Background/Aims: We aimed to determine the concordance rate and clinical predictors of eosinophilic esophagitis (EoE) in patients with endoscopically suspected eosinophilic esophagitis (ESEoE) findings. Methods: From June 2006 to December 2009 in Gangnam Severance Hospital, we prospectively enrolled the patients of endoscopically suspected eosinophilic esophagitis (ESEoE), and then we retrospectively reviewed and analyzed clinical features and endoscopic findings. Results: We found 17 patients of ESEoE, and 5 of them were finally confirmed as an EoE by histology (diagnostic concordance rate 29.4%). We added two more patients previously diagnosed as EoE and compared patients of EoE+ (n=7) with EoE- (n=12). Mean age was 56.0 (range 36-70) and 51.0 (range 36-68) years old, respectively. In EoE+ group, there were 5 males and 2 females and 6 males and 6 females in EoE- group. The symptoms of EoE+ patients were dysphagia (n=5), food impaction (n=3), foreign body sensation in esophagus (n=2), chest pain (n=1), and heartburn (n=1). EoE- patients complained food impaction (n=5), heartburn (n=4), chest pain (n=2), foreign body sensation in esophagus (n=2), and dysphagia (n=1). The endoscopic findings of EoE+ patients were furrows (n=6), rings (n=5), exudates or nodules (n=3), and friability (n=1). EoE- patients showed rings (n=10) and furrows(n=7). Univariative analysis showed that `a symptom of dysphagia`, `presence of exudates or nodules`, `more than 2 suggestive endoscopic findings` were significantly different between two groups. Conclusions: In ESEoE, diagnostic concordance rate was 29.4%. In addition, `symptom of dysphagia`, `exudates or nodules`, `≥2 endoscopic findings` are more suggestive of EoE in ESEoE patients. 견갑골 골절에 대한 임상적 분석

Background: Scapular fractures are uncommon, usually the result of major blunt trauma and frequently overlooked on initial supine chest radiography. Certainly, the emphasis in the initial workup of patients with scapular fractures is to recognize that these patients have a high degree of associated injury. Methods: Of l0898 blunt trauma patients who were treated in Chung-Ang Gil Hospital from Jan. 1995 to Dec. 1996, 96 cases with scapular fractures were analyzed retrospectively. Results: 1) The most common age group was third decade. 2) The ratio of male to female was 2.6:1. 3) Most common cause of injuries were car accidents(74%). 4) Most common part of fractures of scapula was body portion(45%) and 2nd most common part was neck portion(25%). 5) Scapular fractures had commonly associated fractures. Concomitant rib fractures were 40% and clavicular fractures were 27% of the patients. 6) Associated injuries often led to delayed recognition and treatment of the scapular fractures. Conclusion: This study demonstrates that scapular fractures are commonly associated with thoracic injury, that are clinically more significant than the scapular fractures. After blunt trauma, patients with scapular fractures should be examined cautiously and promptly about concomitant ipsilateral regional skeletal or soft-tissue injuries.

Clinical criteria have established for idiopathic hypereosinophilic syndrome(HES): persistent eosinophilia of 1500 eosinophils/㎣ for at least 6 months or death even within 6 months with signs and symptoms of HES; no evidence for parasitic, allergic, or other recognized causes of eosinophilia despite careful evaluation; and signs and symptoms of organ system involvement or dysfunction, such as congestive heart failure, hepatosplenomegaly, central nerveous system disease. Because cerebral hemorrhage in HES has not been reported yet in Korea, we report a case of hypereosinophilic syndrome with peripheral blood eosinophilia, with biopsies of skin and duodenum showing diffuse eosinophilic infiltration, and multiple organ dysfunction in a 49 year old man died of cerebral hemorrhage three months after the occurrence of the disease.

In this paper, The implementation of Design Space Explorer which is one of the synthesis tools in AHS(An Automatic Hardware Synthesizer) system is discussed. Design Space Exploror adopts a three-demensional structure in contrast to the existing two-demensional structure. Design Space Explorer makes the three-demensional design space by calculating execution speed and the cost of the design system according to design level. So, The system designer can get the optimal design condition by comparing each design level in the three-demensional design space.

Solar Cell의 효율개선에 관한 연구

This paper treats of the characteristics of series resistance and shunt resistance, which is the internal resistance of solar cell, in order to improve the conversion efficency of solar cell. In due consideration of series and shunt resistance, the author contrived to seek a quantitative analysis of I-V characteristic curve of solar cell, fill factor and conversion efficiency. The author also tried to infer reduction degree of the actual conversion efficiency compared with the one under the ideal conditions.

6502 Microprocessor用 簡易型 In-Circuit Emulator

One of the simplest ways of debugging the microcomputer system is removing CPU and directly debugging thorugh the CPU socket. But this method often need some transformation of the hardware. So, it may damage the system more or less. In this study, a simplified In-Circuit Emulator by which debugging can be performed through I/O slot of the microcomputer system, is proposed. In this experiment, the Apple Ⅱ microcomputer is used as the host and target system. This system can be used in the other type of computers which have 6562 CPU by way of forming the universal I/0 port on it.

두개저부 골절에 대한 임상적 분석

188 consecutive cases with basilar skull farctures(BSF) out of 2676 head injury patients who were treated in Chung-Ang Gill Hospital from July 1993 to June 1995, were analyzed. These type of fractures are difficult to diagnose by ordinary X-ray examinations on routine head computed tomography(CT), and are frequently inferred by clinical signs. Therefore it's diagnosis is sometimes delayed or missed in initial assessment of trauma patients at emergency department. BSF are different from cranial vault fractures in several aspects other than difficulties in the diagnosis. It more commonly involves the cranial nerves(CN), makes cerebrospinal fluid(CSF) fistulas, and leads to central nervous system(CNS) infections if the CSF fistulas are not detected and treated early and properly. The authors reviewed the clinical features, radiological findings, rate of delayed diagnosis, complications and outcomes of BSF. The most common feature of BSF was otorrhea(64.4%), and other features were rhinorohea(39.4%), raccoon eye(32.4%), and hemotympanum(24.5%). The fractures lines were detected by ordinary skull radiographs and diagnosed as BSF in only 6.4%. The high resolution skull base CT confirmed the fractures in 62.2%. Clinical diagnoses were made in 14.9%. Commonly combined craniofacial lesions were cranial vault fractures(51.1%), intracranial hemorrhages(46.3%), and facial bone fractures(34.0%). Most(89.7%) of CSF leakages were noted within 24hours after accidents, and most(87.7%) of the leakages were ceased by conservative management within 2 weeks, but 5.1% that did not respond to conservative treatment and lumbar CSF drainage, required invasive operative repair. The rate of meningitis was 3.2%, and the prophylactic antibiotics had no beneficial effect to decrease the infection rate. Facial nerve was the most frequently involved cranial nerve, and vestibulo-cocchlear, occulomotor, and olfactory nerve were involved in decreasing order. The onset of facial palsy was immediate in 31.8% and the rest of them were delayed more than 24 hours after head injury. Of 188 patients, 21 cases(11.2%) had delayed diagnosis of BSF.

The acoustic impulse response was studied in order to evaluate qua1ity Initially, the linearity and effects of sample holding conditions on the acoustric signals were demonstrated. Secondly, acoustic signals were obtained at points whlch divide the equator of the sample into 24 equal parts. As a result of the ana1yses, it become evident that the impulse waveform of acoustic signals induced by impact was transmitted along the equator with uniform velocity. The lower transmission velocity in the time domain agreed with the lower peak frequency domain, both theoretically and practically.
