        This study was conducted to identify the effects of rural-fishing village health insurance system and the whole national health insurance system on the number of out-patient and in-patient cases and the hospital bed utilization, and the number of hospital persons in a university located in Choong Chung Nam Do. The results were as follows. 1. The number of out-patients was increased by 6.8% and 18.8% after the implementation of the rural fishing village health insurance system and the whole national health insurance system respectively. The number of out-patient visit cases was increased by 31.5% and 63.5% after implementation of the rural fishing village health insurance system and the whole national health insurance system respectively. 2. The number of in-patients was increased by 13.8% and 22.8% after the implementation of the rural fishing village health insurance system and the whole national health insurance system respectively. The number of bed days was increased by 27.1% and 32.8% after the implementation of the rural fishing village health insurance system and the whole national health insurance system respectively. 3. The hospital bed utilization rates were 77.3% and 98.4% before and after the rural fishing village health insurance system respectively, and 102.6%, after the whole national health insurance system. The bed rotation rate was increased from 23.6 times before the rural fishing village health insurance system to 27.0 times after the system. After the whole national health insurance program, it was also increased to 29.1 times. The average length of stay was 11.9 days before the rural fishing village health insurance system and 13.3 days after that, and 12.9 days after the whole national health insurance system. 4. Before the rural fishing village health insurance system the physicians per 100 beds was 19.0 persons. The physicians per 100 beds was increased to 25.2 persons and 29.3 persons, after the rural fishing village health insurance system and the whole national insurance system respectively. Before the rural fishing village health insurance system the nurses per 100 beds was 26.8 persons. The nurses per 100 beds was increased to 35.4 persons and 38.3 persons, after the rural fishing village health insurance system and the whole national health insurance respectively. Before the rural fishing village health insurance system the manpower per 100 beds was 101.6 persons. The manpower per 100 beds was increased to 128.6 persons and 138.9 persons, after the rural fishing village health insurance system and the whole national insurance respectively.

        치아의 견인을 위한 버튼 접착시 오염이 인장강도에 미치는 영향

        We already know that it is very difficult to obtain an 'isolated field' for direct bonding during the surgical exposure of unerupted teeth. The aim of this in-vitro study is to simulate the clinical situation of forced eruption and to evaluate the tensile strengths of preligatured button with several types of contamination which can happen during the surgical exposure of unerupted teeth. Four orthodontic direct bonding systems were used. (Ortho-One, Rely-a-Bond, Ortho-Two, PhaseⅡ) Each material was divided into four groups(n=20) : Group 1. (Control, no contamination), Group 2. (Rinse etching agent with saline instead of water), Group 3. (Blood contamination of etched surface for 30 seconds), Group 4. (Blood contamination of primed surface for 30 seconds) 320 bovine anterior permanent teeth were divided into the above mentioned 16 groups. Enamel surface was flattened and ground under water coolant. Pre-ligatured buttons were prepared to the same form. (Cut 0.25 ligature wire 10 cm in length. Twist the ligature wire 30 times clockwise. Mark the wire 15mm and 35mm points from button. Make a loop sticking two points together and twist the loop 6 times counterclockwise.) The bonded specimens were stored at 37℃ saline solution for 3 days. then the tensile strength of each sample was measured with Instron universal testing machine, crosshead speed of 0.5mm/min. the following results were obtained : 1. As compared to control groups (Group 1) of each material, Rely-a-Bond had a significantly lower mean tensile strengths than other material. (p<0.01) 2. In Group 2. of Ortho-One and Rely-a-Bond, the mean tensile strengths decreased about 7.7% and 11.1%, respectively with statistical significances. (p<0.05) 3. In Group 2. of Ortho-Two and PhaseⅡ, the mean tensile strengths did not decrease. 4. In Group 3. of Ortho-One, Rely-a-Bond, Ortho-Two, and PhaseⅡ, the mean tensile strengths decreased about 60.8%, 56.1%, 60.2%, and 46.0%, respectively with statistical significances. (p<0.01) 5. In Group 4. of Ortho-One and Rely-a-Bond, the mean tensile strengths did not decrease. 6. In Group 4. of Ortho-Two and PhaseⅡ, the mean tensile strengths were decreased about 20.95% and 22.28%, respectively with statistical significances. (p<0.01) There were formations of a hump shaped mass from bonding resin under blood contamination which disturbed direct bonding procedure. According to Reynolds, the proper bond strength for clinical manipulation should be at least 45N or about 4.5Kg.F. According to these results, it can be concluded that Ortho-One could be used during surgical exposure of unerupted teeth. In any case, blood contamination of the etched surface should be avoided, but the blood contamination of primed surface of Ortho-One may not decrease bond strength. Just 'blowing-out' is enough to remove blood from primed surface of Ortho-One. You can verify the clean surface of the primer of Ortho-One after blowing out the blood contamination.

