of subjects did not know the meaning of sun protective factors(SPF). Women, indoor workers, subjects less than 40 years old, with less sun-exposed, fair colored skin, or with skin type I, II, III tended to know the deleterious effects of sunlight and the meaning of SPF and used more sunscreen. Conclusion: High risk population-men, outdoor workers, subjects oider than 40 years old, with more sun-exposed, dark colored skin, or skin type VI, V could be targeted with campaigns that promote attitudinal and behavioral changes.

        Comparison of Melanoma Subtypes among Korean Patients by Morphologic Features and Ultraviolet Exposure

        Background: Genetic alterations have been identified in melanomas according to different levels of sun exposure. Whereas the conventional morphology-based classification provides a clue for tumor growth and prognosis, the new classification by genetic alterations offers a basis for targeted therapy. Objective: The purpose of this study is to demonstrate the biological behavior of melanoma subtypes and compare the two classifications in the Korean population. Methods: A retrospective chart review was performed on patients found to have malignant melanoma in Severance Hospital from 2005 to 2012. Age, sex, location of the tumor, histologic subtype, tumor depth, ulceration, lymph node invasion, visceral organ metastasis, and overall survival were evaluated. Results: Of the 206 cases, the most common type was acral melanoma (n=94, 45.6%), followed by nonchronic sun damage-induced melanoma (n=43, 20.9%), and mucosal melanoma (n=40, 19.4%). Twenty-one patients (10.2%) had the chronic sun-damaged type, whereas eight patients (3.9%) had tumors of unknown primary origin. Lentigo maligna melanoma was newly classified as the chronic sun-damaged type, and acral lentiginous melanoma as the acral type. More than half of the superficial spreading melanomas were newly grouped as nonchronic sun-damaged melanomas, whereas nodular melanoma was rather evenly distributed. Conclusion: The distribution of melanomas was largely similar in both the morphology-based and sun exposure-based classifications, and in both classifications, mucosal melanoma had the worst 5-year survival owing to its tumor thickness and advanced stage at the time of diagnosis.         셀레나이트로 유도되는 백내장에서 선삼 성분에 따른 백내장 억제효과의 비교 분석

        Purpose: To compare the protective effects of saponin and non-saponin Sun-ginseng extract fractions in a selenite-induced rat cataract model. Methods: A total of 101 Sprague-Dawley rat pups were divided into four groups by treatment: Sun-ginseng, saponin fraction, non-saponin fraction, and control. For induction of cataracts, sodium selenite 15 nmol/g was injected subcutaneously in 13 day-old rat pups. Sun-ginseng extract 100 μg/g (Group I, Ginseng Science, Seoul, Korea), saponin fraction 100 μg/g (Group II), non-saponin fraction 100 μg/g (Group III), and phosphate buffered saline (Control group) were injected intraperitoneally every two days for a total of seven injections. The rats were sacrified and their lenses were dissected and photographed at day 7 and 14, and the cataracts were graded according to the ratio of the cataract area to the total lens area. The blind method was used for the evaluation of the cataract area. Results: At day 14, cataract formation rates (CFR) were 33.3% in group I, 76.4% in group II, 41.2% in group III, and 77.7% in the control group. The mean cataract area (MCA) was 13.4±20.8% in group I, 14.4±11.7% in group II, 5.7±7.7% in group III, and 15.8±12.1% in the control group. Group III showed statistically significant results compared with those of control group (CFR p=0.001, MCA p=0.001). We observed significantly lower incidence and smaller mean cataract area in Group I and Group III at day 7 compared with the control group (Group I, CFR p=0.018; Group III, CFR p=0.032, MCA p=0.005). Conclusions: The protective effects of Sun-ginseng extract are caused by the components in the non-saponin fraction, not by those in the saponin fraction, in a selenite-induced cataract rat model.         Protective Effect of Baicalin Against Carbon Tetrachloride–Induced Acute Hepatic Injury in Mice

        <P>This study examined the effects of baicalin, a bioactive flavonoid isolated from <I>Scutellariae Radix</I>, on carbon tetrachloride (CCl<SUB>4</SUB>)-induced liver injury. Mice were treated intraperitoneally with 0.5 ml/kg CCl<SUB>4</SUB> and different groups of animals received 25, 50, 100, and 200 mg/kg baicalin. At 24 h after the CCl<SUB>4</SUB> treatment, the level of serum aminotransferases and lipid peroxidation was significantly elevated, whereas the hepatic glutathione content was decreased. These changes were attenuated by baicalin. The histological studies showed that baicalin inhibited the portal inflammation, centrizonal necrosis, and Kupffer cell hyperplasia, which are the three most common characteristics of CCl<SUB>4</SUB>-induced liver damage. The serum level and mRNA expression of tumor necrosis factor-α were markedly increased by the CCl<SUB>4</SUB> treatment but suppressed by baicalin. The mRNA and protein expression levels of inducible nitric oxide synthase and heme oxygenase-1 increased significantly at 24 h after the CCl<SUB>4</SUB> treatment. Baicalin attenuated the increase in the protein and gene expression of inducible nitric oxide synthase but augmented the increase in those of heme oxygenase-1. These findings suggest that baicalin protects hepatocytes from the oxidative damage caused by CCl<SUB>4</SUB>, and this protection is likely due to the induction of HO-1 expression and the inhibition of the proinflammatory mediators.</P>

