Primary Hepatosplenic Tuberculosis in an Immunocompetent Adult and Domestic Literature Review

Hepatic or splenic involvement of tuberculosis without other disseminated lesions is a very uncommon form of extrapulmonary tuberculosis, especially in an immunocompetent adult. We report a case of a 25 year-old-man who developed primary hepatosplenic tuberculosis not associated with the lung or other distant organs. He was initially diagnosed with esophageal candidiasis in local clinic. A computed tomographic scan of the abdomen showed hepatosplenomegaly and multiple microabscesses in the spleen. Our initial diagnosis, based on the clinical feature and radiologic findings, was hepatosplenic candidiasis. However, histopathology of the liver specimens revealed chronic granuloma with central caseous necrosis, strongly suggestive of tuberculosis. Although rare, splenic tuberculosis should be considered in the differential diagnosis of splenic abscess, especially in countries where tuberculosis is endemic.

The purpose of this treatise is to inquire into the degrees of fatigue affecting a sampling of students of Cheju National University. For this treatise, sheets of questionnaire covering 40 items were given to a sampling of 444 university students made up of both males and females. On the basis of the responses collected I present the following findings. The summary follows: 1 The highest rate, or 82.43% of the university students polled say that they must take deep breaths because they "feel heavy in the chest" ; on the other hand, the lowest rate, or 31.08% of them say that they sometimes take medicines because of the condition of slight migraine. 2. The highest rate, or 83.30% of the male students polled say that they cannot concentrate their attention on studios. And the lowest rate, or 14 60% of them say that they often feel dizzy. 3. The classification by age group shows that the highest rate, or 86.20% of the 19-year-old students polled mention that they feel pains in their bodies, and the lowest percentage of them say that they sometimes take medicines on account of the condition of slight migraine. 4. The highest rate, or 100% of they 20-year-old male students say that they have to take deep breaths because they "feel heavy in the chest" ; on the other hand, the lowest rate, or 21.10% of them say that they do not talk with other people, but mostly listen to others talk because they lack the desire to talk. 5. However, 100% of the 21-year-old male students polled say that they have no appetite, and the lowest rate or 5 60% of them say that they feel pains or feel as though "paralyzed in their heels." 6. To the Identical questions, the highest rate of the 18-year-old female student's polled say that they do not wish to talk with other people because they feel something wrong with their bodies ; on the other hand, the low rate, or 24.40% of them say that they have amnesia with a short retentive memory. 7. The highest rate, or 88.80% of the 19-year-old female students polled say that they "feel heavy in their heads" and cannot think well , on the other hand, the lowest rate, or 29.90% of them say that they suffer from insomnia. 8. But the highest rate, or 100% of the 20-year-old female students polled say that they have anorexia ; on the other hand the lowest rate of them say that they mostly listen to others talk because they are unmotivatied 9. In at this research,it seems that an accurate understanding of what constitutes the degrees of fatigue is required. A correct behavior is encouraged such as deep breathing to alleviate "heavy feeling in the chest" A habit of taking light exercises to relieve fatigue is no less to be encouraged. This research on the degrees of fatigue shows well that there is a blind-point in the health policy for the young students who take medicines simply because they are seized with fatigue or because they tend to act unreasonably to relieve fatigue. The alleviation of fatigue to insure good health is possible through a proper health guidance program based on the belief that "a sound min dwells in a sound body." A study such as this should he conducted on a continuing basis to further refine its methalogical accuracy and to enhance its reliability and validity. The students of Cheju enjoy as high a standard of living as those on the mainland, and they are well aware of the need to avoid fatigue, thanks to the media-sponsored health management counseling including use of recreational facilities, enhancing hysical fitness, and reliance on medical insurance. But the abuse of tonic drinks and toxic as well as hallucinogenic agents has given rise to a host of serious social problems. Furthermore, diversionary indulgences ostensibly to relive stress often lead to wrongful deviation or behavior. It is believed that the results of an inquiry of the degrees of fatigue such as undertaken in this study are to be properly assessed for the benefit of the general health of college students, athletes or non-athletes, and of youth engaged in manual labor all of whom constitute the backbone of a rapidly changing industrial society such as ours.

The purpose of the study was to undertaken to analysis the Grab starting technique between national representive team(3) and Cheju's representive team(3) with 3D cinematography. The conclusions obtained were as follows: Cheju's showed more elapsed time than national representive team on starting block. and more vertical than horizontal direction of COG position, more concentrated vertical velocity & acceleration than forward direction. Therefore It is necessory for cheju's to train balanced trust force in horizontal and vertical direction in grab starting technique.

There are limited data on the duration of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) RNA in respiratory specimens after resolution of coronavirus disease 2019 (COVID-19)-associated symptoms/signs. We determined duration of SARS-CoV-2 virus shedding in symptomatic patients after remission of symptoms. We investigated the duration of SARS-CoV-2 RNA detection using real-time reverse transcriptase polymerase chain reaction for SARS-CoV-2 in nasopharyngeal/oropharyngeal swabs or sputum or saliva. Six patients were included in the final analysis. The median (range) duration of SARS-CoV-2 viral detection after hospitalization was 34 days (22 to 67). After resolution of symptoms/signs, SARS-CoV-2 RNA was detected for median (range) of 26 days (9 to 48). Among the six patients, one had persistent detection of SARS-CoV-2 RNA until day 67 of hospitalization, which was 30 days after symptom resolution. This case represents the longest duration of SARS-CoV-2 detection, and highlights the need for long-term follow up of COVID-19 patients despite resolution of symptoms to confirm SARS-CoV-2 clearance.

Relapsing polychondritis is a rare multisystem rheumatic disease, characterized by recurrent and potentially destructive inflammatory lesions of cartilaginous structures. All types of cartilage & other proteoglycan-rich structures may be involved, resulting in auricular chondritis, laryngotracheal chondritis, ocular symptoms, vasculitis, cardiac abnormalities, skin lesions and glomerulonephritis. The disease may be associated with another connective tissue and autoimmune diseases such as rheumatoid arthritis, systemic lupus erythematosus, Sjogren`s syndrome and systemic vasculitis. We experienced a 69-year-old female patient who had been previously diagnosed as Sjogren`s syndrome, presenting respiratory tract involvement, episcleritis, auricular chondritis and vestibular dysfunction.
Previous Long-term Care Facility Admission as a Risk Factor for Scabies in a Medical Facility

The study aimed to elucidate simple and effective risk factors for scabies infection in the medical environment for early detection and prevention of exposure to other patients and medical staff. We conducted a case-control study of patients who were diagnosed with scabies among hospitalized patients between 2008 and 2019 in an acute-care teaching hospital. Each case was matched according to sex and age with two randomly selected controls without scabies during hospitalization. Clinical characteristics of cases at the time of hospital admission were compared to those of the control group. The scabies group included 34 patients and 68 patients who were included in the control group. After adjusting for confounding factors, previous long-term care facility admission was only associated with scabies (adjusted odds ratio, 5.44; 95% confidence interval, 1.46–20.27; P = 0.012). Careful examination, particularly for patients with previous long-term care facility admission, might be useful for the early detection of scabies.
