        알레르기성 비염에 대한 한약 및 양약 국소비강외용제의 효과 비교 : 체계적 문헌 고찰

        Objectives : The purpose of this study is to evaluate the effectiveness and safety of topical nasal application of Herbal medicine compared with Western medicine in the treatment of Allergic Rhinitis(AR). Methods : Electronic databases including Cochrane library, PubMed, EMBASE, CNKI, KMBASE, KISS, NDSL, OASIS, KISS and KJTK(Korean Traditional Knowledge Portal) were searched by the keywords such as ‘allergic rhinitis’, ‘nasal sprays’, ‘herbal medicine’, ‘plant extracts’, and ‘external application’. The quality of each RCTs was assessed by Cochrane Collaboration of ‘Risk of bias(RoB) Tool’. Results : 19 RCTs were finally selected from 1419 references screened. 19 RCTs were compared with the effects of topical nasal application of Herbal medicine and Western medicine. Based on the symptom scores from 13 RCTs, topical nasal application of herbal medicine generally has a better effect on relief of AR. The two treatments have similar effects on improving the level of specific factors like IgE, IgG, IL-13, uLTD₄ in blood and urine. 8 RCTs showed adverse effects(AEs) in both groups and severe AEs were not reported. Conclusions : This study shows that topical nasal application of herbal medicine can improve symptoms and related factors of allergic rhinitis. Well-designed RCT studies with low risk of bias should be conducted to confirm these findings.

        응급실로 내원한 비의도적 의약품 중독

        Purpose: The social environment of easy access to medicines and arbitrary personal decisions leading to overdose aggravate unintentional medicine poisoning. This study aimed to investigate the characteristics of patients who visited emergency departments with unintentional medicine poisoning and reasons for poisoning based on age group. Methods: We retrospectively collected patients who experienced unintentional medicine poisoning based on data from the national injury surveillance system between 2013 and 2016. Subjects were classified into three groups based on age (0-14 years, 15-64 years, and ≥65 years). We identified sex, insurance, time of poisoning, place, alcohol co-ingestion, hospitalization, death, and reason for poisoning in each age group. Results: A total of 27,472 patients visited an emergency department with poisoning during the study period; 1,958 patients who experienced unintentional poisoning were enrolled in this study. Respiratory medicine was the most frequent medicine in those younger than 15 years of age, and sedatives and antipsychotic drugs were the most common in patients older than 15 years of age. In total, 35.1% of patients older than 65 years were hospitalized. The most common reasons for poisoning were careless storage of medicine in those younger than 15 years of age and overdose due to arbitrary decisions in those older than 15 years of age. Conclusion: Unintentional medicine poisoning has distinct characteristics based on age group, and strategies to prevent poisoning should be approached differently based on age.

        “내 상처에 네 손을 넣어라”: 체리에 모라가의 Heroes and Saints가 말하는 몸 없는 물질성

        With the playwright’s proses that energetically emphasize the recovery and redefining of Chicanx identities and motherland, Cherríe Moraga’s Heroes and Saints usually has been interpreted as a play that represents political and religious possibilities enabled by bodiless Chicana divinity. Cerezita Valle, the play’s heroine who does not have a body under her neck due to prenatal pesticide poisoning, transfigures herself as La Virgen de Guadalupe in order to purge the polluted Madre Tierra(mother-land) and exhort the people of San Joaquin Valley to form joint confrontation. Her absent body, a powerful symbol representing Madre Tierra, expands boundlessly and works as a useful vehicle for delivering urgent political messages, incorporating diverse and divergent Chicanx communities. This article concurs with the previous studies that read Cerezita’s bodiless body as a Chicanx hero-saint who represents the encompassing and caring mother-land required by mestizas living in a world polluted by pesticides and global capitalism. However, this article also argues that Cerezita’s body cannot be read only as an immaterial symbol. Cerezita is a complicated figure which reformulates various dichotomies: political hero and religious icon, Virgin Mary and La Malinche, and ethereality and corporeality. Moraga transfigures Cerezita into La Virgen de Guadalupe with a heavy price; the playwright highlights that Cerezita yearns for female sexuality and desire, thus making it clear that the heroine should be reduced to a religious/political icon only when she painfully abandons her disabled physical corporeality. Nevertheless, by doing so, the play makes the audiences recognize Cerezita’s bodily materiality on stage that cannot be seamlessly packed up into compelling metaphors. The bipolar, ambivalent vibration of the heroine illuminates the possibility of imagining the concrete and the indefinite simultaneously, suggesting the countermeasure against and the ethical representation of the prevalent ‘slow violence’ of the global environmental crisis.