        적외선열화상장치를 이용한 Buchanan plugger 표면의 온도상승 분석

        This study was performed to evaluate the temperature rise on various position of the Buchanan plugger, the peak temperature of plugger's type and the temperature change by its touching time of heat control spring. The heat carrier system 'System B' (Model 1005. Analytic Technologies, USA) and the Buchanan's pluggers of F, FM, M and ML sizes are used for this study. The temperature was set to 200℃ which Dr. Buchanan's "continuous wave of condensation" technique recommended on digital display and the power level on it was set to 10. In order to apply heat on the Buchanan's pluggers, the heat control spring was touched for 1, 2, 3, 4 and 5 seconds respectively. The temperature rise on the surface of the pluggers were measured at 0.5 mm intervals from tip to 20 mm length of shank using the infraed thermography (Radiation Thermometer-IR temper, NEC San-ei Instruments, Ltd, Japan) and TH31-702 Data capture software program (NEC San-ei Instruments, Ltd, Japan). Data were analyzed using a one way ANOVA followed by Duncan's multiple range test and linear regression test. The results as follows. 1. The position at which temperature peaked was approximately at 0.5 mm to 1.5 mm far from the tip of Buchanan's pluggers (p<0.001). The temperature was constantly decreased toward the shank from the tip of it(p<0.001). 2. When the pluggers were heated over 5 seconds, the peak temperature by time of measurement revealed from 253.3±10.5℃ to 192.1±3.3℃ in a touch for 1 sec, from 218.6±5.0℃ to 179.5±4.2℃ in a touch for 2 sec, from 197.5±3.0℃ to 167.6±3.7℃ in a touch for 3 sec, from 183.7±2.5℃ to 159.8±3.6℃ in a touch for 4 sec and from 164.9±2.0℃ to 158.4±1.8℃ in a touch for 5 sec. A touch for 1 sec showed the highest peak temperature, followed by, in descending order, 2 sec, 3 sec, 4 sec. A touch for 5 sec showed the lowest peak temperature (p<0.001). 3. A each type of pluggers showed different peak temperatures. The peak temperature was the highest in F type and followed by, in descending order, M type, ML type. FM type revealed the lowest peak temerature (p<0.001). The results of this study indicated that pluggers are designed to concentrate heat at around its tip, its actual temperature dose not correlate well with the temperature which Buchanan's "continuous wave of condensation" technique recommended, and finally a quick touch of heat control spring for 1 sec reveals the highest temperature rise.

        코크스 폐수를 이용한 활성슬러지 모델 파라미터의 민감도 분석

        Activated sludge model (ASM, IAWQ Task Group) was developed as a general mathematical model to express major biological processes in municipal wastewater treatment. Therefore this model could be applied for the design of a wastewater treatment plant and used for finding optimal conditions in plant operation. Since biological processes adopting biodegradation have been also used to treat lots of industrial wastewater such as coke's wastewater, it is worthy to examine the applicability of ASM on the industrial wastewater treatment. In order to evaluate this possibility, the sensitivity analysis of seven parameters (Y_H,μ_H, b_H, k_h, K_S, K_(O,H), K_x) in ASM was conducted with regard to the SCOD (soluble chemical oxygen demand) removal in coke's wastewater treatment. It has shown that μ_H (maximum specific growth rate) of ASM was the most sensitive one in the SCOD removal process, and relative sensitivity was decided in order of μ_H >> b_H, K_S > Y_H, K_(O,H), K_h, K_x, consequently. Based on the sensitivity analysis and calibration procedure of ASM with the data from coke's plant, the decided parameter values were valid for the proper simulation of this industrial wastewater plant, showing adequate fitting between real and simulated data. In summary, the mathematical model of ASM was suitable for the simulation of coke's wastewater treatment plant.

