우울 장애의 치료에 있어서 dothiepin-sertraline 병합과 dothiepin 단독 요법의 비교

The dysfunction of either or both noradrenaline and serotonin system are important in the pathophysiology of depression. Previous reports have suggested that there may be an important interaction between these two systems. Recently, some investigators have suggested that the combination of tricyclic antidepressants(TCAs) and selective serotonin reuptake inhibitors(SSRIs) would produce a rapid synergistic effect on down-regulation of either or both of these two systems and that this combination may produce a more rapid and absolute antidepressant effect. We compared the treatment efficacy, treatment associated side effects, treatment satisfaction, and the quality of life between the combination therapy of dothiepin-sertraline as well as the therapy of dothiepin alone in the treatment of major depressive disorder and dysthymic disorder. In our study, the combination therapy of dothiepin and sertraline produced a more rapid and absolute antidepressant effect than dothiepin alone. And the patients with combination therapy experienced relatively high treatment satisfaction than the patients with dothiepin therapy. The patients' quality of life improved more rapidly in the combination therapy, especially, in the health perception, social behavior, and life satisfaction, than dothiepin alone. These results support the hypothesis that the combination of TCA and SSRI may produce a rapid synergistic effect on either or both norepinephrine and serotonin system, and more rapid antidepressant effect and high treatment satisfaction.

A case of infiltrative type of hepatic tuberculosis is presented. Ultrasonography revealed a very ill-margined, heterogenously low echoic lesion in the right hepatic lobe. CT scans demonstrated a very ill-defined, geographic, hypodense lesion with minimal contrast enhancement mimicking cholangiohepatitis or infiltrative tumor in the right hepatic lobe. 본 증례는 복부 초음파상 간 우엽에 경계가 불분명하고 불균질한 에코로 보이고 컴퓨터 전산화 촬영상 경계가 불분명한 저음영의 조영증강이 미미한 소견을 보여 간 종괴로 오인했던 침윤적 간 결핵에 대한 예를 보고하고자 한다.

지방의회 의정활동 분석과 발전 방안 연구

Based on the theoretical discussion of local council activities, the main analysis was set as organization, operation, policy-making function, and enforcement agency check function, and the purpose of this study was to in-depth analyze the first to eighth parliamentary activities of the Jeongeup City Council. To conduct the study, organizational and operational aspects were examined through human status such as age, education, and job, and policy-making functions were examined focusing on factors such as ordinance rule proposals, consent and approval processing, and research groups. As for the function of checking the executive agency, analysis was performed with administrative affairs audit and investigation, corrective questions, committee systems, and activities. As a result of the analysis, most of the general characteristics were higher education than college graduates, the proportion of female lawmakers was small, and job diversity was insufficient. In addition, the number of bills, ordinance and rule initiatives, and approval rates were high, and the active activities of member research groups, the number of corrective questions, and the characteristics of diversification were shown. Through the analysis results, the following research implications were presented. First, it is necessary to revitalize and specialize in the activities of the parliamentary committee, and to this end, it is necessary to strengthen job training and revitalize workshops to strengthen expertise. Second, in order to strengthen the expertise of legislative activities, it is necessary to revitalize the policy support officer system that is performing the assistant function. Third, in order to elect local council members with various expertise and strengthen their expertise in parliamentary activities, it was suggested to strengthen the activities of local council members' banquet. Fourth, it is necessary to revitalize the parliament's own research group to strengthen the expertise of legislative activities. 이 연구는 지방의회 의정활동에 대한 이론적 논의를 토대로 주요 분석 국면을 조직과 운영, 정책결정 기능, 집행기관 견제기능으로 설정하여 정읍시의회 제1대-제8대까지의 의정활동을 심층적으로 분석한 후 지방의회 의정활동 강화방안을 제시하는데 연구목적을 두었다. 연구 수행을 위해 조직 및 운영 측면은 연령·학력·직업 등 인적 현황과 의회 구성 요인을 통해 살펴보았고, 정책결정기능 측면은 조례규칙안, 동의 및 승인 처리, 건의 및 결의안 처리, 연구단체 등의 요인을 중심으로 살펴보았다. 집행기관 견제기능 측면은 행정사무감사 및 조사, 시정질문, 위원회 제도 및 활동 등으로 분석을 수행하였다. 분석결과 일반적 특성은 대부분 대졸 이상의 고학력이었고, 여성의원 비중이 적었으며, 직업의 다양성이 부족하였다. 또한 의안 수, 조례·규칙 발의 및 가결비율이 높고, 의원연구단체의 활발한 활동, 시정질문 건수의 증가 및 다양화의 특성이 나타났다. 분석결과를 통해 다음과 같은 연구의 시사점을 제시하였다. 첫째, 의회위원회 활동의 활성화와 전문화가 필요하며, 이를 위해 전문성 강화를 위한 직무연수 강화, 워크숍 등이 활성화가 필요하다. 둘째, 의정활동의 전문성 강화를 위해 보좌기능을 수행하고 있는 정책지원관 제도의 활성화가 필요하다. 셋째, 다양한 전문성을 가진 지방의원 선출 노력이 필요하고 의정활동 전문성 강화를 위해 지방의회의원 연찬회 활동 강화를 제시하였다. 넷째, 의정활동의 전문성 강화를 위해 의회 자체 연구모임 활성화가 필요하다.