        신경병증성 통증에 대한 소건중탕의 임상적 고찰: 후향적 환자군 관찰 연구

        Objectives: This study aimed to analyze the effectiveness of Sogunjung-tang on neuropathic pain. Methods: This study was conducted on 50 cases of patients with neuropathic pain who were treated in LUA clinic from Jun 2021, to Oct 2021. All patients were treated with Sogunjung-tang, and in some cases, acupuncture or moxibustion was also performed. We measured the efficacy of treatment with a numerical rating scale (NRS), frequency, and duration of pain before treatment, after 1 week, and 3 weeks. Results: Both average pain NRS and worst pain NRS significantly decreased 1 week and 3 weeks after treatment compared to pre-treatment. The frequency and duration of pain did not differ after 1 week of treatment but showed a significant difference after 3 weeks. Conclusions: This study suggests that herbal medicine treatment with Sogunjung-tang reduces pain intensity, frequency, and duration in patients with neuropathic pain.

        Efficacy and safety of blood stasis based herbal medicine for patients with traffic accident: A prospective observational study

        Background: Blood stasis (BS) is commonly used for pattern identification in traumatic injuries, including traffic accidents (TAs). Various studies have identified the efficacy of Korean medicine treatments for TA patients, but studies focusing on the BS-based herbal medicine (BSHM), including Tongdo-san (TDS), are rare. Methods: This was a single-center, prospective observational study, conducted from August 24 th , 2018 to December 27 th , 2018, which included 40 TA patients. Participants underwent routine Korean medicine treatments including acupuncture, electronic moxibustion, cupping, physical therapy, and herbal medicine. In the herbal medicine treatment, participants took BSHM with more than 3 days including taking TDS. The primary outcome measures were the scores from a 100 mm visual analogue scale (VAS) and numerical rating scale (NRS). Secondary outcome measures included scores from EuroQol-5 dimension (EQ-5D) and EQVAS questionnaires, the BS and cold/heat indices, and safety assessments. Results: There were significant improvements in the VAS, NRS, EQ-5D, EQ-VAS scores, and BS index after treatment. In the sub-analysis, VAS, NRS, EQ-5D, and EQ-VAS scores were higher in groups with a higher BS index. The moderate and severe BS index groups showed more improvement than the minor BS index group, and there was a significant difference in the EQ-5D scores. There was no significant differences observed in cold/heat index groups scores. Conclusion: BS is associated with TA-related symptoms. BSHM, including TDS, may significantly reduce BS, pain, and discomfort.

        교통사고 중상자의 한방치료 경험에 대한 질적 연구

        Objectives: The purpose of this study is to understand the experience of traffic accident patients with serious injury treated with Traditional Korean Medicine (TKM). Methods: This study was conducted on the basis of grounded theory. We recruited 10 participants who had been hospitalized in Dongguk University Bundang Oriental Hospital after traffic accident. Raw data was collected by in-depth interview, and analyzed by using constant comparison. The coding(open-axial-selective) was performed in order to create paradigm and theoretical model, which can explain main theme of this study. Results: Core phenomenon of this study was 'Experiencing TKM treatment as an alternative' and core category was 'Searching for the answer to the management of traffic accident sequela through TKM treatment' Conclusions: For patients with serious injury, 'Experiencing TKM treatment as an alternative' has meaning as the following: First, The absence of accurate radiography and surgical management makes it hard for patients to choose Korean medicine hospital immediately after traffic accident. Second, Effectiveness of TKM treatment should be evaluated in the long term. Third, TKM treatment is superior to Western medicine treatment in managing long-term patients who suffered from serious injury involved in traffic accident. Finally, Cooperative service of the Western medicine and TKM will provide more satisfactory medical care for traffic accident patients.