        상악 유측절치 부위의 유치 과잉치에 관한 증례보고

        과잉치란 치배 형성기 이상으로 유치열기와 영구치열기에 정상보다 많이 발생하게 된 치아를 말한다. 유치 과잉치의 빈도는 0.03~1.9%로 상악 유측절치 부위에 호발하고 대부분은 정상 치아 형태로 맹출하며 정상 시기에 탈락하므로 보호자들이 인식하지 못하는 경우가 많다. 과잉치의 발생에 대해서는 현재 치판의 과활성으로 인해 제 3의 치배가 발생한다는 가설이 널리 받아들여지고 있고 유치과잉치가 발생할 경우 영구치열기에는 정상 또는 비정상적인 형태의 계승 과잉치가 존대한다고 보고된 바 있다. 유치 과잉치의 약 75%는 정상적으로 맹출하고 탈락하기 때문에 특별한 처치는 필요하지 않지만 계승 영구치 맹출에 영향을 주는지를 관찰하기 위하여 정기적인 구강 및 방선학적 검사가 필요하다. 본 두 증례의 경우 정상 치아 형태의 유치 과잉치가 유전치부 발육 공간을 이용하여 상악 유측절치 부위에 맹출하였고 방사선 사진 검사 결과 유치 과잉치 하방에 매복된 계승 과잉치가 관찰되었으므로 이에 보고하는 바이다. Most supernumerary primary teeth erupt in the primary lateral incisor region and are reported to occur in 0.03 to 1.9% in the primary dentition. Most parents fail to recognize them because of their normal shape, eruption and exfoliation. Several theories have been proposed for the etiology of hyperdontia and the dental lamina hyperactivity theory is most widely accepted. When a supernumerary primary tooth develops. it is reported that there also exists a supplemental or rudimentary form of a succedaneous supernumerary tooth. Supernumerary primary teeth do not need specific treatment as 75% of them erupt and exfoliate normally. Only periodic radiographic exams are necessary to find out whether there is any problem with the eruption of the succedaneous tooth. In the following two cases, a supplemental supernumerary primary tooth with a loss of the anterior developmental space was observed at the maxillary lateral incisor region. Radiographic examination revealed an impacted succedaneous supernumerary tooth at the palatal side of the supernumerary primary tooth.

      • 전로제강 슬래그(BOF-Slag)를 이용한 고온탈황에 관한 연구

        고온 석탄연료가스의 탈황에 BOF-Slag의 이용가능성을 실험해 보았다. BOR-Slag의 온도, 유량, 탈황제의 특성등이 H₂S 제거효율에 미치는 영향을 조사해 본 결과, 최적의 H₂S 제거효율은 700℃에서 이루어졌고 입자크기가 작을수록 황포집능은 향상되었으나 0.214-0.631mm에서는 큰 영향이 발견되지 않았다. 조업온도 800℃이상에서는 탈황제의 응집으로 내부물질전달저항이 증가하여 탈황제의 반응성이 저하되었다. Experiments have been made to test the practical feasibility of using BOF-Slag to desulfurize hot coal-derived fuel gas. In this study, the effects of particle size, temperature, flow rate and sorbent characteristics on the H₂S removal efficiency of BOF-Slag were investigated. Experimental results showed that the removal efficiency of H₂S was optimum when the temperature was about 700℃ and the smaller sample size, the better sorbent capacity had and the sample size in the range of 0.214∼0.631mm didn't influence. When the temperature was above 800℃, the reactivity of sorbent has lowered because agglomeration of sorbent increased intraparticle transport resistance.