임신부에서 측두하악장애의 진단과 치료

In this case report, I discussed the diagnosis and treatment of two pregnant women with temporomandibular disorders(TMD) who visited the Department of Oral Medicine, PNUH. Also, I reviewed some investigations of diagnosis and treatment of TMD in pregnant women. The obtained results were as follows; 1. No single X-ray diagnostic procedure for TMD results in radiation dose that threatens the well-being of the developing embryo and fetus. 2. Most non-steroidal anti-inflammatory drugs(NSAIDs) have commonly used because these drugs are considered to be nonteratogenic, but these agents are not recommended for routine use after 3rd trimester. 3. Electro-acupuncture stimulation therapy(EAST) is contraindicated for 1st trimester, and ultrasonic deep heat therapy, microwave deep heat therapy, low level laser therapy, myo-monitor are not contraindicated for pregnant women but clinician must consider some risk of adverse fetal effects. 4. The occlusal stabilization splint may be used for pregnant women, if it is fabricated indirectly. 5. Surgical treatment is contraindicated for pregnant women.

Assessment of Collagen-Induced Arthritis Using Cyanine 5.5 Conjugated with Hydrophobically Modified Glycol Chitosan Nanoparticles: Correlation with 18F-Fluorodeoxyglucose Positron Emission Tomography Data

Objective: To evaluate the potential and correlation between near-infrared fluorescence (NIRF) imaging using cyanine 5.5 conjugated with hydrophobically modified glycol chitosan nanoparticles (HGC-Cy5.5) and 18F-fluorodeoxyglucose-positron emission tomography (18F-FDG-PET) imaging of collagen-induced arthritis (CIA). Materials and Methods: We used 10 CIA and 3 normal mice. Nine days after the injecting collagen twice, microPET imaging was performed 40 minutes after the intravenous injection of 9.3 MBq 18F-FDG in 200 μL PBS. One day later, NIRF imaging was performed two hours after the intravenous injection of HGC-cy5.5 (5 mg/kg). We assessed the correlation between these two modalities in the knees and ankles of CIA mice. Results: The mean standardized uptake values of 18F-FDG for knees and ankles were 1.68 ± 0.76 and 0.79 ± 0.71, respectively, for CIA mice; and 0.57 ± 0.17 and 0.54 ± 0.20 respectively for control mice. From the NIRF images, the total photon counts per 30 mm2 for knees and ankles were 2.32 ± 1.54 x 105 and 2.75 ± 1.51 x 105, respectively, for CIA mice, and 1.22 ± 0.27 x 105 and 0.88 ± 0.24 x 105, respectively, for control mice. These two modalities showed a moderate correlation for knees (r = 0.604, p = 0.005) and ankles (r = 0.464, p = 0.039). Moreover, both HGC-Cy5.5 (p = 0.002) and 18F-FDG-PET (p = 0.005) imaging also showed statistically significant differences between CIA and normal mice. Conclusion: NIRF imaging using HGC-Cy5.5 was moderately correlated with 18F-FDG-PET imaging in the CIA model. As such, HGC-Cy5.5 imaging can be used for the early detection of rheumatoid arthritis. 권위주의 체제의 비용을 통한 신호 전달

이 글은 1975년 월남에서 억류되었던 공관원들의 석방을 위한 한국 정부의 외교를 "비용신호이론"으로 설명한다. 월남 패망 이후 철수하지 못한 한국 공관원은 외교관 특권을 인정받지 못한 채 임시혁명정부에 의하여 억류되었다. 그중 외교관 신분을 가졌던 내무부 소속 공관원 1명과 중앙정보부소속 공관원 2명은 반혁명 죄목으로 수감되었다. 이들은 월남 당국의 심문은 물론 2차례에 걸쳐 북한 요원들로부터 심문, 전향권유, 그리고 "타의망명"으로 표현되는 강제북송위협까지 받았다. 이들은 한국 정부의 석방 노력으로 1980년 4월 석방되었다. "비용신호이론"은 한국 정부의 석방교섭을 이해하는데 두 측면에서 도움을 준다. 첫째, 한국 정부는 억류된 외교관과 수감된 공관원을 석방시키기 위한 교섭을 진행하면서 철저하게 언론보도를 통제하였다. 최초 구출계획에 있었던 언론공개의 조건이 모두 충족되었지만, 한국 정부는 조용한 외교에 의존하였다. 권위주의 정권이 공약을 이행하지 않기 때문에 치러야 하는 국내 정치적 비용은 민주주의 국가보다 작다. 따라서 한국 정부가 공관원의 억류ㆍ수감 사실을 공개하고 구출을 위한 노력을 천명한다고 하더라도, 한국 정부가 월남 정부에 보내는 신호의 신뢰성이 높아지지 않는다. 둘째, 한국 정부의 의중을 임시혁명정부와 이후 통일 월남에 전달하는 과정에서 한국 정부가 감당하는 비용이 점차 증가하였다. 한국 정부가 비용이 포함된 신호를 보내자 월남이 반응하였다. 이후, 거액의 이면 금전거래와 남파 간첩 송환까지 수용할 의사를 보이자, 석방 교섭이 급속하게 진행되었다.

