Background: In Korean medicine, carpal tunnel syndrome is treated by stimulating the acupoints around the wrist. Although a deep understanding of anatomy and guidance is needed to stimulate these acupoints to avoid undesirable side-effects, currently there are no published guidelines for acupotomy treatment. The aim of this study is to evaluate the effectiveness and safety of fluoroscopy-guided acupotomy compared with conventional acupotomy treatment. Methods: This is a randomized, patient-assessor, patient blind, parallel clinical trial. A total of 30 patients will be enrolled at Wonkwang University Gwangju Hospital, and will be allocated to either an experimental group or a control group. The experimental group will be treated using fluoroscopy-guided acupotomy and the control group will be treated using the conventional acupotomy method. Results: The primary outcome measure will be identification of a cross-section area of the median nerve measured by ultrasonography, and the secondary outcome measure will be the alleviation of pain measured by the Visual Analogue Scale, improvement in the Nerve Conduction Study, Tinel test, Phalen’s test, EuroQol 5-dimension scale, and Boston Carpal Tunnel Questionnaire score. Safety components will be measured by monitoring vital signs, electrocardiographs, blood tests, general chemical tests, urine tests and pregnancy tests. In addition, observations for adverse effects will be performed during the trial. Conclusion: This study will provide a more effective, and less harmful way of treating carpal tunnel syndrome compared with conventional acupotomy. Fluoroscopy-guided acupotomy will help practitioners to be accurate in direction and depth of the needle for treating carpal tunnel syndrome.
Case Series Study on the Use of BU Pharmacopuncture Treatment in Patients with Acute Lumbar Sprain

Objective: The purpose of this study was to investigate the clinical effects of BU pharmacopuncture therapy consisting of bear’s gall(fel ursi) and ox bezoar(bovis calculus) on acute lumbar sprain. Methods: 12 patients diagnosed as acute lumbar sprain in 6 designated local Korean medicine clinics from October 2017 to February 2018 were treated by BU pharmacopuncture. Several acupoints in abdomen and lumbar region were selected by clinicians at their own discretion. The effectiveness of the therapy was evaluated using VAS and ODI. After that we reviewed the medical records of all these patients to evaluate the effectiveness and safety of the therapy. Results: The VAS and ODI scales were significantly decreased after BU pharmacopuncture therapy. And no major complications and adverse effects were reported. Conclusion: BU pharmacopuncture showed rapid pain relief in patients with acute lumbar sprain. It is possible to shorten the treatment period of acute lumbar sprain and prevent progressing to chronic back pain in advance. To establish the effects of BU pharmacopuncture therapy, more succeeding clinical and laboratory studies are needed.
26-Week Repeated Dose Oral Toxicity Study of KCHO-1 in Sprague-Dawley Rats

Objectives: KCHO-1(Mecasin), also called Gamijakyakgamchobuja-tang originally, is a combination of some traditional herbal medicines in East Asia. This medicine has been used mainly for alleviating neuropathic pains for centuries in Korean traditional medicine. KCHO-1 was developed to treat pain, joint contracture and muscular weakness in patients with amyotrophic lateral sclerosis. This study was carried out to investigate the chronic toxicity of KCHO-1 oral administration in rats for 26 weeks. Methods: Sprague-Dawely rats were divided into four groups and 10 rats were placed in the control group and the high-dose group, respectively. Group 1 was the control group and the remaining groups were the experimental groups. In the oral toxicity study, 500 mg/ kg, 1,000 mg/kg, and 2,000 mg/kg of KCHO-1 were administered to the experimental group, and 10 ml/kg of sterile distilled water was administered to the control group. Survival rate, body weight, feed intake, clinical signs, and visual findings were examined. Urinalysis, ophthalmologic examination, necropsy, organ weight, hematologic examination, blood chemical examination and histopathologic examination were performed. Results: Mortality and toxicological lesions associated with the administration of test substance were not observed in all groups. Conclusion: NOAEL(No observed adverse effect level) of KCHO-1 is higher than 2000 mg/kg/day. And, the above findings suggest that treatment with KCHO-1 is relatively safe.
The Effects of Acupotomy Therapy on Carpal Tunnel Syndrome: A Report of 4 Cases

The aim of this report was to show the effects of acupotomy in patients with carpal tunnel syndrome. Four patients were treated with acupotomy twice. Visual analogue scale (VAS), Tinel`s sign, Phalen`s test, Boston carpal tunnel syndrome questionnaire (BCTQ), muscular strength test, and a cross-sectional area of median nerve was measured using ultrasound before and after treatment. In all 4 cases, the VAS score, BCTQ score and cross-sectional area of median nerve, all decreased and muscular strength test score increased. Tinel`s sign and the Phalen`s test changed from a positive to a negative in most cases. This report shows that acupotomy is an effective treatment for carpal tunnel syndrome. Further larger are needed to fully evaluate the beneficial effects of this treatment.