        변색 실활치에 대한 carbamide peroxide gel의 표백효과

        The bleaching of discolored nonvital teeth is conservative treatement that satisfy the cosmetic desire. The most common method for this treatement, walking bleaching, is using 30% hydrogen peroxide and sodium perborate. Many alternatives are suggested for preventing the external cervical root resorption that is the common complication of the nonvital teeth bleaching with 30% hydrogen peroxide. The same extent of oxidation reactions as that resulted by the bleaching with the application of 30% hydrogen peroxide and sodium perborate can also be acquired more safely by materials that contain 10% carbamide peroxide, used primarily for the bleaching of vital teeth. Therefore, this study was performed to evaluate the efficacy of 10% and 15% carbamide peroxide bleaching gel in nonvatal teeth bleaching. The internal bleaching of intentionally discolored teeth was performed in vitro with 10% carbamide peroxide (Group 1), 15% carbamide peroxide (Group 2), mixture of distilled water and sodium perborate (Group 3), and mixture of 30% hydrogen peroxide and sodium perborate (Group 4). The bleaching materials were refreshed following 3, 6, 9 and 12 days. To evaluate the bleaching effect, the color change of the crowns was measured at 1, 2, 3, 4, 7 and 15 days of bleaching using the colorimeter. The results were as follows : 1. L^* and ΔE^* values were increased with time in all bleaching agents(p<0.01). 2. There was no significant difference in L^* and ΔE^* value among bleaching agents. 3. Δ^* value higher than 3 was shown after 3 days of bleaching with 10% carbamide peroxide gel, 1 day with 15% carbamide peroxide gel, 4 days with mixture sodium perborate and distilled water and 4 days with mixture sodium perborate and 30% hydrogen peroxide, respectively. These results revealed that the use of 10% and 15% carbamide peroxide bleaching gel in non-vital teeth bleaching is as effective as mixture of distilled water and sodium perborate and mixture of 30% hydrogen peroxide and sodium perborate. Accordingly, carbamide peroxide could be used clinically to bleach discolored non-vital teeth.

        Geosynchronous Magnetic Field Response to Solar Wind Dynamic Pressure

        The present study examines the morning-afternoon asymmetry of the geosynchronous magnetic field strength on the dayside (magnetic local time [MLT] = 06:00~18:00) using observations by the Geostationary Operational Environmental Satellites (GOES) over a period of 9 years from February 1998 to January 2007. During geomagnetically quiet time (Kp < 3), we observed that a peak of the magnetic field strength is skewed toward the earlier local times (11:07~11:37 MLT) with respect to local noon and that the geosynchronous field strength is larger in the morning sector than in the afternoon sector. That is, there is the morning-afternoon asymmetry of the geosynchronous magnetic field strength. Using solar wind data, it is confirmed that the morning-afternoon asymmetry is not associated with the aberration effect due to the orbital motion of the Earth about the Sun. We found that the peak location of the magnetic field strength is shifted toward the earlier local times as the ratio of the magnetic field strength at MLT = 18 (B-dusk) to the magnetic field strength at MLT = 06 (B-dawn) is decreasing. It is also found that the dawn-dusk magnetic field median ratio, B-dusk/B-dawn, is decreasing as the solar wind dynamic pressure is increasing. The morning-afternoon asymmetry of the magnetic field strength appears in Tsyganenko geomagnetic field model (TS-04 model) when the partial ring current is included in TS- 04 model. Unlike our observations, however, TS-04 model shows that the peak location of the magnetic field strength is shifted toward local noon as the solar wind dynamic pressure grows in magnitude. This may be due to that the symmetric magnetic field associated with the magnetopause current, strongly affected by the solar wind dynamic pressure, increases. However, the partial ring current is not affected as much as the magnetopause current by the solar wind dynamic pressure in TS-04 model. Thus, our observations suggest that the contribution of the partial ring current at geosynchronous orbit is much larger than that expected from TS-04 model as the solar wind dynamic pressure increases.
