민간의료보험 형태가 보건의료 형평성에 미치는 영향: 선진국의 경험을 통한 정책적 함의

본 연구는 최근 활발히 진행되고 있는 민간의료보험 역할 설정에 대한 논쟁을 보건의료 형평성의 관점에서 해석하였다. 민간의료보험 역할에 관한 기존 연구는 주로 효율성의 관점에서 진행되었으나 본 연구는 보건의료정책의 주요한 목표인 형평성의 차원에서 민간의료보험 역할 확대가 갖는 영향을 평가하고자 하였다. 본 연구에서 보건의료 형평성은 보건의료 재원조달 형평성과 보건의료 서비스 이용 형평성으로 구성하였다. 또한 민간의료보험의 형태를 보충형, 부가형, 대체형, 중복형으로 구분하고 각각의 형태가 보건의료 형평성의 두 가지 차원에 미치는 영향을 선진국의 경험을 통해 해석하였다. 재원조달 형평성의 경우, 주요한 의료서비스에 대한 공공지출을 통한 보장이 존재하는 캐나다(부가형), 네덜란드(대체형), 영국(중복형)의 경우 민간의료보험은 일종의 ‘사치재’로 고소득층에 의해 주로 구매되므로 그 자체로는 누진성을 띠게 된다. 의료서비스 이용에 대한 본인부담이 높은 프랑스(보충형)에서는 민간보험 가입이 광범위하게 확대되어 민간보험 재원조달이 역진적인 것으로 나타났다. 의료서비스 이용 형평성의 경우, 본인부담의 크기가 소득계층별로 의료이용의 차이를 야기할 수 있는 캐나다와 프랑스의 사례에서 민간보험은 민간보험을 구매한 계층의 의료이용을 증대시키는 것으로 기능하고 있었다. 공급자에 대한 보상에 영향을 주는 영국과 네덜란드 사례에서는 민간보험 구매층과 비구매층의 이용 서비스 분리현상이 나타났다. 이러한 선진국의 경험을 통해, 현재 확대되고 있는 본인부담 보충형의 민간보험은 역진적인 재원조달을 야기할 수 있고, 민간보험 구매능력이 있는 고소득층의 의료이용을 증대시켜 의료서비스 이용 형평성을 저해할 수 있음을 정책적 함의로 제시하였다. This study attempts to analysis the impact of private health insurance on equity in health care system. In this study, equity in health care system has two dimensions. One is equity in health care financing, the other is equity in health care utilization. The types of private health insurance can be classified complementary type, supplementary type, substitute type and duplicate type. This study expects that each type of private health insurance has different effect on equity in health care system. Through reviewing the experience of developed countries, I draw upon some policy implications about the relationship of private insurance type and equity in health care system. First, the expansion of complementary private insurance could make more regressive health care financing system in Korea. Second, in Korea, there is the bundle of the sector of National Health Insurance coverage and non-coverage. Therefore complementary/ supplementary type insurance can similar function that those who purchase the private insurance use more health care services.
The Latent Structure L2 Literacy Ability and Intercultural Sensitivity of Korean EFL Students

This study explored L2 literacy ability and intercultural sensitivity of Korean late elementary to early middle school students learning English as a foreign language. This study investigated the latent variable structure of L2 literacy abilities, including fluency, vocabulary, reading comprehension, and writing abilities, and intercultural sensitivity which involves interaction engagement, respect for cultural differences, interaction confidence, interaction enjoyment, and interaction attentiveness. The present study employed a non-experimental, correlational design. One hundred twenty-two 5th and 6th grade elementary students and one hundred forty 7th and 8th graders in middle school in Korea participated in this study. The results of confirmatory factor analysis indicated one measurement model of L2 general literacy ability and intercultural sensitivity; these two latent factors are correlated with each other. In addition, four indicators of literacy ability (fluency, vocabulary, reading comprehension, and writing) were strong predictors of L2 learners’ literacy achievement. Likewise, four indicators (interaction engagement, respect for cultural differences, interaction confidence, and interaction attentiveness) were highly correlated to intercultural sensitivity, but interaction enjoyment was not correlated to intercultural sensitivity. Therefore, interaction enjoyment was removed from the measurement model of literacy and intercultural sensitivity.
Reading global literature with different grade levels and genders