전북지역 아동 성 폭력 범죄의 특이성 분석과 예방

아동 성 폭력은 가까운 친족에 의해 범해지는 경우가 많고, 또한 지속적으로 행해지는 특성을 갖고 있어 피해아동의 정신적 피해와 가족들이 겪는 고통은 매우 크다. 아동 성 폭력은 피해 아동과 가해자가 밝히지 않는 한 사실을 밝히기 어렵고, 피해 아동이 자신을 보호할 수 있는 능력이 없고, 성폭력 범죄의 일반적인 특성 가운데 가해자와의 관계를 보면 최소한 가해자와 피해자간의 안면이 어느 정도 있는 사이에서 발생하는 경우가 60% 이상 차지하고 있는 것으로 나타나고 있다.1 특히 아동 성 폭력의 경우에는 친족에 의해 범해지는 경우가 많고, 이러한 피해는 성인이 된 이후에도 지속되는 것으로 나타난다. 본 연구에서는 전북ONE-STOP 센터에서 조사되어진 성폭력 피해아동 139명을 대상으로 전북지역의 아동 성 폭력 범죄의 특이성을 분석하여 전북지역의 아동 성 폭력 범죄에서 나타나는 범행의 특이성을 피해자의 면식여부, 피해자, 가해자 등을 중심으로 비교분석하여 예방책을 마련하고자 하였다.

『보뮈뉴의 한 남자』에 나타난 신화적 세계

Cette étude a pour l’objet d’examiner le monde mythique dans Un de Baumugnes, le deuxième volume de la trilogie Pan de Giono. Nous pouvons remarquer trois thèmes suivants : ‘la nature, Pan, TOUT’, ‘la Terre-Mère’ et ‘la parole et la musique’. Dans Un de Baumugnes, beaucoup d’éléments et des êtres prennent chacun la valeur dynamique dans leur substance, forme et geste. Giono les transforme en êtres vivants et féminins. Chaque élément de la nature est très proche de l’image complète de la nature. C’est ainsi que Giono représente la nature ou toutes les choses en matière. Il ne se contente pas d’écrire, il veut intensivement comprendre et goûter le monde avec ses sens. Les éléments que Giono a décrits sont recréés comme la propriété de la vitalité débordante par anthropomorphisme en tant que ‘Pan’, c’est-à-dire, un concept de ‘toutes choses ou le grand Tout divin’ étymologiquement. Les êtres- Angèle, Philomène et Albin- sont aussi semblables à la nature-Pan. Ils se manifestent avec l’image naturelle et féminine. En particulier il faut nous remarquer qu’Albin, un de ces trois personnes, apparaisse maternellement. Avec l’air de l’harmonica Albin le voyant ressuscite Angèle et la Douloire mourant. La musique de guérir provient de son pays natal Baumugnes. Baumugnes où l’homme et la nature s’harmonisent prend la valeur de l’espace symbolique, à savoir, cet-endroit là où le son de la nature est justement la musique. Donc Albin qui est originaire de Baumugnes est Baumugnes lui-même, dès lors son pays qu’il raconte, c’est précisément l’histoire et toute l’histoire. La musique salvatrice qui s’exprime par odeur, son et couleur de la nature devient la parole du vent comme un grand orchestre cosmique. Elle diffuse et infiltre la nature de Baumugnes dans la Douloire. En véhiculant les sens de Baumugnes-nature, cette mélodie fait relever la Douloire. Les impressions sensorielles établissent comme ça la communication avec la totalié cosmique. C’est ainsi que la musique d’Albin qui mêlée de son et de parole est complexe et polyphonique, donc elle a tellement la pouvoir magique et mystérieuse. Cette parole intemporelle, ça signifie la langage poétique qui unit l’Homme à la Nature. Alors il communique avec la nature et parvient à faire partie de la vie universelle. En fait, cela nous permet de constater que l’air de la Monica d’Albin est la musique verbale dépassant la langage humaine. Giono, qui peint nature le son de la nature pareille à la musique, doit être musicien, poète et sorcier. Les éléments mythiques que nous avons découverts en cette oeuvre réactualisent un mystère. Enfin ils font remonter à nous jusqu’au temps sacré, dans ce processus récurrent nous pourrions rêver la nouveauté du commencement du Temps-un concept de la pureté-avec la vitalité et la créativité cosmique.
