한은정,김윤범,Han, Eun-jeong,Kim, Yoon-bum 대한한방안이비인후피부과학회 2003 한방안이비인후피부과학회지 Vol.16 No.1
Maxillary sinus retention cysts are probably common findings on PNS series. Many patients remain asymptomatic in the presence of these lesions, but a variety of local and systemic symptoms have been associated with them. A patient presented with symptoms of chronic rhinosinusitis had a finding of both maxillary sinus retention cysts on PNS series. He has taken the Gal-gen-tang and treated by acupunture & infra-red. After about 4 months' treatment, his symptoms resolved and PNS series were normal. In conclusion, Maxillary sinus retention cysts may become clinically important when they occur in the setting of symptoms compatible with chronic rhinosinusitis, or when the diagnosis is in doubt.

Objectives : Gal-Geun-Tang (GT) has been described from SANGHAN in Korean traditional medicine and known to act against cold, fever, hypertension, and nasal catarrh. However, little has yet been learned about the effect of GT on immune function. In the current study, in vitro and in vivo immunomodulatory activity of GT (water extract) was investigated. Methods : Water extract of GT induced in vitro proliferation of spleen cells and significantly increased their proliferative responses during anti-CD3 activation. Using purified splenic T and B cells, it was revealed that GT has a mitogenic activity to B cells and promotes their proliferation induced by lipopolysaccharide, whereas T cell proliferation was not triggered and GT was rather inhibitory to T cell activation caused by anti-CD3 antibody. In the presence of antigen presenting cells (APC), GT addition resulted in a significant increase of IFNγ and IL-4, but not IL-2, production. However, addition of high concentration (1,000㎍/㎖) of GT led to a marked reduction in T cell cytokine production and under such condition, GT facilitated apoptosis of T cells when examined by flow cytometry with propidium iodide staining. Results : In vivo immunomdulation of GT was also investigated using a mouse model. Following keyhole limpet hemocyanin (KLH) immunization, GT (1 ㎎/day) was orally administered for 9 days. Cell numbers in thymus, spleen and peripheral blood were not altered by GT administration, indicating that such dose is not immunotoxic. Cell numbers in draining lymph nodes (LN) and ex vivo Ag-specific proliferation of LN cells were significantly elevated by GT administration. However, any preferential stimulation of T or B and CD4+ or CD8+ T cell subpopulations was not observed in a flow cytometric analysis of LN cells. This result shows that GT does not promote in vivo B cell proliferation while GT enhances Ag-specific proliferation of LN cells, unlike what was observed in vitro. Conclusions : For a further understanding of in vivo immunomodulatory activity of GT, ex vivo cytokine production of LN cells obtained from KLH-immunized mice was evaluated. Ag-specific IFNγ production was significantly higher in GT-treated mice when compared to PBS-treated control mice. In contrast, IL-4 production in GT-treated group was comparable to control group unlike to in vitro data. In addition, GT administration did not result in any significant differences in serum levels of Ig (IgM, IgG1 and IgG2a) between GT-treated and control groups. Taken together, these data strongly support that GT promotes immune response, more profoundly type 1 helper T cell (Th1) activity and GT may be applicable for treatment of intracellular parasite infection such as viral diseases.

Objectives : This study was performed to investigate the protective effects and mechanisms of Salvia miltiorrhizae Radix extract (SME) on endotoxin shock. Methods : We used two models; LPS-induced sepsis model for in vivo model, and murine peritoneal macrophages responses for in vitro. SME was administrated orally to mice. After 1 hr, LPS was injected intraperitoneally. Survival rate was checked each time per 12 hr for 5 days. Mice were sacrificed 3 hr after LPS injection, then blood samples and organs were harvested. Cytokines secretion was measured by ELISA. Organs tissues were observed with microscope. Murine peritoneal macrophages were cultured for 1 hr either in a medium alone or in a medium that contained SME, as indicated. Then, the cells were treated with LPS for 24 hr. mRNA levels of cytokines were measured by real-time RT-PCR. Cytokine levels in the supernatants were measured by ELISA. The amount of nitrite was measured by using the Griess method to evaluate NO production. The cell lysates were analysed by Western blotting using antibodies for iNOS and β-actin was used as an internal control to monitor equal protein loading. Results : SME improverd the survival rate of mice model. SME inhibited the secretion of inflammatory cytokines and organs damages on Endotoxin Shock model. SME suppressed cytokine expression, cytokine secretion,NO production, iNOS expression in LPS-induced murine peritoneal macrophages. Conclusions : The results suggest that SME has protective effects on endotoxin shock through suppression of inflammatory cytokines, organ damages, NO production and so on.

Objectives : Recently, the demands for the effective and safe depigmentating and anti-aging agents of the skin have increased due to the medical, pharmaceutical and cosmetic reasons. The aim of this study is to search new materials from the traditional herbal medicines which inhibit the aging process of skin in vitro. Methods : Human dermis cell (HS68) was used to test the effect of Rhus verniciflua Stokes (RVS). 80% ethanol or water extracts were screened for their inhibitory activities against elastase. Elastase inhibition effect was tested by microplate reader instrument. And MMP-1 suppression effect of RVS was tested by western blot. These cells were investigated the viability by MTS assay. And also the inhibition effect of tyrosinase by RVS was tested. Results : RVS (final concentrstion 1 ㎎/㎖ appeared over 30% of inhibition of elastase activity. So we are investigated anti wrinkle effects of Rhus verniciflua Stokes look through MMP-1 inhibition activity, also Extracts of RVS showed higher anti-tyrosinase activity than arbutin as final concentration 1 mg/ml. These results suggest that herbal medicines could be strong potential sources of inhibition of anti-aging and whitening effects for the skin.Colclusions : RVS was the best suppressor candidate of elastase activity among other control oriental drugs. It was found that RVS did not have toxicity to cells. We found proper concentration of RVS to treat in HS68 culture and investigated not only wrinkle care effect but also whitening effect of RVS.

Objectives: This study aimed to verify the classification of atopic dermatitis into the digestive and respiratory disorder on the basis of a literature study. Methods: We searched for the term "atopic dermatitis" in KISS, RISS, NDSL, DBPIA, and OASIS. On further filtering the searched paper further by including the terms "Korean medicine", "literature study" and excluding "acupuncture", we found eight papers relevant to the literature study of atopic dermatitis. The reviewed papers included keywords of Oriental medicine such as Sub-yeol (濕熱), Bi-heo (脾虛), Poong-sub (風濕), Hyeol-heo (血虛). We classified keywords as "digestive system" or "respiratory system" and compared the clinical symptoms of classified Each category keywords. Results: Atopic dermatitis was classified into four categories : acute digestive disorder, chronic digestive disorder, acute respiratory disorder, and chronic respiratory disorder. The four categories of atopic dermatitis showed differences based on region, dermal symptoms, associated symptoms, and affected age group. Conclusions and Discussions: This study suggests a new theory of Atopic dermatitis classification. The theory is similar to that stated in former classification; however, it focuses on the digestive and respiratory disroder. All of atopic dermatitis into digestive disorder show both digestive symptoms and dermal symptoms. However, atopic dermatitis into respiratory disorder shows only respiratory symptoms. Thus, this study can establish a relationship between Western and Oriental medicine's study of atopic dermatitis using keywords such as "digestive disorder" and "respiratory disorder".