The purpose of the present study is to evaluate the effects of reading global literatureon students’ literacy ability and intercultural sensitivity in regard to students’ genderand grade levels, with the same data as in Bae’s (2013) study. This study is a onegrouppretest-posttest design. Participants were 131 Korean students from the 5th to 8thgrades, and they read and studied picture books of global literature for 15 weeks. Therewas no gender difference in the effects of reading global literature on L2 literacyachievement and intercultural sensitivity development, as demonstrated by latent meananalyses. On the other hand, there were some differences in intercultural sensitivityachievement for different grade levels. Only 7th grade students did not show asignificant improvement in intercultural sensitivity, especially the sections on respectfor cultural differences, interaction confidence, and interaction attentiveness. Furthermore, the interaction attentiveness measure of intercultural sensitivity sawstatistically significant improvement in the other three grade levels.
수종 Laminate Veneer용 레진 시멘트의 변연누출 비교

The purposes of this study were to evaluate the microleakage of 3 porcelain laminate resin cements bonded to enamel & dentin with 3 dentin surface treatment. Enamel was treated with 37% phosphoric acid and dentin was treated with Gluma (Bayer Dental, Germany), All-bond(Bisco. Co.U.S.A.) and G-C Fuji Ⅱ (G-C co. Japan). After Porcelite (Sybron/Kerr, U.S.A.), Choice (Bisco. Co. U.S.A.) and Laminabond (Shofu Co. Japan) resin cements were luted to each porcelain veneer specimen, the specimens were stored in water at 37℃ for 24hr, and were thermocycled at 4℃ and 60℃ for 100 cycles with dwelling of 1 minute in each methylene blue dye bath. Then, specimens were sectioned centrally with a low speed diamond saw and the microleakage of each specimen was evaluated with a light microscope. Th results were as follows. 1. A greater microleakage at the tooth/composite resin interface occured for 3 resin cement groups than porcelain/composite resin interface(p<0.05). 2. A greater microleakage at the dentin/composite resin interface occured for 3 resin cements groups than enamel/composite resin interface(p<0.05). 3. The microleakage difference between Gluma treated group & All-bond treated group was not statistically different, but between Gluma treatd group & Glass ionomer treated group and All-bond treated group & Glass ionomer treated group was statistically different(p<0.05). 4. The microleakage difference at tooth/resin interface among Porcelite group, Choice group and Laminabond group was not statistically different, but the microleakage difference at porcelain/resin interface between Porcelit group & Choice group and Porcelite group & Laminabond group was statistically different(p<0.05).
2015 개정 교육과정이 적용된 초·중등 영어교과서의 읽기 지문 연계성 분석

The present study explored to examine the continuity between 6th grade of elementary and 1st grade of middle school English textbooks with 2015 Revised National Curriculum. Five 6th grade elementary school English textbooks and eleven 1st grade middle school English textbooks were analyzed through the readability, text lexical and syntactic features, and cohesion by using Coh-Metrix tool. For this analysis, five categories of the Coh-Metrix measures were used including basic descriptive results of texts, lexical and syntactic features, cohesion, and readability scores, and 23 measures were involved in total. The results showed that the continuity between 6th grade of elementary and 1st grade of middle school English textbooks was not well controlled in the perspective of text quantity. The other measures of lexical and syntactic features and cohesion also indicated the inconsistent level increasing between two different grade levels. However, the readability of 1st grade middle school English textbooks was higher than that of 6th grade elementary school English textbooks. All these results showed that the continuity of English textbooks for two different grade levels was not suitably organized. Pedagogical implications and future research directions were discussed.