        급성 백혈병 환자에서 수두양 피부병변을 보인 파종성 Fusarium 감염 1예와 국내 파종성 Fusarium 감염증에 대한 고찰

        최근 면역저하환자의 증가와 함에 국내에서도 점차 침습성, 파종성 Fusarium 감염증 보고가 증가하고 있다. Fusarium 감염은 아스페르길루스 감염과 유사한 임상양상을 보이지만 특징적인 피부병변을 동반하고 혈액배양에서 균이 분리되는 경우가 더 많다. 저자들은 급성 림프구성 백혈병으로 진단받고 치료중인 30세 남자 환자에서 중증의 호중구 감소 상태에서 비특이적인 피부병변을 동반한 파종성 Fusarium 혈류감염을 경험하였기에 국내 증례 고찰과 함께 보고하는 바이다. 환자의 초기 피부병변은 구진에서 시작하여 주로 소포성 병변과 일부 통증을 동반한 결절 등 여러 형태로 나타났고, Tzank 도말검사 양성으로 비정형 수두로 오인되었다. 이후 피부병변은 농포로 변하였고, 혈액배양 및 피부 조직검사에서 파종성 Fusarium 감염으로 진단받았다. 환자는 liposomal amphotericin B, voriconazole 병합요법과 G-CSF 투여, 과립구 수혈을 받았으나, 호중구 감소증에서 회복하지 못하고 급성 호흡곤란증후군과 다장기 기능부전으로 사망하였다. 본 증례는 파종성 Fusarium 감염의 경우에도 수두양 병변으로 비특이적인 양상으로 나타날 수 있으므로, 감별진단을 위해 적극적으로 혈액배양과 피부조직검사를 시행해야 함을 보여준다. Recently, we experienced a case of disseminated fusarial blood stream infection with varicelloid skin eruptions, who had suffered from persistent neutropenic fever after salvage chemotherapy for relapsed acute lymphocytic leukemia (ALL). Neutropenia continued in spite of G-CSF and granulocyte transfusion, and he failed to respond to conventional amphotericin B initially, and then liposomal amphotericin B and voriconazole combination therapy. Disseminated fusariosis can be diagnosed by blood cultures in 50% of patients and present skin lesions in more than 80% of patients. So, typical skin lesions are important clue to diagnose of the disseminated fusariosis. However, many skin lesions in immunocompromised hosts are due to various infectious and non-infectious causes. So, this case shows that it is important to obtain biopsy specimens of skin lesions for histopathologic examination, culture and staining. Here, we present our case with the review of the literatures reported in our country, so far.

        흰쥐의 발치아에 이식된 태아 치아싹의 발육

        정상적인 치열의 형성 및 치아의 생성과정을 분석하고 실제로 임상에 적용하기 위해 치아의 발생과정을 연구하는 것은 매우 중요하다. 본 연구는 이식된 태아 치아싹의 석회화가 가능하고 정상적인 구조로 발육할 수 있는지 밝히기 위해, 그리고 악골 자체가 새로운 이식 공여부로 사용가능한지 알기 위해 흰쥐의 발치와에 치아싹을 이식하여 치아의 형성을 살펴보고자 하였다. 이를 위하여 성숙한 흰쥐의 상악 제 1구치를 발거한 발치와에 13.5일된 태아쥐의 치아싹을 이식한 후 4,8주 후에 희생하여 조직학적 및 방사선학적으로 관찰하여 다음과 같은 결과를 얻었다. 1. 이식된 치아싹에서 상아질과 백악질, 치수, 치주인대의 소견을 갖는 석회화된 조직이 형성되었다. 2. 형성된 경조직 및 연조직은 정상적인 치아형성과정에 비해 발육이 늦었으며 유착 및 골양상아질(osteodentin)과 같은 비정상적 조직이 관찰되었다. 3. 석회화된 조직은 8주내에 악궁으로 맹출하는 소견이 관찰되지 않았다. The developmental process of a tooth is being presented as an important study subject to analyze formation of normal dental arch and tooth. The purpose of this study was to see the formation of tooth from a tooth bud transplanted in a white rat regarding that the jawbone could be used as a new donor site of a trasplant. The first molar of a matured white rat was extracted and the tooth bud of a 13.5 day rat embryo was transplanted. The histological and radiographical results after 4 and 8 weeks respectively are as the following. 1. Calcification in dentin, cementum, pulp and periodontal ligament was formed from the tooth bud transplanted in the alveolar socket. 2. The development of hard and soft tissue was delayed compared to the normal tooth formation and abnormal histologic features such as ankylosis and osteodentin were found. 3. The formed hard tissue did not erupt into the jaw within 8 weeks.