        온침의 온도변화 특성에 대한 문헌 고찰 연구

        Objectives: The purpose of this study is to organize the research methods and results of studies related to the temperature of the warm needle for systematic utilization of warm needling technique. Methods: This study used the databases of nine (Pubmed, Science Direct, Cochrane Central, 4 Korean databases, CNKI, CiNii) to analyze temperature-related studies of the warm needle from 2000 to June 2019. Results: A total of 19 papers were included. Of these, 15 were used for mugwort, 2 for high frequency, and 1 for both mugwort and high frequency, and the other one for a ceramic heater. The maximum temperature rises as the amount of moxibustion increases. It is also affected by the density of moxa and the ignition part. There were 16 papers using stainless steel needles and 4 papers using a needle made of gold or silver to compare. In the area of the needle, the closer it is to moxibustion, the hotter it is. Compared to stainless steel needles, gold and silver needles showed almost twice the temperature. The effects of environment and radiant heat should be considered during warm needle procedures. Conclusions: There are various experimental methods such as warm needle technique materials, methods, measuring parts, measuring instruments, etc. The results were also very diverse. When setting the heating source, ignition part, size of moxibustion, etc. of warm needles, it should be implemented in a way that takes safety and validity into account. Considerations for temperature characteristics, radiant heat, etc. of warm needles will be needed when making warm needle apparatus.

        Effectiveness of Acupuncture for Scoliosis: A Systematic Review

        Scoliosis is a 3-dimensional change of the spine, presenting 10° above Cobb angle. Various acupuncture methods are being increasingly performed to correct scoliosis. However, no systematic review has been published. Therefore, we report on the effectiveness of acupuncture on mild scoliosis in this systematic review. We searched various databases for acupuncture treatment for scoliosis published before June 2021. The primary outcome was Cobb angle, and the secondary outcomes were patient-centered scores. Six studies were identified. RCTs that compared combination therapy with acupuncture, and other treatment methods and showed significant improvement in the Cobb angle, and effective rate, but, with low quality evidence. Acupuncture monotherapy resulted in significant improvement in the Cobb angle and had high heterogeneity compared with other treatment methods, but no significant improvement in the effective rate was observed, and the quality of evidence was low. Regardless of the type of scoliosis, acupuncture monotherapy or combination therapy with acupuncture showed significant improvement in the Cobb angle, and the quality of evidence was moderate. No serious adverse events were observed in the 6 studies reviewed. Acupuncture is considered safe, and it is more effective when performed with other conventional treatments.

        Rapid, sensitive and specific detection of Brucella canis by real-time PCR assay using HybProbe

        We designed and evaluated a specific HybProbe set for identification of Brucella canis, which is based on a specific single nucleotide polymorphism (SNP) of the omp25 gene. Real-time PCR assay using this probe specifically identified B. canis reference and field strains, however, the other Brucella species and general bacteria strains were not amplified. This real-time PCR assay can re-confirm specific amplification in melting curve analysis. The average melting temperature values for generation of B. canis strains was 67℃, however, that of the other Brucella species was 62℃, due to a single base-pair mismatch at the binding site of the probe. The detection limit appeared to be 30 fg of DNA and 100 CFU by serial dilution of whole blood inoculated with B. canis. In addition, use of the buffy coat resulted in approximately 50 times higher sensitivity than whole blood directly. This technique is widely applicable for use in diagnostic laboratories and is readily discriminative with regard to small amounts of biomass.
