소화성 궤양에서 삼제 병합 요법의 저용량에 의한 Helicobacter pylori 제균률과 궤양 치유효과

Helicobacter pylori is the major cause of histologic chronic gastritis and is strongly associated with peptic ulcer disease, gastric cancer and lymphoma. Several recommendations for the management of the Helicobacter pylori infection have been developed. However, there has been no clear therapeutic regimen for the optimal treatment of Helicobacter pylori infection. This study was desinged to evaluate the eradication rate and the ulcer healing rate of the triple regimen with low-dose clarithromycin(0.5g/day) in peptic ulcer patients. Forty three patients with gastric ulcer(n=15) and duodenal ulcer(n=28) were enrolled. The patients were treated either with omeprazole 20mg b.i.d. amoxicillin 0.5g b.i.d. and clarithromycin 250mg b.i.d. (low-dose regimen group, n=29) or with omeprazole 20mg b.i.d., amoxicillin 1.0g b.i.d. and clarithromycin 0.5g b.i.d.(high-dose regimen group, n= 14) for 2 weeks. After 4 weeks, the patients were evaluated for the eradication of Helicobacter pylori and the healing of ulcer. And gastritis scores according to the Sydney system were evaluated before and after 4 weeks from the end of the triple regimen. There was no significant difference in the eradication rate and the ulcer healing rate between the low-dose regimen group, group and the high-does regimen group. In the low-does regimen group, the mean initial scores of chronic inflammation and activity were decreased after the eradication of Helicobacter pylori. In conclusion, the low-dose triple regimen is as effective as the high-dose triple regimen on the eradication of Helicobacter pylori and the ulcer healing rate in peptic ulcer disease.

Background/Aims: Balloon dilatation is a useful alternative to surgery in patients with benign pyloric stenosis. However, little data are available on the long-term outcome of the procedure. This report was attempted to determine the safety and efficacy of endoscopic balloon dilatation for 14 patients with gastric outlet obstruction caused by duodenal ulcer. Methods: Review of medical records or telephone interview was performed retrospectively. Results: Follow-up was conducted for median 18.5 months (3-48 months). Gastric outlet strictures had a median diameter 6 mm (range, 2-9 mm). Five (35.7%) patients had active ulcer. 12 mm to 18 mm balloons were inflated a median of 1 times (range, 1-4 times) for a median of 4 minutes (range, 1 11 minutes). Thirty-two procedure (1.5/patient) were performed; 9 patients (64.3%) had one treatment and 5 patients (35.7%) had multiple treatment. Immediate symptomatic relief was achieved in 13 patients (92.8%) and 7 patients (50%) achieved sustained symptomatic relief. Dilatation failed only in 2 patients (14.3%) ultimately and both recovered by palliative bypass surgery. No complication was noted during treatment. Conclusions: Endoscopic balloon dilatation is safe and effective for most patients with gastric outlet obstruction induced by duodenal ulcer. And due to limitation of retrospective aspect of this report, further prospective, randomized studies must be performed.

<P>This article investigates numerically carrier-phonon interaction and nonequilibrium energy transfer in direct and indirect bandgap semiconductors during sub-picosecond pulse laser irradiation and also examines the recombination effects on energy transport from the microscopic viewpoint. In addition, the influence of laser fluence and pulse duration is studied by using the self-consistent three-temperature model, which involves carriers, longitudinal optical phonons, and acoustic phonons. It is found that a substantial non-equilibrium state exists between carriers and phonons during short pulse laser irradiation because of time scale difference between the relaxation time and the pulse duration. It is clear that the two-peak structure in carrier temperature exists and it depends mainly on laser pulses, fluences, and recombination processes. During laser irradiation, in particular, the Auger recombination for Si becomes dominant due to the increase in the carrier number density, whereas for GaAs, the Auger recombination process can be ignored due to an abrupt increase in SRH recombination rates at the initial stages of laser exposure.</P>

Splanchnic artery aneurysms, previously thought to be rare, are being diagnosed with incresed frequency be- cause of development of noninvasive imaging techniques. The pathogenesis of visceral aneurysms is varied and may be secondary to arteriosclerosis(30%), trauma(25%), inflammation(11%), gestational alteration, surgery, infection, medial necrosis, collagen vascular disease, arteritis, and congenital anomalies. Clinical manifestation of visceral artery aneurysms is vague and not specific. The diagnosis and treatmet for them are difficult. Preoperative arteriography may be helpful in emergency situation to detect the aneurysm and visualize multiple aneurysms. Surgical ligation, resection and/or reconstructive surgery are first choice of treatment for them. Otherwise, transarterial embolization has also been reported for them recently. We report a case of hemoperitoneum due to left gastric artery aneurysmal rupture and a rare complication of splanchnic artery aneurysm "Double rupture". The patient was treated successfully with simple ligation of the ruptured vessel.

<P>Telomerase activity appears to be associated with cell immortalization and malignant progression. Understanding how telomerase activity is regulated in vivo is important not only for understanding the molecular biology of telomerase but also for the potential clinical application of anticancer drugs. This study evaluated telomerase activity and quantified the expression of human telomerase reverse transcriptase (hTERT) mRNA and human telomerase RNA (hTR) using a real-time reverse transcriptase-polymerase chain reaction (RT-PCR) method before and after the exposure of cisplatin and 5-fluorouracil (5-FU) in two head and neck squamous cell carcinoma (HNSCC) cell lines. Two human HNSCC cell lines (PNUH-12 and SNU-899) were studied. Cell cytotoxicity, the change of telomerase activity, and hTERT mRNA and hTR expression by 5-FU and cisplatin exposure were assessed by MTT assay, TRAP assay, and real-time RT-PCR, respectively. In two cell lines, after cisplatin exposure, the telomerase activity and hTERT mRNA expression decreased, but hTR expression increased according to the concentration of drug. However, in both cell lines, the telomerase activity and hTR did not show any significant change after 5-FU treatment, but the expression of hTERT mRNA decreased. These results suggest that there may be other important regulating mechanism except hTERT mRNA as the regulation factor of telomerase activity in HNSCC cell lines.</P>

Dynamic Filling Characteristics of a Capillary Driven Underfill Process in Flip-Chip Packaging

<P>This study investigates the dynamic flow characteristics of capillary-driven underfill flows in a flip chip package. In the present study, we used two different bump arrays using Sn-2.5Ag solder balls with 80 μm and 100 μm diameters on commercially available flip chips, which have different pitches of 150 μm and 180 μm. First, we measured surface tension and viscosity with a rheometer and a tensiometer, respectively, and conducted an experimental visualization of the dynamic filling behavior of the underfill flows. From the captured images, we estimated the filling times, which can be affected by two important factors: bump arrangements and resin viscosities. In addition, we conducted a FVM (finite volume method)-based numerical simulation using commercial CFD code (Fluent v. 6.3.26), and compared its numerical results to both the experimental data and the analytical solutions given by the previous model described by Wan <I>et al.</I> (2005). The numerical predictions and analytical solutions estimating filling time were in good agreement with the experimental data, and the increase in spatial density of solder bumps allowed the flow to fill more slowly due to the increase in flow resistance. We conclude that the non-Newtonian characteristics and bump arrangement are very important factors in the design of flip-chip packaging.</P>