Introduction: This case report assessed the effects of Korean medicine treatments on a chronic migraine patient who did not respond to triptan. Case Presentation: The patient received Korean medicine treatments, such as acupuncture, moxibustion, cupping, and herbal medication, during a hospitalization period. The effects of treatments were assessed with the Numerical Rating Scale (NRS) and migraine attack frequency. The intensity and attack frequency of migraines were significantly reduced after the complete treatment. Conclusions: For migraines, Korean medicine such as acupuncture and herbal medication could be an alternative to conventional therapy.

Objectives This study is a preliminary study for safe and accurate cervical Hyeopcheok acupuncture by comparing the depth of cervical Hyeopcheok acupoint with cervical spine rotation. Methods Researchers took ultrasonographic images of cervical Hyeopcheok acupoint during full rotation of cervical spine, 45 degree rotation and neutral condition in a prone position. 4 healthy volunteers were recruited. Results No significant difference in needling depth of cervical Hyeopcheok acupoint was observed. However, there were a differences in anatomical structures’ shape such as muscles. Conclusions There are no significant difference in needling depth, but anatomical structures’ shape were changed. This result mean that if patient rotate cervical spine during acupuncture treatment, it can occur complications. Thus, when acupuncture treatment on cervical Hyeopcheok acupoint, practioner should make subject’s cervical spine alignment corrected and be careful not to change posture during the procedure. Objectives The purpose of this study is to carry out a survey on satisfaction for Korean medicine treatments in traffic injury patients. Methods Satisfaction survey was carried out on the 237 traffic injury patients who had been treated by Korean medicine treatments in Department of Korean Rehabilitation Medicine, Wonkwang University Gwangju Medical Center from February, 2017 to August, 2017. The questionnaire form of preceding study was used to assess the satisfaction degree of patients. All data were coded by SPSS v20.0 (IBM Corp., Armonk, NY, USA) and analysed by descriptive maneuver, x2-test, T-test and correlation. Results Among the 237 cases, the first hospital to be selected for traffic injury treatment was Korean medicine hospital in 91 cases. And 97 patients were transferred from the western medicine hospital. The main reason for choosing Korean medicine hospital was that the patients prefer Korean medicine treatments than western medicine treatments. 85.2.% of patients answered that their symptoms were improved, and 75.1% of patients answered that they were satisfied with Korean medicine treatments. The satisfaction degree of cupping treatment and acupuncture was relatively high, but the degree of Herbal medicine and physical treatment was relatively low. There were no statistically significant differences in satisfaction degree with sex and age. The satisfaction degree, improvement of symptoms, intention to revisit and intention to recommend was significantly correlated each other. Conclusions The degree of improvement of symptoms and The satisfaction degree were revealed relatively high. Additional and repetitive studies are needed to understand the patient's position and improve the quality of Korean medicine treatments for traffic accident patients. 성향정기산가미방을 중심으로 한 한의약 치료를 통해 단기간에 호전을 보인 전순환계 뇌경색 후 발생한 지남력장애 및 현훈을 주소로 한 환자 1례에 대한 보고

Introduction: The aim of this study was to report the effect of Korean medicine treatments on a stroke patient with acute cerebral infarction. Method: A 63-year-old male with anterior and middle cerebral artery infarction had symptoms of dizziness and disorientation. The patient was treated with Korean medicine therapy, including the herbal medicine “Sunghyuangjunggi-san-gamibang.” Treatment progress was assessed using the Korean version of the Modified Barthel Index (K-MBI) and the Korean dizziness handicap inventory (K-DHI). Results: After 14 days of treatment, the K-DHI score decreased from 78 to 7 and the K-MBI score increased from 18 to 24. The patient’s main symptoms were improved after the treatment, and no side effects were observed. Conclusion: Korean medicine treatment, including Sunghyuangjunggi-san, might be a recommended therapeutic option for dizziness and disorientation in stroke patients.
